INSTRUCTIONS 1 ) Review the attached packet for CE and CME session approvals. 2 ) Sign in AND sign out by tapping your badge on the electronic scanners outside the workshop session rooms. Attendees must be present for the ENTIRE session to receive credit. 3 ) Access the online evaluation at an onsite CE/Evaluation kiosk (beginning Monday afternoon at Registration) or at www.TheNationalCouncil.org/Conference for up to 30 days (May 10, 2013) after the conference. INSTRUCTIONS

1 ) Review the attached packet for CE and CME session approvals.

2 ) Sign in AND sign out by tapping your badge on the electronic scanners outside the workshop session rooms. Attendees must be present for the ENTIRE session to receive credit.

3 ) Access the online evaluation at an onsite CE/Evaluation kiosk (beginning Monday afternoon at Registration) or at www.TheNationalCouncil.org/Conference for up to 30 days (May 10, 2013) after the conference.

Enter your badge number (located on your onsite name badge) and create a password.

Complete the demographic information requested.

Complete the evaluation for each session you attended.

Select which certifi cate(s) you need (see list on next page). Print a receipt.

For individuals applying for the National Association of State Boards of Accountancy’s (NASBA) CPE credits at the National Council for Community Behavioral Health-care’s 2013 National Council Conference, April 8-10, 2013 in Las Vegas, NV: The delivery method of educational instruction is “Group-Live.” Sessions are approved for the following fi elds of study: business management and or-ganization, fi nance, personal development, personnel and human resources, and specialized knowledge and applica-tion (ex. healthcare, HIPAA compliance). The Program Level is Intermediate. For prerequisites, attendees need a basic understanding of how the public behavioral health system works and how services are delivered and paid for. No advanced preparation is required. An attendee can earn up to 13.5 CPE credits for attending the full conference. In addition, an attendee can earn 6.5 CPE credits for attending a qualifying preconference university or 17.0 CPE credits for attending Three Days for Docs.

CE and CME Credit must be requested and online evaluations completed within 30 days of the conference – by May 10, 2013.
