Heraldry of tHe College

The upper portion of the badge of the College consists of a golden lion in a passantstance superimposed on the chief which is red in colour.

The lion represents our nation of Singapore. Red is the colour of blood which is theprecious liquid on which life depends and which is our mission to save.

The lower portion of the badge consists of a green cross superimposed on a whiteshield. The green cross symbolizes the humanitarian mission of our College. White isthe colour of purity which is our aim.

On the green cross is superimposed an open book and a caduceus. The open bookrepresents knowledge and continued learning and the caduceus is the recognizedsymbol for the art of healing.

The year ending 31 March 2017 was eventful and saw many positive developments. The promise of a better future for family medicine continues to shine brightly. I am pleased to report that the 25th Council will complete its term with the successful completion of some key initiatives.

Membership and Community Engagement The membership of the College as of 31 March 2017 increased from 1892 to 1998. Although we have been seeing year on year increases in membership, the rate of increase still falls below the number of new medical practitioners who qualify for membership. Our Council continues to invest effort and resources to engage medical students and increase the visibility of the good work that our College is doing. Our collaboration with the Joint Committee on Family Medicine Singapore (JCFMS) to co-organize career seminars has now become an annual affair and the highlight of the activities of student Special Interest Group. We believe our on-going effort to engage medical students will bear fruit in the next few years. As it is, family medicine residency programs are experiencing unprecedented increases in the number of applicants compared to the earlier years. Our GDFM course has also seen unprecedented increase in numbers of applicants. Our effort to engage the media in promoting a positive image of family medicine is making good headway. We worked with our consultants at Weber Shandwick to highlight the important role played by our College as a national body representing family doctors and advocating high standards of patient care. As a result, our College was featured positively in many newspaper articles, radio and television broadcasts. These were all very well received.

Assets I am reassured to see that our College’s financial situation remains robust. This is testament to the good stewardship of our Council. Our financial reserves provide stability and security for the operations of our College. We do not receive direct funding from the government. Although we are a charity, the funding we receive from donations is negligible. We must remember lessons from the past when we saw the College’s reserves dwindled in a downward spiral which could have ended in insolvency. I hope the future Councils and our membership will continue to build and safeguard our reserves. This will ensure that our College will always be a viable champion of family medicine, way into the future.

Courses conducted The Graduate Diploma in Family Medicine, the Master of Medicine Family Medicine (College Program) and the Fellowship of the College of Family Physicians Singapore (By Assessment) are the signature programs run by our College and they continue to do well. The curriculum and delivery of these programs are constantly reviewed and improved over the years. We need to keep these programs relevant by adapting to the new education environment. More importantly, we need to ensure that our training programs continue to improve patient care by being responsive to the changing needs of our patients and our community. We need to be aligned with national healthcare priorities. From time to time, we will need to deliver a concentrated upgrade to our programs beyond the constant cycles of Improvements. This will ensure that our programs will always be relevant and beneficial towards improving patient care.

PartnershipsOur College have helped to foster excellent relationship with the Singapore Medical Association and the Academy of Medicine Singapore. The three professional bodies hold regular dialogues to keep each other updated of one another’s activities. On matters that require a whole of medical profession approach, we had taken joint action. This had resulted in positive developments in improving the newly developed ethical code and the on-going engagement to reverse negative trends in the relationship between Third Party Administrators and medical practitioners. The College continues to have a good working relationship with the Ministry of Health and the Singapore Medical Council. We are consulted regularly on policy matters. We had made many suggestions which were well appreciated and had resulted in improvements to proposed regulations.

Major Developments The 25th Council had initiated some key developments that will have a major impact on the future development of family medicine in Singapore.

The FAmily Medicine for OUr Singpaore (FAMOUS) Project was instituted as a concerted effort to understand present trends and the likely future landscape of family medicine. The interim findings were reported at the World Family Doctor Day 2017 dinner and the June 2017 issue of our College Mirror. The findings and recommendations will have a major impact on family medicine development and provide guidance for our efforts to improve the state of family medicine in Singapore.

Enhancement of the GDFM was another landmark development that was initiated in close collaboration with the Primary and Community Care Division of the Ministry of Health. The revamp and review of the GDFM curriculum is timely in view of the changing healthcare needs of our population and the increased intake in recent years (from 114 in 2014 to 235 applicants in 2017). The review of the curriculum by the College appointed Committee had been completed and presented to the Joint Committee on Family Medicine Singapore. The revamp will see enhancements in areas of need such as chronic disease management, geriatrics and mental health. Applicants who wish to acquire competency in community hospital practice can choose to take additional modules and be credited with a Certificate of Competency. This additional certificate will be issued by the College and recognized by the community hospitals. An implementation taskforce will be set up to operationalize the new program in academic year of 2018.

