The CRCC team consists of: Team Leader

Intake Worker and Service Facilitator

Partnership With Carers (PWC) - Disability Workers

Partnership With Carers - Mental Health Workers

Partnership With Carers -Young Carer Workers

CRCC – broad overview

Carer must live in the Eastern Region Carer providing significant caring role We provide short term and emergency respite Not income/means tested Self referrals accepted Further eligibility criteria applies to each program We can not duplicate another service

Anyone can ring for information or to discuss a possible referral

Partnership With Carers - Disability

• Aim: to provide immediate and short term respite to carers of someone with a disability

• Eligibility: A primary carer of a person who has a severe or profound disability and requires assistance with one or more core living activities (self care, mobility and communication)

PWC - Disability Continued

• PWC workers can:

Support and Plan together with the carer

Assist the carer to access community services and supports

Partnership With Carers - Mental Health

Aim: The purpose of the Mental Health Respite Program is to provide care arrangements with the primary purpose of giving the carer a short break from their usual caring role.

Eligibility Criteria:Carers of an individual with a mental illness or psychiatric illness. A Carer must be providing an active, regular and significant caring role, including assistance with living skills and emotional support

• PWC workers can:

Support and Plan together with the carer

Assist the carer to access community services and supports

Partnership With Carers - Young Carers

• Young Carer Definition: A Young Carer is aged 25 years and under who provides a significant support to an individual who has a disability, chronic illness, mental illness, alcohol or other drug issue or is frail aged

• Aim: to help Young Carers balance their caring role with school and with life
