Dundee-Crown Girls Basketball

Feeder Tryout Information

When: Sunday Feb. 16th at Dundee-Crown High School in the

fieldhouse, enter at the athletic entrance

Time: 12-2 pm for Grades 5th – 8th

Cost: This will be announced after teams are selected

Other important information: Arrive 20 minutes early to fill out

a waiver and registration form, make sure parent/guardian is

present. Please make sure to wear basketball attire and bring

water.

Jr. Eagles Girls Basketball

2020 Jr. Eagles Girls Basketball Information

Mission: The goal of the Jr. Eagles Girls Basketball Feeder program is to prepare girls to play basketball at Jacobs High School. “Current” 5th- 8th graders who anticipate attending Jacobs HS will have an opportunity to meet girls from other schools and develop the core skills necessary to compete at the high school level.

When: March, April, May 2020 Practices: 5th/6th: 4:15 to 5:30 pm @ Dundee Middle School (Tuesdays and Thursdays) - - - - - - - - - - - - - - - - - - - - - - - - - - - - 7th/8th: 7:00 to 8:30 pm @ Westfield and/or Jacobs HS (Tuesdays and Thursdays). **Note: Starting April 7th, practices will move to Dundee Middle School for 7th/8th grade for the remainder of the season** Games: Saturdays during April and May @ Dundee Crown High School/Rakow Center/Randall Oaks Rec Center. Game times and dates will be announced at a later date.

RSVP: Please RSVP by Sunday, March 1st by contacting me via phone call, text, or email @ [email protected] or (773) 706 4347. In this RSVP email, text, or call, please include:

1. Your daughter’s first and last name 2. Current grade level 3. Grade School

Tryouts: Monday, March 2nd, 2020 @ Dundee Middle School. Please arrive 15 minutes early to register. Money will be collected after teams are announced at the first parent meeting. There is a potential for cuts at ALL levels. All players are expected to attend tryouts. Each player will be notified whether or not she made the team through email or phone call by Jonathon Reibel, no later than Friday, March 6th. Parent meeting will be held at the first practice and it will not take longer than 10 minutes. We will send out a confirmed date and time for the parent meeting, once we finalize the practice schedule.

Fee: $250, payable upon team selection; includes jersey and all games. (Information regarding payment will be discussed at tryouts)

Tryout Times: Monday, March 2nd @ Dundee Middle School 5th /6th grade: 5:30 - 7:00pm 7th /8th grade: 7:00 - 8:30pm NOTES:

✓ This is a competitive program. Playing time in games for all levels is dependent on ability, attitude and understanding of team offensive and defensive strategies and concepts.

✓ Every effort should be made to attend all practices and games. We understand that spring sports may interfere, but with adequate and timely communication, your coaches can be flexible.

2020 Golden Eagles Youth Spring Training Clinic

Join Head Coach Jamie Murray, the coaching staff, and current Jacobs High School players. Experience the passion for the game while

working together to build fundamentals.

When: Saturday March 14th from 2:00pm- 4:00pm (1:45pm check in)

Grades: 1st-8th (walk-ins are welcomed)

Cost: $25 includes a tee-shirt

Where: Jacobs High School - Competition Gym

What to bring: baseball pants, glove, a bat, helmet, catching gear, gym shoes, and water.

Areas that will be covered: Throwing/Catching, hitting, base-running,

pitching and infield/outfield.

WAIVER AND RELEASE OF ALL CLAIMS/CONSENT FORM

ACTIVITY: Spring Clinic (March 14th, 2020)

Please read this form carefully and be aware that in signing this form you will be waiving and

releasing all claims for injuries you or your child might sustain arising from your child’s participation

in the activity identified above (the “Activity”).

The Participant intends on participating in the Activity. The Parent/Guardian agrees to assume the full risk

of any injuries, including death, damages, or losses which the Participant may sustain as a result of

participation in any and all activities connected with or associated with the Activity.

The Parent/Guardian does hereby fully release and discharge the Board of Education of Community Unit

School District No. 300, including its Board members, employees, agents, and volunteers from any and all

claims from injuries, including death, damages, or losses which the Participant or Parent/Guardian may

have or which may accrue on account of participation in the Activity.

The Parent/Guardian further agrees to indemnify, hold harmless, and defend the Board of Education of

Community Unit School District No. 300, including its members, employees, agents, and volunteers from

any and all claims, liabilities, or costs (including attorneys’ fees) resulting from injuries, including death,

or damages to property, sustained by the Parent/Guardian or the Participant and arising out of, connected

with, or in any way associated with the Activity.

In the event of injury while participating in the Activity, Parent/Guardian grants permission to the School

District to seek emergency treatment for the Participant if the School District or Participant reasonably

believes that emergency treatment is necessary.

It is the express intent of the Parent/Guardian that this waiver and release shall bind the members of the

Participant’s and Parent’s/Guardian’s spouse, family, heirs, assigns, and personal representatives and shall

be deemed as a release, waiver, discharge, and covenant not to sue the Board of Education of Community

Unit School District No. 300.

If any term, covenant, condition, or provision of this waiver and release is held by a court of competent

jurisdiction to be invalid, void, or unenforceable, the remainder of the provisions shall remain in full force

and effect and shall in no way be affected, impaired, or invalidated.

I have read and fully understand the above waiver and release of all claims.
