Reading HealthPlex for Advanced Surgical and Patient CareWEST READING, PENNSYLVANIA

Reading HealthPlex for Advanced Surgical and Patient Care features the latest technologies, a focus on the patient/family experience, and the enhanced use of green space.

The 8-story tower consolidates campus-wide surgical services including pre-admission testing, a surgical center with 24 operating rooms including four hybrid rooms and two daVinci System robotic surgery rooms, and an eight-room procedural suite. Sixteen new emergency department treatment rooms with a third CT scanner aid in patient care for what is the 7th busiest ED in the nation. Five new trauma bays located adjacent to the emergency department and operating rooms provide critical care with minimal transport. The upper floors house 150 new patient rooms equipped with smart room technologies.

Co-locating surgical services on the lower levels provides more efficient patient and staff flows, but the resultant programs and adjacencies demand very large footprints. By taking advantage of the topography, a significant portion of the procedure platform is below grade and covered by the amenity of an accessible green roof. The sloping site also provided the opportunity to create a new dedicated entry and public space, activating 7th Avenue. Maximized daylighting and sweeping views of the green roofs and neighboring public gardens contribute to the healing nature of the space.

Mercy Hospital JeffersonCRYSTAL CITY, MISSOURI

Mercy Hospital Jefferson is located in Crystal City, Mo. The existing campus was constructed in 1953 with semi private rooms. Over the years it was expanded to contain approximately 491,556 SF of space in buildings ranging from 1 to 5 stories. In 2013 Bates was hired to expand the existing campus with a 90-private-bed inpatient tower, new public admitting and waiting rooms, and an expanded emergency department. A new stand-alone cancer center will be constructed on adjacent property. An addition to the existing central utility plant will include new emergency generators and cooling towers for the hospital. Other significant renovations include: a new cardiology diagnostic department, fully renovated LDR and Postpartum department, new gift shop and chapel, new outpatient clinics, renovation of the existing cancer center into outpatient behavioral health, and various renovations and updates throughout. The new building is a 4-story structure with the capacity to expand vertically an additional 3 stories. Materials will conform to the Mercy Guidelines and Brand Standards. The new inpatient tower will be connected to the existing facility through new north and south galleries which are constructed with glass to allow natural light to flood the public spaces. New entry canopies will be constructed at the hospital, clinic, and emergency department entries as part of an entire rebranding of the facility. This project will be completed by the end of 2017.

Boulder Community Foothills Hospital ExpansionBOULDER, COLORADO

This major expansion of a 10-year-old greenfield hospital added 124,000 SF to the existing hospital, and included renovating 85,000 SF of existing space. A new 48,000 SF medical office building and a 64,000 SF parking garage was also added to the campus as part of the expansion.

The key goals of the project were to design to LEED Silver, architecturally tie the campus together, and ensure continuous smooth operation of the existing facility while undergoing the expansion. This last piece proved most challenging on the vertical expansion of 19,200 SF over the existing ED. Minimizing disruption called for careful planning and coordination, resulting in a 13-phased plan that successfully met the hospital’s design, budget, and schedule goals. The additions allowed the hospital to increase to 130 beds, as well as expanded emergency, imaging, surgery, and support services, and added a cath lab.

Mary Bridge Adolescent Behavioral Unit at MultiCare’s Tacoma General HospitalTACOMA, WASHINGTON

Located in Tacoma, Washington, the Mary Bridge Adolescent Behavioral Health Unit at MultiCare’s Tacoma General Hospital is a 27-bed inpatient unit that serves the unique needs of adolescents between 13 and 18 years of age with 24-hour inpatient care. To meet the growing demand of behavioral health needs of the community, this facility provides access to adolescents and their families in a mental health crisis.

The Clark/Kjos Architects (CKA) team, consultants, health system care team, and construction manager participated in a rigorous lean workshop. CKA summarized the most current research, a literature search of more than 100 articles, of behavioral health facility environmental factors, which can be summarized by three categories of the environment:

• Physical: Functional spaces for sufficient and varied activity and consultation• Social: Spaces that promote positive behaviors• Symbolic: Signals of trust, collaboration, refuge, and respect

This information served as confirmation to the group that the ideas that were brought forward by the group were in alignment with current research.

