The System Change Group is made up of individuals from services across the city who work with clients with multiple
and complex needs. Members have a commitment to making change happen; creating solutions to blocks in
the systems that are experienced by these clients.

Creation of an active network of cross-sector Agents of Change by merging the System Change Group and Homeless Health Board.
Client informed lapse leaflet now available in dry houses.
Adult safeguarding: Improved internal procedures within drug & alcohol services (see case study below).
Improved access for dual diagnosis: key performance indicators and recommissioning expectations around joint working between drug & alcohol services and mental health services.
