    POST INFORMATION

    This is a new post created at the James Cook University Hospital to support the generalhistopathology reporting service.There are currently ten consultants (nine whole time equivalent posts and one part timepost) at James Cook University Hospital and two whole time equivalent consultants at theFriarage Hospital NorthallertonThe post is advertised on a whole time (10 PA basis), although any candidate who is unableto work full time will be eligible to apply and, if appointed, modification of the job content willbe discussed in consultation with consultant colleagues and the Medical Director.

    The Trust is part of the recently formed North of England Cancer Network which is made upof the old Northern Cancer Network and the Cancer Care Alliance.

    The Regional Spinal Injuries Unit provides a comprehensive spinal cord injury service for theNorth of England (population 3.4 million).

    It also covers a range of highly specialist tertiary services extending to 1.5 million people inTeesside and parts of Cumbria, Durham and North Yorkshire with leading expertise in heartdisease, cancer, trauma, neurosciences, renal services and spinal injuries.

    The Trusts core business is to provide secondary healthcare to:

    Around 274,000 people living in Middlesbrough and the local area of Redcar andCleveland

    124,000 people in an area stretching from the North Yorkshire Moors to the centralPennines, the borders of York district in the south and the borders of Darlington in thewest.

    In 2009/10 the Trusts budget is 418.1m and it employs almost 7,000 full and part time staff.

    Hospital at NightThe Trust has developed an at night wireless communication system between ward andmedical staff via a night co-ordinator. This allows on-call clinicians receiving ward requestsfor help to prioritise calls, using supplied clinical observations, through a PDA, gives accessto protocols on patient management and also allows data collection on patient and wardactivity and response times to seeing patients and completing tasks. This system has beenawarded the prestigious HSJ Award for e-technology and has been short-listed for a BTaward.In January 2006 the Trust achieved Practice Plus status for work on Improving WorkingLives. This is an important recognition for commitment to good working practices and, asequally important, how staff find working in the Trust.

    THE JAMES COOK UNIVERSITY HOSPITAL

    One of Europes most modern hospitals, The James Cook University site is the largest of itstype and has a wide range of specialities. This hospital was opened in October 2003 and has acapacity of 1,007 beds. Based on the original site of South Cleveland Hospital, all acute andelective medical and surgical specialties are accommodated. Tertiary services, in addition tocardiology and cardiac surgery, include renal medicine/haemodialysis, neurosurgery, vascularsurgery and the regional spinal injuries unit.

    This site contains a modern central operating suite containing 19 well-equipped theatres,with two recovery areas. This is immediately adjacent to an Adult Surgical Day Unitcontaining a further three theatres and also to the Paediatric Day Unit. The 16 beddedgeneral ICU is across the corridor. Two additional theatres for gynaecology and obstetrics

    are in the Central Delivery Suite and two more in the Ophthalmology Day Unit.

    The Accident and Emergency Department on this site is the designated Regional TraumaCentre for Teesside, South Durham and North Yorkshire. The hospital has a 24-hour AcuteAdmissions Unit.

    There is a 21 room imaging department equipped with a full range of mainly digitalmodalities, a trust-wide PACS system and a purpose built 6 bed Radiology Day Unit. AMedical Physics Department is also on this site together with isotope imaging.

    The central clinical laboratory provides a comprehensive pathology service with electroniclinks to wards and departments. Electronic requesting is being piloted.

    There is a purpose-built Endoscopy centre.

    The hospital has a comprehensive tertiary level fertility service and is also under the remit ofthe Gynaecological Cancer Centre, the Cancer Care Alliance of Teesside, South Durhamand North Yorkshire.

    The hospital caters for just less than 4000 deliveries in purpose built delivery and lowdependency suites. The birth rate is increasing. Fetal medicine and high dependencydelivery referrals are accepted.

    There is a regional tertiary neonatal intensive care unit with 9 intensive cots, which acceptsboth local and regional admissions. It supports a neonatal transport service and has a strongacademic record. Any babies born at the Friarage hospital (c1100 deliveries per year)

    requiring intensive care are transferred to this unit.

