ACIP Recommendations

The Pfizer-BioNTech COVID-19 vaccine (2 doses, 10µg, IM) is recommended for children 5 through 11 years of age in the U.S. population under an Emergency Use Authorization.

This applies to all children 5-11, including those with underlying conditions and previous COVID infection.

See the clinical considerations presentation with important details on implementation and administration language for this pediatric population as reviewed by the committee.

Safety and Efficacy Summary

• ~3,000 children vaccinated with the Pfizer vaccine during clinical trials • 90.7% effective against symptomatic COVID disease• no deaths, no case of myocarditis, no Bell’s palsy, and no anaphylaxis. • There have been 0 deaths related to myocarditis from the COVID-19 vaccine in

adolescents and adults, a statistic many voting members felt compelling when deciding their vote.

• Systemic and local reactions were less severe in the 5-11 cohort compared to older children and adolescents.

The committee felt that the safety and efficacy data were clear, and that benefits overwhelmingly outweigh the risks of COVID disease (ie. MIS-C, unknown effects of long term COVID, etc.)

Schedule and timing

• Administered intramuscularly as a primary series of 2 doses (0.2 mL each) 3 weeks apart to individuals 5 through 11 years of age.

• The 4-day grace period applies to this formulation.• There is no maximum interval between doses. Regardless of length of time

since 1st dose, no need to restart series. • For those who age from 11 to 12 years old, mid series:

• While both formulations are allowed under their EUA, CDC recommends giving patients the dose based on their age at the time of vaccination. Doing otherwise, however, would not be considered an error.

• No recommendations for an additional dose for an immunocompromised child.

• No booster recommendation for this age group.

Vermont Implementation

• Initial allocation of 15,900 doses prepositioned across the state for school and community clinics.

• Vermont has a proven track record of effectively using large community clinics for rapid vaccine distribution.

• Opened registration for school based and community clinics on 11/3. Almost 16,000 kids have registered so far.

• Many pharmacies received their allocation of pediatric vaccine this week.

Current landscape

Demand and capacity to vaccinate far exceeds supply.

• Uptake at community clinics has exceeded the states expectations.• Almost 16,000 have registered for state-run community and school located clinics. This is more than a

3rd of the state's population of 5 through 11-year-olds.• Due to this high uptake, the state is obligated to fulfill all those second dose needs.• We are asking for additional vaccine at the highest levels, but this is not a simple or quick

process.• At this time, we do not have enough vaccine to support routine ordering for PCP offices.• Large community clinics are the fastest way to get your patients vaccinated. Please refer kids

that can get vaccinated at a pharmacy or community clinic to the state website. There they will be able to sign up via the state system and see which pharmacies are offering vaccine in their area. https://www.healthvermont.gov/covid-19/vaccine/vaccines-children

• We are using IMR, registration data, and geographic distribution to assist with internal decision making.

First Pediatric Pfizer Order

• Tonight (11/09), Pediatric Pfizer vaccine will be made available in your VIMS catalog. • To the best of your ability, determine the number of doses you can use as FIRST DOSES

in 2 weeks (administration between approx. 11/17 and 12/01). Place an order for this amount tomorrow November 10, as a COVID-19 Only Order from the VDH DEPOT.• Even if you determine the minimum order (100 doses) or more is needed for this time frame, do NOT

place a direct order. We may not be able to fulfill your need this first week, and initial orders may all come from the Depot.

• Second dose needs are being taken into consideration when planning. Do not hold doses. • Confirmation for delivery quantity, and delivery date (between 11/15 and 11/18), will go out from the

[email protected] email no later than Thursday night. Expect changes to requested order quantity. Do not expect all two weeks of vaccine next week.

• Guidance may change before the next order date (11/17), so please be mindful of our communications.

• Some Healthcare partners participating in community vaccination efforts currently have pediatric vaccine available. Please refer to communication through the [email protected] today and tomorrow for up-to-date guidance.

• If you are receiving a transfer from your umbrella organization, you will not place an order in VIMS and should work through your organization.

COVID-19 Vaccine ordering, general reminders

• Do not order other routine vaccines on Wednesdays, reserve for COVID-19 orders.

