A Diabetes Self Management Educaon (DSME) Course is coming to Mount

Roskill in February 2016. You can refer paents with diabetes to the course.

Friends/Whanau/family are welcome to aend as well.

You can refer a paent by compleng the DSME referral form and faxing it to

Auckland PHO on 379 4024 or emailing the referral in your outbox documents

to Wayde@aucklandpho.co.nz. If you have any quesons or queries about