    There is an established Academic Centre embracing postgraduate medical education, NurseTraining School, the School of Radiography and library facilities. It encompasses two largelecture theatres with the latest audio-visual aids and a considerable number of classrooms andseminar rooms, including ward facilities for teaching and examining. There is a new library withcomprehensive IT suite.

    A dedicated undergraduate teaching centre has been commissioned recently and there areplans to have a dedicated simulation-training centre on site in the near future.

    An accredited major trauma receiving hospital, it contributes to UK Trauma, Audit &Research Network (UK TARN). It occupies a leading UK position in its survival rates fromblunt trauma as evidenced from UK TARN.

    THE FRIARAGE HOSPITAL

    The Friarage Hospital at Northallerton, situated approximately 23 miles from The James Cooksite, merged with South Tees Hospitals NHS Trust in April 2002. It provides a wide range ofsecondary services to its local population and is well known for its friendly atmosphere.

    A fully equipped district general hospital with 314 beds, it provides a 24-hour accident &emergency service and offers inpatient and outpatient facilities in general surgery includingcolorectal, breast (including screening) and urology services, orthopaedics and trauma.Medical specialties include acute medicine, diabetes and endocrinology, rheumatology,gastroenterology, respiratory and rehabilitation medicine. Inpatient and outpatientobstetrics/gynaecology, paediatrics and adult psychiatry services are on site

    Cardiology, encompassing CCU and cardiography, functions as a self-contained unit but is partof the cardiology directorate. There are two cardiologists in post. Increasing integration of thecardiology services across the two sites is planned.

    The hospital is furnished with a well-equipped medical library with facilities for electronicsearching of medical databases and access to the Internet. A full programme ofpostgraduate educational meetings is held on this site.

    Support services on this site include haematology, chemical pathology, histopathology,microbiology, physiotherapy, occupational therapy and speech and language therapy.Patients requiring cardiothoracic or neurosurgical services, nuclear medicine or MRIscanning are referred to other hospitals including The James Cook University Hospital andLeeds General Infirmary (45 miles).

    The Ministry of Defence Hospital Unit (MDHU) has been based at the Friarage since July1999 with contribution of military doctors, nurses and other staff who are integrated into thehospitals services.

    EDUCATION, TEACHING AND RESEARCH

    The modern Academic Centre at The James Cook University Hospital is housed within twolinked buildings. They contain two large lecture theatres, which have audio-visual links.There are many seminar rooms of varying sizes, which are used for teaching,demonstrations, lectures and conferences. There is a new library which is well equippedwith most of the general and specialist journals relevant to the Trusts clinical activity. Thereis a large IT suite of over 40 stations within the library complex.

    The Academic Centre also houses the Clinical Trials Unit which has recently been extendedwith links to the unit at the Royal Victoria Infirmary in Newcastle. Clinicians who areinterested in developing a clinical trials programme are encouraged to contact the Research& Development Department.

    There are close links with the College of Health at Teesside University for the training ofnurses and midwives, radiographers and physiotherapists.

    There is an active postgraduate district and divisional curriculum on site based in thepostgraduate education centre under the direction of a university clinical tutor. There is an up-

    Northallerton, one of Englands best-loved country towns is on the doorstep of the YorkshireDales.

    LOCAL AMENITIES

    Leisure/RecreationThere are major shopping centres at Teesside Park (Stockton), Middlesbrough andDarlington. Newcastle and Gateshead Metro Centre are within reach, as are the historicalCities of York and Durham. Hartlepool Marina and Hartlepool Historic Quay and MaritimeMuseum are part of a multi-million pound development also only a short distance away. Thehistoric spa town of Harrogate is within easy reach of Northallerton.

    There are excellent facilities in the area for most sports, including golf, swimming, squash,rugby, cricket and tennis. There is an excellent indoor tennis centre (David Lloyd) close tothe Durham University campus in Teesdale (4 miles from the James Cook UniversityHospital) and there are excellent golf courses at Wynyard and Romanby near Northallertonwith driving range facility. The Tees Barrage has allowed the creation of simulated whitewater canoeing and rafting centre in Teesdale that attracts canoeing enthusiasts from allover the UK.

