To work for Jämtland Härjedalen and following all the practical and concrete examples of new solutions that are being tested and put into action here has been tremendously rewarding. It is incredibly positive that so many leading IT companies have opted to work together in the region to find the right solutions to complex problems. There is no doubt that we are at the forefront in E-health; it gets confirmed time and time again when we meet colleagues across Sweden.

We now hope that more people want to join us on this journey of discovery - for with more smart brains and perspectives we will become even more creative.

Östersund is home to the only e-health hub of its kind in Scandinavia. A hub where public and private organisations really cooperate. Where sometimes it can take only six months from ideation to testing in real life. On a practical level, we have 800m2 of office space and 30 offices to rent. This is where recurring inspirational seminars are held and where the proximity to decision-makers is tangible.

And yet, the recipe for success is simple: we sit together in the same place, take breaks together and exchange difference experiences. Heathcare’s challenges end up directly with developers from the private sector. Thoughts flow freely and ideas become solutions. And yes, it becomes good business. For both large and small companies alike.

It is no coincidence that the E-health hub was created and is being run successfully right here in Östersund, Jämtland Härjedalen. The circumstances of the region provide the optimal combination of challenges and enterprise.

• This is Sweden’s most sparsely populated county, with a surface area the size of Holland and Belgium put together.

• It has Sweden’s largest demographic challenge with a large proportion of older people.

• Tough financial situation; just like many other county councils and regions, Region Jämtland Härjedalen is in the red.

• Short decision-making cycles and good cooperation between the public and private sectors.

We have to make changes in Jämtland Härjedalen, and we have to make them now. That is why progress in e-health is happening quicker here than anywhere. Large and small IT companies alike have realised it, the EU and World Health Organisation (WHO) have understood it, and that is making other public actors take notice of what we are doing.

The E-health hub is open to everybody who is interested and involved in e-health, both small organisations with cutting-edge expertise and larger, multinational companies with a breadth of skills. But it is also open to new and curious entrepreneurs.

Microsoft has chosen to have a place with us, to be part of the progress being made here. Likewise, the patient record system company Cambio, which is a listed company with its head-office in Stockholm, has chosen to be part of the E-health hub. Phoenix Idea, Noblu, Telia and ImagineCare are a few more of our active partners.

Several other companies that have offices in Östersund, such as Tieto and Atea, have chosen to rent a few office spaces with us to be part of the network. And another of our sector colleagues, Sigma, has all of its Östersund office space located in the E-health hub. National e-health development departments and the start-up company We&Sports contribute greatly to our network. Both Region Jämtland Härjedalen and the municipality of Östersund continuously use the workspaces they have here. The E-health hub of Jämtland county has become a magnet that attracts a breadth of competence within the IT and e-health fields.

Östersund has a cluster of IT companies that both compete and collaborate and that are currently growing and employing. There is solid knowledge within a wide range of IT professions, and people with IT expertise are moving to the city, while further skills are being developed through IT education programmes such as those offered by Mid Sweden University and Lärcentrum.

Jämtland Härjedalen has a unique position within e-health - largely due to the wonderful cooperation between the public and private sectors in the area. Vältel is a successful Interreg project that has enabled several IT companies to carry out advanced product development in close cooperation with healthcare. Some of the companies that have taken this opportunity are ImagineCare, Intel, MyMowo and Dynamisk Helse.

VälTel creates test beds in two countries

The Interreg project VälTel offers businesses a route into the care and welfare services and the health and medical care services of the municipalities and regions of central Scandinavia. It includes four unique test beds which have been used to their full potential for the last three years. The goal is that ideas, products and processes can be tested on the right target group, so that they can then be scaled up and sold to a larger market. Many sub-projects and tests have been carried out within the scope of Vältel, such as Skill Aid, Home monitoring, Digital evening visit and Important mothers.

”Vältel is a project that has contributed to the digital transformation of health and medical care and social care and welfare. Tight budgets sometimes make it difficult to be able to make large changes, but via VälTel people have had the possibility to test and carry out short pilots. It’s better to test efficiently for a shorter period and learn, rather than not test at all. This approach has worked well and often created the conditions for continued implementation”

Norweigian company Dynamik Helse has created an education app called Skill Aid for personnel who work within elderly care. In conjunction with the municipality of Östersund, the app has now been developed and tested in care homes for persons with functional disabilities. The evaluation shows good results and the municipality of Östersund has now purchased the app for further development.

Within the scope of a healthcare project in Hammarstrand and Stugun, a number of patients with heart failure, high blood pressure, diabetes, COPD and mental health issues had the chance to measure their health indicators at home, in combination with physical activity on prescription. First, a working hypothesis was developed which was then tested on a smaller scale and evaluated. Thereafter, the project was continually scaled up, with regular reviews during which problems were identified and solved when they arose. One of the reasons for the success of the project was that patients could monitor themselves and take control, with healthcare personnel online as virtual support.

