EXECUTIVE SUMMARY 95

PIKA International, Inc. conducted a Time-Critical Removal Action (TCRA) on the Arroyo 96 Exiting Parcel 3 near Gate 209 of the Open Burn/Open Detonation Unit Kickout Area 97 Delineation from June 16 to August 7, 2008. The activities conducted under the TCRA 98 included surveying the arroyo and sidewalls with hand-held analog geophysical 99 equipment, sub-surface removal of all munitions debris (MD) and Munitions and 100 explosives of concern (MEC) within the arroyo, and surface clearance of the arroyo 101 sidewalls. 102

The work was performed on behalf of the U.S. Army Corps of Engineers (USACE), Fort 103 Worth District, under Contract No. W912BV-04-D-2016, Delivery Order DY02. All work 104 was accomplished in accordance with the Statement of Work, USACE Safety Manual 105 (EM 385-1-1, USACE 2003), 29 CFR1910.120, the Approved Site-Specific Work Plan 106 (MKM, May 2008) with its integral Accident Prevention Plan and Work Plan Addendum 107 (MKM, June 2008), Engineering Pamphlet (EP) 385-1-95a, Basic Concepts and 108 Considerations for Ordnance and Explosive Operations (USACE 2003), Data Item 109 Descriptions (DIDs), and other USACE guidance documents. 110

Exactly 84 grids were cleared and quality control checked, with over 15,000 digs 111 completed, resulting in the removal of over 1,200 pounds of MD and 57 MEC items. 112 Over 1,900 man-hours were recorded with no lost-time accidents or property damage. 113

1.0 INTRODUCTION 115

This site-specific final report was prepared by PIKA International, Inc. (PIKA) to 116 describe the activities conducted under the Time-Critical Removal Action (TCRA) 117 Addendum to the Final Kickout Investigation Work Plan, MKM Engineers, Inc. 118 (MKM), June 2008. The Work Plan (WP) Addendum guided execution of the field 119 work by delineating the extent of the open burn/open detonation (OB/OD) Unit 120 Kickout Area at Fort Wingate Depot Activity (FWDA), McKinley County, NM. The 121 WP Addendum was prepared to include the Munitions Response Site (MRS) 122 which is the Arroyo Exiting Parcel 3 near Gate 209 of the OB/OD Unit Kickout 123 Area Delineation. The WP Addendum (MKM, June 2008) describes the tasks 124 required at this location that is part of the FWDA OB/OD Unit Kickout Area 125 Delineation Munitions Response Area (MRA). Per the statement of work (SOW), 126 the WP was executed on the 3 miles of the Arroyo Exiting Parcel 3 near Gate 127 209. 128

The activities described under the Statement of Work (SOW) were required to 129 achieve compliance with the Resource Conservation and Recovery Act (RCRA) 130 Permit No. NM 6213820974, issued by the New Mexico Environmental 131 Department (NMED), for the FWDA facility, and the Programmatic Agreement 132 among the United States Army, the Navajo Nation, the Pueblo of Zuni, the New 133 Mexico State Historic Preservation Officer, and the Advisory Council for Historic 134 Preservation for Environmental Restoration Activities to be undertaken at FWDA 135 and Associated Project Lands. 136

1.1 Project Authorization 137

PIKA performed this project on behalf of the U.S. Army Corps of Engineers 138 (USACE), Fort Worth District (CESWF). Authorization for performance is 139 contained in Contract No. W912BV-04-D-2016, Delivery Order DY02, issued to 140 MKM Engineers, Inc. (MKM) by USACE, CESWF on May 30, 2006. The contract 141 was novated to PIKA in May 2008. 142

