Is the aim of the health care system to maximise QALYS? An investigation of "what elsematters" in the NHSOffice of Health EconomicsThe appraisal of health care technologies by NICE focuses on cost effectiveness, usually

measured in terms of incremental cost per quality adjusted life year (QALY) gained. According tothis research, however, cost effectiveness does not appear to be the dominant consideration inNHS decisions about resource allocation. The authors examine what factors affect NHSdecisions, and suggest the perspectives of NICE and the DH are very different with clearimplications for efficiency across the health care system.http://www.ohe.org/publications/recent-publications/list-by-date-20/detail/date////is-the-aim-of-the-health-care-system-to-maximise-qalys-an-investigation-of-what-else-matters-in.htmlRegistration with the Office of Health Economics website needed to access this report.Registration is free.

The National Programme for IT in the NHS: an update on the delivery of detailed carerecords systemsNational Audit OfficeThis report concludes that the rate at which electronic care records systems are being put inplace under the NPfIT is falling below expectations and the core aim that every patient shouldhave an electronic care record under the Programme will not now be achieved. Even wheresystems have been delivered, they are not yet able to do everything the Department intended. Moreover, the number of systems to be delivered has been significantly reduced, without acommensurate reduction in cost.http://www.nao.org.uk/publications/1012/npfit.aspx

Other Health Technology AssessmentsHysterectomy, endometrial ablation and Mirena for heavy menstrual bleeding: asystematic review of clinical effectiveness and costeffectiveness analysisBhattacharya, S et al.This study found that despite longer hospital stay and time to resumption of normal activities, more women were satisfied after hysterectomy than after endometrial ablation. There is somesuggestion that Mirena is potentially cheaper and more effective than first-generation ablationtechniques, with rates of satisfaction similar to second-generation techniques; there is limitedevidence to suggest that hysterectomy is preferable to Mirena. Although the most cost-effectivestrategy, hysterectomy may not be considered an initial option owing to its invasive nature andhigher risk of complications.

A pragmatic single-blind randomised controlled trial and economic evaluation of the useof leukotriene receptor antagonists in primary care at steps 2 and 3 of the national asthmaguidelines (ELEVATE study)Price, D et al.This study found that leukotriene antagonists are unlikely to be a cost-effective alternative toinhaled corticosteroids, at 2005 prices, as initial asthma controller therapy at step 2 of the BritishThoracic Society (BTS) guidelines, but evidence suggests that they are clinically equivalent tolong-acting 2-agonists (LABAs) as add-on to inhaled corticosteroids in terms of quality of life aswell as secondary measures; furthermore, they could be an alternative to LABAs at step 3 of theBTS guidelines.Health Technol Assess 2011;15(21)

It builds on, but supersedes, the approachset out in the 2007 National framework for responding to an influenza pandemic, taking accountof the experience and lessons learned in the H1N1 (2009) influenza pandemic and the latestscientific evidence. Many other organisations and individuals have extensive experience of thechallenges that can be posed by a pandemic. The government is keen to ensure that thisexperience is fully reflected. People Powered HealthNational Endowment for Science, Technology and the ArtsPeople Powered Health is a new programme to support the design and delivery of innovativeservices for people living with long term conditions. The programme focus is the concept ofco-production - that people's needs are better met when they are involved in an equal and reciprocalrelationship with professionals, working together to get things done. The first stage is an open callfor ideas. NESTA will work with the most promising applicants to develop their ideas beforeselecting partnerships that will receive investment and support to put them into practice. By

November 2012, NESTA want each locality to have redesigned a service or a pathway forpatients with long term health conditions that put most or all of the characteristics of co-productioninto practice.http://www.nesta.org.uk/areas_of_work/public_services_lab/people_powered_health

National Audit of Falls and Bone Health in Older PeopleRoyal College of PhysiciansThis audit shows that there is unacceptable variation in the quality of NHS services for care andprevention of falls and fractures. In many areas, there is a major gap between what NHSorganisations state they provide, in terms of commissioning, protocols or structure; and whatclinical audit reveals in terms of actual care provided. The audit shows that older patients withfractures do not routinely receive key aspects of care for falls prevention or bone health, needlessly exposing them to a greater risk of further falls and fractures.Report and executive summary available at:http://www.rcplondon.ac.uk/resources/national-audit-falls-and-bone-health-older-people

Pain Management Interventions for Elderly Patients With Hip FractureAgency for Healthcare Research and QualityThe authors conclude that overall, there is limited evidence about the comparative effectiveness,

benefits, and adverse events of pain management interventions used for elderly patients with hipfracture. There is moderate strength of evidence that nerve blocks reduce pain and the incidenceof delirium, and evidence of low strength indicates that preoperative traction does not improverelief from acute pain. For all modalities, the evidence is generally inadequate to estimate harmsand the incidence of adverse events.http://www.effectivehealthcare.ahrq.gov/index.cfm/search-for-guides-reviews-and-reports/?productid=679&pageaction=displayproduct

Addition to inhaled corticosteroids of long-acting beta2-agonists (LABA) versus anti-

Differences favouring LABA in lung function, functional status and quality of life scores are generally modest. There is some evidence ofincreased risk of adverse events with LABA. Integration this time? Liberating the NHS and the role of local governmentLocal Government Improvement and Development

The report summarises findings from a historical overview of arrangements and experiences ofjoint working between local government and the NHS since the NHS was founded in 1948. Thisoverview generated 10 principles of integration which are then used as a framework foranalysing the relevant provisions of the Governments current proposals. Finally, some of therisks to the successful application of the emerging framework are examined. These arise from theimplementation of the wider arrangements for commissioning and accountability in the NHS.http://www.idea.gov.uk/idk/core/page.do?pageId=27388210