The term of the 25th Council comes to an end with great promise and expectations of the future. I would like to thank my fellow Council Members for their dedication and the personal sacrifices that they had made to ensure the wellbeing of our organization. We also have a dedicated army of volunteer faculty in all the College training programs. They work selflessly and tirelessly to pass on their skills and knowledge to the next generation of family physicians. They epitomize the true meaning of professionalism. Such commitment from altruistic and like-minded members is the lifeblood of our organization.

Last but not least, special thanks is accorded to our long suffering Secretariat staff, who labour quietly and efficiently in the background to ensure that all goes well for our College.

It is an unreserved honour and privilege to be part of this movement for the good of our fraternity and our patients. On behalf of the Council, I would like to thank everyone who had helped our common cause in one way or another.

Family Practice Skills Course (FPSC)

CONTINUING PROFESSIONAL DEVELOPMENT/ MEDICAL EDUCATION PROGRAMMES AND COURSES

The College promotes Continuing Professional Development (CPD) and Continuing Medical Education (CME) through the Family Medicine Modular Course, Family Practice Skills Course, MMed (Family Medicine) Programme B, Family Medicine Membership and Fellowship Programme.

The Institute of Family Medicine (IFM), an organ within the College, plays a significant role in developing the contents and syllabi of these programmes and courses.

During the year the IFM reviewed, updated and edited the notes as well as their e-learning components for Modules 1, 2, 3 and 8 of the Family Medicine Modular Course to make them more relevant and user-friendly to the course participants.

The Family Medicine Modular Course consists of 8 modules of knowledge acquisition and is a requirement for doctors who are preparing for the Graduate Diploma in Family Medicine (GDFM) Programme and the Master of Medicine (Family Medicine) College Programme. Each year, four modules are covered.

For the year April 2016 - March 2017, there were 169 trainees registered in the GDFM course, 35 trainees in the MMed(FM) College Route and 19 trainees in the Fellowship [FCFP(S)] programme.

During this financial year, the College conducted four Family Practice Skills Courses.

A typical FPSC template is a course with six units of distance learning with 30 MCQs test, conducted in two seminars, and also workshops conducted over two afternoon sessions typically, one on Saturday afternoon and one on Sunday afternoon.

E-LearningThe E-Learning programme launched in 2003 has been incorporated into the GDFM training programme. An E-Learning component for each module was introduced, consisting of therapeutic notes, case scenarios, assessment by MCQs and a case review to be put up by a tutorial group.

Accreditation of core CME pointsThe College is represented by Dr Tan Tze Lee in the Singapore Medical Council (SMC) CME Co-ordinating Committee. The College works closely with the SMC to ensure that the quality of the CME events is acceptable. It continues to advise SMC on the accreditation of core Family Medicine CME events through its panel of Family Medicine (FM) CME advisors appointed by the College.

In the year under review, the College Mirror has seen a revamp in its masthead and content layout. The quarterly publication continued in its mission of putting together timely news to update the family doctor fraternity, under the leadership of Dr Low Sher Guan Luke, Dr See Toh Kwok Yee and Dr Chung Wai Hoong Irwin and their editorial teams.

During the current term of office, April 2016 to March 2017, the Council helped facilitated several feedback sessions and also took part in the meetings with the Ministry of Health. We also tried to provide support and representation on the various committees that arose out of the numerous feedback sessions.

The Council also sought to continue to assist and help our members in the various ways possible and also solicit feedback from our members as well.

Annual General Meeting 2016The College AGM was held on 30 July 2016 at College of Medicine Building (COMB). The minutes have been circulated.

Commencement of Academic Year 2016The Commencement Ceremony was held on 30 July 2016 at College of Medicine Building (COMB).

There were 169 trainees in the GDFM course, 35 trainees in the MMed(FM) College Programme and 19 trainees in the FM Fellowship [FCFP(S)] Programme.

Convocation 2016The Family Medicine Convocation Ceremony was held on 19 November 2016 at NUSS Kent Ridge Guild House and the Guest-of-Honour was Dr Lam Pin Min, Minister of State for Health.

15 doctors were conferred the MCFP(S). Of which, 11 were awarded the MCFP(S) by Election on 19 November 2016, 2 were awarded the MCFP(S) by Election on 4 January 2017 and 2 were awarded the MCFP(S) by Assessment on 19 November 2016. 66 GDFM graduates and 54 MMed(FM) graduates were recognised.

COMMENCEMENT OF THE ACADEMIC YEAR The Commencement of the academic year for 2017 will take place at the College of Medicine Building, on 29 July 2017.

GRADUATE DIPLOMA OF FAMILY MEDICINE (GDFM) The Graduate Diploma in Family Medicine (GDFM) is a vocational training certification for primary care doctors. The aim of this 2-year part-time trainee programme is to train primary care doctors to practise Family Medicine at an enhanced level to meet the needs of the child, the adolescent, the adult and the elderly. The course emphasises on basic clinical, diagnostic and management skills essential to general practice.

