This Addendum forms a part of the contract documents and modifies the original Contract Documents, Specifications and Drawings for Construction, Volumes 1 & 2, dated August 7, 2018 and any previous Addenda.

Bidders must acknowledge receipt of this addendum by noting in the space provided on page 2 of the Bid Form included in the contract documents. Questions will be accepted until end of day on Thursday January 31, 2019.

General

1. Permitting: The City/Engineer has secured the floodplain development permit for the pole barn, a copy of which will be provided to the successful bidder. The Contractor is responsible for all other permitting.

1. Replace Davis Bacon wage decision ID51 dated 09/07/2018 with revised wage decision dated 01/18/2019, attached.

2. Replace General Special Provision GSP 0001 1.13.C (Construction Staking Item C) with the following: Construction Staking will be provided once free of charge for each alignment, separately. I.E.: Four (4) site visits for staking, one per alignment, shall be provided without additional charge. Re-staking shall be at the Contractor’s expense.

Example: PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014. Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.

Survey Rate Identifiers Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier. Survey wage rates are not updated and remain in effect until a new survey is conducted. Union Average Rate Identifiers Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier. A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based. ---------------------------------------------------------------- WAGE DETERMINATION APPEALS PROCESS 1.) Has there been an initial decision in the matter? This can be: * an existing published wage determination * a survey underlying a wage determination * a Wage and Hour Division letter setting forth a position on a wage determination matter * a conformance (additional classification and rate) ruling On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the

Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.) and 3.) should be followed. With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Washington, DC 20210 The request should be accompanied by a full statement of the interested party's position and by any information (wage payment data, project description, area practice material, etc.) that the requestor considers relevant to the issue. 3.) If the decision of the Administrator is not favorable, an interested party may appeal directly to the Administrative Review Board (formerly the Wage Appeals Board). 2. Project Scope

a. Schedule I – Lift Station & H2S Mitigation Improvements

Paul stated that existing pumps will remain, and damaged portion to be repaired. New pump is to be additional pump. For clarification: Pump 2, including slide rail and pump mount shall be removed and surrendered to the City. New pump, including new slide rail and pump mount, shall be installed in its place.

b. Schedule II – Wastewater Collection System Improvements

3. Environmental/Archeological Monitoring Requirements

a. SWPPP – Contractor is responsible for submitting a Stormwater Pollution Prevention Plan (Incidental to work, Per ISPWC 1002).

i. NOI – Notice of Intent required for disturbance over 1 acre.

b. Archeological – Monitoring required for excavation of all previously undisturbed areas.

Archeological services provided/coordinated by TD&H.

i. Coordinate monitoring with RPR. Contractor responsible to provide 48 hours notice to RPR prior to beginning excavation.

c. Asbestos Concrete Handling Abatement and Disposal

i. All removal and disposal shall be completed according to EPA regulations.

Incidental to bid item. Must be compliant with ISPWC/EPA.

4. Funding Agency Requirements

a. Project Sign per outline in the Supplementary Conditions

i. Project sign is a 4x8 designed with funding agency logos. Template will be provided at preconstruction meeting (or before).

b. Davis Bacon Wage and Labor Compliance shall be complied with.

i. Employees to go through Wage Interviews.

ii. Contractor shall submit certified pay rolls weekly. Updated wage decision will be issued with Addendum 1. Will be discussed in detail at preconstruction meeting. If there are any questions or if you anticipate that an additional classification will be needed please let CEDA know.

c. Section 3 Compliance – the last page of the Post-Award Contract Forms in the contract manual has the steps to comply. Voluntary reporting for contracts exceeding $100,000. Angie Edwards (CEDA) will work with contractors to complete.

d. American Iron and Steel – This project is partly funded by Rural Development; materials shall comply with A.I.S. requirements.

5. Site

a. Contractor is responsible for inspecting the site to assess conditions (General Special Provisions 1.3.B.1 and others) before bidding.

b. See General Special Provision 1.7.

c. Waste Site and Staging Areas

i. The contract work shall be performed within the rights-of-way. Individual service work may be coordinated with homeowners.

ii. The City has properties that may be used for staging upon written request. Contractor responsible for protection of staging areas and restoration to pre-project conditions.

The City may have space available for rock and dirt, dependent on amount, for temporary waste site.

d. Site Tidiness (General Special Provision 1.17) – The contractor is responsible for maintaining a safe and tidy site.

6. Construction

a. Pipe installation by trench excavation is required for bidding.