• VIMS COVID-19 reconciliation must happen every Wednesday, even if you do not intend to order.• Adjust out doses first, such as waste before completing a “COVID-19-Only Reconciliation”

• After reconciling on Wednesday, practices may place an order for direct delivery or as a transfer from the depot. Wednesdays are the cut off for all COVID-19 orders. Delivery will occur the following week. • Do not adjust doses into your inventory. Orders and Depot transfers will populate into your

inventory automatically by the end of day. If transferring out, enter into VIMS on the same day it occurs.

• If you receive vaccine transfers from your umbrella organization, you will not place an order in VIMS and should work through your organization.

Distributor or Depot Delivery?

• The Health Department preference is to utilize distributor deliveries as much as possible. • COVID-19 vaccine ships directly to you via FedEx or UPS, from McKesson (for Moderna and

Janssen) and Pfizer, just like routine vaccines. No set delivery date but tracking info will be available in VIMS.

• Ancillary supplies arrive in separate packaging within a day or two of the vaccine delivery. • Minimum order quantities apply.

• When clinic planning, order minimums, storage, or other concerns prevent a distributor delivery, Vermont utilizes its vaccine Depot. • The Depot delivers vaccine and ancillary supplies to sites needing less than the minimum order

quantities. • Because the Depot has limited resources, deliveries for the week following a Wednesday order are

batched together geographically. You will get verification of the delivery date by e-mail. • When vaccine is not in limited supply a three-week order is preferred to limit the number of

• VIMS will not allow both a Depot and Distributor order at the same time. • If all the products you order don’t meet the required minimums, submit everything as a Depot

order. During the approval process we will update all qualifying orders to Distributor.• This will be reflected on the e-mail you receive the Thursday following your order.

• Generally, please submit orders from the distributor if possible.• When ordering in multiples of the minimum quantities: Pfizer 12+: 1170 doses (195 vials) | Pfizer

• Thermal shipper contains dry ice. Have goggles and waterproof insulated gloves available (rubber garden gloves will be sufficient).

• Unpack container according to instructions that come with shipper.

• Place vaccine into ultra cold freezer or refrigerator only. • Pfizer 5 though 11 COVID-19 vaccine can not be stored at regular

freezer temperatures!• If placing vaccine in the refrigerator mark with beyond use date.

• Discard dry ice in well ventilated area.

• Return the temperature monitoring device using the temperature monitoring return kit included in your thermal shipper.

• Dispose of single use thermal shipping container.

Deliveries from the Vaccine Depot

• Vaccine will arrive at refrigerated temperatures.• Vaccine must be placed in refrigerator at time of delivery. • Vaccine will be labeled with beyond use date.• Ancillary supplies will arrive with the vaccine.

Pediatric Pfizer Storage and Handling

• Vials are labeled with manufacture date and not expiration date.

• Vials are good for up to 6 months after manufacture date in ultra cold freezer.

• Once refrigerated, vials can be stored for up to 10 weeks. The 10-week refrigerated beyond use date should be recorded on the carton at the time the vaccine is placed in the refrigerator.

• May be stored at 8°C to 25°C (46°F to 77°F) for a total of 12 hours prior to dilution.

• After dilution, the vial should be held between 2ºC to 25°C (35°F to 77°F). Vials should be discarded 12 hours after dilution.

Vial labels and cartons may indicate that a vial should be discarded 6 hours after the first puncture. The information in the Fact Sheet supersedes the number of hours printed on vial labels and cartons.

Expiration Date• Associated with a specific lot number and might be extended as new data is obtained.• Change is reflected in VIMS and communicated by the Immunization Program.• NOTE: The date reflected on the Pfizer pediatric vial is the Manufacture date. NOT the expiration.

Add 6 months to this date.

Beyond Use Date• When Pfizer vaccine is moved out of ultracold freezer storage and into refrigerated temperatures, it

has a Beyond Use Date that should be documented clearly.• Tracking and adherence to a Beyond Use Date is the practice's responsibility and will not

be reflected in VIMS.

Beyond Use Time• All COVID-19 vaccines have Beyond Use Times associated with dilution/vial puncture, after which

Administration - Overview

The Pfizer BioNTech Vaccine is administered IM as a primary series 2 doses (0.2mL each) 3 weeks apart in those 5 through 11 years of age.