This work was accomplished in accordance with (IAW) the guidelines set forth in 143 the SOW and applicable Department of Defense (DoD) guidance for munitions 144 and explosives of concern (MEC) clearance operations. All work was 145




accomplished IAW the SOW, USACE Safety Manual (EM 385-1-1), 29 146 CFR1910.120, the approved site-specific WP (MKM, May 2008) with its integral 147 Accident Prevention Plan (APP) and WP Addendum (MKM, June 2008), 148 Engineering Pamphlet (EP) 385-1-95a, Basic Concepts and Considerations for 149 Ordnance and Explosive Operations, Data Item Descriptions (DIDs), and other 150 USACE guidance documents. 151

1.2 Objectives and Scope 152

The primary objective of this project was to perform a magnetometer, flag, and 153 dig removal operation on the Arroyo Exiting Parcel 3 near Gate 209 of the FWDA 154 OB/OD Unit Kickout Area Delineation, extending to the north for 3 miles, as 155 shown in Figure 1-1 in Appendix A. 156




2.0 TECHNICAL EFFORT 157

All work executed was accomplished in a manner that ensured the health and 158 safety of the workforce and the public at large. Munitions and MEC are a safety 159 hazard and may constitute an imminent and substantial endangerment to the 160 public, environment, and site personnel. Additionally, all work was conducted 161 under the guidance of PIKA’s subcontracted cultural resources personnel 162 following the guidelines in the SOW. 163

Using the mag, flag, and dig technique, a sub-surface removal was performed in 164 the Arroyo Exiting Parcel 3 near Gate 209 of the FWDA OB/OD Unit Kickout Area 165 Delineation and extending north for 3 miles. The mag, flag, and dig team 166 removed the arroyo bottom and cleared the surface of the sidewalls up to the 167 high water line by forming a line abreast and traversing the 3-mile length of the 168 arroyo, while maintaining a spacing of approximately 5 feet between sweep 169 personnel. The arroyo sidewalls were cleared of near-surface anomalies, and any 170 large or deep anomalies detected in the sidewalls of the arroyo were marked 171 with pin flags and addressed separately at the end of the mag, flag, and dig 172 removal action. The objective of the surface sweep was to locate exposed and 173 near-exposed MEC and MD that posed a hazard or obstruction to subsequent site 174 operations and use. 175

2.1 Equipment 176

Hand-held analog geophysical equipment was used to conduct the geophysical 177 investigation. Unexploded ordnance (UXO) personnel used the Schonstedt GA-178 52Cx Heliflux magnetometer (52 CX) to locate of ferrous anomalies and the 179 Whites Spectrum XLT (XLT) to locate non-ferrous anomalies. 180

The depth of detection for a given item depends on the size and orientation of 181 the target item, as well as the characteristics of the soil. Prior to the use of both 182 the 52 CX and the XLT at the FWDA facility, a test plot was established and PIKA 183 conducted equipment calibration and proveout at the test plot area. The goal of 184 this task was to demonstrate that the hand-held analog geophysical equipment 185 would provide the best available technology for the detection of large metal 186 material located on the surface and to the depth of detection. 187




2.2 Site Preparation 188

Limited tree and brush clearing was conducted by PIKA before geophysical 189 detection began. Whenever possible, the surveys were diverted around trees to 190 avoid tree or brush removal. For this investigation, the term “clearance” was 191 defined as removal of obstructing branches, limbs, stalks, leaves, or other above 192 ground growth; no plant roots or root zone soils were removed. The vegetative 193 clearance for this investigation included removing obstructing branches, limbs, or 194 stalks because they impeded equipment being utilized. All brush clearing was 195 done in coordination with the Zuni and Navajo cultural and archeological 196 subcontractors. 197

2.3 Site Removal Action 198

PIKA performed the grid surveys using hand-held analog geophysical equipment 199 to complete the removal of all detected anomalies in the 3-mile portion of the 200 arroyo. The floor of the arroyo was cleared of metal items discovered during the 201 removal action to the detection limit of the instruments described in Section 2.1 202 of this document. The arroyo sidewall surfaces were cleared from floor level to 203 the high water line using the same methodology. Because of safety 204 considerations, survey and removal in unstable sidewalls was limited. Hand-held 205 analog geophysical and visual inspections were used to verify the removal 206 actions from a minimum of 10% of the 3-mile arroyo to meet quality control 207 (QC) requirements. Initial and daily instrument tests were performed IAW MR-208 005-05, Geophysical Investigation Plan (USACE 2003). 209