    There is easy access to the coast, the North Yorkshire Moors National Park, the Penninesand Dales. There is an extensive range of recreational and cultural facilities in theimmediate vicinity. The Cleveland hills offer opportunities for a variety of outdoor pursuits,including rambling, gliding and hang-gliding. There is active support for the arts in theregion, both at professional and amateur level, with a wide range of cultural events andactivities available. These include theatre, ballet, opera and all types of music, as well as avariety of museums.

    There are multi-screen cinemas in Middlesbrough, Teesside Park (Stockton) and HartlepoolMarina with bowling facilities at Teesside Park. Middlesbrough, Stockton, Yarm, Stokesley,Great Ayton and Northallerton have a wide range of family friendly eateries. The majority of

    small towns and villages surrounding Middlesbrough and Northallerton have excellentrestaurants offering a variety of different cuisine.

    The enthusiastic spectator can follow all football, rugby and cricket. Middlesbrough FootballClub has a superb new ground on the developing Middlehaven site, close to the banks of theriver Tees (Riverside Stadium) regularly featuring Premiership football.

    HousingThere is a choice of housing in comfortable residential areas in the many pleasant rural andurban areas in the surrounding countryside and within easy reach of the trust. Stokesley,Great Ayton, Eaglescliffe, Yarm and the villages surrounding Northallerton offer a range ofquality houses at sensible prices. A range of executive houses is available, includingexclusive developments at Wynyard, Nunthorpe and surrounding villages. Marske and

    Saltburn offer the opportunity to live by the sea and there are many beautiful village locationsat the edge of the Dales National Park area accessible from Northallerton.

    EducationThere is a complete range of educational facilities ranging from nursery to higher education,with Universities at Teesside, Durham, Sunderland and Newcastle, offering a wide choice ofdegree courses. The local secondary schools are comprehensive and many in Nunthorpe,Stokesley, Yarm, Eaglescliffe and Northallerton enjoy an excellent national reputation with

    TransportRoad links are excellent in all directions allowing quick and easy commuting. Due to thegood road system and ease of travel, many consultants live outside the traditional 10 milesdistance.

    Road: The A19 links to the A1 (M) and the A66 links Teesside to the beautiful Lake Districtand Cumbria. Travel time to the Lake District is just 1.5 hours by car. Whitby andYork are just 1 hour away

    Rail: Darlington and Northallerton railway stations are only a short driving distance away,from which London is 2.5 hours away and Edinburgh 2 hours.

    Air: Durham and Tees Valley International Airport is also about twenty minutes fromMiddlesbrough and has frequent services to London, Amsterdam, Dublin and a largenumber of holiday destinations both in the UK and abroad.

    Newcastle and Leeds/Bradford International Airports are within easy driving distance.

    THE SPECIALTY & R.C.PATH APPROVED JOB DESCRIPTION

    INTRODUCTIONThis is a new whole time consultant Histopathologist post to the South Tees Hospitals NHSFoundation Trust at the James Cook University Hospital, Middlesbrough. The appointee willbe expected to deliver a general histopathology reporting service, to be the lead or deputylead pathologist for a cancer tumour site multidisciplinary team, to develop a subspecialistdiagnostic interest that complements the existing consultant expertise within the departmentand to lead on service developments pertaining to the diagnostic areas of interest. Aninterest in, or willingness to develop an interest in skin or head and neck pathology would bedesirable.There are currently ten consultants (nine whole time equivalent posts and one part timepost) at James Cook Hospital and two whole time equivalent consultants at the FriarageHospital, Northallerton.

    Any candidate who is unable for personal reasons to work full-time will be eligible to beconsidered for the post. Job shares will be considered. If such appointments are made,modification of the job content will be discussed on a personal basis with the Trust inconsultation with consultant colleagues.

    THE AREATeesside is a major centre of industry with substantial new infrastructure development and issurrounded by beautiful countryside including the North Yorkshire Moors National Park, theNorth East coastal beaches with Teesdale and the Pennines to the west. There are excellentand largely uncongested road links within Teesside itself, also providing fast access to theA1 and motorway network so that the centres of Newcastle, Durham, Leeds and York are allwithin easy reach. London is within two and a half hours by rail using the east coast mainline from Darlington and Durham Tees Valley Airport is within twenty minutes drive from TheJames Cook University Hospital. The national cycle network route 65 links Middlesbroughand York.

