Introduction
The following directory had been updated to identify Specialist Palliative Care Services available for patients with end stage heart failure within hospital, hospice and community environments in the Merseyside and Cheshire region. It aims to provide relevant contact details, which will be reviewed and updated annually.
The directory was originally complied in September 2008 by and has been updated to include the Palliative and End of Life Integrated Clinical Networks (ICNs) co-ordinated by their chairs and the original membership group (appendix 1)
