Introduction

The Tribal Nursing Home & Assisted Living Facility Directory lists the information about each tribally operated nursing home and assisted living facility. The information includes the tribal affiliation, facility name, address, certification, number of beds, and year opened. Additionally, information about the Quality Improvement Organizations (QIO) and Long-Term Care Ombudsman programs is outlined. As recommended by the Uniting Nursing Homes in Tribal Excellence (UNITE) group, staff contact information is not included due to high turnover. This document is updated annually. Please report corrections to [email protected].

Long-Term Care Ombudsman

Long-Term Care (LTC) Ombudsman programs help resolve health, safety, welfare, and advocate for the rights of individuals living in long-term care facilities, like nursing homes and assisted living facilities. Each state manages an LTC Ombudsman program. The program is directed by the Older Americans Act to:

• Identify, investigate, and resolve complaints made by or on behalf of residents; • Provide information to residents about LTSS; • Ensure that residents have regular and timely access to ombudsman services; • Represent the interests of residents before governmental agencies and seek administrative, legal, and

other remedies to protect residents; and • Analyze, comment on, and recommend changes in laws and regulations pertaining to the health, safety,

welfare, and rights of residents.

Quality Improvement Organizations

Quality Improvement Organizations (QIO) aim to improve the effectiveness, efficient, economy, and quality of services delivered to Medicare beneficiaries. Additionally, QIO’s are required under Sections 1152-1154 of the Social Security Act, and their core functions are to:

• Improve quality of care for beneficiaries;• Protect the integrity of the Medicare Trust Fund by ensuring that Medicare pays only for services and

goods that are reasonable and necessary and that are provided in the most appropriate setting; and• Protect beneficiaries by addressing individual complaints, such as provider-based notice appeals and

violations of the Emergency Medical Treatment and Labor Act (EMTALA).

There are two types of QIO programs, Beneficiary and Family Centered Care (BFCC-QIOs) and Quality Innovation Network (QIN-QIOs). Each have separate directions:

Beneficiary and Family Centered Care (BFCC-QIOs) Quality Innovation Network (QIN-QIOs) • Help Medicare beneficiaries exercise their right to

high-quality health care• Manage beneficiary complaints and quality of care

reviews• Manage beneficiary appeals of a health care

provider’s decision to discharge them from thehospital or discontinue other types of services

• Bring Medicare beneficiaries, providers, andcommunities together in data-driven initiatives to:

o increase patient safety,o make communities healthier,o better coordinate post-hospital care, ando improve clinical quality