The programme is planned to accommodate the busy doctor’s schedule as almost all courses that require in-person attendance are conducted outside regular office hours. The course consists of the following: l 8 modules of Family Medicine Modular Course (FMMC) with each comprising of 4 workshops, 1 tutorial, online case studies and multiple choice assessments l 3 Practice Management Courses and 1 elective Family Practice Skills Course

The examination is conducted by Division of Graduate Medical Studies (DGMS), National University of Singapore (NUS).The standard set is a pass in all three segments: l Written paper I - Applied Knowledge Test: 100 MCQ questions (2 hours) l Written Paper II - Key Features Problems (KFP): 10 questions (1 hour) l Skills Assessment in the Objective Structured Clinical Examination (OSCE) - 10 stations of 11 minutes each, comprising 3 minutes for reading and 8 minutes for assessment (2 hours)

The GDFM(NUS) is a title or qualification approved by the Singapore Medical Council for display by Registered Medical Practitioners.

The 15th GDFM Examination was held on 24 June 2016 for the written papers and 2 & 3 July 2016 for the OSCE. A total of 66 of the 94 candidates passed, giving a passing rate of 70.21% for the GDFM Examination 2016

MASTER OF MEDICINE (FAMILY MEDICINE) [MMed(FM)]

There are currently two programmes to prepare doctors for the MMed(FM) examinations.

Family Medicine Residency Programme With effect from 2011, Programme A is organised into a 3-year Residency Program with a greater emphasis on formative assessment and training.

The programme aims to equip the resident with educational and clinical experiences in accordance with ACGME-I standards, which focuses on structured training and close supervision. The resident will undergo clinical rotations in the hospitals and polyclinics, call-back sessions and teaching sessions that combine foundation teaching, continuity clinics, skills workshops/attachments and community medicine.

MMed(FM) College Programme is a structured training programme designed for mature practicing family physicians in Singapore. The programme is jointly organised by the College of Family Physicians Singapore and the DGMS, NUS. This 1-year part-time course provides a comprehensive and structured training programme for doctors working in primary care who have completed the 8 modules of the Family Medicine Modular Course (FMMC) to prepare them to sit for the MMed(FM) examinations.

Family Practice Skills Course (FPSC)

The Family Practice Skills Courses (FPSC) continue to serve the continuing professional development needs of the General Practitioner/ Family Physician. Each skills course includes one or more of the following components in which the family physician can choose to participate in:

l Distance Learning with verification (6 CME points upon attaining minimum pass grade of 60% in online MCQ Assessment) l Seminars (2 CME points per seminar) l Workshops (1 CME point per workshop) l Small group activities including clinical attachments, tutorials, ward-rounds (for some courses)l Assessments which can be written, oral, demonstration and/or clinical (for some courses)

Duties and Responsibilitiesa) To review financial reports from the ED and Secretariat staff.

b) To review budgets prepared by Secretariat staff and the various committees.

c) To ensure regular and accurate monitoring and accountability for funds, and report to the Council on any financial irregularities and concerns.

d) To make financial recommendations to the Council, and to provide feedback on any financial matter specifically designated by the Council.

e) To oversee short and long-term investments of the College.

f) To advise the ED and other appropriate staff on financial priorities and information systems, depending on committee member expertise.

Observations and Decisions by the Committee for the Financial Period under Review1. The committee noted the agreed framework of financial processes for the College Property Workgroup and will actively monitor this aspect if there is a property acquisition eventually.

2. The committee advised the Secretariat on the fixed deposit placements in terms of the selection of the tenure and the financial institution. Fixed deposits were placed with: l HL Bank

Four issues of Singapore Family Physician were published in FY 2016 (1 April 2016 to 31 March 2017) to date. Report for the period under review

1. We are pleased to review that there was no staff resignations for the last financial year.

2. The pipeline of medical education and training activities undertaken by College remain strong and the committee will be looking into strengthening the Secretariat team in anticipation of increase work in the near future. There is also a need to look at increasing manpower at the Secretariat to carry out its mission better and more effectively in future.

In the financial year from 1 April 2016 to 31 March 2017, the College derived its income mainly from membership fees, sponsorships and fees from its courses and seminars. The 3 items together accounted for 84.5% of its total income.

Fees from courses and seminars accounted for 65.9% of the total College income, sponsorships accounted for 7.2% of the total income and subscriptions accounted for 11.4% of the total income.

In the financial year from 1 April 2016 to 31 March 2017, the College’s consolidated total income was $2,412,323; the consolidated total expenditure for the same period was $ 1,658,019. The consolidated net surplus for the financial year from 1 April 2016 to 31 March 2017 was $749,108.

I am pleased to report that the College continues to be on a firm financial standing for the financial year 1 April 2016 to 31 March 2017.
