Pre- and Post-Hospitalization CoverageAll related expenses for up to 30 days before and up to 60 days after hospitalization are reimbursed under Health Forever.

Please note that these expenses must be related to the condition for w hich the individual is hospitalised.

ICU BenefitsOften hospitalization expenses shoot up in case there is a need to undergo intensive care treatment. In case the individual is

admitted for 7 days or more to the Intensive Care Unit of the hospital, w hich has got a minimum of 300 beds, a lumpsum

a. Cataract, Benign Prostatic Hypertrophy, Hysterectomy for Menorrhagia, Fibromyoma, Hernia, Hydrocele, Congenital

Internal Diseases, Fistula in Anus, Piles, Sinusitis are not payable for all Insured Persons.

b. Any heart, kidney and circulatory disorders are not payable for all Insured Persons suffering from

hypertension/diabetes. If these diseases are pre-existing at the time of proposal they shall not be covered even during

subsequent period of renew al.

Citibank w ill charge an annual transaction fee of Rs.195 per individual per policy. How ever if you book the policy online, this

fee of Rs.195 w ill not be charged. The premium mentioned above w ill be reduced by 10% if you cover 3 or more members of

your family.For example, if you buy Health Forever for yourself, your spouse and 2 children, the net premium payable w ill be

10% less than the total cost as per the table above

Claims

You can make a claim under your Health Forever coverage in tw o w ays:

1. Get admitted to a hospital that is part of the Health Forever netw ork, call the 24-Hour toll-free helpline (1800 345 88 99) and

take an authorization at least 72 hours in advance. Thus you avail of Health Forever's Cashless hospitalization benefit. Royal

Sundaram w ill settle the bills directly w ith the hospital.

In case of an emergency hospitalization, call Medicare w ithin 48 hours after being admitted to take the authorization.

2. If admitted into any other hospital, all bills along w ith supporting documentation are to be sent to Royal Sundaram for a

reimbursement.

Enrollment Once you enroll for the plan, the applicable premium depending upon the plan chosen by you w ill be debited to your Citibank

Credit Card or Bank Account. Coverage begins from the date Citibank pays premium to Royal Sundaram tow ards your Health

Forever Plan. Royal Sundaram w ill issue the policy certif icate along w ith your Cashless Health Card and send it to the Indian

mailing address w hich you w ould be providing in the form.

Exiting the planShould you w ish to cancel your Health Forever coverage, you just need to w rite to Citibank N.A, P.O. Box 4380, Anna Salai

P.O, Chennai 600 002. In case you cancel your coverage before the end of a full policy year, Royal Sundaram w ill refund

part of the premium paid as per applicable short period rates.

Policy Exclusions The Company shall not be liable to make any payment under this Policy in respect of any expenses w hatsoever incurred by

any Insured Person in connection w ith or in respect of:

1. Pre-existing Conditions: Any medical condition or injury, w hich existed before the commencement of the policy period

(even if unknow n to the insured) or for w hich care, treatment or advise w ere sought, recommended by or received from a

Doctor. Complications arising from a pre-existing medical condition or injury w ill be considered part of such pre-existingcondition or injury. Pre-existing conditions w ill be covered after f ive years of continuous insurance w ith us.

2. 30 Days Waiting Period: Any disease contracted by the Insured Person during the f irst 30 days from the commencement

date of the cover. This exclusion shall how ever, not apply if in the opinion of Panel of Medical Practitioners constituted by the

Company for the purpose, the Insured Person could not have know n of the existence of the Disease or any symptoms or

complaints thereof at the time of making the proposal for insurance to the Company.

3. First Year Exclusions: During the f irst year of the operation of the Policy the expenses on treatment of:

w aived for Insured Persons holding an existing Health Insurance Policy, provided the Policy is renew ed before the expiry

date of the existing Health Insurance Policy.

4. Treatment arising from or traceable to pregnancy/childbirth.

5. Circumcision unless necessary for treatment of a disease not excluded hereunder or as may be necessitated due to an

accident.





a. Ionising radiation or contamination by nuclear fuel or from any nuclear w aste from burning nuclear fuel or

b. Radioactive, Toxic, explosive or other dangerous properties of any explosive nuclear machinery or part of it.