To ensure positional control of the mag, flag, and dig operation, the site was 210 divided into flexible 200-foot by 200-foot grids (“U” shaped from one side wall, 211 across the bottom, and back up the opposite sidewall) and each corner marked 212 with a wood stake. The layout of each grid is illustrated in Figures 2-1 and 2-2 in 213 Appendix A. The stake was labeled using an alphanumeric system. The grids 214 were further divided using movable 0.25 inch by approximately 200-foot lengths 215 of line to divide the flexible 200-foot by 200-foot grid into 5-foot search lanes. 216 Hand-held detectors were used to process each search lane systematically and 217 mark anomaly locations. The grid sheets completed in the field are included as 218 Appendix B. 219




Items detected on or near the surface and the arroyo floor subsurface during the 220 investigations were marked with two crossed pin flags. The UXO Technician 221 locating the item contacted the UXO Technician III (Tech III) Team Leader for 222 confirmation of the identity of the item. Once the identity of the item was 223 established, the Tech III would determine whether the item was MEC; if 224 identified as MEC, the Tech III would determine whether it was unfuzed and 225 acceptable to move. If a suspected item was identified as MEC, a digital photo 226 was taken and the position was recorded by using the established grid sheets 227 and global positioning system (GPS). All information for the MEC item was 228 recorded on a MEC Tracking Log. The MEC Tracking Logs completed during the 229 investigation of the arroyo are included in Appendix C. Data associated with 230 MEC locations included: 231

• Item number assigned; 232

• Grid number where the item was found; 233

• Date; 234

• Item description; 235

• Location of item in coordinates; 236

• Whether the item was safe to move; and 237

• Where the item was stored. 238

All items were excavated by a two-person UXO team, then transported to and 239 stored in earth covered magazines (ECMs), as described in Section 2.4. None of 240 the discovered items were deemed unsafe for movement. 241

Each grid was determined to be cleared after all anomaly locations marked 242 during the grid search were investigated and cleared, and a minimum of 10% of 243 the grid area was re-investigated and no further anomalies were discovered. If a 244 MEC item was located during this 10% QC inspection, the grid was re-245 investigated and any anomalies removed. Once the removal was completed, the 246 QC inspection was repeated. 247




A final quality assurance (QA) inspection was conducted by the USACE Ordnance 248 and Explosives Safety Specialist (OESS). The QA inspections were recorded on 249 Form 948; completed forms are included in Appendix D. 250

Rainfall events caused flooding in several of the grids following the 10% QC 251 inspection, but before USACE QA inspection. These grids were 100% re-252 surveyed by the Senior UXO Supervisor (SUXOS) and the UXO Quality Control 253 Specialist (UXOQCS) to ensure that material was not washed into the grids 254 following the initial removal action. QA inspections were conducted following the 255 re-surveying effort. 256

2.4 Storage of Removed Items 257

All MEC removed from the investigation area was stored under the “Conditional 258 Exemption” (CE) provision in the Munitions Rule (MR) (62 FR 96621) 259 promulgated by the Environmental Protection Agency (EPA). The State of New 260 Mexico has adopted the MR and 40 CFR Part 266 (NMAC 20.4.1.700) that sets 261 forth two approaches for the storage of waste military munitions (WMM): CE 262 Storage and Permitted Storage. The ECMs met the requirements for CE storage 263 IAW the TCRA Explosive Safety Submission (ESS) for the Emergency Interim 264 Measure of Arroyo Exiting the OB/OD FWDA, McKinley County, NM (June 2008). 265

