Te Washington Center or Internships and Academic Seminars is dedicated to providing interns with an exceptional experiential

learning experience. In the Washington Centers marketing eorts, we seek to highlight the experiences o current Washington

Center interns to demonstrate the importance o an internship program to potential interns, unders, sponsors, etc. First-hand

accounts o your experience in the program are integral to others understanding o the components and value o the program.

4.0 Authorization

I you agree to the ollowing statements, please check boxes afer reading.

Troughout the course o the program, I acknowledge that I may be photographed, quoted, or videotaped as part o my

participation in Te Washington Center program. Furthermore, I acknowledge that materials submitted by me to Te

Washington Center become the property o Te Washington Center and may be excerpted in whole or part or use in

Washington Center materials.

I urther acknowledge that this inormation may be utilized in a variety o orms to market Te Washington Center and its

programs. Tis marketing may be or both internal and external audiences. I authorize Te Washington Center and its duly

authorized agents to use my name, image, likeness, and quotes or reasonable purposes in marketing and promotional materials

which may include, but not be limited to, brochures, videos, websites, advertisements, news articles, posters, college, local and

national news organizations on my behal to inorm them o my participation in this program and o my relationship to and

participation in other newsworthy events.

I do not give Te Washington Center permission to utilize my image, materials, or likeness in the marketing materials.
