CHECKLIST Provincial Duty MUST BRING: PRC FORMS (Managed DR, Cord Care, OR Scrub, & OR Circu) Index cards (Managed DR, Cord Care, OR Scrub, & OR Circu) Complete Type A Standard Uniform with (Cap / Badge) Scrub suits, bonnet and mask Smock gown OR/DR slippers (must not be used in the dormitory) Sterile Gloves (at least 5) IMPORTANT STUFF Students are expected to bring complete articles/supplies; no one is allowed to leave the dormitory and hospital premises for personal purposes Toiletries good for entire stay in the province o Soap, shampoo/conditioner, tooth brush, tooth paste o Underarm deodorant o Razor/Shaver for boys o Sanitary napkins/linens and light make up for girls o Facial tissue and 1 pack wet tissues Night clothes i.e. Pajamas Everyday comfortable casual clothes enough for 1 week Rubber slippers to be used in the dormitory Maintenance Medications :for ASHTMA OR ALLERGIES when indicated REMEMBER All articles, clothing, supplies must be placed inside one luggage. Minimize your baggage allowance. Avoid bringing too my bags for the travel. Each student may only bring one luggage and one bag (back pack) Bottled water and finger-foods for the travel.

*subject to disciplinary acts, student will be sent home prior to the completion of provincial duty
