Curb Ramp Compliance Post Construction Report instructions
1. The report must be filled out electronically. Hand written reports or scanned copies will not be accepted.
2. These reports are to be completed for curb ramps on the state trunk network – state highways and connecting highways. This is for any curb ramp installed or updated on the state trunk network which includes permit work that installs/updates curb ramps as well.
3. If a Local Program project intersects with the state trunk network then a report is to be completed for those curb ramps - information on the local road system is optional and should be kept with project records but do not need to be submited to the region bike/ped. coordinator.
4. When entering the Lat/long for a curb ramp location use decimal degrees and show to 6 decimal places (i.e. 99.999999). DO NOT use degrees-minutes-seconds. Correct Format =43.073962,-88.913757. Note: Longitude should be a negative number.
5. The report is to be filled out after a curb ramp(s) has been constructed.
6. Save the report using the following file naming convention: Project ID_XX (ex. 9999-99-99_01), when
the re are multiple reports for a project, report sheets submitted need to include the Project Id and number sequence (e.g. 01, 02, 03). DO NOT send multiple reports in one file. Correct Format = 3301-01-72_01.pdf and 3301-01-72_02.pdf.
7. Curb Ramp Location – Each curb ramp location field is numbered 1, 2, 3 or 4 and corresponds to a location on the intersection graphic provided. In the intersection graphic, place the corresponding number (1, 2, 3 or 4) in the correct location. Do not use double digits or letters.
8. Attributes – fill in all fields, do not leave them blank. If a measurement is asked for provide a numericvalue. Only provide N/A in fields with that option.
9. The Construction Project Manager is to verify the report is complete. Email the completed report to theRegion Bike/ Pedestrian Coordinator for the project.
