BO433269, last known of Armbridge, Beaver County, was levied a civil penalty of $500 for operating on a lapsed or expired license. (09/19/18) Centre County Joshua Kinner, license no. CM014753L, of Pittsburgh, Allegheny County was levied a civil penalty of $1,000 based on practicing on a lapsed or expired license. (07/10/19) Bucks County Kimberly J. Harrelson, license, no. CO263373, of Newtown, Bucks County, was suspended for failing to pay a previously imposed civil penalty. (10/01/19) Dauphin County Donald L. Dearing, license no. CO279085, of Middletown, Dauphin County, was suspended pursuant to the Order of the Court of Common Pleas of Dauphin County dated October 14, 2019, which the court issued under section 4355 of the Domestic Relations Code. CO303474, of LaBelle, Fayette County, was granted permission to sit for the cosmetology licensure examination and, if licensure requirements are met, to have his license granted and immediately placed on probation for no less than three years. (09/19/19) Luzerne County Stephanie Almeida Forty, of West Hazleton, Luzerne County, is subject to a $1,000 civil penalty for aiding and abetting the unlicensed practice of cosmetology. (06/05/19) Mercer County Shakiya H. Easterling, license no. CO282679, of Sharon, Mercer County had her license suspended for two years based upon pleading guilty to two felonies. (01/02/19) Philadelphia County Laura Persons, of Philadelphia, Philadelphia County, will be granted a nail technician license on probation for no less than one year based on her criminal history, upon successful completion of the examination. (05/13/19) Jessica Schaefer, license no. CO237617L, of Philadelphia, Philadelphia County, was suspended pursuant to the Order of the Court of Common Pleas of Philadelphia County dated October 17, 2019, which the court issued under section 4355 of the Domestic Relations Code. CO269560, of Greensburg, Westmoreland County, had her license suspended for no less than 2 years with such suspension to be immediately stayed in favor of no less than 2 years of probation based on her criminal convictions. (12/26/18) Out of State Alysia A. Buck, PA license no. CQ117275, of Somerdale, New Jersey, was suspended for failing to pay a previously imposed civil penalty. (10/17/19) PA Board of Crane Operators Clearfield County Todd R. Rees of Morrisdale, Clearfield County had his application for crane operation granted and his license placed on probation for no less than three years based on not being able to operate a crane with reasonable skill and safety by reason of mental or physical illness or condition or physiological or

(10/24/19) PA Board of Dentistry Butler County Deron T. Kovac, DMD, license no. DS027813L, of Cranberry Township, Butler County, had his application for an anesthesia permit- restricted II denied. (07/23/19) Cambria County Meagan E. Alexander, license no. DH069678, of Portage, Cambria County, had her license placed on probation for one year based on her guilty plea to a felony. (09/24/19) Dauphin County Gregory John Arendacs, license no. DS022495L, of Hershey, Dauphin County, was indefinitely suspended, retroactive to May 13, 2016, for failing to comply with a previous agreement entered into with the Commonwealth. (09/25/19) Harold Joseph Harper, DMD, license no. DH008966L, of Millersburg, Dauphin County, had his license to practice as a dentist temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (12/17/19) Out of State Joshua A. Robbins, DDS, PA license no. DS038424, of Tomball, Texas, was indefinitely suspended for no less than five years based on disciplinary action taken by another state. (10/09/19) Miranda Whylly Smith, DDS, license no. DS030450L, of Spring Hill, Florida, had her license reprimanded and was ordered to pay a civil penalty of $2,000 because she had her license to practice dentistry disciplined by the state of Florida for improperly delegating professional responsibilities to unlicensed individuals and for administering anesthesia without a permit. FD011457L, of Portage, Cambria County, was levied a $300 civil penalty and assessed $966.18 in costs of investigation, because he failed to maintain a copy of the written agreement, statement of funeral goods and services, signed by the family representative or agency official, as well as the funeral director, for six years. FD012487L and FS012487L, of Scranton, Lackawanna County, was indefinitely suspended, retroactive to February 14, 2019, because he engaged in gross immorality and was convicted of a crime of moral turpitude. (12/04/19) Luzerne County Jeffrey A. Fierro, license nos. FD012061L and FS012061L of Mountain Top, Luzerne County, was actively suspended for a minimum period of three months, Andrew J. Piskel, license nos. FD0111391L and FS015392, Hazleton, Luzerne County, and Fierro Funeral Service, Inc., license no. FR000517L, Hazleton, Luzerne County, were levied a civil penalty in the amount of $2,000 and $976.44 towards the costs of investigation, because Mr. Fierro engaged in gross incompetency, negligence or misconduct in the carrying on the profession, and Mr. Piskel and Fierro Funeral Service, Inc. both failed to give a family member or agency official arranging for the funeral a written agreement form statement of funeral goods and services at the time of arrangements. (12/04/19) McKean County Sam Cummings Jr, license nos. FD013590L and FS013590L, of Kane, McKean County, was levied a civil penalty of $1,000; and costs of investigation of $500; because he failed to provide an itemized statement and total cost via a written agreement to a decedent's family prior to the decedent's cremation, and further failed to secure a signature from the decedent's family on the agreement. (12/04/19) Philadelphia County Vince N. Baker, F.D. and Baker Funeral Home, Ltd., license nos. FD013127L, FS013127L and FF000110L, of Philadelphia, Philadelphia