6. Any treatment relating to change of life.

7. The cost of spectacles, contact lenses and hearing aids.

8. Dental treatment or surgery of any kind unless requiring hospitalization.

9. Convalescence, general debility, a `run-dow n' condition or rest cure, external congenital disease, defects or anomalies,

sterility, venereal disease or intentionalself injury.

10. All expenses arising out of any condition directly or indirectly caused by or associated w ith Human T-Cell Lymphotropic

Virus Type III (HTLB-III) or Lymphadenopathy Associated Virus (LAV) or the Mutants Derivative or variations of Deficiency

Syndrome or any syndrome or condition of a similar kind commonly referred to as AIDS. (Information available in the Policy

Summary booklet).

11. Charges incurred at Hospital or Nursing Home primarily for diagnostic, X-ray or laboratory examinations or other

diagnostic studies not consistent w ith or incidental to the diagnosis and treatment of the positive existence or presence of

any ailment, sickness or injury, for w hich confinement is required at a Hospital/Nursing Home as defined.

12. Expenses on vitamins & tonics unless forming part of treatment for injury or disease as certif ied by the attending

Physician.

13. Voluntary medical termination of pregnancy during the f irst 12 w eeks from the date of conception.

14. Injury or disease directly or indirectly caused by or contributed to by nuclear w eapons/materials or radioactive

contamination.

15. Injury or disease directly or indirectly caused by or arising from or attributable to War, Invasion, Act of Foreign Enemy,

Warlike Operations (w hether War be declared or not).

16. Any claim or expense of any kind caused directly or indirectly by:

Any routine or preventative examinations, fascination, inoculations or screening.

18. Outpatient treatment charges.

19. Sex change or any treatment w hich results from, or is in any w ay related to, sex change.

20. Hormone replacement therapy.

21. Renal dialysis, except w here this is in connection w ith acute secondary failure and is part of the intensive treatment.

22. The treatment of psychiatric, mental or nervous conditions and insanity.

23. Any cosmetic, plastic surgery, aesthetic or related treatment of any description, w hether or not for phychological

reasons, unless medically necessary as a result of an accident.

24. Use of intoxicating drugs/alcohol and the treatment of alcoholism, solvent abuse, drug abuse or any addiction and medical

conditions resulting from, or related to, such abuse or addiction.

25. Any treatment received in convalescent homes, convalescent hospitals, health-hydros, Nature Cure clinics or similar

establishments.

26. Any stay in hospital for any domestic reason or w here there is no active, regular treatment by a specialist.

27. Any treatment received outside India.

28. Industrial disaster.

29. Any Ayurvedic, Homeopathic, Naturopathy or any other form of local medication.

30. Complication of surgery, therapy or treatment administered on the Insured Person w hich is not prescribed or required by

a Registered Medical Practitioner/Registered Medical Institution in their professional capacity.

31. Taking of drugs unless it is taken on proper medical advice and is not for the treatment of drug addiction.

32. Any fertility, sub-fertility or assisted conception operation.

33. Any person w hilst engaging in speed contest or racing of any kind (other than on foot), bungee jumping, parasailing,

ballooning, parachuting, skydiving, paragliding, hang gliding, mountain or rock climbing necessitating the use of guides and

ropes, pot holing, abseiling, deep sea diving using hard helmet and breathing apparatus, polo, snow and ice sports and

activities and similar hazards.

34. Any person w hilst engaging in aviation, w hilst mounting into or demounting from or traveling in any aircraft other than as

a passenger (fare paying or otherw ise) in any duly licensed standard type of aircraft anyw here in the w orld. ["Standard

type of Aircraft" means any aircraft duly licensed to carry passengers for hire or otherw ise by appropriate authority

irrespective of w hether such aircraft is privately ow ned or chartered or operated by a regular airline or w hether such an

aircraft has a single engine or multiple engine"].