Eight (8) earth-covered magazines (“igloos”) within the explosive storage area B 266 were designated for CE storage of WMM. MEC was stored within igloo B-1038. 267 MD and scrap metal removed from the investigation area were also stored within 268 the CE igloos. MEC was segregated based on compatibility groups, as defined in 269 DOD Ammunition and Explosives Safety Standards, 6055.9-STD (DOD 2004), as 270 it was placed in the CE igloos. 271

2.5 Waste Management 272

Items removed from the arroyo were separated by type (MEC, MD, or scrap 273 metal) and placed within the CE igloos (see Section 2.4). MEC is planned to be 274 treated within the Corrective Action Management Unit (CAMU) at a later date. 275 The CAMU is included in a Class III permit modification that is currently being 276 reviewed by NMED. Following treatment of MEC within the CAMU, the MEC and 277 MD will be processed through a thermal convection system (TCS) to enhance the 278




recycle value of the metal. The TCS is a non-permitted activity. All of the metal 279 (MEC, MD, and scrap) will then be shipped off site to an approved metals 280 recycling facility. 281

2.6 Explosives Accountability 282

Explosives were received and stored within the CE igloos for the duration of the 283 project. Given that no items were required to be blown in place, the explosives 284 were sent to another project site at the end of field activities. Explosive 285 Accountability Records and shipment documentation are included in Appendix E. 286




3.0 SAFETY 287

The APP developed as part of the WP generated by MKM for the Kickout 288 Delineation (MKM, May 2008) was used for the arroyo investigation effort. 289

During the course of work, PIKA personnel were involved with activities that 290 exposed them to chemical, physical, and biological hazards, which PIKA 291 controlled through the use of engineering, administrative, and personal 292 protective equipment controls. Additionally, because the operations conducted 293 during this project were governed by the OSHA hazardous waste standard in 29 294 CFR 1910.120 and 1926.65, MKM developed a Site Safety and Health Plan 295 (SSHP) for this project. 296

During the SSHP development, MKM conducted an activity hazard analysis (AHA) 297 of the tasks listed below: 298

• Mobilization and site set-up; 299

• Location surveying and grid layout; 300

• Brush clearance; 301

• Geophysical surveying; 302

• Anomaly investigation; 303

• Quality control inspections; 304

• Demolition of MEC items; 305

• Inspection/handling of MD and non-MD scrap; and 306

• Site tear-down and demobilization. 307

The results of the AHA were expressed in the MKM Certification of Activity 308 Hazard Analysis (CAHA) forms presented in an attachment to the APP. 309 Additional information on the activity hazard assessment process and the hazards 310 associated with the tasks presented below were presented in Section 3.0 of the 311 FWDA SSHP. 312




As required by OSHA in 29 CFR 1910.120(i) and 1926.65(i), PIKA provided all 313 site personnel with hazard information training prior to their participation in site 314 operations. The content of this and all required site-specific training was outlined 315 in detail in Section 5.2 of the SSHP. 316

3.1 Emergency Response Training 317

All PIKA personnel potentially involved with responding to an on-site emergency 318 were briefed on their roles and responsibilities as part of the initial indoctrination 319 training. During this training, PIKA personnel were briefed on the procedures in 320 the emergency response and contingency plan presented in Section 15.0 of the 321 SSHP. 322

3.2 Supervisor/Employee Safety Meetings 323

PIKA conducted a Site and Task Hazard Briefing prior to the deployment of 324 personnel each day. This briefing was conducted by the SUXOS and augmented 325 by the UXOSO. During this briefing, all PIKA and subcontractor personnel were 326 briefed on the tasks to be conducted that day, the hazards associated with the 327 tasks, and the mitigation methods that would be employed by site personnel to 328 reduce or eliminate their risk of exposure. 329

3.3 Safety and Health Inspections 330

Daily inspections were conducted by both the SUXOS and the UXOSO to ensure 331 that site operations and personnel were complying with the APP, SSHP, and 332 other regulatory requirements. 333

