Physician engagement: the first step for guiding patient medical home transformation

When surveyed, more than 50% of Calgary West Central PCN member physicians were unsure about the purpose of patient medical homes and where to begin. Given the complex nature and distribution of their member physician practices, the challenge was to determine how they can engage its member physicians to begin or continue their journey toward becoming a patient’s medical home.

Hear about the strategies and tactics used for change management that is helping the PCN move toward having 100% of their physician members engaged in medical home activities.

Learn about how the Addiction and Mental Health Central Clinic (AMHCC) partnered with the University of Calgary’s Central Family Medicine Teaching Centre (CFMTC) to create a process for handling complex mental health patients as they transition back to primary care for mental health management. This session demonstrates that family medicine clinics can manage patients with stable complex mental health problems by establishing good communication and working together to provide comprehensive care to patients.

Building the internal muscle for change required to transform primary care in Alberta

In this session, Dr. Wise will present his experiences, observations and published findings related to health system and primary care practice redesign, patient-centered care implementation, process improvement, practice facilitation, and building a culture for continuous improvement. These findings are derived from work with more than 100 primary care practices and multiple health systems in the United States over the last decade, along with sponsored research of patient-centered medical home implementation. In addition, a brief overview of Lean Thinking process improvement techniques and their application in support of practice redesign will be provided.

Leading social systems: a focus on partnership in organizational life

This two-hour workshop is a dynamic, yawn-free experience that focuses on the worlds of Tops, Middles, Bottoms and Customers (consumers/clients/patients) during turbulent times in organizations. We will get to the heart of partnership in organization life - what makes partnership so critical, what gets in the way of it developing, and what the role of leadership is in making it happen. This visceral approach enables the learning to get beyond the head level to where it really hits home and sticks.

Managing large scale change in a primary care, multi-clinic environment*

Hear about how the Department of Family Medicine created a quality improvement infrastructure model that would support four very unique, autonomously run clinics through their journey of primary care evolution. By realizing the need for leadership support to lead the significant changes such as EMR implementation, office efficiency principles, and a culture shift to measurement and accountability, the role of the Quality and Informatics Manager was created.

Mirella will share about the creation of the Quality and Informatics team, strategies used and lessons learned in the management of large scale change using this model.

CHANGE ALBERTA: expanding diet and exercise interventions in primary care in Alberta

Metabolic syndrome (MetS) is estimated to occur in 1 in 5 people and is the basis of cardiovascular disease, stroke, diabetes and their complications, which are major contributors to rising health care costs. Hear about the C.H.A.N.G.E. Program, an evidence-based lifestyle intervention that combines scientific evidence, the patient’s family doctor, and the expertise of team-based care to create a personalized plan that is monitored at regular intervals.

Using EQ5D5L for optimal multidisciplinary team composition

A distinguishing feature of PCNs in Alberta is the provision of multidisciplinary healthcare. The question is what is the optimal number of types of disciplines over what number of visits to produce positive changes in overall health as measured by EQ-5D-5L?

The evaluation team at South Calgary PCN has begun to identify the combined impact of multiple providers and number of visits on EQ-5D-5L VAS Scores in four groups of chronic patients. Hear about the processes undertaken and first calculations made in this ongoing work.

Time2Talk (T2T): 2 years and thriving!

The creation of a medical home within the PCN environment challenges us to serve the whole person – medically and psychologically. PCN’s are in a strategic place in a patient’s life to identify mental health issues and invite the patient to collaborative care so that we can address mental health concerns at the earliest possible opportunity.

The South Calgary Primary Care Network took on this commitment and created “T2T” (Time to Talk). Join us to hear about our journey in moving from creation to pilot project to a fully integrated mental health service located within the PCN.

Caregivers: an integral part of the care team

80-90% of community care is being provided by unpaid caregivers, who need support and resources to prevent depression, isolation and becoming overwhelmed. As the PCN teams work together to provide comprehensive care – the family caregivers need to be incorporated into the care plans. This session will look at caregiving in Alberta – the issues, impacts and concerns that effect the over 600,000 caregivers in our province. This session will also highlight the programs offered by the Alberta Caregivers Association, for both caregivers and care providers, to learn about resources, supports and developing self-care attitudes and practices.

