    Introduction

    A. Background

    The Family systems approach views the family as a system composed of interrelated parts that

    work together to achieve a common goal. This implies that a change in a part of the system or each

    individual members of the family can bring about change to the whole family system and functioning.

    A family systems approach to common clinical situation can facilitate a new level of understanding of a

    patients problem which aids in leading to a better medical outcome.

    B. Statement of Objectives

    General Objective

    To be able to understand the structure and dynamics of a family through the use of assessment

    tools under the family systems approach, and apply it in the identification of the familys problems as

    well as the recommendations on how to address and deal with the identified problems.

    Specific Objectives

    1. To describe the family according to their structure and phase of their family life cycle

    2. To draw the familys genogram

    3. To describe the family lifeline

    4. To draw and describe the family mapping

    5. To describe the clinical biography of the family

    6. To identify the health, emotional, and socio-economic problems of the family

    7. To determine the presence/absence of family dysfunction by using the APGAR

    Methodology

    Instrumentation

    The report made use of the different tools in family assessment such as the construction of the

    family genogram, family mapping, and family lifeline, as well as the family APGAR and SCREEM.

    Data Collection Procedure

    The group was assigned to a family in Dasmarinas, Cavite to conduct an interview regarding the

    familys background . The interview was conducted and questions were asked according to the guidelines

    provided by the Department of Community Medicine.

    The APGAR tests were conducted through one-on-one interviews of the members of the family

    who were then present at the time of the interview.

    Data Analysis

    The information gathered during the interview were tabulated and analyzed by following the

    guidelines given for each assessment tool used.

    Family Lifeline

    The Estocado family was established in 1989 with the union of Irene and Jaime Estocado

    Jr, whose roots were from Bicol. There was no information given whether they had a civil or

    church wedding. They have been residing in Dasmarinas, Cavite for seven years already but

    before the family settled in Dasmarinas, Jaime lived in Bicol. After the couple got married in

    1898, Irene gave birth to their eldest daughter, Jhemaryn whos already a Business Management

    graduate of National College of Science and Technology. She gave birth to their first child on the

    same year they got married. The couple had another 4 children and Irene gave birth under the

    care of a midwife. She gave birth to her second child Evangeline on 1991 and was followed by

    the birth of her third daughter Shiela on 1993. She gave birth to another girl on 1997 and her

    names Zara Mae. Fi nally, she gave birth to her youngest and only son Andrew Spencer on 2000.

    Based on the family interview that was conducted, we found out that the family is

    nuclear. They all live together under one roof. Hence, no other relatives live with them. They

    also do not have any household helper and their home is composed only of the family members

    (7). Although there are instances that the grandmother visits them and stays with them for some

    time. There were no recorded deaths aside from the brothers and sisters of both of the parents.

    There were no recorded marriages as well because most of the children are still adolescents. The

    family is very supportive of each other. The mother has a close relationship with her children;

    they spend most of the time together at home. The older daughters are very industrious, they

    make sure that they support and help their family even in simplest ways. The eldest daughter

    whos already 21 has not built a family yet and is still supporting the family. The father on the

    other hand, though quite busy working was never aloof or distant to his family.

    Figure 3, shows the three generations of (surname) family. The first generation shows the

    both parents of Mr. Jaime and Irene (surname) (figures 1, 2 and 3). The (Jaimes surname)s

    second generation consists of eight (8) members as well with Jaime Jr. as the 4 th child of Mr.

    Jaime Sr. and Virginia (figure 1). (irenes maiden name)s second generation consists of eight (8)

    members with Irene as the 6 th child of Mr. Arturo and Gloria (figure 2). Furthermore, figure 3

    shows the third generation of (irenes maiden name and Jaimes surname) with 5 members and 2

    live births.

    Based on personal communication with the index person namely Mrs. Irene Estocado,

    there is a history of high blood pressure on her mothers side. Fortunately, Irene does notmanifest any clinical and/or biological symptoms of high blood pressure. Among the eight

    siblings, only Rosemary (Irenes sister) diagnosed to have high blood pressu re and diabetes

    (figure 1). The relative normal blood pressure of Irene may be attributed to her lifestyle and

    active participation to several community projects.