St. Joseph’s Hospital Health Center Combined Heat & Power (CHP) PlantSYRACUSE, NEW YORK

In 2008, St. Joseph’s broke ground a monumental $220 million expansion which would “max out” the capacity of the existing electricity infrastructure. The hospital was faced with the decision to add a costly power line or look for alternatives to meet their power needs.

Combined heat and power (CHP), generating both electricity and thermal energy from a single fuel source, would meet the 451-bed hospital’s goals. Significantly more efficient than traditional power generation, CHP can dramatically reduce utility costs while allowing the hospital to continue normal operations in the event of a power outage.

The team designed a $15 million CHP system to work in conjunction with the utility that includes a 4.6 megawatt gas-fired turbine and a 45,000 pounds/hour heat recovery steam generator. Locating an open site for the plant was challenging on the dense, urban campus. The design-build team tucked the plant into an underutilized open space suspended above a loading dock and under a three-story block of patient rooms.

Projected to generate 25 million kWh annually, the plant provides a majority of St. Joseph’s electricity. CHP allows the hospital to generate its own power and steam, improve reliability, lower costs, and reduce greenhouse gases.

Providence Health Services — Breeze Advanced Ambulatory Care Center Tenant Build-OutTORRANCE, CALIFORNIA

The project is a 3-story, 106,000 SF, advanced ambulatory care center housing the City of Hope Medical and Surgical Oncology/Infusion Center and Radiation Oncology Center; Providence Health Services (PHS) Ambulatory Surgery Center, Orthopedic Clinic, Genetics Clinic and Building Administration and Conference Center; and the Robust for Life Clinic. The project also includes a main waiting area, coffee shop, concierge for all patients, and building support areas.

A series of Lean process improvement meetings were held to discuss reinventing the patient process from registration to surgery to discharge. The following improvements were developed: 90 to 95 percent of patients are preregistered eliminating the traditional registration desk and admitting process; Entry waiting was modified to enable concierge staff to register those patients who were not preregistered; and ambulatory surgery patients are brought directly to a pre-op position. Trials of the new registration process at an existing facility decreased the registration process from nine minutes to four.

Design Achievement—Envisioned as the hospital of the future, the Cleveland Clinic Avon Hospital leverages flexibility and technology while incorporating an interdisciplinary team approach to both medicine and services. Simultaneous to design, medical staff developed a work flow process to coordinate with interdisciplinary teaming, e-hospital, and technological advancements. The design, which is informed by process mapping of patient, material, and support services, results in a 15 percent reduction of space compared to an average acute care hospital in the United States. Visioning sessions involving enterprise leaders and researchers in the analysis of multiple mock-ups of patient rooms led to the final selection of a same-handed room configuration. Within the room, the patient conveniently controls lighting and temperature, as well as personal information systems through an interactive television. The judicious use of acoustic treatments and enclosure of the interdisciplinary work stations creates a quiet patient environment.

Scope Summary—This project included a 225,000 SF inpatient bed tower expansion of a an existing family heath and ambulatory surgical center. The scope added 126 inpatient beds, 12 intensive care beds, two new operating rooms, a pharmacy, a lab, dietary services, and an expansion of the emergency and imaging departments. Developed on an environmentally sensitive site that contains wetlands, the parking expansion represents one of the most extensive uses of permeable pavers in the United States. DLR Group|Westlake Reed Leskosky provided process mapping, medical programming and planning, architecture, structural engineering, and interior design.

Eastern Maine Medical CenterBANGOR, MAINE

The Eastern Maine Medical Center modernization 361,000 SF project enables the hospital to adapt to emerging technology and modern procedures while accommodating increased demand on a limited footprint.

