Letter from the Cancer Committee Chair

The Mid Coast Hospital Center for Cancer Care is a Commission on Cancer-accredited program offering a cross-disciplinary, wide-ranging approach to patient care.

In 2019, Mid Coast Hospital made several key developments in the cancer care we provide to our community, including improvements in breast cancer navigation, a new tobacco cessation and prevention program, and increased access to clinical trials.

Treatment opportunities will also continue to expand in 2020 through our relationship with the MaineHealth Cancer Care Network. In 2019, MaineHealth received a federal grant for its new Lifespan Program, which will allow patients at Mid Coast Hospital to participate in advanced clinical trials right here at home.

As we look ahead, we are proud of the advancements we have made in the past year and for our patients’ ongoing trust in the compassionate, comprehensive care we provide daily. We look forward to continuing our legacy of exceptional cancer care in 2020, and are grateful, as always, for our community’s continued support.

Creating Opportunities for Prevention & Screenings CANCER PREVENTION EDUCATIONAs part of its commitment to building a healthier community, the Mid Coast Center for Community Health & Wellness holds regular free lectures regarding cancer prevention.

Food for Health is a series offered in the spring and fall that provides cooking demonstrations and education showing how chronic diseases can be prevented, treated, and reversed with a plant-based diet. Topics this year included:

• Healthy Diet: Separating Fad from Fact • Food and Inflammation • How a Plant-Based Diet Reduces Risk of Infectious Diseases

From January through June 2019, 508 members of the community attended a Food for Health lecture, which is more than double the 2018 attendance. Due to the popularity of the series, more Food for Health lectures were held in 2019 than in the previous year.

PROMOTING COMPREHENSIVE TOBACCO CESSATION AND EDUCATIONIn fall 2019, as part of its efforts as a Maine District Tobacco Prevention Partner, Mid Coast Hospital launched a new tobacco prevention and cessation program. The BeFree Tobacco Prevention & Treatment Program seeks to prevent all types of tobacco use among youth, including e-cigarettes, prevent exposure to secondhand smoke in public spaces, and provide treatment for those who use tobacco.

BeFree supports members of the Midcoast community in their efforts to quit using tobacco by:

• Increasing community members’ awareness of resources that can help them quit • Providing free tobacco cessation counseling • Gifting all participants with a free quit kit

SKIN CANCER SCREENING REVEALS CANCER RISKEach year, Mid Coast Hospital holds a community skin cancer screening aimed at promoting early detection and prevention of skin cancer. Offered free of charge to the public, this year’s event was held on Thursday, June 13, 2019.

Of the 34 individuals screened, 6 had positive findings requiring biopsy or follow up. They were given names of local dermatologists and advised to seek further medical care.

All who participated were also given education materials on skin cancer prevention and treatment. The screening was a collaboration of the American Academy of Dermatology, the American Cancer Society, and Mid Coast Center for Community Health & Wellness, as well as Mid Coast-affiliated dermatology experts.

MID COAST HOSPITAL 2019 CANCER CARE ANNUAL REPORT

Treating the Total Patient PSYCHOSOCIAL DISTRESS SCREENING During patients’ initial consult, the Mid Coast Hospital Center for Cancer Care asks they complete the National Comprehensive Cancer Care Network (NCCN) Distress Thermometer, in order to assess their psychosocial needs. The NCCN Distress Thermometer rates patients’ distress on a scale of 1 through 10, and asks them to answer questions about concerns unrelated to physical health, such as family and emotional problems.

The provider and nurse navigator review and discuss the results with each patient, and referrals are made as needed. Patients in a crisis situation are referred to the Mid Coast Hospital Emergency Department.

In 2019, the Mid Coast Hospital Center for Cancer Care increased its number of distress screens given over the previous year, as well as the number of referrals to the hospital’s oncology-focused social worker. The majority of patients that scored a 5 or higher on the screening were referred to receive social work services.

ENHANCING CASE REVIEW PROCESS In 2019, following a recommendation from a Commission on Cancer surveyor, the Mid Coast Hospital Center for Cancer Care expanded its Case Review and Tumor Conference process in an effort to take a more holistic view of patients.

This expansion included involving more members of each patient’s care team in the twice monthly meetings, in an effort to take a more all-encompassing view of the patient.

The meetings now include consistent attendance of the patient’s navigation team, comprising three nurse navigators and one social work navigator, as well as a palliative care provider and inpatient services. The meetings are also now more comprehensive, with staff discussing more in-depth patient information that contributes to care, such as the results of their psychosocial distress screening and financial and transportation needs.

In addition, Interdisciplinary Team meetings are now held twice weekly, and the Medical Infusion Center team holds a daily huddle to help stay connected to patients and their conditions and symptoms.

Building the Continuum INCREASING ACCESS TO CLINICAL TRIALSDue to Mid Coast Hospital’s membership in the MaineHealth Cancer Care Network (MHCCN), a coordinated system of care comprising 12 MaineHealth member and affiliate hospitals, our cancer patients have access to world-class, coordinated cancer treatment. It is supported by a grant by The Harold Alfond Foundation.

That access expanded in an exciting way in September 2019, when the MHCCN received a six-year, $5.1 million award from the National Cancer Institute Community Oncology Research Program (NCORP). The grant allowed the cancer network to launch its Lifespan Program, the only oncology program in Northern New England to enroll patients in National Cancer Institute clinical trials at every stage of cancer from prevention to survivorship, and from pediatric to adult. The trials will have a range of objectives, from reducing the incidence of cancer to improving the way cancer care is delivered to patients.

The Lifespan Program will allow Mid Coast Hospital patients, and patients from other MHCCN hospitals, to participate in advanced clinical trials in their own communities, and provides a wonderful and potentially transformative care opportunity for patients in rural areas.

Additionally, in 2019, five Mid Coast Medical Group-Cancer and Blood Disorders patients participated in a clinical trial through the Maine Cancer Genomics Initiative, which offered them a comprehensive genomic profile that could be used to identify second-or third-line therapies.

We are grateful the MaineHealth Cancer Care Network Lifespan Program will make more innovative treatment opportunities a convenient possibility for our patients in 2020.

MAKING ADVANCEMENTS IN PATIENT NAVIGATIONPatient navigation in cancer care refers to individualized assistance offered to patients, families and caregivers to help overcome barriers and facilitate timely access to quality medical and psychosocial care.

In 2019, Mid Coast Hospital began participating in an initiative by Project ECHO, an organization dedicated to giving providers in rural areas the knowledge they need to provide high-quality specialty care.

Specifically, Mid Coast Hospital is participating in a Project ECHO program focused on breast cancer navigation. The program assists care sites throughout the state of Maine without a breast-specific program in reviewing patient cases for genetic testing eligibility.

Each month, our Mid Coast Hospital Breast Cancer Navigator joins a Project ECHO video meeting as an expert panel member. The initiative is being evaluated by the Maine Cancer Care Network, and, depending on its success, may be expanded to include other yet-to-be-determined projects.