Cultural transformation: what does this really mean?

Those who have implemented the Patient’s Medical Home will tell us this work is about cultural transformation. What does this really mean? How do we as interdisciplinary providers, health and community leaders affect cultural change? What role can each of us individually and as team members play to support this journey? Join this closing session to glean practical lessons that will support you and your team’s contribution to accelerating primary care in Alberta.

Presenter BiographiesRichard Alarie, MA, R. PsychRichard works as the clinical supervisor for the Addiction and Mental Health Central Clinic (A&MHCC). He is a registered psychologist and completed his post-secondary education at Lakehead University obtaining a Masters of Arts in Clinical Psychology. Over his career, he has worked in a number of community settings providing crisis, outreach and clinic-based mental health services. In his current role, Richard is involved in overseeing daily clinical operations, including intake, staff supervision and program development. Richard has been involved in the collaboration between A&MHCC and the Central Family Medicine Teaching Centre since October 2014.

Lori Apostal, MBA, BScN, RNLori is the Executive Director for the Wood Buffalo PCN. With over 25 years of nursing experience, Lori has worked in a variety of clinical, education and administrative positions in both acute care and primary care settings. In 2013 Lori returned to her home town of Fort McMurray to take on her role with the PCN. She is passionate about bringing health care resources into her community and has a special interest in providing access to the First Nation’s communities in the municipality of Wood Buffalo.

Shannon Berg, PhDShannon Berg is Executive Director, Primary Health Care, Addiction and Mental Health with Alberta Health. She has more than 30 years of experience in the health care industry in a variety of roles and settings. She is a registered occupational therapist, holds a PhD in Health Services Research, a clinical assistant professor at the University of British Columbia Department of Occupational Science and Occupational Therapy, an associate with Vancouver Coastal Health Research Institute’s Centre for Clinical Epidemiology and Evaluation, and adjunct faculty at University of Alberta’s Faculty of Rehabilitation Medicine.

Phil Cady CD, BSW, MALT, DSocSci (Cand)Phil Cady is President of Cognitive Leadership Strategies West (CLSWEST) and a founding partner of the LEADS Learning scholar practitioner network. He is an Associate Faculty member in the MA in Leadership (Health Care Specialization) and MBA programs at Royal Roads University, and the Conference Board of Canada’s Director’s College. Phil also serves on several organizational boards and frequently volunteers his services to non-profit and social-mission-driven organizations. He is an active Doctor of Social Sciences student at RRU and is studying and researching in the area of complexity science and social systems leadership in a health care context. Phil has brought leadership and learning opportunities to organizations of all sizes and complexity from across the world. Don Chan, MD, CCFP, FCFPDr. Chan is a graduate of the University of Calgary’s Faculty of Medicine, and completed his Family Medicine residency at the University of Alberta. Since then, he has been in a community-based family practice in Edmonton since 1989. When he’s not practicing, he is a clinical lecturer with the University of Aberta’s Department of Family Medicine and is currently sits on the board of the Edmonton North Primary Care Network. Additionally, Dr. Chan has been a physician mentor with the Alberta Medical Association’s Physician Office System Program.

Mirella ChiodoMirella Chiodo is the Quality and Informatics Manager for the Department of Family Medicine at the University of Alberta. Her responsibilities include leading quality improvement and patient safety initiatives and electronic medical records meaningful use at the four Academic teaching clinics. In addition, she has maintained a role as Alberta AIM Faculty assisting physicians and their teams focus on access, efficiency and clinical care improvements for the past 7 years. Mirella is also certified in the Prosci change management methodology.

Patrick Griffin, MS, RSWPatrick Griffin is the Clinical Supervisor for “Time to Talk”, the mental health program for the South Calgary PCN. Patrick graduated from The Columbia University School of Social Work (NYC) and had done post-graduate training at the CG Jung Institute (Chicago) and the Psychosynthesis Center (WI). His career in adult mental health has included direct patient care, administration, clinical supervision and program consultation & design. Patrick is a Sessional Instructor in the Faculty of Social Work at the University of Calgary.