Natural light, decentralized nurses’ stations, adaptable-acuity single patient rooms, private bathrooms, patient lifts, and respite areas are all incorporated into the design. With expansive views of the adjacent Penobscot River, the tower supports a main lobby, cafe, heart and vascular care space, surgical suite, NICU, two floors of private patient rooms, and a newly renovated labor and delivery center.

Thoughtful planning of the inpatient spaces enables better access and coordination of care for patients of all ages. Micro stations provide staff observation while limiting the disruption to patients and families. A flexibly designed NICU accommodates multiple birth siblings with connected patient rooms.

The new lobby incorporates design elements reflective of Maine’s natural beauty. The new cafe provides an event space as well as a area of relaxation for patients, families, and staff.

Glenbrook Hospital, John and Carol Walter Ambulatory Care CenterGLENVIEW, ILLINOIS

Glenbrook Hospital, a 173-bed member of the Northshore University Hospital system in suburban Chicago, has achieved in 20 years a total modernization and expansion of its diagnostic and treatment areas, as well as privatization of all its beds. The hospital has become more outpatient-focused with the new John and Carol Walter Ambulatory Care Center (ACC). Outpatients and staff now have clarity of entry at the opposite end of the hospital from inpatients and surgical procedure patients. The three level, 150,000 SF ACC connects to the hospital at the lower level for physician services (offices and conference space), staff, and infrastructure systems. The first floor links to all patient and public functions and amenities in the hospital. The ACC comprises the Kellogg Cancer Center, the Neurological Institute, the Urologic Center for Health, the Cardiovascular Institute, and physical therapy. While providing a fresh face to the campus, the interior has significant exposure to the outside with its atria-focused entry and waiting areas. The ACC is surrounded with lush landscaping, including the cancer center patient garden. The project has a LEED Silver rating.

The Penn Presbyterian Medical Center (PPMC), which is part of the University of Pennsylvania Health System (UPHS), had not undertaken a significant new construction project in at least 25 years. The existing ambulatory and acute care interiors had become outdated, the circulation systems needed clarifying, and upgrades to the patient and family experience were necessary. UPHS—which includes the Hospital of the University of Pennsylvania (HUP) and Pennsylvania Hospital, in addition to PPMC—mandated the move of its trauma operations from HUP to PPMC, and that provided the impetus for both renovation and new construction.

The new 647,000 SF Stamford Hospital uses the patient-centric philosophy of the Planetree model as the basis for its design. EYP Health partnered with the clinicians and administrators to balance high tech and high touch, never losing sight of the client’s goals to personalize, humanize, and demystify health care. The new facility includes the most progressive technology possible, as well as indoor and outdoor yoga sites, green roof-top terraces for caregivers, and quiet spaces for respite, therapeutic touch and sound therapy. The hospital made it a priority to collaborate closely with the City of Stamford and neighboring communities to inform the planning and design of this new campus that enhances the community. The resulting collaborations between EYP Health, the project team, and the community led to the creation of this Planetree Designated Patient-Centered hospital in Stamford.

As the primary provider of pediatric specialty care in Arkansas, Arkansas Children’s Hospital is expanding their inpatient and outpatient services to meet the growing needs of the community. The scope builds on their existing ambulatory care services delivered at the Lowell Clinic.

The project uses vernacular materials such as glass, stone, wood, and metal panels. These materials flow inside the building creating a feeling of continuity. A colorfully lit glass stair creates a “Beacon of Hope” on the northeast corner. The southwest corner provides beautiful views to the rolling hills. Ample natural light is brought into all the public spaces. A theme of “Discovery and Delight” infuses colors and whimsical sculptural graphics reinforcing wayfinding and creating identifiable destinations.

Key program elements include 28 inpatient beds in the ED, 30 exam rooms, and 3 vertical ED stations; 30 clinic exam rooms, and 3 treatment rooms; 5 operating rooms; imaging; diagnostic/ancillary services; full service kitchen and cafeteria; coffee shop; family house respite area; helipad and refueling station; and central plant.