ImagineCare provided part of the solution via their mobile app, which the participants used to note down their results. For patients who do not have or do not want to use a mobile phone, InTel’s system Ihap complemented ImagineCare’s app for delivering results via a technical solution to the healthcare service.

During pregnancy, pregnant women can experience reduced opportunities for movement. To address this, the exercise company MyMowo together with Region Jämtland Härjedalen has developed and tested a virtual exercise programme, a digital service, for pregnant women within Region Jämtland Härjedalen. The test mainly shows good results.

The closest care is that which the patient/user is able to administer themselves – self-care - along with the support provided by the municipalities and regions to facilitate this. More accessible, closer care together with new approaches can mean that health and welfare resources can be better used and go further. In Region Jämtland Härjedalen, the strategic work has already begun to take shape and it was an obvious choice to involve care services, decision-makers and enablers from the very beginning.

E-health issues are a high priority in Sweden and projects are ongoing in many areas. The Government has initiated investigations and the Swedish Association of Local Authorities and Regions (SALAR) has also raised e-health as a central focus area. Jämtland Härjedalen got involved immediately. In our region, we make the nationally formed mandates more concrete, and create our own tests and projects based on the needs of the market.

National initiatives make rapid progress in Region Jämtland Härjedalen

SALAR and Vinnova have together initiated the LEDA for smarter welfare programme, which aims to increase speed, benefits and quality in the introduction of new digital solutions. Östersund is one of eleven cities participating in the programme and has, for example, introduced GPS watches with an embedded safety alarm.

Instead of a traditional alarm that only works in the proximity of the home, a user can use the GPS watch to sound the alarm from their home, neighbouring village or Spain. The idea behind the GPS watch is to promote good health without the user needing to feel tied to their home. There has been great demand from users, and 350 GPS alarm watches are now in use.

Experience so far has been positive, and the preliminary results show that older citizens in Östersund need less support than their peers in other parts of the country.

In Sweden’s 290 municipalities, there are 700 GPS watches with an embedded safety alarm within health and medical care. 350 of these are in Östersund.

Just outside Östersund in Brunflo, Region Jämtland Härjedalen together with a doctor from one of the medical centres has implemented a pilot study for Doctors via app. The doctor, whose normal place of residence is the island of Öland, has been able to accept healthcare appointments via an app while there, instead of at the medical centre. Since the medical centre’s patient record system is able to handle app appointments and since the experience has been good, the pilot has transitioned into regular practice. The positive experience is shared by many of those involved: both doctors who for different reasons cannot physically be present at the medical centre, and patients who for different reasons prefer an app-based consultation. This may be patients with mental health issues that make them unwilling to be around a lot of people, or also patients for whom it is difficult to take time from work to travel to and from the medical centre.

What are the actual challenges within healthcare and how can they be solved? To answer these questions employee shadowing, a perhaps somewhat unconventional method, has been used within a project. In practice this has involved a Microsoft employee accompanying a nurse in Östersund to observe how the working day looks, what the workflows are, what works well and what is more complicated. The objective of the study is to find out how the nurse works today to be able to draw conclusions about how they could work in the future. Digitalisation is not just about making old information digital, rather it is about creating new ways of working and new processes.

The municipality of Härjedalen has the challenge of three medical centres, one in each of Funäsdalen, Hede and Sveg, huge geographical distances and difficulties in getting emergency doctors. At the E-health hub in Östersund, a number of companies have developed solutions that mean that the doctor can be available virtually, online.

At the medical centres, patients book appointments as usual and the nurse receives patients there, while the doctor is located at the hospital in Östersund. Using video technologies, the doctor can examine the patients, for example their eyes, ears and throat, in fact with higher resolution visibility than in real life. With the help of the nurse, the patient takes the tests needed and becomes part of the normal hospital flow. The doctor, for their part, can cover more medical centres without needing to travel between them. The objective is to reduce the number of patients that need to visit the hospital and instead treat them within primary care. In the future, appointments at primary care centres can be shifted to the home.

Region Jämtland Härjedalen is currently an important participant in several international e-health projects, such as VälTel. The region is also involved in the projects C3 Cloud and HS monitor, both of which are Horizon2020 projects. It also acts as an mHealth hub, a project initiated by the World Health Organisation (WHO) and the International Telecommunications Union, ITU.

Nowadays most people own a mobile phone. Irrespective of how rich or poor we are, the mobile phone is always at hand and it can be used to deliver medical services. Mobile healthcare can be provided via phone conversations, text message or satellite. There is an enormous potential for development in the area, WHO and ITU have identified opportunities and a large project has been initiated. The centre for the development initiatives is located in Andalusia, Spain, but the E-health hub in Östersund is its remote hub. This is where information about the world’s best m-health solutions is being gathered and shared.

