    1. OBJECTIVES:

    Evaluate the performancelevel of secondary students.

    Determine the competencies

    they have mastered andleast mastered from first tofourth grading periods.

    2. The DSAT shall be given on

    the following dates:March 2, 2011

    First and Second Year

    March 3, 2011Third Year and Fourth year

    3. The test shall cover all the

    learning areas and shallbe administered to 40

    students per year level

    through randomsampling.

    6. FUNCTIONS AND RESPONSIBILITIES:

    6.1 Assistant Chief Examiners:

    - Assist CE in the distribution of test

    materials and test forms.

    - Monitor and supervise the administration

    of the test.

    - Consolidate, submit and validate the testresults of the school/district assigned

    to him/her.

    6.2 ROOM EXAMINERS:

    -Receive test questions from the

    principals.-Account sampled pupils/students given

    by the CE and check against the form 1 of

    the year level assigned.

    -Distribute test questions and answersheets prepared by the school.

    -Administer the test.

    -Prepare and submit list of

    examinees to the ACE.

    -Check answer sheets usingthe key to corrections.

    -Prepare test results andsubmit same to the ACE.

    7.ROOM ARRANGEMENT:

    There shall be 40 examinees

    in a room arranged into

    columns of 4s and rows of

    10s, 20 boys and 20 girls.

    8. REPORTS/FORMS TO BE SUBMITTED:

    A. DAT Form 1

    - to be prepared in 1 copy

    - to be submitted to ACE

    B. DAT Form 2

    - for school frequency to beprepared by the ACE and to be

    submitted to the CE.

    C.DAT Form 4- Indicate the name of examinees,

    grade/year and section.

    D. DAT Form 5

    - to be prepared by the ACE in 1copy

    9. SUBMISSION DATE:

    DAT FORM 1

    - to be submitted to the ACE on

    or before March 31, 2011 for thesecondary .

    DAT FORM 5

    - to be submitted by the ACE to

    the CE on April 1 to 16, 2011.

    REMINDERS:

    1. Test papers are to be given to theschools after the test.

    2. Replacement is NOT allowed in casethere shall be absent pupils in theafternoon.

    3. Test scheduled stipulated in the

    memorandum should be strictlyfollowed.

    4. In case of absent pupilsincluded in the sampling, the

    first choice for replacementshould be the pupil listedbefore the one sampled and thesecond choice, should be the

    one listed after the onesampled.

    5. Test results should betreated with confidentialityuntil recorded results besent to the field through amemorandum.
