   • Obesity may lead to diabetes and hypertension

 • Hypertension, high insulin levels, high triglycerides,

and excess body fat will increase the risk of coronary artery disease (CAD).

Review of Evidence Relevant to the Proposed Intervention: 

  • Heart disease is the leading cause of death in the United States,

Michigan, and Kent County • The average weight of males in Kent County is 197 pounds and is

considered “more than average.” • Kent County has an obesity rate of 28.1 percent

 • The adult diabetes rate in Kent County is recorded as 8.6 percent

 • In 2009, the percent of residents in poverty in 2009 was 14.6

percent for Kent County revealing a need for free screenings for health promotion and disease prevention

Brief review of findings: 

Male residents of Kent County ages 35-45 have been identified to be at an increased risk of heart disease.

 Top risk factors identified:

• Age• Sex

• Family History• Smoking

• Hypertension• Hyperlipidemia

• Diabetes• Obesity

• Sedentary lifestyle• High stress

Target Population

Male residents of Kent County are targeted for this intervention secondary to the devastating effects that potential disability can have on the family.  Males in this age group are likely to be child rearing and income building.  The potential loss of life and income can pose a significant loss in security. Targeting this population could pose challenges because the age group may feel that they are young, healthy and do not need any intervention.  Any developed programs would need to be disguised as fun, men centered activities with a healthy theme.

Community Intervention

The focus is on young adult men ages 35-45 •  Hosting the event will be the David D. Huntington

YMCA in downtown Grand Rapids with an accessible and  convenient location

 •  The event will be held in the Spring when the weather

is nice and people are motivated to be active after the slow winter months

Primary Prevention Involving Community Sponsors

•  Evaluation for risk of heart disease • Screening for blood pressure, high cholesterol, and

diabetes• Nutritional information and demonstrations• Exercise and physical activity information and

demonstrations• Heart Disease information and education• Education and support regarding smoking cessation

Secondary Prevention

In an effort to minimize the impact of a disease if realized during the screening process, the following options are available: • Follow-up appointment to be made with the participant's

primary care physician to address concerns • Visiting SpectrumHealth.org or contacting Spectrum Health at

1-616-391-5999 to be set up with a primary care physician at a convenient location

• Additional services and education through Cherry Street Health Services by visiting Cherrystreet.org or contacting (616)965-8200.

• Visiting Metrohealth.net for physician services

  • Tie-in event to draw participants• Tournament to be held in YMCA parking

Event Logistics

• Those in attendance will visit booths and participate in screenings and education.

 • Raffle tickets will be given at each booth for

participation and entered into a drawing. • Prizes will be donated by our sponsors and include a 3

month YMCA membership, tickets to sporting events at the Van Andel Arena and White Caps games, workout equipment or gift certificates donated by sporting goods store, sports and energy drinks, energy bars.

• Unpredictable weather – rain and possible very cold weather• Spring Break- due to harsh winters families will be traveling • Limited Public Transportation- Limited to GRATA or Go bus.• High Religious Population – Church events causing scheduling

conflicts• Growing Hispanic Population – possible language barriers and

food preparation differences (Kent County, 2011)

 American Heart Association 2020 Impact Goal is, “to improve the

cardiovascular health of all Americans by 20 percent while reducing deaths from cardiovascular disease by the

year 2020” (American Heart Association, 2011, para. 1).

Desired Outcomes:

By the Year 2020, Males in Kent county between the age of 35-45 will exhibit:

  

• 20% fewer rates of BMI’s greater than 25.• 20% fewer rates of smoking• 20% fewer rates of diabetes

• 20% fewer rates of hypertension• 20% fewer rates of hyperlipidemia• 20% increase in physical activity
