    OBSERVATIONS

    It was observed that a welder was grinding the pipe-line without required PPE (face shield)

    It was observed that the waste skip/bin on the deck is generating bad odour.

    It was observed that welding operation was carried on without necessary fire controls.

    A personnel was seen smoking while working in the tunnel area

    A worker was seen standing under the elevated (massive) load on the deck area.

    It was observed some skips/waste bin are overfilled and not disposed/emptied.

    A worker was observed smoking at the stern area.

    It was observed that two personnel (from one of the supply boats) were seen fishing at the bow area of the vessel in an open area without life jacket.

    A welder was seen running a maintenance work on a hand-tool while it was operative; which could lead to cutting of finger(s)

    Basket was used to transfer two personnel (a person at a time), on and of the barge; which made the basket unbalance, which could lead to easy slip of the passenger from the basket

    It was observed that there are various marshy areas on the deck due to water that drips down from air conditioners; which can cause slip hazard and can also generate insect with time.A worker was transferred with basket in an unsafe way; it is mandatory the basket be balance during transfer to avoid slip/fall/drop of personnel.

    It was observed that housekeeping in the engine room was not at its best; presence of trash is obvious therein.

    Dangerous SIMOP was carried out; overhead lifting took place while some welders working on the deck.

    During transfer to and from the cargo barge, workers were seen standing and pooling the tagline without harnesses anchored to a safe place.

    It was observed that the colour coding of some lifting gears and some tools have not been completed; Today being the effective date of new colour coding.

    Various marshy areas on the deck due to water that drips down from air conditioners have not been attended to

    A Worker, operating automatic grinder on piplines did not clear away from the pipeline drifting towaords him during lifting operation.A worker was seen jumping from one level to another , even throught there is stairway which could have been used.

  • it was observed that riggers working with "NPCC YAS" were working at the unprotected edge without work-vest.

    it was observed that a rigger was standing under the pipeline and taking tagline from the pipeline being lifted.5 workers were seen working at the elevated level and the unprotected edge without work vest, the location is (lifeboat area) in between the bow and portside.

    WHAT DID WE ACTUALLY DO TO CORRECT IT?NPCC safety must ensure thorough induction of the personnel on-board the marine spread.The substandard attitude was immediately corrected and the supervisor was informed at the same time.The involved personnel was corrected and effect of his negligence was discussed.

    NPCC should ensure that there are at least 2/3 people on the basket while transferring, to maintain proper balance

    Pending the removal of the waste, insecticide and deodorant can be used and plastic sheet may be used to cover the skips.

    A tiny line/robe may be devised to the leaking point in the a/c and direct it straight into a bucket on the ground/surfaceEnsure that there are at least 2/3 people on the basket while transferring, to maintain proper balance. To strictly adhere to ADMA procedures for welding and equip welding foreman with hot-work checklist.Assign dedicated housekeeper for engine room for proper housekeeping from time to timeWhenever SIMOP would be done, safety should come first; the welders should have cleared and stopped the work for few moments, for lifting to be done.It is advised that riggers working at the edge of supply vessel (that is not guarded), should use safety harnesses anchored to a safe zone.Smoking is not allowed in non-designated area. All workers should be reinducted in this regard.It is highly recommended that the colour-coding should be completed ASAP.

    All personnel should stay clear off the lifting perimeter. And also the banksman should watchful of load movement.It is advised that housekeepers should be divided into groups and assign each group to different areas. A line/robe may be devised to the leaking point in the a/c and direct it straight into a bucket on the ground/surfaceSmoking is not allowed in non-designated area. All workers must follow company smoking policy.all fabricators/ welders must keep safe distance from moving/ shifting pipeline.it is advised that always use proper excess and exit routes and use stairways accordingly to avoid injuries.

  • All riggers must wear work vest during expose to edges of barge near sea during lifting/ shifting operation TO/FROM DLB 750 and CB barges.All riggers must use tagline sticks or approperiate devices to catch the taglines and avoid suspended load above.All riggers/workers must wear work vest during expose to edges of barge or working on extended or unprotected plateforms potentially falling hazard into sea.
