CreatingaHealthierCommunityTogetherNorthHartford,CT’sdesignationasafederalPromiseZone(NHPZ)in2015mobilizedarenewedspiritofcollaborationamongcityleaders,localorganizationsandcitizensonbehalfofNorthHartford’s24,000residents.TheNorthHartfordTripleAimCollaborative(NHTAC)waslaunchedin2017astheHealthandWellbeingworkgroupfortheNHPZ.
Buildingonthesignificantworktodate,theCollaborativeiscurrentlyexpandingitsreachandimpactbyharnessingtheincreasingconvergenceofpartnersandresourcescommittedtocreatingahealthier,morevibrantcommunityforall.Vision:Livingourbestlivesincommunity.Purpose:Greateralignmentandcollectiveactionamongresidents,organizations,governmentandinvestorsworkingtogethertoimprovehealth,wellbeing,andvalueofinvestment.GuidingPrinciples1. AlignmentTheCollaborativeservesastheHealthandWellbeingworkgroupfortheNorthHartfordPromiseZone(NHPZ),andalsoalignswithotherlocaleffortstoadvancehealthandwellbeingthroughoutthecity.Inaddition,theCollaborativeisthedesignatedleadfornationalinitiativessuchasTrinityHealth’sTransformingCommunitiesInitiative,InvestHealth,andWellville.
CollectiveActionThe10-yearfederalPromiseZonedesignationhascreatedasenseofurgencytoaddresssignificantinequitiesinhealthandwellbeinginNorthHartford.WithdirectionandfinancialsupportfromtheLeadershipCouncil,theCollaborativeisengagingresidents,community-basedorganizations,governmentandfundersto:• Connectresidentsto
integratedclinicalandcommunitysolutions;
• Improvecommunityconditionssuchashealthyfoodaccessandsafety;
• Buildthefoundationforlifelongwellbeingbystrengtheningprotectivefactorsamongchildrenandfamilies.
CollectiveCapacityOneofthebenefitsoftheCollaborativeistocoordinatetheeffortsofmultiplepartnerstoincreasecollectivecapacity,impactandsustainability.Thisincludeshostingworkgroupsthatdevelopsharedstrategies,resourcesandsupportforkeyfunctionalareassuchas:• Dataandmeasurement;• Communityengagement;• Advocacy;• Learningandevaluation;• Communication;• Capacitybuildingand
technicalassistance.
BackboneSupportCoordinatingsupport(“backbone”)isco-fundedbySaintFrancisHospital’sWellBeing360initiativeandtheUnitedWayofCentralandNortheasternConnecticutandstaffedbytheNHTACDirector,withadvisorysupportfromWellville.
CreatingValueandSustainabilityWiththeCityofHartfordHealthandHumanServicesasthelead,theCollaborativewasrecentlyselectedasa“referencecommunity”fortheHealthEnhancementCommunity(HEC)Initiative,whichispartofConnecticut’sStateInnovationModel(SIM)aimedathealthcaredeliveryandfinancingreforms.TheCollaborativewillparticipateinthedevelopmentofnewcommunity-centeredmodelsthatencouragemulti-stakeholderpartnershipsacrossthestatetoworktogethertopreventdisease,improvehealth,andlowercosts.Thesenewmodelswillcreatepathstofinancialsustainability,potentiallythrough“sharedsavings”fromhealthinsurersandotherpayersthatrewardspecificoutcomes.

CurrentExamplesofActionPortfolioCollaborativemembersandpartnersarelaunchinganexpandingportfolioofactionstoachievethreecommunity-leveloutcomes:longerlife,greaterwellbeing,andimprovedvalueofinvestment(impactperdollarspent).Afewcurrentinitiativesinclude:
WellBeing360:Throughinvestmentsofmorethan$450,000since2016,thisTrinityHealth/SaintFrancisinitiativehashelpedlaunchtheNorthHartfordTripleAimCollaborative;startadiabetespreventionprogram(DPP)attheYMCAinHartford;improvehealthyfoodaccessthroughtheNorthEndFarmersMarket,Knox’surbanfarminginitiative,andtheCookingMattersnutritiontrainingprogram;reducegunviolencebyexpandingthecrisisresponseteamatHartfordCommunitiesThatCare;andpreventopioiddeathsbypartneringwithGreaterHartfordHarmReductionCoalitiontoincreaseaccesstonaloxone.
