Introduction
During the last years Telefónica I+D (the R&D branch of Telefónica, the major Spanish telecom operator) has been working in the development of an open platform for the delivery of new services for the user at home.
Telefónica I+D has developed a complete OSGi (Open Services Gateway initiative) platform to deploy and manage integrated services to the residential users market. This platform is based on the existence of a Residential Gateway (RG) at user’s home.

Introduction
The platform allows the provision of any kind of home related services such as: home control and automation, technical alarms management, e-learning, on-line gaming, home networking as well as tele-assistance and even tele-medicine services.
Particularly, some of these services are well applicable to segments of population (users) with special needs (such as handicapped or elderly people) to support them in overcoming the barriers they find in their daily life at home (tele-assistance – e-home care).

System Components
Monitoring Center:It hosts all the sw applications, databases to manage the service. It is also in charge of managing the user profiles, communicationsand all the security issues. Front desk to monitor and watch the monitored users.
Home:Residential Gateway (RG) o Pasarela Residencial: Element, remotely managed, is the interface between the public network access (ADSL) and the internal private networks (for home control, communications, entertainment, etc) where different terminals are connected . It also host some sw for the services.Set-top-Box (STB): Connection to the TV set, in order to use thisas user interface (for videoconferencing and video display). Alarms: Emergency panic button, flood detector, motion/presencedetector, glass-break detector, smoke and gas detectorsMedical devices: Devices to monitor vital parametersOther devices: Lighting, HVAC, gate electronic lockers etc.

The user is able to take a panic button he can press to set up an alarm in case he needs some assistance.
The remote care service proposed allow the detection of a lack of activity of the user lasting more than a configurable period of time that is considered to be abnormal for this particular user; thus detecting potential accidents or problems inside the care receiver home.
This functionality is supplemented with remote surveillance cameras which allows the remote supervisor to watch clearly the location of the person in case of emergency. Possible the sequential scan of the distinct cameras

DomoCare: ServicesVideoconferenceAn important service proposed is a simple videoconferencing system, based
on SIP, that uses the TV set to establish the videoconference session.
Medical examination could eventually be replaced by the videoconference interview.
The supervisor also is able to send messages to the TV Screen (for instance: for deaf aid)

DomoCare: ServicesAgenda and reminder: Handling of event or appointments
This component allows to handle and mange any kind of messages related to medical appointments, medicines taking etc . The registry can be introduced and modified by the supervisor at any time.The inputs are registered in a server database and the messages flow is timely managed by the application by sending SMSs, emails, messages in the screen of the TV set, sirens at home etc., It includes an acknowledgement mechanism. In case the reminder is not confirmed an alarm notification will be produced

Monitoring/capturing of medical parameters
The system offers the possibility of remotely taking measures from different medical devices, located at the user’s home. In this way, it is possible to monitor a person with health problems, controlling some medical parameters as glucose, heart beat, blood oxidation level etc.
A large variety of medical devices can be used, provided that a standard communication interface (serial, USB,…) to connect it to the Residential Gateway is available.

This option allows the execution of some home control actions (remote or local) over home elements such as lighting, air-conditioning, appliances, etc.

DomoCare: ServicesMultimedia Contents shown in the TV setThe care receiver will have access to contents specifically designed for him. These contents may include exercises, explanations, informational sessions about illness and diseases, therapeutical session and so on. The contents are TV like videos that will be displayed in the TV set through a decoder.

Conclusions
Telefónica has developed a Home Care system well applicable to segments of population with special needs (such as handicapped or elderly people).
It is flexible, and encompasses quite a lot of features to improve the remote care of people while at home from a Monotoring Center.