• Invert vial gently 10 times to mix (do not shake)• May contain white to off white particles

• Dilute vaccine, by adding 1.3ml sterile 0.9% Sodium Chloride• ONLY use diluent that comes with vaccine, do not substitute

• Equalize pressure in vial by removing 1.3ml air• Invert vial 10 times to mix, (do not shake)• Record the date and time you punctured the vial for dilution

• Store at refrigerated temperatures• Discard any unused vaccine 12 hours after dilution

• Draw 0.2ml of vaccine for IM injection. Use the low dead volume 1ml syringes that come in your ancillary kit to draw the vaccine.

• Verify that the vial has an Orange cap and label border and states “Age 5y to<12y”

• Do NOT use vaccine intended for 12 years and older (Purple Cap) for individuals 5 through 11 years.

• Dilute vaccine only with the diluent provided (sterile 0.9% Sodium Chloride)

• After dilution with 1.3mL of diluent, the vial will contain 9-10 doses at 0.2mL.

• Do NOT add more than 1.3mL of diluent.

Regardless of the volume remaining in the diluent vial, it must be used for a ONE-TIME dilution. After 1.3mL withdrawal, the remaining diluent must be discarded.

Administration – Tips

• Do NOT pool vaccine from multiple vials to make a full dose.• Pedi Pfizer ancillary kits contain 1” needles. CDC is not sending any 5/8” needles in the

pediatric ancillary kits. See Administering Vaccines: Dose, Route, Site, and Needle Sizefor more information.

• COVID-19 vaccines may be administered without regard to timing of other vaccines.• This includes simultaneous administration of COVID-19 vaccine and other vaccines on the same day.• See CDC Best practices for more information on coadministration of vaccines.

• No additional doses or boosters have been approved for 5 though 11.• Give the Pfizer vaccines based on age at the time of administration.

• For those who age 11 to 12 mid series, either vaccine is allowed under the EUA.• CDC recommends giving them the dose based on their age at the time of vaccination. • Doing otherwise is not considered an error.

Planning to use Pediatric COVID-19

• Your office may plan to use the pediatric COVID-19 vaccine through:• Regularly scheduled appointments• A COVID-19 vaccine clinic for your patients• An offsite clinic for community members

• Planning larger clinics will be challenging in the first several weeks while the state is under allocation and must continue providing vaccine for community clinics and second dose needs.

• We recommend prioritizing your children and youth with special health needs (CYSHN) patients in the early administration of your pediatric vaccine.

• When vaccine is available to you, please consider coadministration with other pediatric vaccines and flu.

• Vials have 9-10 doses, must be used within 12 hours of puncture.• Waste guidance has not changed. Be thoughtful with planning, but do not turn anyone away.• 15 to 30-minute observation time post vaccination.

Grant Funding is Available

The American Academy of Pediatrics Vermont Chapter, in collaboration with the Vermont Department of Health’s Immunization Program, is providing funding to support expanded influenza and/or COVID vaccination by healthcare practitioners or organizations.

• Health Care Practices/Organizations may receive $1000 per clinic held (up to a maximum of $5000 total).

• Applications may include clinics held retrospectively starting September 1, 2021. • Applications will be reviewed on receipt, and funds will be distributed upon receipt of the

final project report. • Applications must be received by November 19, 2021. • Immunization clinics that are part of this grant must be held and reported on by March

31, 2022.

National Town Hall on Children’s Vaccine for Parents

Reed Tuckson and The Black Coalition Against COVID hosted a Town Hall event which featured doctors answering parent questions about COVID vaccine, and AIM’s own Michele Roberts discussing the rollout. Vermont is launching an initiative to increase sensory supports for children with disabilities and special health needs to ease their anxiety while getting their COVID-19 vaccine.

AAPVT Family Forums

In the Spring, when the EUA was approved for 12 and above, AAPVT held forums where parents could connect with local pediatricians to receive information and ask questions. AAPVT will hold these forums again for the EUA approval of 5 and above!

The goal is to have a meeting in each district with local pediatric clinicians. The AAPVT website has all currently scheduled sessions and will be updated as new sessions are confirmed. There is also a graphic for the conversations, which you can share with your patients.

Pfizer TrainingPfizer Vaccines US Medical Affairs continues to host daily Medical Updates & Immunization Site Training for All Providers with a Focus on Emergency Use Authorization of the Pfizer-BioNTech COVID-19 Vaccine for Children 5 through 11 Years of Age.