3.4 Exposure Data 334

As required by MR-080, Monthly Status Report (USACE 2003), the MKM Project 335 Manager (PM) coordinated with the MKM Corporate Environmental Safety and 336 Health Manager (CESHM) on a monthly basis to ensure the acquisition and 337 presentation of exposure data. This included the number of man-hours 338 expended on the contract, any reportable accidents that occurred during the 339 month, and any reportable accidents that may have accumulated since the 340 project start. Exposure logs are part of the weekly reports (Appendix E) 341




submitted to the client. The exposure data collected for this project are as 342 follows: 343

• Man-Hours Recorded in Direct Support for Project - 1,910 344

4.0 QUALITY CONTROL 348

Under the oversight of the Unexploded Ordnance Quality Control Specialist, PIKA 349 implemented a three-phase inspection process with full documentation. The 350 following subsections outline these measures 351

4.1 UXO Quality Control Specialist (UXOQCS) 352

The UXOQCS was responsible for verifying that all project activities complied with 353 the approved plans and procedures. The following QC-related monitoring 354 responsibilities for the definable features of work for the project were carried out 355 by the project UXOQCS: 356

• Coordinated with the CESWF and OESS to ensure that QC objectives 357 appropriate to the project were set and all personnel were aware of these 358 objectives; 359

• Maintained a QC log to document details for field activities during QC 360 monitoring activities to serve as a memory aid in preparation of the Daily 361 QC Report; 362

• Coordinated with PIKA's Quality Assurance Manager (QAM) to ensure that 363 QC procedures were being followed and were appropriate for achieving 364 data validity sufficient to meet QC objectives; 365

• Conducted periodic QC surveillances of all site activities using a three-366 phase inspection process and recorded the findings in the Daily QC Report 367 for the Preparatory, Initial, and Follow-up QC Report; 368

• Reported noncompliance with QC criteria to PIKA’s QAM, PM, and SUXOS, 369 identified the root cause of the nonconformance, and documented these 370 nonconformances on the PIKA Nonconformance Report; 371

• Initiated a Rework Items List on nonconformance areas that had to be re-372 accomplished to meet quality specifications; 373

• Conducted QC meetings as required by the CESWF and recorded meeting 374 outcomes in the Daily QC Report; 375




• Coordinated with the responsible parties to initiate the proper corrective 376 actions to be taken in the event of a QC deviation and documenting these 377 actions on the Corrective Action Request (CAR); and 378

• Ensured that lessons learned were documented and forwarded to the 379 PIKA QAM for analysis. 380

4.2 Three-Phase Inspection Process 381

The UXOQCS ensured that a three-phase control process was implemented for 382 each definable feature of work. Each control phase was important for obtaining a 383 quality product; however, the preparatory and initial inspections were particularly 384 invaluable in preventing problems. Production work was not performed on a 385 definable feature of work until a successful preparatory and initial phase 386 inspection had been completed. 387

4.2.1 Preparatory Phase Inspection 388

A preparatory phase inspection was performed before beginning each definable 389 feature of work. The purpose of this inspection was to review applicable 390 specifications and verify that the necessary resources, conditions, and controls 391 were in place and compliant before the start of work activities. 392

The UXOQCS verified with the client that all pre-requisite submittals had been 393 delivered and approved. The UXOQCS met with the SUXOS and the staff 394 responsible for the definable features of work. He then generated and used a 395 Preparatory Phase Inspection Checklist customized to fit the specific feature of 396 work and site conditions. 397

4.2.2 Initial Phase Inspection 398

An initial phase inspection was performed the first time a definable feature of 399 work was performed. The purpose of the inspection was to: 400

• Check the preliminary work for compliance with established procedures 401 and contract specifications; 402




• Verify inspection and testing and establish the acceptable level of 403 workmanship; 404

• Check safety compliance and review minutes of the preparatory phase 405 meeting; and 406

• Check for omissions and resolve differences of interpretation. 407

The UXOQCS was responsible for ensuring that all discrepancies between site 408 practices and approved specifications were identified and resolved. The UXOQCS 409 also monitored the MEC operations and any response actions conducted at the 410 worksite. 411

