5. In the last 14 days have you:
Travelled outside Saskatchewan but within Canada?
Had NON-close contact with a confirmed or probable case of COVID-19? (see definitions)
Spent prolonged time (i.e. >15 min) with people outside your extended household where you were not able to maintain physical distance or use appropriate PPE† for the situation ?
* If you have received clearance by occupational health, employee health, or public health as no
longer a risk following exposure or positive test, you can answer no.
** Current list is available at screening stations
† Appropriate PPE includes cloth masks for non-medical situations

If you answer YES to question 4** or 5 you MAY work but must self-monitor

*If you have an approved travel exemption, please proceed but be prepared to provide documentation
** If you have been in an area of concern (Q4) and are unable to maintain a social distance (2 metres) while at work, please don a mask even if working in a
non-clinical area

Support Person If you answer YES to question 1 or 2
you will NOT be permitted to visit at this time Contact HealthLine 811 and
self-isolate until you receive further direction
If you answer YES to question 3 you will NOT be permitted to visit until 14 days:
after your return to Canada
after your last contact with confirmed or probable case
You must self-monitor and, if you develop symptoms, isolate and call HealthLine 811 immediately
If you answer YES to question 4 or 5 you MAY visit if you don’t have symptoms
and you need to wear a mask
You must also self-monitor and, if you develop symptoms,
isolate and call HealthLine 811 immediately