The clinic welcomes patients in a hotel-like atrium with natural light filtered through a grand staircase in the center. The resulting space melds a dynamic interplay of natural light, colors, and textures with primary clinic space. The Wilf Family Center is an intellectual center for children’s health care as well as an entertainment space for families and children. It is specially designed to accommodate patient needs and medical equipment, so families can watch movie premieres, attend special performances, and meet celebrities, including athletes from the Minnesota Vikings. The engineering scope included lighting design, MEP, low voltage, and telecommunications systems designed to support the highly technical and medical requirements of the space.

In addition to the conference center, HGA designed and upgraded the MEP systems for the existing facility and the new ED/patient tower addition to the main 400,000 SF hospital. HGA’s engineering services involved facility analysis and system upgrades along with new construction integrating with the overall hospital infrastructure. The engineering scope included replacing the medium voltage utility and emergency electrical distribution systems, optimizing the central plant design, and designing new engineering systems to support the expansion and existing facility.

Designed to create outstanding patient experiences and to provide more convenient access to cancer care, the new 85,000 SF, $44 million center offers oncology, behavioral health, nutrition, therapy, acupuncture, yoga, and more. The program includes exam rooms, patient conference areas, and family education, while treatments include radiation oncology, HDR, cyberknife, and PET/CT. Amenities include cosmetology retail, a cafe and break area, art and music therapy and peaceful healing gardens, walking trails, and a water feature for respite.

The input gathered from patients and families for more customization and comfort were incorporated into HGA’s design through larger consult rooms to accommodate families; larger chemotherapy spaces adjustable for privacy; and technology to involve long-distance relatives in care planning.

To promote caregiver collaboration while saving space and reducing cost, flexible work environments are located in the administrative and clinical areas. With no one person “owning” their space, multiple discrete work environments are provided.

Akron Children’s Hospital Kay Jewelers PavilionAKRON, OHIO

In response to significant increases in patient volumes, Akron Children’s Hospital recently expanded its campus with a 368,000 SF critical care tower. The tower features a 100-bed NICU; a six-bed, high-risk obstetrics program; a 39-bay pediatric emergency department with 14 fast-track and five behavioral health rooms; six outpatient surgical suites; and clinical space for pediatric sub-specialty programs.

The design team was mindful of the need to build in flexibility, so NICU rooms can be converted into PICU rooms; and two floors of shell space are available for future use.

Taking its design cues from the park that flanks the entry to the hospital, the design team created a fanciful treehouse setting in the lobby that appeals to children and extends the outside inside. An expanded Ronald McDonald House of Akron; six-level, 1,200-space parking deck; and a new entry rounded out the program for the new LEED Gold facility.

The Department of Veterans Affairs Outpatient Clinic in Tallahassee, Fla., is a developer-led, design-build, lease-back facility designed around a “Green Campus in the Park.” Hoefer Wysocki’s design creates a warm, non-institutional, healing environment that balances the technological and code/criteria-driven features that support the delivery of twenty-first century health care. The design includes multiple pavilions, which are all varied within the building’s symmetrical composition.

The facility includes mental health, women’s care, cardiology, surgery, endoscopy, dermatology, urology, ENT, podiatry/orthopedics, dental, diagnostic imaging and other specialty services. The exterior and interior design provide a cohesive color palette, a variety of materials, and a compositional balance, which establish an engaging and lasting impression for all who visit the facility. The William P. Clements Jr. University Hospital (CUH) is the centerpiece of the nationally acclaimed University of Texas (UT) Southwestern Academic Medical Center located in Dallas. The Tower 3 expansion will house 292 beds concentrated within the neurology, brain and spine services. The increase will bring the total complement of inpatient beds to 752. CUH Tower 3 will house advanced diagnostic and treatment areas such as MEG Scanner, gamma knife, neuro operating rooms, neuro interventional suites and a neuro ICU-based CT scanner. It will also incorporate specialty nursing units such as neuro ICU, acute stroke, spine/orthopedics, and epilepsy.

