EROSION CONTROL NOTES:
1. THE PROPOSED PROJECT REQUIRES A NATIONALPOLLUTANT DISCHARGE ELIMINATION SYSTEM PERMIT
(NPDES) FROM THE UNITED STATES ENVIRONMENTAL
PROTECTION AGENCY (EPA) A STORM WATER
POLLUTION PREVENTION PLAN (SWPPP) WILL BE
PREPARED AS A CONDITION OF THE NPDES PERMIT.
THE SITE CONTRACTOR IS RESPONSIBLE FOR FOLLOWING
THE CONDITIONS OF THE NPDES PERMIT AND SY/PPP.
2. THE SITE CONTRACTOR IS RESPONSIBLE FOR
ESTABLISHING AND MAINTAINING SUITABLE EROSION AND
SEDIMENTATION CONTROL DEVICES ON SITE DURING
CONSTRUCTION AS REQUIRED TO PREVENT SILT FROM
LEAVING THE SITE INCLUDING CONSTRUCTING
ENTRANCES AND PERIMETER SEDIMENT CONTROLS. SILT
WILL NOT BE ALLOWED BEYOND CONSTRUCTION LIMITS.
3. EROSION CONTROL SHALL, AT MINIMUM. BE INSTALLED
IN THE LOCATIONS SHOWN ON THE PLANS. IT IS THE
RESPONSIBILITY OF THE CONTRACTOR TO INSTALL
ADDITIONAL EROSION CONTROLS AS NECESSARY.
CONTRACTOR SHALL OBTAIN ALL NECESSARY PERMITS
FOR ALL GRADING AND OTHER LAND DISTURBING
ACTIVITIES PRIOR TO CONSTRUCTION.
5. THE CONTRACTOR IS RESPONSIBLE FOR THE CLEANUP
AND REMOVAL OF ANY BUILDUP OF SEDIMENT WHICH
ESCAPES FROM THE SITE.
6. CONTRACTOR IS RESPONSIBLE FOR CLEANING SILT AND
DEBRIS OUT OF ALL STORM DRAINAGE STRUCTURES
UPON THE COMPLETION OF CONSTRUCTION.
7. THE CONTRACTOR IS RESPONSIBLE FOR REMOVING ALL
TEMPORARY EROSION CONTROL MEASURES AFTER
CONSTRUCTION IS COMPLETE AND ALL DISTURBED
AREAS HAVE BEEN STABILIZED.
8. THE CONTRACTOR IS RESPONSIBLE FOR ALL COSTS
ASSOCIATED WITH ANY FINES LEVIED AGAINST THE SITE
FOR VIOLATIONS OF EROSION CONTROL REGULATIONS.
9. CONTRACTOR SHALL PROVIDE TEMPORARY GROUND
COVER FOR ALL AREAS 'MTH EXPOSED SOIL WHICH WILL
NOT BE DISTURBED BY GRADING OPERATIONS FOR A
PERIOD OF 14 DAYS OR MORE.
10. IF WORK ON THIS PROJECT IS SUSPENDED FOR ANY
REASON, THE CONTRACTOR SHALL MAINTAIN THE SOIL
EROSION AND SEDIMENTATION CONTROL FACILITIES IN
GOOD CONDITION DURING THE SUSPENSION OF WORK.
11. SPRINKLE OR APPLY DUST SUPPRESSORS TO KEEP
DUST WITHIN TOLERABLE LIMITS AT THE CONSTRUCTION
SITE.
12. FILTER FABRIC SHALL BE INSTALLED IN ALL CATCH
BASINS, WITH PERIODIC CLEANING AND/OR REPLACING
OF THE FILTER FABRIC, WHEN NECESSARY, THROUGHOUT
THE DURATION OF THE CONSTRUCTION.
13. CONTRACTOR RESPONSIBLE FOR PROTECTINGSTORMWATER MANAGEMENT AREAS FROM EROSION AND
SEDIMENT DURING CONSTRUCTION.
14. CONSTRUCTION ENTRANCES SHOULD BE INSPECTED
WEEKLY AND AFTER HEAVY RAINS OR HEAVY USE. THE
CONSTRUCTION ENTRANCE SHOULD BE TOPDRESSED
WITH NEW STONE ONCE THE ENTRANCE BECOMES
COMPLETELY CLOGGED WITH MUD AND SOIL PARTICLES.