    Moreover, Ms. Irene also elaborated that Jaime Jr., aside from his vice of smoking and

    casual alcohol drinking, complains back muscle pains and insomnia. He drinks alcohol every

    night so as to soothe himself to sleep.

    For the third generation, Jaime and Irenes children 1 st child, Jhemaryn, had a history of

    diarrhea in 2002 for drinking contaminated water from the faucet. Evangeline (19 years old), on

    the other hand, had a drug allergy specifically (_________________nasa interview un eh sorry d

    ko jot down). Approximately 2 years after giving birth to Evangeline, Mrs. Irene Estocado

    became pregnant with her 3 rd child. On the 4 th month of her conception, she had a miscarriage

    and was told to be as a result of namatanda . On her supposed to be the 5 th and 6 th month of

    pregnancy, she still do not have her regular menstruation and her suprapubic area still gets bigger

    and had thought that maybe her child could have been twins. However, on her 7 th month of

    gestation, she again had a miscarriage. Shiela (17 years old) had a motor accident in 1998 and

    was brought to the hospital. Fortunately when Shiela was examined, no major wounds, injury or

    fracture incurred. Zara Mae (12 years old) is susceptible to acute diseases such as colds, cough

    with or without fever. Lastly, their only son, Andrew Spencer, appears to be the healthiest so far

    among his siblings.

    Figure 3 also shows that the first child, Jhemaryn, is closest and has strong bonds with

    her mother. While Andrew Spencer, the youngest and only son, is closest and has strong bonds

    with his father. Evangeline and Shiela are close to each other but both have conflicts with ZaraMae, who was described as moody and often misunderstood.

    Socioeconomic profile

    Four members of the family work and contribute to the total family income. Mr. Jaime

    Estocado Jr. works as a warehouse manager at E-mart and earns about Php 25,000.00 a month.

    Their eldest also works at this place, specifically the Human Resources and is currently earning

    Php 10,000.00 a month. Mrs. Irene Estocado sells ihaw-ihaw (barbecued chicken small

    intestines, porks clotted blood, and etc.) occasionally and usually earns approximately P 500.00.

    This can be considered as an additional income for the family. Their second child, on the other

    hand, just started as a trainee at KFC six days before the interview, and her monthly earning can

    not be established at the moment and will not be included in the total family income.

    The total family income of Estocado family is about Php 25,000.00 a month and

    designation of money for the basic necessities such as water, electricity, food and school fees are

    handled by Mrs. Irene.

    House and Environment

    Housing

    The Estocado family resides in a house situated in Dasmarinas, Cavite. It is a duplex up-and-

    down house owned by a relative of the husband. The family are staying on the first floor of a unit of the

    duplex for free. The materials used in the construction of the house are reinforced concrete, iron, and

    wood, which can withstand different weather conditions. Pieces of plywood and metal scraps were also

    used in the construction of a mini sari-sari store owned by the family, which is located right in front of

    their house.

    The house has two rooms, one where both husband and wife and their youngest child (Andrew)

    sleeps, and another for the other four children. It also has a common bathroom, a kitchen, and a living

    room.

    Appliances Owned

    The family owns appliances which they use mainly for cooking and food storage such as a gas

    stove and a refrigerator. They also own other electronic appliances such as a television unit, DVD player,

    a laptop, and electric fans for entertainment and comfort. All these are currently functioning well,

    except for the refrigerator.

    Water

    The drinking water used by the family is mineral water delivered to them in gallons once every

    week from Pala-Pala. They consume about four containers of mineral water per week and they also

    sell the water that is delivered to them to their neighbors. Tap water supplied by the Dasmarinas

    Water District is for other water usage.

    Garbage Disposal

    Garbage is collected periodically by garbage collectors and are disposed of by compost

    pit method.

    Excreta Disposal/ Bathroom

    Sanitation

    Animals Raised

    No farm animals are being raised by the family although they do have a pet dog that they play

    with every now and then. They feed the dog with their leftovers and they let it sleep inside their house

    every night. According to the children, their family is very fond of dogs especially their father. In fact,

    just a day after their previous pet dog died, their father brought home a new dog given by their fathers

    friend. Although the children say that the dog is maamo , it can pose a threat to them of getting bitten and

    acquiring rabies, since the family never had their dog brought to a veterinarian for necessary vaccinations.

