• Hear a bad echo on the call?
• Echo is usually caused by multiple connections to a single event.
• Close all but one browser/tab and the echo will clear up.

Oniel Delva, BA, CTT+Communications Manager
Oniel Delva is the Communications Manager for the CROWNWeb OCT team. Since joining the CROWNWeb team in October 2008, he has spearheaded the development of CROWNWeb educational events and materials, and published more than 35 CROWNWeb articles in a numerous national magazines and journals.
Michael Seckman, CTT+Training Manager
Michael Seckman is the Training Manager for the CROWNWeb OCT team. Mr. Seckman has led the development of CROWNWeb online training modules, online help systems, and quick start guides since December 2007. Additionally, in his role, Mr. Seckman oversees the deployment of instructor-led webinar and video training efforts.

• What is the difference between a Saved and a Submitted CMS-2728 form?
Saved CMS-2728 forms can still be edited
Saved CMS-2728 forms can be deleted if needed
Saved CMS-2728 forms contain data that cannot be received or reviewed by CMS
Saved CMS-2728 forms are NOT SubmittedCMS-2728 forms
• Currently, there are nearly 10,000 CMS-2728 forms in a SAVED status in CROWNWeb – ready to be reviewed and SUBMITTED.
• Facilities must remember that their CMS-2728 forms are not complete until they are SUBMITTED.

• CROWNWeb triggers a “Near Match In-Scope” or “Near Match Out-of-Scope” message if key identifiers do not match.

Resolving Patient Near Match

• CROWNWeb allows users to view and compare data in CROWNWeb vs. data entered, if “Near Match In-Scope.”
• CROWNWeb informs users to contact their ESRD Network for assistance, if “Near Match Out-of-Scope.”
• If a single patient’s records are entered twice in CROWNWeb, they must be merged. Note: Fictitious Patient Data.

• PART data are obtained from information entered on the Admit/Discharge, Patient Attributes, and Dialysis Treatment Information screens in CROWNWeb.
• PART data in CROWNWeb help with verifying the accuracy of:
Patient census
Admit/Discharges status
Patients’ current treatment summaries
Patients’ current attending physicians
• Users should follow the proper patient admit process to ensure that patients missing from the PART screen are entered in CROWNWeb.
