1.0 PURPOSE

To gather reliable data through inspection, observation, and inquiry to identify opportunities for improvement. Also, items/activities, which exhibit exemplary compliance or an innovative approach to ES&H processes, should also be identified.

2.0 SCOPE

This procedure is applicable to Bechtel projects/facilities.

3.0 DEFINITIONS

3.1 CSM - Construction Site Manager

3.2 FM - Facility Manager

3.3 ES&H - Environmental, Safety, and Health

3.4 ES&HEP - Environmental, Safety, and Health Execution Plan

3.5 ES&HR - Environmental, Safety, and Health Representative

3.6 Supervision - Includes all personnel who have responsibility for controlling the actions of others

4.0 REFERENCES

5.0 RESPONSIBILITIES

When an activity is defined for a certain position in this procedure, a designee may perform the responsibility.

5.1 CSM/FM - The CSM/FM has the overall responsibility for the implementation of this procedure and is responsible for supporting it and for ensuring that all project/facility entities actively participate.

The CSM/FM is responsible for providing the personnel, facilities, and other resources necessary to effectively accomplish this procedure.

The CSM/FM and the management team will lead by example, modelling the behaviour expected from all employees performing work.

5.2 ES&H Representative - The ES&HR, in conjunction with the CSM/FM, is responsible for implementing and administering this procedure.

The general administration of the onsite assessment process is the responsibility of the ES&HR.

5.3 Supervisor - Supervision will be thoroughly familiar with this procedure and with their individual responsibilities regarding its implementation and enforcement.

6.0 REQUIREMENTS

6.1 Daily/Routine Assessments

6.1.1 Supervisors who direct the work activities of employees are responsible for assessing the work processes within their area for unsafe conditions, practices, and at risk behaviour.

6.1.2 The ES&HR will conduct on-going inspections of the work activities. Deficiencies will be reported to the responsible supervisor.

6.2 Monthly Formal Assessments

The ES&HR is responsible for co-ordinating monthly safety assessments. The format and participation of personnel will be planned by the ES&HR in conjunction with the Zero Injury Team. The Bechtel Self-Assessment Process will be utilised to assess and continually improve the requirements of the ES&HEP. Monthly assessments should involve a cross section of the work group and include the monitoring of subcontractor activities. From time to time, the Zero Injury Team members should participate. This type of assessment can be documented on a form similar to the one included as Exhibit A.

6.3 CSM/FM’s Assessments

6.3.1 The CSM/FM (or a designee during the CSM/FM’s absence) and the ES&HR will conduct an assessment of a safety system included in the ES&HEP on a monthly basis.

6.3.2 The ES&HR and CSM/FM will agree on which safety system will be assessed each month. The assessment format will be in accordance with the following guidelines:

· Select at least one safety system such as the Safety Task Analysis Risk Reduction Talk (STARRT)/Job Hazard Analysis (JHA) system, earth fault system, etc. and visit an area. While in the area, (1) observe the use of the system by employees, (2) physically check the equipment in use, and (3) contact employees regarding their knowledge of the system.

· Based upon the information obtained, develop corrective action items or plan an in-depth assessment to determine the root causes of barriers, problems, or deficiencies.

· Involve employees, supervisors, and others, as necessary, to further define root causes. Compile recommendations and effect improvements.

· Periodically involve other supervisors in this process.

6.4Corporate ES&H Assessments

6.4.1 Corporate ES&H assessments are conducted on a periodic basis to assess the application of ES&H systems, and to make recommendations for improvement.

6.4.2 Corporate assessments may encompass the entire ES&H process or be limited to a critical few. Assessments will be performed utilising the Bechtel Self-Assessment Process computer software. Key areas of assessment will be:

· Safe Work Observation Process

· Training and Education

· Responsibilities

· Hazard Communications Programme

· Record Keeping

· Accident Investigations

· Safety Task Analysis

· Injury and Illness Trends

· Process Safety Management (if applicable)

· Zero Injury Team

6.4.3 The Regional ES&H Services Manager or his/her designee is responsible for conducting at least one comprehensive assessment of the project/facility ES&HEP with a follow-up review each year.

6.5Assessment Instructions

6.5.1 Prior to the arrival of the ES&H Services personnel (approximately 2-3 weeks), an assessment announcement letter will be sent (Exhibit B). This letter will outline the parameters and specifics regarding the

corporate assessment, and request that the project/facility start preparation by conducting a self-assessment on the elements of their ES&HEP.

6.5.2 Typical protocol for the assessment will be as follows:

· Opening meeting/in briefing

· Records review

· Personnel interviews

· Physical conditions tour

· Close-out meeting/out briefing

6.5.3 All corporate assessments will be formally documented. The project/facility will be left with a graphical summary analysis report that identifies deficiencies and safe operations/ conditions. A clear plan of action must accompany each assessment that identifies all action items and responsible employee for the action

NOTE: The CSM/FM is responsible for ensuring that all action items are closed out in a timely manner.

Format for the assessment is outlined in the Environmental, Safety, and Health Manual. Prior to the assessment, you should start conducting a self-assessment, utilising the self-assessment tool (located in the Appendix of the BESH manual) to evaluate your current compliance with your own Environmental, Safety, and Health Plan. As a reminder, the purpose of conducting this assessment is not to find fault or criticise, rather to offer innovative ideas and recommendations for continuous improvement.
