a national quality system for children's healthcare which encompasses care provided through the Medicaid program and the Children's Health Insurance Program (CHIP)”

CHIC PROJECT BACKGROUND• Infrastructure grant – not for
provision of direct services• Joint effort between Idaho and
Utah• National evaluation:
o Effectiveness of the demonstration programs
o Impact of demonstrations on healthcare quality of children enrolled in Medicaid or CHIP
o Increase in transparency and participant choice
o Impacts on costs of care

information technology in children’s healthcare• Develop/enhance practice
improvement partnership(s) in Idaho and Utah• Evaluate the impact of using a
medical home approach to deliver children’s healthcare

information technology in children’s healthcare deliveryo Develop interface with public health
immunization data through IHDEo Incorporate Idaho services and
resources into the Medical Home Portal
o Develop systems to extract/report quality measures from EHR
o Develop Pediatric Patient Summary (PPS)

Medical Home Demonstrations - Visiono Primary care and sub-specialty child health
practices in Idaho and Utah function as effective medical home teams providing high quality, coordinated, and collaborative care in partnership with parents and with particular attention to children and youth with special healthcare needs.
o Consistent with the Patient-Centered Medical Home definition from the State Medical Home Collaborative groups in Utah and Idaho respectively.

Medical Home Demonstrations - Approacho Selected pediatric practices in Utah and
Idaho will receive:• LEARNING COLLABORATIVE SESSIONS• Embedded medical home coordinator
(shared with 1 or 2 other practices)• Help in selecting, training, and
supporting a family partner• Consultation and assistance from QI
practice coach• Support in building infrastructure and
systems to improve efficiency, support care coordination, and measure clinical and service quality
• Opportunities to share experiences, resources and lessons learned with other practices

Medical Home Demonstrations - Status
o Utah has a target start date of March 2011• 12 pediatric practices have been selected
(9 primary care and 3 sub-specialty)• Interviewing for medical home
coordinators and practice coach • Awaiting final input from Multi-payer
demonstration which would allow extra support
o Idaho is participating in development work with Utah to start Medical Home Demonstrations March 2012 in at least 3 pediatric practices

Learning Collaborative Sessionso Not just CME, measures and
demonstrates quality improvement
o Open to practices which care for children whether in the medical home demonstration or not
o At least one annually in Idaho starting 2011 with LC on Asthma
o CHALLENGE: delivery to multiple (3-5) locations around Idaho
