Already boasting a worldwide reputation thanks to football and music, Liverpool’s time as European Capital of Culture in 2008 has led to a new sense of confidence. Recent developments include the Liverpool One retail area and a 11,000-seater arena which has brought international entertainment names back to the city.

From the wide, flowing Mersey to the countryside and beaches beyond the suburbs, Liverpool sits at the centre of a vast area of beauty, culture and entertainment.

Liverpool’s waterfront, home to the iconic Three Graces, Albert Dock and Pier Head, has UNESCO World Heritage Site status. The architecture is world-class, with more listed buildings than any city outside London, and two cathedrals which are major attractions in their own right - the Anglican Cathedral and the Metropolitan Cathedral. Aintree University Hospital in Liverpool provides general acute services to a population of 330,000 in North Liverpool, South Sefton and Kirkby and specialist services to a population of 1.5 million across the North West and North Wales.

Employing more than 4,000 staff, Aintree was authorised as a Foundation Trust on 1 August 2006, and has more than 13,000 public and staff members. Since becoming a Foundation Trust more than £100m has been invested in Aintree’s site. It was the first hospital in Liverpool to be rated Good by the Care Quality Commission (CQC).

Aintree provides high quality emergency and planned care to meet the needs of the local community. Aintree has dedicated assessment services including an Acute Medicine Unit, Ambulatory Emergency Care Unit, Acute Frailty Unit and a Surgical Assessment Unit. In addition to this, Aintree provides specialist services for Merseyside, Cheshire, South Lancashire and North Wales. These include major trauma, complex obesity care (including bariatric surgery), head and neck surgery, upper GI cancer, hepatobilliary, endocrine services,

respiratory medicine, rheumatology, ophthalmology and alcohol services. The Sefton Suite is Aintree’s Private Patient Unit.

Aintree is a recognised centre for multidisciplinary health research and education, and is a founder member of Liverpool Health Partners and a member of the North West Coast Clinical Research Network. Aintree collaborates closely with the University of Liverpool, Edge Hill University, Liverpool John Moores University, the University of Chester and other NHS organisations. The hospital hosts the Staff College at Aintree, which supports leadership development for senior NHS staff. Emergency Medicine at AintreeAintree is currently in the middle of a comprehensive transformational change programme designed to improve the way we manage non-elective patients attending the Accident and Emergency Department.

A key driver behind this model is the Trust’s commitment to optimising consultant-led care during peak demand hours to ensure rapid assessment and treatment and enable patients to receive the most appropriate care and treatment at the earliest possible time in their journey.

The Accident and Emergency Department is located within our purpose built, state of the art Urgent Care and Trauma Centre (UCAT) and comprises the following:

• Ambulance handover area

• 3 major trauma resuscitation rooms with dedicated high performance CT scanner

• 3 high dependency resuscitation rooms

• Majors assessment and treatment area

• Minors area comprising a number of consult exam rooms

• Observation Ward

• Dedicated X-ray department

• Mobile decontamination unit

• Link-bridge to The Walton Centre for Neurology for transfer or major trauma patients.

Also located within UCAT are the following services:

• Acute Frailty Unit• Urgent Care 24 (OOH GP) This post forms one of a team of 16 Consultants in Emergency Medicine. The existing members of the team are as follows:

The senior medical team is supported by one Associate Specialist, six StRs, three Staff Grades, two part-time Clinical Assistants and twelve SHO equivalents and seven FY1. There are also two Advanced Nurse Practitioners and seven Emergency Nurse Practitioners who assist in the management of acutely ill medical patients presenting to the Trust.

Job planThis post attracts 10 programmed activities per week. There may be the opportunity to increase this by negotiation and any special interests will be considered.

At present the on call commitment is 1 in 16 (non-resident) with 1 in 5 weekend working.

The job plan for the first three months will be based on a provisional/indicative timetable. A formal job plan will be agreed between the appointee and the Clinical Director within three months of the commencement date of the appointee.

The supporting professional activities (SPAs) allocation within the provisional job plan is an illustrative guide only and will be finalised following individual agreement with successful candidates. Approximately 1.5 SPAs will be provided as standard within a full-time contract.

Regular job planning meetings will be undertaken with the post holder to ensure that the appointee is remunerated for any additional activities undertaken and that the job plan accurately reflects the overall workload undertaken by the post holder.

RotaA self-rostered annualised rota is currently in operation.

The standard shifts for Consultants in Emergency Medicine are as follows:

Monday to Friday: 08:00 - 16:00 09:00 – 17:00 11:00 – 19:00 16:00 – 00:00 (with on-call after midnight)

19:00 – 02:00 Saturday & Sunday: 08:00 – 17:00 12:00 – 21:00 16:00 – 00:00 (with on-call after midnight)

The post holder will be expected to contribute to the Major Trauma Team Leader rota Monday to Friday however the following shifts will be covered by the Cheshire and Merseyside Trauma Network:

Monday to Thursday: 23:00 – 08:00Friday to Sunday: 21:00 – 09:00Saturday & Sunday: 09:00 – 21:00

There will be the opportunity to contribute to additional trauma shifts if desired.

All out of hours duty after 22:00 is recognised at an enhanced rate (this will be reviewed in March 2017).

Duties

Specific duties of the role include:• Reception, resuscitation, diagnosis and initial

treatment of all emergencies as appropriate.• Referral of patients to other specialties, transfer of

patients for admission to assessment units, to other hospitals within or outside the Trust, referral as out-patients or back to the community.

• Provide high quality consultant delivered care to patients including clinical leadership and supervision of junior medical staff, ensuring the highest standards of clinical care are provided to all patients.