    Individuals and families moving into the area have a wide choice of affordable housing incomfortable residential areas within the towns, or in the numerous small villages and markettowns of the surrounding countryside.

    First rate educational facilities are available, from nursery schools through to TeessideUniversity and the new Stockton campus of the University of Durham. In addition to stateeducation there are a number of excellent and affordable private schools within easy reach.

    Cultural and recreational activities are well provided within Teesside itself and in Durhamand Newcastle to the north and York to the south, all within easy reach.

    THE TRUST

    South Tees Hospitals NHS Foundation Trust is a people friendly organisation, with acomplement of over 280 consultants and 7000 staff in all, based on two sites The JamesCook University Hospital in Middlesbrough (JCUH) of 1,070 beds, and the smaller FriarageHospital in Northallerton (FHN) some twenty miles to the south.

    There are the usual DGH specialties of general medicine with an associated acuteassessment unit, general surgery, orthopaedics and trauma, diabetes and endocrinology,respiratory medicine, gastrointestinal medicine, geriatrics, obstetrics, gynaecology andpaediatrics. At JCUH there are regional and sub-regional specialties of urology, nephrology,radiotherapy and oncology, dermatology, rheumatology, clinical genetics, immunology,

    neonatology, cardiology and cardiothoracic surgery, ENT, ophthalmology, maxillofacialsurgery, plastic surgery, vascular surgery, upper GI surgery, neurology and neurosurgery.The regional spinal injuries unit serving a population of over three million is also based at theJCUH.

    JCUH is a major trauma centre and provides the main A&E service for Teesside. There are22 operating theatres with separate day case facilities and two eight bed adult generalintensive care units with a separate intensive care unit for cardiothoracic surgery andpaediatric.

    JCUH is a teaching hospital and houses a large academic centre with excellent facilities foreducation, teaching and research.

    THE DIVISION OF PATHOLOGYThe Pathology Division groups the laboratory disciplines of cellular pathology, haematology,biochemistry, microbiology and virology. The laboratories, offices and mortuary at JamesCook University Hospital are located in modern facilities purpose built in 2002.The pathology laboratory at the Friarage Hospital is a new build that opened in July 2006.

    CELLULAR PATHOLOGYThis is a friendly and supportive department with an emphasis on open two waycommunication. All staff are encouraged to take a team based approach to problem solvingand service development and there is a weekly semiformal housekeeping meeting.

    The department is regularly visited or contacted over the phone by clinicians who want todiscuss unusual or interesting cases and excellent clinical-pathological relationships havebeen established with many of the service users.

    The main histopathology laboratory at James Cook University Hospital is spacious and openplan with three tissue processors and microtomes that allow conventional or large blockcutting. There are two cut-up stations both with height adjustable, downdraft ventilated

    AFOS benching and a MACROPATH imaging system to allow a permanent record ofspecimen dissection and sampling. There are plans to replace the current LEXICOM digitaldictation system with voice recognition. There is a separate high risk cut-up facility, cryostatand staining area for high risk specimens or frozen sections.The department is considering moving to rapid tissue processing with automated embeddingas part of a managed service contract.

    Immunohistochemistry is fully automated with two Ventana Benchmark machines purchasedin 2009. There is an extensive repertoire of immunoantibodies available and facilities for skinand renal immunofluorescence.

    Neuropathology and perinatal pathology are provided on site. The Royal Victoria Infirmary atNewcastle provides electron microscopy services. A contract is in place with Medical

    Solutions for molecular studies such as FISH for Her2 status. Other molecular pathologytests have been purchased from Leeds.

    There is a large, airy cytology screening room adjacent to the main laboratory, which isfurnished with ergonomic screening stations and microscopes. Gynae cytology samples areprepared and processed in the main lab area. The department is working as part of theTeespath network with the University of North Durham providing the processing unit forLiquid Based Gynae Cytology specimens. Slides are transported back to James Cook forreporting. Non-gynae cytology samples at James Cook are processed both routinely and byLiquid Based Cytology using a CYTYC Thin Prep 2000.

