Demystifying Lab Values

Parameters measuredAny one of a number of measurements of the serum concentration of a given compound; often

part of an automated examination that determines several speci�c parameters from a single sample

of serum.The test in a “nutshell”

Exactly what each “panel” covers is determined by the particular machine or laboratory used.

Normals may vary widely between labs and/or machines. Typical tests include:

Electrolytes (Na+, K+, Cl-, HCO3

-, Ca2+, PO4-, Mg2+) – the basic “building block” chemicals

of the body; alterations in the electrolytes can result in, as well as be the result of, numerous

disease processes. Blood sugar – important to measure in the diagnosis of diabetes, as well as in altered levels

of consciousness. Low blood sugar (hypoglycemia) may result in unusual neurological or

behavioral abnormalities. Kidney function (BUN, Cr) – measure the excretory function of the kidney.

Uric acid – elevated in gout. Commonly measured on multi-panel tests.

Laboratory tests are a part of just about all medical and surgical work-ups.It is imperative that all members of the healthcare team have a baseline understanding of these tests. A basic understanding of the different types of lab tests, their role in medical decision making, and an ability to recognize and deal promptly with abnormal values is an essential skill for all healthcare professionals.

The purpose of these worksheets is to allow you, the healthcare provider, to rapidly access need to know information regarding chemisty profiles in the easiest possible fashion. The chemistry profile worksheets will layout:

• � Parameters measured – a list of what tests are commonly obtained as part of any given “test.”• � Test in a “nutshell” – a terse discussion of what the test covers, including why it may be ordered.

• � Normal values – where available, typical normal values for laboratories in the United States and Canada have been summarized; remember, normals vary from hospital to hospital.• � Panic values – levels that most laboratory experts consider “potentially life-threatening.” The laboratory generally contacts the provider, hospital floor, etc. immediately when a value in this range is noted. Generally, if the recipient of this information is not the ordering provider (e.g., physician, physician assistant, nurse practitioner), the person who first receives the “message” from the lab should immediately contact the provider. (Note: there may be other less “life-threatening” results that need to be given to the primary provider as soon as possible; however, these vary from case to case. The best way to deal with this situation is for the primary care provider to include, with standard orders, a list stating something like: “Contact me when: . . .”).

• � Results in a “nutshell” – the major causes of abnormalities in the measured parameter(s). Most of these are summarized in tabular form. This makes it easier for you to rapidly find the information you need and apply it immediately in your practice.

Following the chemistry profile, are case studies specific to chemistry panel lab tests along with their answers to confirm the correct interpretatons.

Amy A. Lutteur, BSN, RN, VA-BC, CRNI, owns and teaches for Home Infusion Nurse®, an educational company that teaches nurses how to perform PICC/Port and PIV care and how to manage infusions. Additionally, she is an active member of a Level 1 trauma center vascular access team. She is board certified in vascular access, as well as in infusion nursing.