15. SEDIMENT BEHIND PERIMETER SEDIMENT CONTROLS
MUST BE REMOVED ONCE SEDIMENT REACHES A LEVEL
OF DEPOSITION EQUAL TO HALF THE HEIGHT OF THE
BARRIER.
CONSTRUCTION NOTES:
IN GENERAL. THE PLANS ARE DIAGRAMMATIC AND ARE
NOT INTENDED TO SHOW EVERY FITTING, CHANGE IN
DIRECTION OR DETAIL OF CONSTRUCTION.
2. THE CONTRACTOR SHALL GIVE ALL NOTICES. COMPLY
WITH ALL LAWS ANO REGULATIONS, OBTAIN ALL PERMITS

PROTECTION OF PROPERTY AT THE SITE ADJACENT
THERETO INCLUDING TREES, SHRUBS, LAWNS.
PAVEmENTS, ROADWAYS. STRUCTURES AND
UNDERGROUND UTILITIES NOT DESIGNED FOR REMOVAL.
RELOCATION, OR REPLACEMENT.
5. THE CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR
THE MEANS. METHODS. TECHNIQUES. SEQUENCES. AND
PROCEDURES OF CONSTRUCTION REQUIRED FOR THE
INSTALLATION OF THIS WORK.
ALL MATERIALS SHALL BE NEW AND FREE FROM
DEFECTS AND OF GOOD QUALITY.
7. THE CONTRACTOR SHALL KEEP THE PREMISES FREE
FROM THE ACCUMULATION OF WASTE MATERIAL AND
OTHER DEBRIS RESULTING FROM THIS WORK.
ALL PAVEMENT MARKINGS, IF ANY, DISTURBED BY
CONSTRUCTION SHALL BE RESTORED AS PART OF THIS
WORK.
THE CONTRACTOR SHALL INSTITUTE ALL SAFETY
MEASURES NECESSARY TO PROTECT THE PUBLIC
SAFETY. THIS SHALL INCLUDE, BUT NOT LIMITED TO,
BARRICADES, SIGNS. LIGHTING, FENCES, POLICE DETAILS.
AND ANY OTHER MEANS AS DIRECTED. NO TRENCHES
ARE TO REMAIN OPEN OVERNIGHT.
10. THE CONTRACTOR SHALL NOTIFY DIG SAFE PRIOR TO
ANY EXCAVATION.
11. ALL METHODS AND MATERIALS SHALL CONFORM WITH
THE MASSACHUSETTS HIGHWAY DEPARTMENT
STANDARDS, TOWN OF SANDWICH D.P.W. STANDARDS,
AND TOWN OF SANDWICH PLANNING BOARD RULES ANO
REGULATIONS.
12. FINAL LAYOUT OF STOP SIGNS AND STREET SIGNS WILL
BE APPROVED BY THE SANDWICH DEPARTMENT OF
PUBLIC WORKS DURING CONSTRUCTION.
UTILITY NOTES:
PRIOR TO REcElviNG AUTHORIZATION TO CONSTRUCT.
ALL MATERIALS SHALL MEET THE STANDARDS AND
RECEIVE SHOP DRAWING APPROVAL. BASED ON
MANUFACTURERS DRAWINGS AND OTHER DATA.
ALL PROPOSED STRUCTURES. PIPES OR UTILITIES
CONNECTING TO EXISTING STRUCTURES, PIPES OR
UTILITIES SHALL BE VERIFIED FOR LOCATION AND
ELEVATION BY THE CONTRACTOR PRIOR TO
CONSTRUCTION. ANY DISCREPANCIES NOTED SHALL BE
REPORTED TO THE ENGINEER.
3. THE CONTRACTOR SHALL BE RESPONSIBLE FOR
MAINTAINING ADEQUATE RECORDS OF THE LOCATION
AND ELEVATION OF ALL WORK INSTALLED. THE
CONTRACTOR SHALL PROVIDE THIS INFORMATION TO
THE ENGINEER FOR USE IN PREPARING AS-BUILTS.
UNLESS OTHERWISE NOTED ON THE DRAWINGS OR IN
THE SPECIFICATIONS. ALL PROPOSED DRAINAGE PIPES
SHALL BE RCP PIPE.
5. ALL DRAIN MANHOLES SHALL BE 4, DIAMETER UNLESS
OTHERWISE NOTED.