    Accessibility to Community Facilities

    The location of their house is fairly accessible to community facilities. Their barangay is located

    across MERALCO and is just one or two kilometres away from health institutions such as DLS-UMC

    (De La Salle University Medical Center). It is also a kilometre away from the town proper and market,

    where the town church is located as well as food establishments. Private and public schools are also

    accessible to the familys children a s well as recreational areas for relaxation and family bonding such as

    SM Da smarinas, Robinsons Place Dasmarinas, and Walter Mart Dasmarinas.

    Their barangay hall is newly constructed with a health center and a basketball court where

    children can play.

    Personal Habit

    The father of the family consumes about two to three beer at night to fall asleep. It was

    also mentioned that he smokes about five times every night.

    Preferred Medicine

    Mrs. Irene plays the mommy doctor of the family. According to her she is able to

    discern whether her sick family member needs to seek greater care. It is only then, will she either

    bring him/her to the hospital or albularyo. Due to financial constraint, the family does not

    regularly visit a family physician. The family also utilizes herbal remedies such as ginger and

    oregano when needed. They occasionally buy vitamins but doesnt consider this as a priority.

    They sometimes dance inside the house for fun and consider this as their exercise.

    Their eldest gets free rice from work which this usually last about half a month. Mrs.

    Irene prepares food for her family often using a gas stove, and occasionally uses charcoal for

    heating foods.

    Food Preferences

    Mrs. Irene, who prepares food for her family believes that vegetables will keep her family

    healthy and thus, these are usually part of their daily meal. They also consume meat such aschicken, pork, beef and fish. She prepares breakfast, lunch, afternoon snack and dinner for her

    family every day.

    The green box represents the parents, Jamie and Irene while the yellow box represents

    their five children (from oldest to youngest) , Jhemelyn, Evangeline, Sheila, Zara and Andrew. In

    general, the family lives in harmony although conflicts, differences and preferences in this

    nuclear family of seven cannot be avoided.

    Although all children have good relationship with both parents, they are much closer to

    their mother since she is the one who is always at home and the one who is more expressive in

    terms of affection. The father fails to establish this kind of bond with his daughters since he

    leaves home early in the morning for work and comes home late in the evening. Meanwhile, he

    is more attentive and affectionate to his only son Andrew. Andrew being the youngest gets

    attention and affection from everyone. The second and third children, Evangeline and Sheila

    respectively, have a closer relationship with each other than to anyone else in the family. On the

    other hand. the eldest Jhemelyn is close to her mother. Jhemelyn also helps her mother Irene with

    the finances of the household. Zara, the youngest daughter doesnt get along that much with the

    rest of her siblings especially, Evangeline and Sheila. Zara is described as moody, sensitive,

    quarrelsome and always misunderstood.Nevertheless, the couple still talks about their problems and shares their thoughts and

    feelings regarding family matters.

    The Estocado family is not as different as is in a common filipino family regarding health

    issues. The family has a common side hypertension problem. Jaime Sr., on the father 's side and

    Lola Gloria on mother's aspect. Lolo Arturo, husband of Lola Gloria, is a casual alcohol drinker

    and smoker which can affect his health leading to more serious problem. Aside from HPN, the

    family on the mother's side rule out a (+) diabetes mellitus on Mario, Ms. Irene's brother. The

    problems regarding this issue can be passed on to the next generation and genetically can overlap

    within the family. We are not certain if Jaime Jr. and Irene is having this problems too.

    Present family lifestyle is not that good. Jaime Jr., father of four and husband of Irene,

    even though not yet ruled out having some health problems has an issue of casual alcohol

    drinking and smoking which can lead to future detrimental health problems. Evangeline have

    some drug allergy and Zara Mae is somewhat immunosuppresed. There are no disabled in the

    family and all are still well functioning as a human being. Common coughs and colds are

    problems especially for the children, a typical Filipino health concern. There are no majorcasualties regarding their health so far, even on both family sides.

    The Estocado family as a whole is not that health challenged the way they discussed and

    bring it to us. They can manage their lifestyle in relation to health. They do not have that main

    issue regarding illness and disease. Maintaining their mind and body physically fit will help them

    become safe and well.

    F. Psychodynamics

    Religion plays an important role in the morality of human person. Mrs. Irene and her

    family is a Roman Catholic. No one in the family is active in any religious organization but Mrs.

