Client Server PDoc Solution The Sullivan Group’s PDoc Solution for the MEDITECH Client Server environment provides facilities with a turn-key solution to satisfy their clinicians’ desire to have a documentation workflow that supports their practice. The suite of nearly 140 chief complaint-specific templates have been vetted by board-certified EM physicians and designed to reduce the time it takes to create a medical record. Additionally, TSG’s PDoc Solution leverages the existing functionality within the MEDITECH system to ensure a high-level of documentation compliance with important Risk, Safety, Quality indicators, thereby creating a more defensible medical record. Drawing from 20 years of research into adverse outcomes and a granular analysis of 350,000 high-risk patients, TSG designed the PDoc Solution to specifically target those clinical areas of risk exposure that have often led to patient safety events and malpractice litigation. After analyzing over 25,000 Chest Pain cases, the evi- dence clearly indicates that there are several documen- tation and clinical practice deficiencies; e.g., TAD Risk Factor Analysis is frequently missing from the medical record.

The Sullivan Group’s PDoc Solution for the MEDITECH Client Server environment provides facilities with a turn-key solution to satisfy their clinicians’ desire to have a documentation workflow that supports their practice. Comprehensive Content

PDoc templates provide physicians with comprehensive clinical content for nearly 70 adult templates and 70 pediatric templates. These templates have been vetted by a group of veteran physicians, board-certified in Emergency Medicine and Pediatric Emergency Medicine.

Efficiency Of Documentation

90% of the basic physical exam can be completed for many complaints with just 3 clicks, giving physicians more time to spend with the patient or drill down on the relevant organ system.

Required Content

Research of the literature as well as a targeted analysis of 350,000 medical records has revealed gaps in physician documentation and practice that contribute to adverse outcomes and medical malpractice claims.

The PDoc templates require documentation of certain critical elements prior to disposition. If documentation is lacking, the physician will be alerted and automatically directed back to the missing critical documentation.

Risk Sensitive Content

There are other elements that may be relevant to high-risk conditions that would be too burden-some to treat as a required field. These elements are flagged for the physician using this unique character: “)(“

Studies have shown that such prompted fields are very successful at elevating these elements in the physician’s consciousness and increasing the level of documentation compliance.

Risk Stratification Tool

The relevant risk stratification tools for each chief complaint are built right into the templates on the “Risk Stratification” tab, such as the PERC calculator and Wells DVT Score. Physicians can use these tools to appropriately rule out diagnoses and have the results write directly to the chart to create a highly defensible medical record should there be an adverse outcome.

Additional clinical decision support elements and access to evidence-based medicine content are accessible via links into the RSQ® Knowledge Base through the “View Protocol” page.