• Be proactive in ensuring that all patients receive rapid assessment, treatment and clinical decision making, whilst ensuring that the Clinical Quality Indicators to deliver safe quality and performance are delivered and sustained.

• Work in conjunction with the nurse coordinator to ensure that there is effective and timely communication between the clinical team; timely intervention and safe care to maintain optimal patient flow.

• When colleagues are absent alternative clinical duties may be arranged to provide appropriate cover.

• Provide teaching and training at undergraduate and postgraduate level and supervise and train the junior and specialty doctors in Emergency Medicine, advanced nurse practitioners and emergency nurse practitioners.

• Be proactive in the clinical governance agenda and take on leadership roles to support continuous learning and improvement, thereby abiding by the clinical and corporate governance policies of the Trust.

• Liaise with clinicians and managers within and outside the department to address issues arising that affect the service.

• Participate in relevant audit and quality assurance processes.

Other duties include: • Provide Consultant input to Emergency Medicine and

oversee and support junior medical staff in this area• Maintain the knowledge and skills essential

for fulfilling the job plan. The Trust supports the requirements for continuing professional development as laid down by the Royal College of Emergency Medicine and is committed to providing time and financial support of these activities. There is a strong programme of continuing medical education, including a monthly Grand Round programme in the Trust. Study leave can be taken subject to the Terms and Conditions of Service. If the appointee is a new consultant, a named consultant mentor will be provided.

• Teaching is strongly encouraged both within the Trust and Emergency Medicine. Emergency Medicine has an unparalleled selection of acutely ill patients with evolving medical problems that provide a rich learning environment for medical students, junior doctors and allied professionals. The post holder will be expected to actively participate in undergraduate and postgraduate teaching.

• Actively develop clinical research interests, particularly in the areas of the primary/secondary care interface and clinical audit.

• Undertake revalidation as prescribed by the General Medical Council, and will be required to undergo multisource feedback and participate in reflective practice.

• The Trust is committed to providing a high quality clinical service and is strongly involved in both audit and clinical governance. It is expected that the post holder will make an active contribution to both of these areas. There is an active post graduate education programme in the Trust with ample opportunity to obtain internal CME.

• Adhere to all Trust Policies and Procedures.

The post holder will be based primarily in the Accident and Emergency Department.

Office space and secretarial support will be provided for the post holder. An up to date computer system will also be provided which will be equipped with internet access and e-mail capability. Appropriate training for the use of this equipment will be available if necessary.

General provisions, summary of main conditions of service and general statement & obligations

The appointment is subject to the Terms and Conditions for Consultants (England) 2003.

1. General provisions

(a) The Appointee will be expected to work with local managers and professional colleagues in the efficient running of services and will share with consultant colleagues in the medical contribution to management. Subject to the provision of the Terms and Conditions of Service, he/she is expected to observe the Trust’s agreed policies and procedures, including Infection Prevention and Control, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instructions of the Trust. The Appointee will be expected to follow the local and national employment and personnel policies and procedures. He/she will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of their patients to be able to contact a Consultant when necessary.

(b) All medical and dental staff are expected to comply with appropriate Trust Health and Safety policies and Infection Control policies.

(c) The Appointee will have responsibility for the training and supervision of junior medical staff who work with him/her and will devote time to this activity on a regular basis. If appropriate, he/she will be named in the contracts of doctors in training grades as the person responsible for overseeing their training and as a main source of advice to such doctors on their careers.

(d) As part of the NHSLA accreditation of the Trust it is mandatory for the appointee to attend the Trust Welcome Event and the Basic/Advanced Life Support Course.

2. Residence

(a) The successful candidate will normally be required to live within 15 miles/30 minutes of his/her base hospital (the hospital where the principal duties lie) and in a location offering easy access to the other hospitals as required.

(b) Where, however, the successful candidate already resides within 15 miles by road of his/her base hospital, he/she will not be required to remove his/her home nearer the hospitals.

(c) Where the successful candidate’s present residence is more than 15 miles by road from the hospital he/she will be required to remove his/her home to meet the residential clause of his/her contract, unless he/she has the written consent of the Trust to the contrary.

(d) The Terms and Conditions of Service states the “removal expenses shall be reimbursed and grants paid only when the Trust is satisfied that the removal of the practitioner’s home is required and that the arrangements proposed are reasonable”.

(e) Therefore, successful candidates are advised not to enter into contractual arrangements for the removal of their home until such time as the formal approval of the Trust is received in writing.

3. Disclosure & Barring Service (previously Criminal Records Bureau)

The successful applicant will be required to undertake an enhanced DBS Disclosure check. Further information on the DBS is available on their website at http://www.dbs.gov.uk. The Appointee will be required to pay for the DBS Enhanced Disclosure.

4. General Statement & Obligations

a) Duties of the PostThe duties above indicate the postholder’s main responsibilities but are not exhaustive. They may be jointly reviewed with the postholder according to experience and to support service development.

b) Business Continuity PlanThe Trust is required to have robust emergency preparedness plans in place, to ensure the continuity of service in certain circumstances, e.g. major civilian disaster, business continuity incident. The Trust expects the post holder to be aware of their role in the event of such a significant disruption to Trust services. As a consequence of such circumstances the post holder may (within their scope of competence) be required to undertake a variation to their normal duties, normal base and / or hours of work as are reasonable and appropriate to maintain essential service provision.

c) Policies & ProceduresAll employees must follow the Trust’s policies and procedures. These cover all areas of work, both clinical and non-clinical. Some important areas applying to all employees are highlighted below. Your induction will cover key post-related policies. All Trust policies are available on the intranet.
