JOB SUMMARY AND PURPOSE

The Program Manager I supervises all e�orts to ful�ll the goals and objectives of the Program, designing plans and operationsaccording to contract and budget guidelines. Intake, assessments, service planning, service delivery, and goal attainmentTraining, supervision, and problem-solving for di�cult casesMeeting grant objectives, adhering to grant guidelinesProvides information to Division Director highlighting progress to reaching set goalsProvides quality service to clients with rental issues.Monitor program compliance to VOALA policies and required procedures; maintains documentation of all programservices delivered to participants.Measures and assess impact of services, supports, and participates in key elements to the program's successE�ectively communicates project expectations to team membersCoordinate outreach and collaboration e�orts with community partners and landlordsOther Duties as directed to accomplish program goals

Quali�cations

REQUIREMENTS:

Must be able to pass a �ngerprint clearance, background check, including criminal history, personal references,employment and education veri�cations

EDUCATION:

Bachelor's degree in Social Work (or related discipline) or a combination of relevant education and experience (4 years)

EXPERIENCE:

Minimum of 3 years of experience in Social Services Program Management, Coordination or SupervisionMinimum of 2 years of direct experience in Sta� Management

KNOWLEDGE:

Strong knowledge of case management ethics, HIPAA, Programmatic standards of operation and clinical engagement,clinical documentation methods, client focused ideology

The Case Manager – Generalist is responsible for helping people who are in di�cult situations with advice, �gure out whatkind of help they need, help them �nd the services they need, create plans for treatment or recovery, work with other healthand human service providers, and keep tabs on client’s progress with treatment plan. DUTIES AND RESPONSIBILITIES

The Case Manager - Generalist provides intensive case management services to eligible clients, meeting participants at leastweekly. The Case Manager is the primary point of contact to coordinate services and care for assigned clients. Each CaseManager handles a caseload of individuals in need of a wide variety of human services resources and provides the full range ofservices available, as described below:

Assign and monitor cases:Review daily referrals and screenings of clientsDesignates households for intake and ensures all supporting documents are veri�edProvides information and referrals for callers and walk-in clients

Conducts comprehensive screenings and assessments with clients to collect functional, environmental, �nancial,employment, housing, educational, and health information, as appropriate, to develop a case plan

Develops support systems to meet clients and family needs; coordinates and facilitates move-in activities, rentalnegotiations, and service referrals as requiredMay make home and �eld visits

Monitors cases on a regular basis to determine quality and e�ectiveness of services providedReview cases with Program Coordinator and Case Management TeamAudit case �le on a regular basis for completeness and accuracyConducts crisis intervention as necessary

Assists case managers by identifying and coordinating a variety of available services necessary for clients and families tomaintain/obtain self-su�ciency and family stabilization

Refers clients to available community providers for therapeutic, social, educational, and medical resources.Provides support to case managers and clients through ongoing case conferencing, counseling, and advocacy

Conducts comprehensive �le reviews and assessment of data collected to assure all necessary documentation is collectedby case managers for services eligibility purposes

Assure all assets and �le documents are maintained and secure for all clients

Counsels during regular case conferences with participants using Motivational Interviewing, Trauma-Informed Care, andSeeking Safety; consults with service team, monitors progress of clients on case plan, assists clients to meet the agreedupon goalsProvide service linkages and support systems to ensure identi�ed needs are met, coordinate with outside service providersActs as advocate for client before judicial, community, social service, and administrative bodies as needed; facilitatessupport groups and client participation as appropriateProvide street outreach, engagement, and crisis stabilization in coordination with other VOALA programsDevelops, performs, and assists with workshops pertaining to client’s needsParticipates and assists with outreach eventsOther duties as required and is subject to change at any time