4.2.3 Follow-up Phase Inspection 412

A follow-up phase inspection was performed each day a definable feature of 413 project work was performed. The purpose of the inspection was to ensure 414 continuous compliance and a high level of workmanship. The UXOQCS was 415 responsible for on-site monitoring of the practices and operations taking place 416 and verifying continued compliance with the specifications and requirements of 417 the contract and approved project plans and procedures. The UXOQCS was also 418 responsible for verifying that a daily health and safety inspection was performed 419 and documented as prescribed in the SSHP. 420

4.3 Documentation and Document Control 421

Documents for this project were prepared, reviewed, approved, and issued IAW 422 the PIKA Corporate Quality Control Plan. A master project file was kept for the 423 project that included plans; drawings; specifications; calculations; procedures; 424 daily, weekly, and monthly progress reports; field reports; audits; and telephone 425 records. 426

Daily SUXOS reports are included as Appendix F. Weekly reports, including 427 photo documentation, are included in Appendix G. 428




4.4 Data Management and Data Quality Objectives 429

All data was managed IAW the PIKA Corporate Quality Control Program. Hard 430 copies and electronic copies of the data were maintained at the field office. 431

Data quality objectives (DQOs) were based on the concept that different data 432 uses require different quality metrics. Data quality was defined as the degree of 433 certainty with respect to precision, accuracy, representativeness, completeness, 434 and comparability required of a data set. 435

DQOs are both qualitative and quantitative statements specifying the data 436 standards required to support the project. The DQOs developed during project 437 planning served as standards against which project objectives were measured. 438 This approach helped to ensure that data of sufficient type, quality, and quantity 439 were collected to document and facilitate appropriate response actions that 440 addressed safety, public health, and the environment. 441

The DQOs for the Schonstedt GA-52 CX and Whites Spectrum XLT included the 442 following: 443

• Prior to use, all analog geophysical equipment will be checked and 444 calibrated against a known ferrous and non-ferrous anomalies. These 445 instruments will be checked using the established geophysical test plot. 446 The purpose of this test/calibration is to ensure that the instruments are 447 operating properly and to appropriately adjust the sensitivity level of the 448 instruments. 449

• Analog geophysical equipment items will be checked against the known 450 source to ensure they are operational and capable of detecting ferrous 451 and non-ferrous objects. Each day the operator will perform this test 452 before placing the instrument into operation. Detectors that fail to detect 453 the simulators or metallic sources in the field will be considered to have 454 failed QC and will be removed from service until repaired. 455

4.5 Equipment Calibration and Testing 456

All instrumentation used on site was response-checked IAW the manufacturer's 457 specifications before and after use each day. If an instrument failed to respond 458




correctly, PIKA removed the device from service until it was repaired IAW 459 manufacturer's specifications. 460

4.5.1 Communication Equipment 461

Before commencing operations each morning, radios and cellular phones were 462 checked. Radios were function checked to ensure batteries were charged and the 463 radios were operational. Cellular phones were checked to ensure they were 464 operational. If communications were lost, either between teams and the 465 command post or off-site to emergency services, work ceased until 466 communications were restored. 467

4.5.2 Analog Equipment 468

Prior to use, all hand-held analog geophysical instruments were checked and 469 calibrated against known ferrous and non-ferrous anomalies. These instruments 470 were checked using the established geophysical test plot. The purpose of this 471 test/calibration was to ensure that the instruments were operating properly and 472 to appropriately adjust the sensitivity level of the instruments. 473

The detectors were checked against the known source to ensure they were 474 operational and capable of detecting ferrous and non-ferrous objects. Each day 475 the operator performed this test before placing the instrument into operation. 476 Detectors that failed to detect the ferrous and non-ferrous sources in the field 477 were considered to have failed QC and were removed from service until repaired. 478

