Case 1A 23-year-old woman developed weakness in her right hand and numbness in her pinky and ring finger after sleeping on her elbows during a long plane trip.

• The ulnar nerve originates from the medial cord of the brachial plexus (C8 and T1 nerve roots). The

• Grossly, the myelinated tissue appears white and is called the white matter. The cell bodies of neurons are darker and are called the gray matter. In the brain, the gray matter is located on the periphery, while the white matter is located on the inside. In the spinal cord, the gray matter forms an “H” shape inside the spinal cord, while the white matter is on the outside. The appearance of the white and gray matter on MRI depends on whether the image is T1-weighted or T2-weighted. On a T1-weighted pulse sequence, the gray matter looks gray and the white matter looks white. On a T2-weighted pulse sequence, this pattern is reversed.

The Myelin Sheath• The myelin sheath is a collection of lipids and

proteins that surrounds each axon. Within the CNS, myelin is produced by oligodendrocytes, while in the PNS it is made by Schwann cells. A single oligodendrocyte can myelinate up to 50 axons, while a Schwann cell myelinates a single axon. The function of myelin is to speed the electrical signal on axons. In myelinated axons, the electrical signal can jump from one node of Ranvier to another, referred to as saltatory conduction. In axons that have lost their myelin, there is a continuous and slower propagation of the action potential along the axon.