6. UNLESS OTHERWISE NOTED, ALL EXISTING UTILITIES
THAT SERVE EXISTING USES/BUILDINGS, INCLUDING
BUT NOT LIMITED TO, DRAIN, TELEPHONE. CABLE AND
ELECTRIC SHALL BE PROTECTED. THESE UTILITIES
AND OTHERS THAT SERVE ADJACENT PROPERTIES
SHALL REMAIN IN SERVICE THROUGHOUT
CONSTRUCTION.
7. ELECTRIC, TELEPHONE, CABLE AND APPURTENANCES
SHALL BE INSTALLED IN ACCORDANCE WITH
REQUIREMENTS OF THE APPROPRIATE UTILITY
COMPANIES.
R. THIS PLAN HAS BEEN PREPARED AS A SCHEMATIC
REPRESENTATION OF THE PROPOSED ELECTRIC,
TELEPHONE AND WATER SYSTEMS. IT IS NOT THE
INTENTION OF THIS DRAWING THAT EVERY DETAIL,
DEVICE, ITEM OR PIECE OF EQUIPMENT BE SHOWN.
FINAL DESIGN AND APPROVAL MUST BE OBTAINED
FROM THE RESPECTIVE UTILITY COMPANIES FOR
LOCATION OF TRANSFORMERS, CONDUITS, AND

9. AT THE COMPLETION OF OPERATIONS, THE

ALL NEW AND EXISTING DRAINAGE STRUCTURES OF
SAND, SOIL OR OTHER DEBRIS.
10. INSTALLATION AND TESTING OF THE WATER MAINS,
SERVICE CONNECTIONS, HYDRANTS AND FITTINGS
SHALL CONFORM TO THE TOWN OF SANDWICH
REQUIREMENTS.
OPERATION AND MAINTENANCE
PLAN:
THE PROJECT INCLUDES AN OPERATION AND MAINTENANCE
PLAN REGARDING THE PROPOSED DEVELOPMENT TO MINIMIZE
IMPACTS TO ENVIRONMENTAL RESOURCES DURING AND
FOLLOWING CONSTRUCTION. THE PLAN INCORPORATES THE
USE OF SILT FENCE/HAYBALES WHERE NECESSARY AND
STORM DRAIN INLET PROTECTION DURING CONSTRUCTION.
DUE TO THE FACT THAT ALL OF THE PROPOSED
CONSTRUCTION TAKES PLACE OUTSIDE OF ONE-HUNDRED
FEET OF WETLANDS RESOURCES. LOCATIONS OF SILT FENCE
AND HAYBALES ARE NOT SHOWN ON THE PLANS. IT IS
THE RESPONSIBILITY OF THE CONTRACTOR. UNDER THE
SWPPP, TO INSTALL SILT FENCE/HAYBALES WHEN
NECESSARY. MORE SPECIFICALLY. THE FOLLOWING
ACTIVITIES SHOULD TAKE PLACE BOTH DURING AND
FOLLOWING CONSTRUCTION, DURING CONSTRUCTION, IT IS
THE RESPONSIBILITY OF THE CONTRACTOR TO MAINTAIN THE
PROJECTS DRAINAGE SYSTEM. FOLLOWING CONSTRUCTION,
IT WILL BECOME THE RESPONSIBILITY OF THE HOMEOWNER'S
ASSOCIATION TO MAINTAIN THE DRAINAGE SYSTEMS.
DURING CONSTRUCTION
1, THE CONTRACTOR SHALL INSPECT THE SITE A MINIMUM
OF ONCE A WEEK AND WITHIN 24 HOURS OF A
1/2-INCH OR GREATER RAIN STORM EVENT TO ENSURE
THAT EROSION OF SLOPES, STOCKPILES, ETC, IS NOT
OCCURRING. IN INSTANCES WHERE EROSION IS
OBSERVED, SILT FENCE/HAYBALES SHOULD BE
INSTALLED DOWN GRADIENT OF ERODING AREAS.
APPROPRIATE REPAIRS TO THE SEDIMENT CONTROLS
WILL BE MADE AS NECESSARY.