    Doctors in training:Specialist registrars are based in the department as part of the Durham-Tees Valley rotation.Foundation Year 2 trainees rotate through the department at four monthly intervals.

    Biomedical Scientists and Medical Laboratory Assistants:The cell path laboratories at James Cook University Hospital, Middlesbrough and theFriarage Hospital, Northallerton are managed by Mrs Jacqui Richards, Specialty ServicesManager.BMS staff based principally in histology at James Cook include, 1 wte BMS3, 1 wte BMS2.5,5.8 wte BMS1 and 1 wte trainee BMS.There are 3 wte AFCband 4 Associate Practioner 5 wte MLAs , and 1wte MLA/MDT co-ordinator.BMS staff based principally in cytology at James Cook include 1 wte BMS3, 2 wte BMS2, 2.7wte BMS1, 2 wte cytoscreeners and 1.5 MLA.

    The mortuary is managed across both sites by Mr Richard Styan and there are 1.5 wteMTOs and 0.5 wte MLA.

    There are 4 wte secretarial and 2.9 wte clerical staff.

    MANANGEMENT ARRANGEMENTSThe clinical director and budget holder for cellular pathology is Dr David Scoones.The clinical director post is remunerated and the appointment is made and performancereviewed through the medical directors office. The clinical director post is not fixed term anddoes not rotate within the consultant team. The present clinical director has been in post for just over a year. The opportunity for succession planning comes at the time of annualappraisal for consultants who express interest in taking on a wider management role.

    The lead pathologist for Cervical Cytology is Dr Adrienne Mutton.The division of pathology is represented at the Trust Management Board by the Chief ofService Dr John Drury who is also consultant chemical pathologist, and the divisionalmanager Dr Steve Duncan. There is a monthly Pathology Executive Board Meeting at whichcellular pathology is represented by Dr David Scoones and Mrs Jacqui Richards, and amonthly Cellular Pathology Directorate Meeting attended by all cellular pathologyconsultants and senior technical staff.

    All consultant histopathologists are members of the North of England Cancer Network.

    THE JOB ITSELF DUTIES OF THE POST

    a) TitleConsultant Histopathologist employed by the South Tees Hospitals NHS Foundation Trust

    b) Principal Duties of the PostSurgical Pathology;

    The successful candidate will be expected to undertake routine case reporting includingfrozen section reporting for a general surgical case mix. The department is a tertiary cancercentre and receives complex resection specimens; there is a move toward complexresections being reported by smaller teams of pathologists and therefore the appointeesreporting duties for such complex cases will be agreed with the clinical director.The appointee will not be expected to provide renal pathology or neuropathological reportingunless the appointee has specific expertise and interest.

    CytologyThere is no requirement in this post to report cervical smear cytology or non-gynae cytology.However, if the successful applicant can demonstrate a specific non-gynae cytology interestand expertise, inclusion in the non-gynae cytology reporting rota will be considered subjectto the needs of the department. Non-gynae cytology reporting includes hot breast FNACreporting for a weekly one-stop breast clinic.

    Anatomical Pathology:The successful candidate will not be expected to undertake adult hospital interest autopsiesunless they have a specific interest. There is no requirement to undertake perinatalautopsies.Coronial autopsies are not part of the NHS duties of the post and any autopsies undertakenat the behest of H.M. Coroner are accommodated using a time shifting mechanism and withthe agreement of the clinical director.

    Clinical SupportThe successful applicant will be expected to provide advice and support to other clinical

    colleagues, to be available to discuss histopathological findings and to contribute to the auditand monitoring of the effectiveness of the clinical services.The appointee will be expected to provide pathology support to one or more cancermultidisciplinary teams.

    Teaching & Training: The directorate is involved in postgraduate training with threeaccredited educational supervisors and other colleagues who act as clinical supervisors.The successful applicant will be expected to contribute to the supervision and training ofmedical, scientific and technical staff within the directorate.Dr Adrienne Mutton is Associate Programme Director for the Durham and Tees ValleySpecialist Registrar Training Programme.An informal journal club takes place attended by both technical and medical staff.There is a monthly departmental CPD meeting attended by all staff to which the appointee

    will be invited to contribute.

