CATHETERIZATION

Purposes:
1. To relieve acute or chronic urinary retention2. To drain urine pre-operatively and post-operatively3. To determine the amount of residual urine after voiding4. To determine accurate measurement or urinary drainage in critically ill patients
Materials:
Sterile gloves Catheter LubricantPlaster Syringe with sterile water Antiseptic solution Rubber sheetCotton balls Pick up forceps Bath blanket for draping
1 2 3 4 51. Washes hands2. Prepares equipments and brings preparation to bedside3. Identifies patient and explains procedure. Adjust bed in comfortable
height4. Provides privacy. Changes top sheet with bath blanket5. Adjusts bed protector, serves bedpan and gives external douche.
Position and drapes patient properly.6. Observes aseptic technique in opening the pack7. Places lubricant on OS. Pours betadine to 2 sterile CBs. Prepare
catheter.8. If foley catheter is used, prepare syringe. Aspirate 10 cc of sterile water.9. Put on gloves aseptically.10. Drapes area with fenestrated sheet11. Puts thumb and fingers to separate minora, paints area with CB soaked
in Betadine. Discards CB12. Adjusts kidney basin near perineum and lubricates catheter about 2
inches.13. Holds catheter at least 3 inches from the tip with the end coiled.14. Locates meatus and inserts catheter.15. If a urine specimen was ordered, direct free end of catheter to the
specimen bottle. Pinches catheter and removes bottle quickly and gently.
16. Inflates the catheter with 10cc syringe17. Removes fenestrated sheet and attach catheter to the urine bag.18. Keep patient dry and comfortable. Secures catheter on thigh.19. Returns used things clean and dry20. Labels specimen properly. Measures all urine withdrawn.21. Document the procedure done including the amount, color and unusual
odor of urine, reaction of patient during procedure including relief if obtained
22. Maintains body mechanics throughout the procedure23. Manifests neatness in the performed procedure24. Receptive to criticisms. Observes courtesy.25. Uses correct English. Shows mastery of the procedure.