2. INSTALL FILTER FABRIC UNDER ALL CATCH BASIN RIMS.
FABRIC SHOULD BE INSPECTED ONCE A WEEK AND
WITHIN 24 HOURS OF A 1/2-INCH OR GREATER RAIN
STORM EVENT TO INSURE THAT DAMAGE TO FILTER
FABRIC HAS NOT OCCURRED. APPROPRIATE REPAIRS
TO THE FILTER FABRIC WILL BE MADE AS NECESSARY.
3. PROTECT BIO-RETENTION AREAS FROM EROSION AND
SEDIMENT DURING CONSTRUCTION, INCLUDING STONE
TRENCH. IN THE EVENT THE STONE TRENCH BECOMES
CLOGGED WITH SEDIMENT DURING CONSTRUCTION,
STONE AND FILTER FABRIC TO BE REPLACED.
CONSTRUCTION ENTRANCES SHOULD BE INSPECTED
WEEKLY AND AFTER HEAVY RAINS OR HEAVY USE. THE

2. THE VORTSENTRY WATER QUALITY UNIT SHOULD BE
INSPECTED FOUR TIMES A YEAR AND CLEANED TWICE
PER YEAR (AT A MINIMUM). CLEANING SHOULD OCCUR
IN THE EARLY SPRING FOLLOWING SNOW MELT.
3. THE VORTSENTRY WATER QUALITY UNIT SHOULD 8E
CLEANED IMMEDIATELY FOLLOWING ANY SPILL OF
HAZARDOUS MATERIALS.
CLEANING OF CATCH BASINS AND VORTSENTRY UNIT
SHOULD BE PERFORMED BY A VACUUM TRUCK AND
MATERIALS REMOVED AND DISPOSED OF INACCORDANCE WITH ALL LOCAL. STATE. AND FEDERAL
REGULATIONS.
5. THE INFILTRATION BASIN. INCLUDING THE RIP-RAP PAD
DISCHARGING TO THE BASIN. SHOULD BE INSPECTED
TWICE A YEAR. ROUTINE MAINTENANCE. INCLUDING
MOWING. SHOULD TAKE PLACE AT LEAST TWICE PER
YEAR AND AS NEEDED. DURING INSPECTIONS, THE
RIP-RAP PAD SHOULD BE INSPECTED FOR ANY
EROSION OR STABILIZATION ISSUES. TRASH AND
OTHER DEBRIS SHOULD BE REMOVED ON A REGULAR
BASIS.
ROADWAY MAINTENANCE PLAN:
STREET SWEEPING
THIS ROADWAY MAINTENANCE PLAN HAS BEEN DEVELOPED
AS A GUIDE FOR YEAR-ROUND MAINTENANCE OF THE
ROADWAYS IN THE PROPOSED SUBDIVISION.
STREET SWEEPING OF ROADWAYS IS THE RESPONSIBILITY OF
THE HOMEOWNER'S ASSOCIATION AND SHOULD BE
PERFORMED TWICE PER YEAR (IN THE SPRING AND FALL).
OR AS NEEDED, THERE ARE THREE TYPES OF SWEEPERS:
MECHANICAL: MECHANICAL SWEEPERS USE BROOMS
OR ROTARY BRUSHES TO SCOUR THE PAVEMENT.
ALTHOUGH MOST OF THE SWEEPERS CURRENTLY IN
USE IN MASSACHUSETTS. BUT IT SHOULD BE NOTED
THAT THEY ARE NOT EFFICIENT FOR PICKING UP
FINE PARTICLES (LESS THAN 100 MICRONS).
REGENERATIVE AIR: THESE SWEEPERS BLOW AIR
ONTO THE ROAD OR PARKING LOT SURFACE.
CAUSING FINES TO RISE WHERE THEY ARE
VACUUMED. REGENERATIVE AIR SWEEPERS MAY
BLOW FINES OFF THE VACUUMED PORTION OF THE
ROADWAY OR PARKING LOT. WHERE THEY
CONTAMINATE STORMWATER WHEN IT RAINS.
VACUUM FILTER: THESE SWEEPERS REMOVE FINES
ALONG ROADS. TWO GENERAL TYPES OF VACUUM
FILTER SWEEPERS ARE AVAILABLE - WET AND DRY.
THE DRY TYPE USES A BROOM IN COMBINATION
WITH THE VACUUM. THE WET TYPE USES WATER
FOR DUST SUPPRESSION. RESEARCH INDICATES
THAT VACUUM SWEEPERS ARE HIGHLY EFFECTIVE IN
REMOVING TOTAL SUSPENDED SOLIDS (TSS) IN
STORMWATER RUNOFF.