Inaddition,SaintFrancisisintegratingasocialdeterminantsofhealthassessmentandreferralprocessinitssafetynetclinicsincollaborationwithCatholicCharitiestohelppatientsimprovesocialfactorsneededforgoodhealthandwellbeing.StrengtheningFamilies:WithfundingfromtheHartfordFoundationforPublicGiving,theOfficeforCommunityChildHealthatConnecticutChildren’sMedicalCenterbegantrainingchildcareprovidersusingastrengths-basedapproachtosupportlocalfamilies.Thegoalistoimproveoutcomesforvulnerablechildrenandfamiliesbystrengtheningthefiveprotectivefactorsknowntoenhancefamilies’capacitytopromotetheirchildren’shealthydevelopment:parentalresilience,socialconnections,concretesupportintimesofneed,parents’knowledgeofchilddevelopment,andchildren’ssocialandemotionalcompetence.ParkerMemorial:Inaddition,theCollaborativehasbeenworkingaspartofacoalitionofresidents,cityagencies,community-basedorganizations,funders,healthsystemsandotherstodeterminehowtheNorthHartford-basedParkerMemorialmaybepositionedasthecoordinatingcenterofanintegrated,trauma-informedsystemtoimprovethelivesoftheentirecommunity.ThecoalitionincludestheCity’sDepartmentofFamilies,Children,YouthandRecreation;HealthandHumanServices;andPromiseZoneAdministrator;alongwithpartnerssuchasConnecticutChildren’sOfficeforCommunityChildHealth,HartfordFoundationforPublicGiving,TheVillageforFamiliesandChildren,andothers.IncludingstrongcommunityrepresentationandleadershipfromtheearlieststageswillensurethatParkerMemorialreflectstheuniqueneeds,strengthsandcultureofthepopulation,andthatthecenteroperateswithanemphasisoninvolvingandempoweringresidentstobuildfamilyresiliencyandwellbeing.

HealthyFoodAccess:Hartfordwasoneof50U.S.citiesselectedtoparticipateinInvestHealth,acollaborativeinitiativeoftheRobertWoodJohnsonFoundationandReinvestmentFund.In2016-2017,amulti-sectorteamledbytheCityofHartfordengagedresidentsandotherstakeholdersinacomprehensiveprocesstoassessthecurrentfoodsystemanddevelopinvestableopportunitiesforimprovement.Retailfoodaccesswasnamedasthetoppriority.Atthesametime,additionaleffortsledbyHartfordCommunityLoanfund(alocalCDFI),HartfordFoodSystem,andothershaveassessedthefeasibilityofananchorsupermarket.Amarketassessmentandresidentfood-purchasingstudyconfirmedtheviabilityofamid-sizedmarketwithafocusonhealthyandaffordablefoods,especiallyqualityfruitsandvegetables,withpricingcompetitiveforadiversepopulationofcityresidents.Withpre-developmentfundingfromtheHartfordFoundationforPublicGiving,continuedworkisnowunderwaytodevelopthe“HealthyHartfordHub”–aproposedmixed-usedevelopmentthataimstoincorporateafull-servicesupermarket,health-promotingservices(suchasaclinic),andpotentialhousingorofficespacetoserveresidentsofHartford,especiallythoseintheNorthEnd,Downtown,andAsylumHillneighborhoods.TheCollaborativewillworkwiththepartnersaboveandwithresidentinputthroughanHHHCommunityAdvisoryTaskForcetoidentifyanddeterminethefeasibilityofvarioushealthpromotingservicesthataimtoimproveresidentwellbeingandbeintegratedwiththenewsupermarket.CurrentExamplesofCapacityBuildingInadditiontotheseandotheractionstoimprovecommunityhealthandwellbeing,theCollaborativeisalsoco-investinginbackbonesupportandotherfunctionalareasneededforsuccess.Forexample:SharedDataandMeasurement:InpartnershipwithDataHaven,HartfordFoundationforPublicGivingandothers,theCollaborativeisdevelopingadataandmeasurementsystemtoestablishsharedgoalsandtotracktheongoingimpactofCollaborativeactionsthroughmetricsthatmattertocommunitymembersandotherstakeholders.DataHavenreceivedgrantsin2015and2018fromtheHartfordFoundationforPublicGiving,SaintFrancis,ConnecticutHealthFoundation,HartfordHealthCare,andotherstofieldthestatewideDataHavenCommunityWellbeingSurvey(in2015and2018).Currentworkisbeingdonetoproduceacomprehensiveregionalcommunityhealthandwell-
beingindexandindicatorsreporttentativelytitledtheGreaterHartfordCommunityWellbeingIndex.Plannedtoincludesignificantstakeholderinput,thecross-sector,collaborativereportisdesignedtohelpmeettheneedsofmultipleorganizationsthroughacoordinatedlocalsource.Stillindevelopment,thevision
