Delta Health Systems and the Delta Consumer Directed Health Plans System are pleased to offer online access to your Flexible Spending Account (FSA) plan information. Online access provides you with greater flexibility in managing your account. By registering for an online account, you will be able to:

• Review FSA account details;

• Obtain real time FSA account balances;

• Enter claims for reimbursement; and

• View the status of your submitted claims.

To start accessing your FSA benefit information online, you will need to follow the below steps.

Accessing Your FSA Detai ls After Logging Into www.deltahealthsystems.com

If Delta Health Systems administers your medical benefits, then you can login to the Delta Health Systems website at www.deltahealthsystems.com to view your FSA information. Simply login under the Member portal. Follow the instruction for either I Am A Returning User or I Am A New User, depending on whether: you previously registered and used the site, or are using the site for the first time.

After your have logged into www.deltahealthsystems.com, you will be directed to the Member Center section.

S T E P 1: In the Member Center under Health & Wellness Tools, click the Flexible Spending icon on the left side of the web page.

I M P O R TA N T : If Delta Health Systems is the claims administrator for your medical benefits, then you can login to the Delta Health Systems website at www.deltahealthsystems.com to view your FSA information. See the section in this Guide, Accessing Your FSA Details After Logging Into www.deltahealthsystems.com, for instruction.

If Delta Health Systems is not the claims administrator for your medical benefits, then you will be able to access your FSA information at www.hrbenefitsdirect.com/delta. See the section in this Guide, Accessing Your FSA Details Through www.hrbenefitsdirect.com/delta, for instruction.

S T E P 2: Under the Flexible Spending icon, click Click here.

S T E P 3: You will be directed to the home page for your employer’s FSA plan.

Please see the section in this Guide, Flexible Spending Account (FSA) Homepage, for instruction regarding how to navigate your FSA plan information.

Accessing Your FSA Detai ls Through www.hrbenefitsdirect .com/delta

If Delta Health Systems is not administering your medical plan benefits, then you will need to visit www.hrbenefitsdirect.com/delta to access your FSA information.

S T E P 1: Go to www.hrbenefitsdirect.com/delta.

S T E P 2: Login.

• Upon registration, your initial username and password will be the same –the first four letters of your last name

and the last four digits of your social security number.

• For example, if your name is John Q. Smith, and your social security number is 123-45-6789; then your initial

username and password will be “Smit6789”.

• After registration, you will be prompted to change your username and password.

S T E P 3: You will be directed to the home page for your employer’s FSA plan.

After you have logged into the associated website to access your FSA plan information, you will be directed to the below home page for your FSA plan.

Your company logo will be located in the top left corner of the web page and will replace the Delta Health Systems logo.

After you have reached your FSA home page, you will be able to access and view different Delta Consumer Directed Health Plans services.

You have several different options regarding how you may view your FSA benefit information. Through the Delta Consumer Directed Health Plans System, you can view:

• A list of your accounts;

• A summary of your accounts; and

• The status of your accounts.

View of your Flexible Spending Accounts (FSAs) Plan Balances

You will find the list of the FSAs in which you are currently enrolled on the right side of your employer FSA home page, under the Flex Enrollment section.

View the Status of your Flexible Spending Accounts (FSAs)

To view the status of your FSAs, such as the amount of the claims that have been reimbursed through your FSA, follow the below steps.

S T E P 1: On your employer FSA home page, under My Account, select the Status tab, then click the benefit status button.

S T E P 2: You will be able to view the details of your FSA election status.

The Delta Consumer Directed Health Plans System offers you the flexibility to create and view your claims online. You can:

• File a new claim for reimbursement from your FSA; or

• View your FSA claims history.

File a New Claim

To seek a claim reimbursement from your FSA, you will need to file a New Claim. To file a New Claim online, follow the below steps.

S T E P 1: On your employer FSA home page, select the Claims Tab, then click the Submit a Claim button.

S T E P 2: Complete the following fields for a new claim:

• Plan* - Select a plan from the pull-down list.

• Provider - Enter the provider’s name.

• Claimant Name - Depending upon who received the service(s), enter either your name, your spouse’s name or your dependant’s name.

• Description - Enter a short description of the care, prescription or services received.

• Service From Date - Select the beginning date of service.

• Service To Date - Select the ending date of service. For a purchase, an appointment or a visit on a single day, the “From” and “To” fields will be the same date.

• Requested Amount* - Enter the dollar amount of your claim (i.e. , the amount of reimbursable expenses).

• Receipt - You must submit a receipt with this submission.

• Notes - Enter any relevant information about this claim.

S T E P 3: Click Submit to enter your claim.

S T E P 4: A Claim Confirmation Receipt report will be displayed.

S T E P 5: If you have another claim to enter after the Claim Confirmation Receipt report is displayed, then click Enter a New Claim. Otherwise, your claim submission is complete, and you can navigate to another page.

View Your Claims History

To view the history of your FSA claims status, follow the below steps.

S T E P 1: On your employer FSA home page, under the Claims Tab click select Claims History.

S T E P 2: Your claims history will be displayed.

S T E P 3: If the claim you are looking for is not displayed, select the Member and Plan Year for the claim you want and click View

Y O U R P R O F I L E I N F O R M AT I O N

The Delta Consumer Directed Health Plans System allows you to view, update or change your FSA profile information. To make changes online to your profile, follow the below steps.

S T E P 1: On your employer FSA home page, select Profile tab, then click the View/Edit Profile button.

S T E P 2: Make corrections to the necessary information.

S T E P 3: Confirm that your email address is correct and always up to date.

S T E P 4: If your employer has provided Direct Deposit Services, then be sure to complete the Bank Account Information section.

• You may add or update your bank account information used for Direct Deposit at any time.

• If the Bank Account Information drop-down box is not shown, then Direct Deposit Services are not available.

Find Your Plan Documents

Some of the forms for your FSA benefits program may be available in the Resource Section. To open a document, click on

the Resource tab and choose the Plan Documents button.

Finding Repor ts

The Reports section allows you to view and print the summary of your FSA activity.

For example, you could run a report to view all of your claims for prescriptions, to view all of your claims for the calendar year, or to identify all of your claims for tax purposes.

You have two reporting options:

• Claim/Check Summary – The Claim/Check Summary lists all of the claims for the calendar year. This report can

identify a claim that is paid by check, automatic deposit or a debit card. This report will list every claim that is

processed regardless of the reimbursed method of payment.

• YTD Plan Summary – The YTD Plan Summary provides a summary of your total election amount, deposit amount,

claim amount, reimbursement amount, denied amount and plan balance.

To begin accessing your transactions through MyBenny.com, follow the below steps.

S T E P 1: Go to mybenny.com

S T E P 2: New users: select “Register”

• Member ID is the SSN.

• Complete remaining sections and return to the home page.

• Login with the email address and password

created during registration.

S T E P 3: Members have access to:

• Cardholder Activity: View “ALL” Activity and Activity Dispute Form