REGARDLESS OF THE TYPE CHOSEN, THE EFFICIENCY OF
STREET SWEEPING IS INCREASED WHEN SWEEPERS ARE
OPERATED IN TANDEM. ONCE THE SWEEPINGS HAVE BEEN
REMOVED FROM ROADWAY SURFACES, THEY SHALL BE
HANDLED AND DISPOSED OF PROPERLY IN ACCORDANCE
WITH ALL LOCAL, STATE. ANO FEDERAL REGULATIONS.
SNOW DISPOSAL, PLOWING AND PAVEMENT REPAIRS
PAVEMENT REPAIRS AND WINTER MAINTENANCE, INCLUDING
PLOWING OF EMERGENCY ACCESS ROADS IS THE
RESPONSIBILITY OF THE HOMEOWNER'S ASSOCIATION, UNTIL
SUCH ROADS ARE ACCEPTED BY THE TOWN.
THE PURPOSE OF THE SNOW AND SNOWMELT MANAGEMENT
PLAN IS TO PROVIDE GUIDELINES REGARDING SNOW
DISPOSAL SITE SELECTION, SITE PREPARATION, AND
MAINTENANCE THAT ARE ACCEPTABLE TO THE
MASSACHUSETTS DEPARTMENT OF ENVIRONMENTAL
PROTECTION (MossDEP) BUREAU OF RESOURCE PROTECTION.
FOR THE AREAS THAT REQUIRE SNOW REMOVAL, SNOW
STORAGE ONSITE WILL LARGELY BE ACCOMPLISHED BY
USING PERVIOUS UPLAND AREAS AWAY FROM ANY
WETLANDS AND RESOURCE AREAS. FOR SMALL. MORE
FREQUENT SNOWFALL EVENTS. THERE ARE ADEQUATE SNOW
STORAGE AREAS LOCATED OFF OF EDGES OF PAVED AREAS
OUTSIDE OF WETLAND RESOURCE AREAS. FOR LARGER
INFREQUENT SNOWFALL EVENTS, OR FOR ADDITIONAL SNOW
STORAGE SPACE, SNOW WILL BE REQUIRED TO BE HAULED
OFFSITE TO A SNOW STOCKPILE AREA MEETING MossDEP
REQUIREMENTS.
THE KEY TO SELECTING EFFECTIVE SNOW DISPOSAL SITES IS
TO LOCATED THEM ADJACENT TO OR ON PERVIOUS
SURFACES IN UPLAND AREAS AWAY FROM WATER
RESOURCES. AT THESE LOCATIONS, THE SNOW MELT WATER
CAN FILTER INTO THE SOIL. LEAVING BEHIND SAND AND
DEBRIS. WHICH CAN BE REMOVED IN THE SPRINGTIME. THE
FOLLOWING AREAS SHOULD BE AVOIDED:
AVOID DUMPING SNOW INTO ANY WATER BODY.
INCLUDING RIVERS. PONDS. AND WETLANDS. IN
ADDITION TO WATER QUALITY IMPACTS AND
FLOODING. SNOW DISPOSED OF IN OPEN WATER CAN
CAUSE NAVIGATIONAL HAZARDS WHEN IT FREEZES
INTO ICE BLOCKS.AVOID DISPOSING OF SNOW ON TOP OF STORM
DRAIN CATCH BASINS OR IN STORMATER DRAINAGE
SWALES OR DITCHES. SNOW COMBINED PATH SAND
AND DEBRIS MAY BLOCK A STORM DRAINAGE
SYSTEM, CAUSING LOCALIZED FLOODING. A HIGH
VOLUME OF SAND, SEDIMENT, AND LITTER RELEASED
FROM MELTING SNOW ALSO MAY BE QUICKLY
TRANSPORTED THROUGH THE SYSTEM INTO SURFACE
WATER.
WINTER ROAD SALT AND/OR SAM) IISF AND STORAGE
RESTRICTIONS.
ROAD SALTS AND SAND SHALL NOT BE STORED ONSITE.
ANY DEICING AGENTS PROPOSED TO BE USED MUSTCOMPLY WITH THE REQUIREMENTS SET FORTH BY THE CAPE.
