There are four key components to ensuring that the risk of developing foot complications in
patients with diabetes is minimised. These are:
1. Patient education
2. The foot protection team
3. The multidisciplinary team
4. A foot care pathway

1. Patient education
•Zorg voor een correcte nagelverzorging: de nagels moeten recht worden geknipt, en de hoeken die tot voorbij de
nagelrand moeten komen, worden gevijld.
•Probeer eelt en eksterogen niet zelf te verzorgen. Vraag hulp aan een gespecialiseerd voetverzorger of een podoloog,
die op de hoogte is van je diabetes, of aan een dokter of diabetesverpleegkundige. Gebruik nooit bijtende zalven of
pleisters die voor dit doel worden verkocht.
•Verschoon iedere dag je sokken. Kies ze niet te klein of te groot, liefst zonder naden.
.
•Draag nieuwe schoenen niet langer dan één uur de eerste dag.
•Controleer de binnenkant van de schoenen op vreemde voorwerpen, plooien, barsten … alvorens ze aan te trekken.

Verminderde wondgenezing = 10-20% infecties eindigt op amputatie
Bevorderd door frequentere traumata met doorbreking van de huidbarrière
Leidend tot (vochtig) gangreen en weefselverlies
Temperatuur meten voet (infectie) Testen van de sensibele neuropathie

• 1/3 van patiënten met diabetes mellitus
• Vaak eerst distaal (asymptomatisch)
• Positieve (branderig, tintelen) en negatieve symptomen (sensibiliteitsverlies, voos gevoel)
• Als hypertensie, dyslipidemie, roken en obesitas: meer pijnklachten

Diabetes Voetconventie
De patiënten met diabetes met een voetwonde/Charcot (of een voetwonde hebben gehad
risicovoet), die een prezorgtraject hebben, of een overeenkomst voor een zorgtrajectdiabetes/diabetesconventie komen in aanmerking voor een aanvraag voor de diabetesvoetconventie
De patiënten met een diabetes voetwonde die door de diabetoloog, vasculaire arts oforthopedist, diabeteseducator of podoloog gezien worden dienen enkel het remgeld van 1 artste betalen
Consultaties podoloog / verpleegkundig consulent zijn onbeperkt gratis!
Enkel en alleen bij consultaties op een erkende diabetes voetkliniek!
Deze overeenkomst is 1 jaar geldig en kan onbeperkt jaarlijks verlengd worden indien nodig

Recommendations
•Perform a comprehensive foot evaluation at least annually to identify risk factors for ulcers and amputations. B
•All patients with diabetes shouldhave their feet inspectedat every visit. C
•Obtain a prior history of ulceration, amputation, Charcot foot, angioplasty or vascular surgery, cigarette smoking, retinopathy, and renal disease andassess current symptoms
of neuropathy (pain, burning, numbness) and vascular disease (leg fatigue, claudication). B
•The examination should include inspection of the skin, assessment of foot deformities, neurological assessment (10-g monofilament testing with at least one other
assessment: pinprick, temperature, vibration), and vascular assessment including pulses in the legs and feet. B•Patientswith symptoms of claudicationor decreasedor absent pedal pulses should be referred for ankle-brachial index and for further vascular assessment as appropriate. C
•Amultidisciplinary approach is recommended for individuals with foot ulcers and high-risk feet (e.g., dialysis patients and those with Charcot foot, prior ulcers, or amputation).

•Refer patients who smoke or who have histories of prior lower-extremity complications, loss of protective sensation, structural abnormalities, or
peripheral arterial disease to foot care specialists for ongoing preventive care and life-long surveillance. C
•Provide general preventive foot self-care education to all patients with diabetes. B
•The use of specialized therapeutic footwear is recommended for high-risk patients with diabetes including those with severe neuropathy, foot deformities, or history of
amputation. B

Risicofactoren
We found a 17% unplanned 30-day readmission rate in this prospective cohort of DFU patients enrolled in a
multidisciplinary diabetic foot service. Only current smoking and hypertension were independent predictors of readmission after risk
adjustment. These findings suggest that implementation of a smoking cessation program maybe beneficial to reduce unplanned
readmissions in DFU patients. They also highlight the complexity involved in achieving comprehensive DFU care and the
unpredictability of readmissions in this unique population of patients. (Courtenay, J Vasc surg in press)