Personnel Test 479

Personnel tests were conducted to ensure that survey personnel had removed all 480 potential interference sources from their bodies. Common interference sources, 481 suck as ballpoint pens, steel-toed boots, or large metallic belt buckles, which 482 could produce data anomalies similar to MEC targets, were removed. 483

Cable Shake Test 484

The purpose of this test was to identify and replace shorting cables and broken 485 pin-outs on connectors. The cable shake test consisted of running the 486




instrument in static mode and shaking all cables while monitoring instrument 487 response. If a short was found, the cable was marked and replaced. 488

4.5.3 Global Positioning System 489

Each unit was checked for adequate battery voltage, correct configuration, and 490 receipt of sufficient data to compute three-dimensional positions. Hand-held GPS 491 units must provide ±10-foot accuracy. Hand-held GPSs that did not meet this 492 standard were considered to have failed QC and were removed from service until 493 they were repaired to meet the standard. 494

All equipment used at the OB/OD Unit Kickout Area Delineation was dedicated 495 solely to the project until the project was completed or until it was no longer 496 needed. If equipment field checks indicated that any piece of equipment was 497 not operating correctly and field repair could not be made, the equipment was 498 tagged and removed from service. 499

4.6 Audits 500

A UXOQCS was on site full time to conduct daily audits of MEC activities and 501 documentation. 502

Audits were performed by the QC specialists who provided information on 503 multiple project tasks, identification of items not initially in the project WP, 504 lessons learned, and/or practices to consider as the project moved forward. 505

As part of the QC, PIKA conducted audits and surveillance at the site to ensure 506 that all procedures and protocols were being followed and that the resulting data 507 were accurate and defensible. Field audits concentrated on both surface and 508 sub-surface sweep procedures, mapping operations, proper documentation, and 509 checks of resulting data for completeness and accuracy within established QC 510 limits. 511

4.7 Quality Control Sampling Inspection 512

Daily audits were performed by the UXOQCS to ensure that all MEC operations 513 and explosive management activities were in total compliance with the WP. All 514




MEC audit and surveillance activities were performed and documented using the 515 Daily QC Reports developed by PIKA. 516

Validation of work procedures was accomplished by conducting QC sampling of a 517 minimum of 10% of the work accomplished. One MEC or MD item located in the 518 grid would have been cause for a QC failure. QC failure was also defined as 519 workmanship that did not comply with basic safety concepts or other industry 520 safety practices. No QC failures were identified during this project. 521




5.0 SUMMARY OF FIELD OPERATIONS 522

PIKA mobilized teams at the site on June 16, 2008. The personnel finalized logistics 523 and met with key personnel in charge of range operations to coordinate field work, 524 enabling the field teams to commence field operations on June 17. Exactly 84 grids 525 were cleared, with PIKA QC evaluation performed prior to turning the grids over to 526 Government for QA. All grids passed Government QA, documented on Form 948s 527 included in Appendix D. Direct support of field operations ended on August 7, 2008. 528

Continued grid staking and layout.

Escorted Zuni and Navajo representatives on West banks of arroyo to map and investigate newly discovered archeological sites.

Received demolition materials and stored in igloos.

Conducted MEC clearance in arroyo.

Ordered supplies for operations.


Per the client, we are not to dig into any of the sidewalls with steep overhangs that could collapse on our workers.

Advised to remain clear of all ant hills

All grids have not been established. The northing and Easting of the last grid was established this week with USACE/Doc Holiday. Will work on final grid numbers next week.

Received Boosters and Detonating cord from WESCO and stored in igloo.

Conducted QC inspections on 7 grids and all passed.


All grids have not been established. Will work on final grid numbers with PM/Cory Woods next week and finalize numbers when we receive the computer from Stafford.

Until we establish actual number of grids, we can not provide accurate completion percentages.

USACOE Safety/Tom Knapp suggested we address the flooding issue with Contracting Officer. We had a flood and grid stakes were washed away Monday night. The major concern is conducting QC/QA inspections after a flood and finding MEC that was washed into completed areas. Addressed this issue with PM/Cory Woods.