Doug Klein, MD, CCFPDr. Doug Klein is an Associate Professor with the Department of Family Medicine at the University of Alberta. His research experience has included both qualitative and quantitative research methods. He is the founder and the Director of the CHANGE ALBERTA Research team. He has been recruited for his experience in primary care research by provincial and national committees. Dr. Klein is a recognized expert on lifestyle interventions in primary care and has several ongoing projects based in primary care in Alberta, including the MOVE program, CHANGE ALBERTA, and a new blog for patients www.changehealth.ca.

Joe Kwan, BScPharmJoe Kwan is a pharmacist and Extended Team Manager for the Calgary Foothills PCN. He has worked in Manitoba, British Columbia, and Alberta in various practice settings including community pharmacy, long-term care, acute care and primary care. Facilitation and interdisciplinary team-based patient care are a passion of his, and he looks forward to sharing his learnings and experiences with all of you!

David MacDonald, BScPT, MBADavid MacDonald oversees the Department of Practice Effectiveness at the Calgary West Central PCN that supports physician members with medical home activities such as panel validation, maintenance, enhanced access strategies, use of disease registries, and screening. David has worked as a physical therapist for 25 years and has been a credited surveyor for CARF as well as surveying organizations in Canada, United States, England and Sweden. He attended Queens University where he received his Bachelor of Science degree in Physical Therapy and his MBA.

Yolanda Martens Van Hilst, RNYolanda Martens Van Hilst is a Registered Nurse and Program Manager for the Calgary Foothills PCN. She has extensive experience in leading interdisciplinary teams and working with a variety of specialty services to improve care for the family physicians and their patients.

Yolanda graduated from nursing in 1993 and moved from the Netherlands to Canada in 1996. She had the opportunity to work in northern BC, North West Territories and Alberta. She has a passion for exploring new and innovative ways of designing and delivering health care services in primary care.

Heather Neumann, RN, BScN The St. Albert and Sturgeon PCN welcomed Heather as their Clinical Manager in February 2015. Throughout most of her nursing career, she has focused on the development of community case managers and facilitating collaboration within health care partners and inter-professional teams. While providing leadership to clinical practice and program development at the PCN, Heather seeks to answer this question: “How will our health system and the care we provide look different when we truly provide patient-centred, collaborative care?” She is a graduate of Foothills School of Nursing (1980) and the University of Alberta (1984).

Martha Nystrom, BSP, MBA Martha Nystrom is the South Calgary PCN Program Manager for the Mental Health programs including “Time to Talk.” Martha graduated from the University of Saskatchewan with a Bachelor of Science in Pharmacy and from the University of Calgary with a Master’s in Business Administration. Her career has involved providing and developing clinical pharmacy services for mental health patients in both hospital and community pharmacy practice settings. She joined the SCPCN following the completion of the pilot phase for the Time to Talk program.

Debra Paches, BScN, RN, GNCCDebra has been a Registered Nurse since 1977, with experience in neurosurgery and medicine. From 1983 to 2014, Debra worked in geriatric rehabilitation at the Edmonton General and Glenrose Rehabilitation Hospital. Debra worked as a staff nurse, charge nurse, unit manager, clinical educator and case coordinator. Debra also worked delivering education programs to introduce the Seniors Delirium Protocol to hospitals in the Edmonton region and developing/delivering education programs for staff (e.g.: N.I.C.H.E.).

Debra joined the Alberta Caregivers Association in 2014 as the Caregiver Navigator Coordinator, charged with developing and delivering the Navigator workshop for Health Providers and facilitating the Caregiver Support Team.

Sophie Piroozfar, MSc, BScSophie Piroozfar works as an Information Analyst in the Evaluation and Business Informatics team in South Calgary PCN. She received her Master’s degrees in Mathematics from Uppsala University (Sweden) and in Economics from KTH Royal Institute of Technology (Sweden). She has completed her Bachelor’s degree in Mathematics as well.

Nandini Raj, BSc., MPHNandini Raj leads the Sustainable Family Practice program at the Calgary West Central PCN which provides member physicians the learning and knowledge transfer to support them with panel and practice effectiveness work in their clinical practices. She also previously led the Alberta Screening and Prevention (ASaP) Initiative for the CWC PCN. Nandini has previously worked in Quality Improvement with Cancer Screening Programs at the Alberta Health Services. She has a Master of Public Health from Boston University, Massachusetts.