County, had their licenses suspended for failing to pay a previously imposed civil penalty. (10/17/19) Price Funeral Home, Inc., license no. FR000921L, and John M. Price, Jr., FD, license nos. FD014657L and FS01657L, of Philadelphia, Philadelphia County, had their licenses suspended for failing to pay a previously imposed civil penalty. (01/24/19) PA Board of Landscape Architects No disciplinary actions taken. PA Board of Massage Therapy Luzerne County Baoquoc N. Nguyen, license no. MSG012812, of Dallas, Luzerne County was temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (12/13/19) Philadelphia County Serguei Vit Bogoutski, LMT, license no. MSG005612, of Philadelphia, Philadelphia County, had his massage therapist license revoked and was assessed a civil penalty of $4,000 for having engaged in sexual intimacies during the professional relationship, as well as sexual impropriety. MD067781L, last known of Biglerville, Adams County, was automatically suspended, retroactive to March 12, 2019, based on her felony under the Controlled Substance, Drug, Device and Cosmetic Act. (06/03/19) Berks County Sean Michael Carver, LAT, license no. RT005203, of Morgantown, Berks County, had his license to practice indefinitely suspended, retroactive to May 21, 2019, because he is unable to practice the profession with reasonable skill and safety by reason of impairment. (10/04/19) Bucks County Frederick Adolph Reichle, MD, license no. MD006646E, of Warrington, Bucks County, permanently voluntarily surrendered his license, because he cannot practice as a medical physician and surgeon with reasonable skill and safety by reason of illness, addiction to hallucinogenic, narcotic or other drugs, or alcohol, or mental incompetence. (12/17/19) Clearfield County Holly Jean Hipps, PA-C, license no. MA051098, last known of Olanta, Clearfield County, was automatically suspended based on her guilty pleas to felonies under the Controlled Substance, Drug, Device and Cosmetic Act. (10/10/19) Crawford County Luis L. Gomez, MD, license no. MD018911Y, of Meadville, Crawford County, permanently and voluntarily surrendered his license to practice as a medical physician and surgeon in the Commonwealth of Pennsylvania, because he violated a lawful order of the Board. (12/17/19) Delaware County Angelic L. Buchanan, LAT, license no. RT004074 of Borough, Delaware County was accessed a civil penalty of $500 based on having continued to practice her profession while her license or certificate was expired from January 5, 2015 through October 6, 2016. (06/14/19) Lackawanna County Paul Stephen Roche, LRT, license no. YM002330L, of Scranton, Lackawanna County, had his license indefinitely suspended retroactive to May 22, 2019 and was ordered to pay costs of investigation of $385 based on being unable to practice medicine with reasonable skill and safety to patients. (07/01/19) Luzerne County Robert E Detato, LRT, license no. YM001268L of West Pittston, Luzerne County, voluntarily surrendered his license, because he was unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, having been convicted of felonious act prohibited by the act of April 14, 1972, known as The Controlled Substance, Drug, Device and Cosmetic Act, or convicted of a felony relating to a controlled substance in a court of law of the United States or any other state, territory, possession or country, or if he or she is or shall become mentally incompetent. (12/17/19) Northampton County Joseph Vincent Episcopio, MD, license no. MD036932L, of Bethlehem, Northampton County, must permanently surrender his DEA Certificate of Registration, and complete 10 total hours of remedial education, because he failed to maintain full and complete medical record and failed to conform to the minimum standards of care when prescribing controlled substances. (12/17/19) Philadelphia County Timothy Fred Shawl, MD, license no. MD036539E, of Philadelphia, Philadelphia County, was suspended for 12 months, immediately stayed in favor of probation; levied costs of investigation in the amount of $6,320.82; and must complete 25 hours of remedial education, because his treatment of a patient fell below the accepted standard of care. (06/25/19) Pike County Fuhai Li, MD, license no. MD428963, of Milford, Pike County, was automatically suspended based on his felony conviction. (07/12/19) Union County Abby Melissa Irwin, MD, license no. MD453761, of Lewisburg, Union County, voluntarily surrendered her license to practice the profession, because she had a license or other authorization to practice the profession suspended, revoked, refused or otherwise disciplined by the proper licensing authority of another state (12/17/19) Westmoreland County Ralph Angelo Capone, MD, license no. MD022529E, of Greensburg, Westmoreland County, was automatically suspended based on his felony conviction. (01/31/19) Michel P. Toret, MD, license no. MD037134L, last known of Jeannette, Westmoreland County, was automatically suspended based on her guilty plea to a felony under the Controlled Substance, Drug, Device and Cosmetic Act. (10/25/19) Wyoming County Robert Adam Campbell, M.D., license number MD444125, of Hudson, Wyoming, had his license suspended with the suspension immediately stayed in favor of probation until at least May 1, 2023, because he is unsafe to practice with reasonable skill and safety. (07/25/19) Out of State David Mark Anderson, M.D., physician and surgeon license MD047517L, of Morgantown, West Virginia, had his license to practice as a physician and surgeon revoked because he was convicted of multiple felonies. MD017972E, of Wilmington (Greensville), Delaware, had his license to practice indefinitely suspended, retroactive to May 21, 2019, and was assessed the costs of investigation because he is unable to practice the profession with reasonable skill and safety by reason of impairment.