The facility was designed with two primary components: podium and tower. The podium houses all diagnostic and treatment departments as well as major support services with a distinct separation of flow between visitor, staff/patient, and service. The tower incorporates a serrated configuration with elevator cores at each end. This 2-story addition and renovation to an existing critical access hospital is the largest in the campus’s history at 50,000 SF. It includes 20 new inpatient beds, a 4-bed critical care unit, a specialty clinic, and 3 procedure rooms—all constructed while the hospital continued to provide full services to the community. The design team led an integrated approach to connect the existing hospital and made wayfinding a campus-wide focus. The materials used for the exterior design are repeated on the interior to reflect the warm palette in the neighboring rural landscape. Natural materials like zinc and terracotta strengthen the connection between the hospital and the community by complementing the existing brick palette. An updated main entry features a reception desk, private registration rooms, and gift shop. The inpatient rooms encompass a hospitality-like quality. Wall bases, desks, and benches are constructed of rift-sawn red oak. A stacked wood wall supports wayfinding while drawing connections to the use of stacked stone on the existing facility. Expansive windows maximize light and the connection to outdoors. The design sought to support numerous studies stating connection to the outdoors promotes healing and shortens recovery, creating a cutting edge facility.

The existing hospital was the sum of five separate building campaigns, undertaken over many years, resulting in a less-than-optimal arrangement of services constrained by aging infrastructure. The priority task of the project was to provide new surgical facilities and significantly increase clinical exam space to better serve the community and relieve pressure on the emergency department.

The architectural goal was to reconnect the hospital to its site and the patients’ and staff’s experience to their wider natural environment. The solution involved both a major new addition to the hospital and a reorganization of the internal departmental relationships. The existing hospital was largely indifferent to its natural setting.

The new hospital features a restored creek at the main entry and expanded views out to the lush marine forest setting. The clinic waiting rooms, the healing garden, and the main waiting/admissions area physically connect the hospital to the scale and grandeur of the Tongass Narrows and the mountains of Gravina Island beyond.

The Health Learning Building designed by Page in collaboration with The S/L/A/M Collaborative is the gateway to the district. Problem-Solving for Thermal Comfort: Where High Design and High Performance IntersectBOSTON, MASSACHUSETTS

Feeling thermally comfortable is one of the aspects that occupants value most in a building, and yet, mitigating localized thermal comfort is rarely a priority during the design process. In many hospital environments, mechanical systems, like perimeter heating, compensate for shortcomings in envelope performance to provide a thermally comfortable environment for its various occupants. Pickering Associates worked with Camden Clark Medical Center to design a 63,000 SF expansion to house their emergency department and new inpatient unit.

The 44-bed emergency department was designed as a split-flow model where the most seriously ill patients are cared for at the ambulance entrance and high acuity patients utilize a walk-in section. The emergency department boasts three new state-of-the-art trauma rooms, CT scanner, diagnostic room, digital x-ray facility, stat lab, and behavioral health wing.

On the floor above, a new 30-bed inpatient unit connects to the existing operating suite. The design includes 15 surgical beds and 15 advanced care beds and allows staff to move patients more quickly to the operating rooms if immediate surgery is required. All patient rooms are private and spacious, each with its own private toilet room with shower.

Pickering Associates provided complete surveying, engineering, and architectural services, guided hospital leadership through the bidding process, and oversaw the project throughout construction with full-time on-site representation.

Memorial Hospital at Gulfport Tower Expansion/RenovationGULFPORT, MISSISSIPPI

The project included a 2-floor addition to an existing 5-story patient tower, creating a new equipment penthouse, relocating the NICU, replacing the exterior skin of the tower and upgrading patient rooms to meet the latest regulatory standards and market demands—all while maintaining functions as an acute care facility with 240 patient beds. When construction commenced, the initial challenge was to create a watertight seal for the existing tower. After extensive value analysis, Roy Anderson Corp (RAC) recommended a 100,000 SF exterior glass curtain wall be installed using a unitized system. This saved eight months in construction. Once the new mechanical penthouse was constructed on the eighth floor, existing mechanical was rerouted and tied into the new system. This freed up space for larger patient rooms and enhanced clinical workstations. Because the rooms and bathrooms were on different footprints from the bottom floors, there was significant utility transitioning. RAC took advantage of times when the patient rooms weren’t at 100 percent occupancy to transition plumbing on a room-by-room basis. To attach the new structure to the top of the tower, contractors hammer drilled more than 100 columns and then poured a concrete base to form stub columns that would support the new structural steel.