Eduardo Salas, PhD Eduardo Salas is a Professor and Allyn R. & Gladys M. Cline Chair in Psychology at Rice University. Previously, he was a Trustee Chair and Pegasus Professor of Psychology at the University of Central Florida where he also held an appointment as Program Director for the Human Systems Integration Research Department at the Institute for Simulation and Training. Before joining IST, he was a senior research psychologist and Head of the Training Technology Development Branch of NAWC-TSD for 15 years. During this period, Dr. Salas served as a principal investigator for numerous R&D programs that focused on teamwork, team training, simulation-based training, decision-making under stress, safety culture and performance assessment.

Dr. Salas has co-authored over 450 journal articles & book chapters and has co-edited 27 books. His expertise includes assisting organizations in how to foster teamwork, design and implement team training strategies, facilitate training effectiveness, manage decision making under stress, develop performance measurement tools and create a safety culture. Dr. Salas is a Past President of the Society for Industrial/Organizational Psychology and the Human Factors and Ergonomics Society, a Fellow of the American Psychological Association and a recipient of the Meritorious Civil Service Award from the Department of the Navy. He is also the recipient of the 2012 Society for Human Resource Management Losey Lifetime Achievement Award, and the 2012 Joseph E. McGrath Award for Lifetime Achievement.

Ernst Schuster, MD, CCFP, FCFP, CCPEDr. Schuster is a full service, hospital based family physician and Clinical Professor for the Department of Family Medicine, University of Alberta and a Senior Medical Advisor and Project Lead for the Practice Review Pilot for the College of Physicians & Surgeons of Alberta.

Dr. Schuster has been clinical faculty of AIM Alberta and an Associate Zone Medical Director for primary care and the rural hospitals in the Edmonton Zone. As a founding Co-Chair, he contributed to the development of Alberta’s primary care networks (PCNs). He has been a Board Member and Speaker of the Representative

Dr. Schuster is dedicated to help improve access, clinical care and process improvement in primary care.

Sarah Willis, RN, BNSarah works for the Central Family Medicine Teaching Centre (CFMTC). Sarah has attained her Bachelor of Nursing with great distinction from Athabasca University at Mount Royal College. She has a diverse nursing career working in family medicine, travel health, postoperative care, occupational health, and community health. Sarah is part of the Leadership Support Team for the CFMTC. She assists with quality improvements and staff education. Sarah has been involved in the collaboration between the Addiction and Mental Health Central Clinic (A&MHCC) and CFMTC, since December 2014.

Christopher G. Wise, MHSA, PhDDr. Wise’s career has been devoted to working with healthcare providers, payers and purchasers to develop, implement and evaluate new models of healthcare delivery that lead to lower population-based healthcare costs and improved quality outcomes. This includes work with Ford Motor Company and General Motors for innovative insurance products with strong medical and disease management foundations, Blue Cross/Blue Shield of Michigan in support of their nationally recognized ‘Physician Group Incentive Program’, and with more than 200 physician practices from 28 separate physician organizations to facilitate their process redesign using the principles and tools of Lean Thinking process improvement.

Dr. Wise is also a co-investigator on several studies to evaluate the limitations and successes of these newer models of care delivery, resulting in several recent publications in peer-reviewed journals. In 2012, Dr. Wise began working with a Michigan health plan, HealthPlus of Michigan, to develop and offer a Practice Facilitation Program to providers in the HealthPlus network, and support those practices in implementing Patient-centered Medical Home capabilities and improving outcomes of the Triple Aim regarding cost, quality and patient experience; during this same time frame, Dr. Wise has been a consultant to the Lahey Health System in Burlington, MA, as the lead facilitator for a year-long “Lean for PCMH Learning Collaborative” to support practice transformation in the Lahey provider network.

Bonnie Lynn Wright, PhD, MScN, BA, RN, CEBonnie Lynn Wright is a nurse, consultant, senior administrator, author, professor, and researcher. In primary care, she has been an evaluator since 2007 for Chinook, Mosaic and currently South Calgary PCNs. She has and does serve on a variety of local and Provincial Committees and Advisory Groups, including the Toward Optimized Practice Evaluation Advisory Group. Bonnie is credentialed with the Canadian Evaluation Society.