    Service DevelopmentThe successful applicant will be expected to contribute to the effective running of thedirectorate by service planning and development to support the changing needs of patientsand clinical users within the Trust and Cancer Network and to meet standards for laboratoryCPA accreditation.

    c) Division of work and job planningIndividual consultant workload is monitored through prospective Cochrane scoring and thereis a weekly reporting rota that aims to accommodate reporting duties, MDT case preparationand attendance, and other meeting and administrative commitments within the agreed DCCsessions for that individual.

    Job planning is subject to annual review usually at the time of appraisal.Any job planning or appraisal concerns that might arise are mediated through the chief ofservice and medical director.

    The proposed consultant Histopathologist job plan is attached (appendix 1).

    For a whole time contract, a minimum of 8 programmed activities of direct clinicalcare will be expected, with 2 PAs of supporting professional activities. Up to 1.0 SPAis allowed to be worked off site at a time and place agreed, with outcomes agreed.These figures are an average per week. A formal job plan will be agreed between theappointee and the Clinical Director, on behalf of the Medical Director, within threemonths after the commencement date of the appointee.

    CONTINUING PROFESSIONAL DEVELOPMENTThe successful candidate will be expected to participate in the Royal College of PathologistsCPD scheme and to plan appropriate study leave in their annual personal development plan.The Trust also runs a comprehensive in-house training programme for generic skillsdevelopment.

    There is a weekly postgraduate lecture with both in-house and external speakers on aThursday lunch time during academic term-time with presentations from cellular pathology.All consultants within cellular pathology are encouraged to participate in systematic auditand to register these audits on the Royal College of Pathologists audit database.A monthly departmental CPD session and journal club is organised by Dr David Scoones.

    RESEARCH and DEVELOPMENTThere is no formal research programme within the department but the cellular pathologydepartment supports research and development within other directorates.An interest in developing research and development within cellular pathology will beencouraged. The James Cook University Hospital has strong links with the Universities ofTeesside, Durham and Newcastle.

    OUT OF HOURSThere is no routine weekend work.There is no on-call commitment.

    ANNUAL LEAVEThe annual leave entitlement excluding Bank Holidays is 32 days. Annual leave applications

    must be received 6 weeks prior to the leave start date.

    APPRAISALThe successful candidate is expected to participate in annual appraisal with the clinicaldirector and to have an annual individual development plan that includes personal andservice objectives.

    MAIN CONDITIONS OF SERVICE

    The terms and conditions are identical to the new Consultant Terms and Conditions ofService of Hospital Medical and Dental Staff (England and Wales) and General WhitleyCouncil Conditions of Service, as amended from time to time.

    1 Any Consultant who is unable to work full time for personal reasons will be eligible tobe considered for the post. If such a person is appointed, modifications of the jobcontent will be discussed on a personal basis in consultation with consultantcolleagues and the Medical Director.

    2. The successful applicant will normally be required to reside within the traditional tenmiles of their base hospital although reasonable requests to extend this are usuallyagreed by the Medical Director.

    3. The appointee will be subject to a medical examination.

    4. The current salary applicable to the post will be in line with the new ConsultantContract.

    5. Relocation expenses will be offered and are negotiated on an individual basis withthe successful applicant.

    6. Secretarial assistance and office accommodation will be made available.

    Travel and SubsistenceTravel and subsistence will be reimbursed for preliminary visits only to those candidates

    selected for interview. Reimbursement is restricted to two such visits, whether they aremade before or after the constitution of a shortlist. In the case of candidates travelling fromabroad, travelling expenses are normally payable from the point of entry into the UnitedKingdom.

    Based on a 10 PA contract with a 8 DCC to 2 SPA split.

    One PA is the equivalent of four hours of time spent in the core hours period.The only agreed fixed sessions to be those of the appropriate multidisciplinary teammeetings for tumour sites.RCPath guidelines for workload (Cochrane) suggest a sessional workload of 40 units.