    Irene is P.R.O last December 2008 of KDBM ( Kababaihan ng Dasmarias Bagong

    Milenyo) in their place wherein the members of this organization is the one who is facilitating in

    the different charity works in the illiterate people of Dasmarias municipality. According to Mrs.

    Irene it does affect her relationship with her family because of this she is telling her family

    especially her children that they are still blessed because they can still eat 3 times a day

    compared to others.

    Regarding health, Mrs. Irene views a healthy person as maliksing kumilos at maganang

    kumain at hind sakitin and in order for her to keep her self healthy she eat nutritious food like

    vegetables, exercise through dancing and resting 4-5 hours a day. Mrs. Irene define a person who

    is sick a nd ill as may masamang pakiramdam at mahina and to her the cause of illness is the

    microorganism that can be found in the food. In some situation, when one of the family got sick

    they go to Albularyo and often times they just do self-prescription and used OTC drugs (over

    the counter). They dont consult the doctors for the reason that the fee cost a lot and healthcondition is not severe because oftentimes the member of the family only has fever. Mr.

    JAIME is smoking and drinks 2 bottle of beer before going to sleep. She seldom exercises

    because according to her she has a lot of thing to do. They relax their selves through watching

    television and simply chatting with each other.

    She took care of his husband and children by providing all what her family need. When

    she is struggling she first consults her first daughter regarding monetary problems. They have a

    good mother-children communication because her children are more open to her than with their

    father. There exists a rivalry between 3 rd child _ and _ . The 2 nd child is more close to

    Sheila. One thing that unite them is when they are watching tv. Each of the members of the

    family has their own roles and responsibilities.

    Based on Shiela Maes personal opinion upon answering the first part of the Family

    APGAR Questionnaire, and the scoring and evaluation done, we can conclude that the family is

    moderately dysfunctional.

    The family Estocado as a whole is a healthy family being that they are able to

    communicate well with each other. Of coarse they cannot avoid small conflicts most especially

    those between siblings but most often they really get along very well. It was also evident that the

    family is loving and is always concerned with the well being of one another. Not only does this

    family have a good relationship with each other but with their relatives and neighbors as well.

    With regards to the community though they are not always active they do participate sometimes

    and share good neighborly relations within the vicinity. As a whole the Estocado family has also

    been very accommodating to strangers.

    Cultural

    The father, Jaime, grew up in Bicol whereas the mother, Irene, in Cavite. Most of their

    lives as a family have been eventful in Cavite. Even though they were living in the modern

    times it was evident that they still believed in the old practices or teachings with regards to

    traditional healers. As per quoting Shiela, 3 rd of the 5 children, there was one incident that a

    sibling who had relentless fever was believed to be victimized by a spiritual element called

    nuno and was brought to the hospital then later on was brought to an albularyo as advised by

    someone who passed by them at the hospital.

    Religious

    Catholicism has been well practiced in the family. They try to make sure they come to

    mass every Sunday as a family and involve themselves in church activities.

    The father, Jaime, as well as the eldest child, Jhemarine, works at E-Mart situated at

    Dasmarias, Bayan. The 2 nd child, Vangeline, is currently a trainee at KFC in Walter Mart. The

    mother, Irene, is a full-time housewife who tends to the needs of the family and at the same time

    sells cheap grilled food when she feels like it.

    Education

    Majority of the household are college undergraduates. Shiela, the 3 rd child, graduated

    high school and plans to go to college and take HRM as a coarse. Vangeline, the 2 nd child, took

    up a vocational coarse at TESDA for a year. Zarra Mae, the 4th

    child, is currently in 1st

    yearhighschool. The youngest, Andrew Samson, is currently in the 4 th grade. Jhemarine, the eldest,

    was able to finish high school. The father, Jaime, and the mother, Irene were both able to

    graduate from high school.

    Medical

    The are no notable illnesses in the family and they rarely get hospitalized. Only the usual

    cough and colds, fever, etc. which are manageable with medicines bought from the pharmacy.

    Irene most often asks the pharmacist what medication she would recommend for such basic

    illnesses. Sometimes Irene, the mother, gives oregano, a herbal medicine, to her children. They

    also seldomly take multivitamins.