7. Construction Staking (General Special Provision 1.13)

a. TD&H to provide construction staking once. If stakes are disturbed, Contractor shall be responsible for fees associated with restaking.

b. Request shall be made in writing. 72 hours (3 working days) advance notice shall be provided.

8. Site Restoration

a. In general, the project site shall be restored to pre-project conditions (General Special Provision 1.19).

9. Schedule

a. Work shall commence within 30 days of the effective date of the Agreement or within 60 days of the Bid Opening, whichever is earlier.

b. 60 working day contract to Substantial Completion, 80 days to Final Completion.

Working days are considered to be Monday through Friday

c. Contractor is responsible for submitting an initial schedule within 10 days after the effective date of the agreement.

d. Weekly Progress Meetings shall be scheduled by the RPR and attended by the Contractor’s project superintendent or manager.

10. Project Construction Cost Range

a. Schedule 1 & 2 Total: $780,000 – $835,000.

11. Addendums

a. Addendum 1 is planned to be issued by the end of the week. It will include the minutes of this meeting. With updated wage rates and questions/responses.

12. Questions? (Questions asked during the meeting are in bold. Answers are in blue and were addressed post meeting by the Engineer)

a. How is the project to be awarded? Will both schedules be awarded to one contractor? Can the contractor bid on one schedule only? See GSP 0001 1.3.C and D. The schedules can be awarded separately or together. A contractor may elect to bid on only one schedule. The City of Stites reserves the right to award the Schedules in the manner they see fit. The low bid may be considered per schedule or total project.

b. Is there a contingency for unexpected utilities? There is a contingency. The Contractor is responsible to protect, preserve, and repair any damage to existing utilities. The City may elect to assist with repairs but are not required to do so. Do you know the water pipe material? To the best of our knowledge, the material ranges across most pressure pipe options. Asbestos concrete seems to be the most common but Poly, Steel, and Ductile are also present. (See attached figure – any locations shown on the figure are believed to be accurate but the Contractor is responsible for all field locates and confirmation of locations.)

c. Is there any allowance for potholing for utilities? Potholing is strongly encouraged as it is to the Contractors benefit. No additional compensation will be allowed for potholing.

d. Is rock excavation expected? None is anticipated. The majority of the work is in pre-disturbed areas and it is expected that previous backfill was performed per historic industry standards.

e. Is there a place for reject material disposal? None has been designated. The Contractor is responsible for all disposals. See GSP 0001 1.15.

f. Do we expect groundwater? Yes.

g. Contractors requested Paul Jones phone number. Paul provided during meeting: (208)935-5621. Please note that all questions and communication shall be routed through the Engineer. Unofficial communications are not grounds for adjustment of the Contract language. See Standard General Condition Article 9, 9.01.

h. Are there special requirements for ground water discharge? Is haul off required? See Division 1000 of the ISPWC. A Construction Site Discharge and/or SWPP Plan shall be submitted to the Engineer for review.

i. Does the contractor provide materials testing? Yes. What is the required frequency of testing? See GSP 0001 1.8 and ISPWC Division 2100.

j. Is the project following ISPWC? See GSP 0001 1.1 and 1.2.

k. Are manhole collars required? Collars are required in all traveled ways. See revised Detail 5-C5.0, for further clarification (part of Addendum 1).

l. How many times is the surveyor coming out for staking? Is it one time for all of it? Staking is planned to be provided for each alignment separately. Therefore, four (4) site visits (one site visit per alignment) for staking shall be provided without additional charge. Restaking shall be at the Contractor’s expense.

m. ITD has a culvert replacement project in Stites that may require highway traffic to be routed along East St. There is a SH-13 Stites Culvert Replacement project which is located at MP 22.1 Stites Grade

Rd on the south end of town. The planned construction window is tied to the water level being below ordinary high water. The successful bidder will be provided contact information to coordinate with ITD’s Contractor and/or Project Representative. The successful bidder will be expected to work with ITD to the best interest of both projects.

n. Is there a carrier pipe/sleeve on the chemical feedline? If so, what is the size? Chemical feedline installation shall conform to manufacturer recommendations.

o. What is the make/model/dimensions of the exhaust fan? Refer to SP 0030 2.1.

p. Will the government shut down affect this project? It is possible. The bidding of the project is planned to proceed as if the Federal Government will reopen without delaying award of the project.

q. Can ground water be pumped into the sanitary sewer? City Code 5-3-15 prevents discharging into the sanitary system.

13. Time for Site Visit: Return within 30 minutes with add’l questions.

a. Lift station yard fence will be open.