The Yale-New Haven Health Long Ridge Medical Center consolidates an array of both practice and related provider-based services into the Stamford community, allowing patients the convenience of accessing care closer to home. The 52,000 SF fit-out features a newly formed Center for Musculoskeletal Care, representing an emerging center of excellence for the system. The new center contains physical therapy and rehabilitation space, imaging, physician practice space, infusion, and laboratory functions.

The building is organized into two areas with multi-specialty practice space and technical diagnostic and treatment space with support functions. The alignment of central waiting areas forms a spine, supporting an on-stage/off-stage organizational model to enhance patient privacy, staff productivity, and efficiency, and foster team-based collaboration. Exam clusters provide flexibility based on patient demand for particular specialties, improving patient and staff throughput and efficiency.

UConn Health John Dempsey Patient TowerFARMINGTON, CONNECTICUT

HKS, Inc., and Steffian Bradley Architects, a partnership, provided master planning and design services for UConn Health John Dempsey Hospital. As the state’s only academic medical center located within a university’s medical school campus, this tertiary care center houses teaching facilities for the medical and dental schools and provides extensive research space.

The project features a new inpatient tower that includes a new surgery suite, emergency department, and main hospital entrance. The new 381,000 SF tower accommodates 169 beds, 13 operating rooms, and a 37-bay emergency department. Renovations to 41,000 SF of the existing facility will provide improved and expanded space for emergency medicine, radiology, pharmacy, cardiology, and dental clinics.

The team worked to develop green solutions for a facility that must remain operational 24 hours a day, 365 days a year and provide cutting edge health care. This required the integration of existing and new mechanical systems to take advantage of the robust health center backbone. The facility is LEED Silver certified.

UCSF Ambulatory Care Center, 5th Floor Heart & Vascular ClinicSAN FRANCISCO, CALIFORNIA

The 5th floor of the Ambulatory Care Center (ACC) building offers sweeping views of downtown San Francisco. The 10,000 SF space houses the now consolidated UCSF-Parnassus Heart & Vascular services ambulatory programs. Taylor Design created an interior clinic space that reflects the contrast of the urban city view with the lush greenery of the surrounding park. The use of contrast is also demonstrated by the on-stage/off-stage concept that delineates the public facing spaces from the staff and clinician-only spaces. Furniture and finishes mirror both the natural hues of the park and the monochromatic hues of the city. The reception desk itself acts as a reflection of the city. The raw concrete columns were left exposed inside the space to contrast the warm wood utilized on the entry wall. Large patterned panels of glass were placed between the wood planks to reflect the breathtaking exterior views and break up the long runs of wood. Artwork used throughout the space drew inspiration from the graphic style of x-ray images of items found in nature.

Sibley Memorial Hospital: The New SibleyWASHINGTON, DC

The New Sibley patient pavilion reflects more than a decade of careful planning and development and is the latest phase of a transformative master plan for Sibley Memorial Hospital, a member of Johns Hopkins Medicine’s Washington, DC campus.

Early in this planning process, the design team met frequently with the strongly engaged local community associations to ensure that the goals for the hospital were met while maintaining the urban design quality which allows the Sibley campus to blend seamlessly into the fabric of the surrounding neighborhoods.

The first phase of the development, completed in 2011, was a new medical building/ambulatory surgery center and parking garage, followed by a multidisciplinary cancer center. As the new front door for the campus on completion in 2016, the 200-bed new patient pavilion creates a medical center destination with all private patient rooms, a new emergency department, and expanded women’s and infant services.