C3-Cloud is a collaborative project within the EU, aimed at developing new software for personally tailored healthcare guidance for people with multiple morbidities. Amongst other things, the software needs to be able to read the information from patient records that is pertinent to each situation, so that different care-givers receive the right suggestions for the medication they should use, and more importantly the medication they should not use for the current patient.

HS monitor is an EU project for cooperation between technology companies and health and medical care. The goal is that the participating companies will develop a technical solution to monitor patients’ blood pressure in the home. A market of a billion people awaits the company that succeeds in getting all the way to a final product as the project will be launched in several European countries.

Why has Sigma chosen to be part of the E-health hub?

Working with projects that impact future healthcare for the better is both fun, exciting and a way to develop our competences. We also believe that this sector will employ a lot of consultants and become a major business area. It is early days, but everybody is interested in more cost-effective healthcare.

How does your company benefit?

As a national consultancy firm we are discussing e-health in several of our locations and offices. Having the opportunity to work in the leading “ecosystem” in the country, with others who know so much about medical and welfare systems feels tremendous. We have been part of projects both at Karolinska University Hospital in Stockholm and Uppsala University Hospital. The work we are doing here in Östersund involves our other Sigma offices and we contribute a future-oriented force to e-health.

Doesn’t it involve quite a bit of work which is unpaid for you as a consultancy firm?

If we want to be involved and lead within e-health then we must invest, and we do that with our time and knowledge. We want to create a better future and that begins with people that are curious and make things happen. That’s how we want to live.

Why has Microsoft chosen to be part of the E-health hub?

For us at Microsoft, the E-health hub is a perfect starting point for achieving more together, which I believe we do. We have several colleagues who take the opportunity to be on-site in Östersund several times a month.

What has been most valuable for Microsoft?

Being part of collaborating on the Close care strategy and collaborating with the municipality and the region. Working together on that has created understanding between different stakeholders because we have bounced ideas, had the possibility to give feedback and created a strategy together.

Any other highlights?

The hackathon/innovation workshop we held during autumn 2019 when those of us who work with solutions invited employees from the region and municipality, students, patients, users and patient organisations. We worked together for two half days to explore concrete problems and identify new solutions. Everybody just forgets their titles and roles and works together.

Why does the E-health hub in Jämtland Härjedalen work so well?

It’s the attitude, the place has drive and entrepreneurship. There’s a spirit of breaking new ground, and we focus on the possibilities rather than the challenges. We focus on how we best solve things together, and here we are not impeded by things that could hold us back, we work through them instead.

Tieto is a part of the E-health hub. Why?

For us it has become a natural place for cooperation, where we sit together with our customers and work in shared projects.

What advantages to you see to being present at the E-health hub?

It is a natural meeting place for local stakeholders, where we can also meet customers from all over Sweden and sometimes from the world. This is where business opportunities are created and project ideas are shaped. We have dialogue with all parties involved, municipalities, health and medical care, the entire region and stakeholders from the business sector.

What does it give you, more concretely?

Increased understanding for the needs of health and medical care services/welfare and social care, and an opportunity to contribute our expertise and experience in important questions. It also provides an arena for demonstrating our products and services. We see that there is a huge need, both today and for the future, and therefore want to be part of the transformation that is underway. Being part of the E-health hub is an efficient way to do this.

What would you say to companies or organisations that have heard about the E-health hub but haven’t taken the step to move in?

Both suppliers and those that have needs are located under the same roof. It is a unique hub and it creates great synergies and ideas for everybody. What’s more, it is driven by the healthcare organisation; it is not the IT departments that are here. Dialogue often takes place directly with doctors, nurses, department managers and other personnel within the organisation.

Why has the E-health hub been successful?

To tackle demographic challenges we must dare to have new ideas and new perspectives. Otherwise, those of us who have worked within healthcare and welfare for many years will probably come up with the same solutions as before. Together with the private sector, we can find new solutions and tackle the challenges. And we have come a long way!

There is a lot of e-health knowledge in Jämtland Härjedalen, why?

We have long distances and a small population. We see that our proximity to decision-makers is an advantage and decision-making cycles are always short. This has meant that we have generated courage and the drive to dare to test our way forward. The Municipal Commissioner and department managers are available when we need to get buy-in or decisions.

Why are people looking at Jämtland Härjedalen?

We have a lot of expertise here. Just look at how Sigma has chosen to place its entire e-health development in Östersund and how national companies come here to test, learn and train. We are already where the rest of Sweden is going to be in ten years.

What would you say to companies that have not yet joined the E-health hub?

Test and try things out in Jämtland Härjedalen now. In ten years you will have customers all over Sweden!
