    Dear Mr. Tatman:

    Postlethwaite & Netterville (P&N) has completed its internal audit procedures and analysis to assist the Academic Records Task Force in its efforts to implement the corrective action plan intended to address the findings in the Louisiana Department of Educations (LDOE) audit. Our procedures included analysis related to academic records review and graduation cohort rate review. The following report provides a summary of our procedures, observations, and recommendations to enhance East Baton Rouge Parish School Systems (EBRSS) compliance and monitoring procedures related to the Academic Records Task Force.

    Our procedures were not designed to constitute a financial statement audit, review, or assurance of the internal controls or information provided. Accordingly, we will not express an opinion or conclusion nor provide any other form of assurance on the completeness and accuracy of the information. Additionally, the projection of any conclusions, based on our findings, to past or future periods is subject to the risk that changes may have occurred during the passage of time that may alter the validity of such conclusions. Furthermore, the projection of any conclusions, based on our findings, to the whole population is subject to the risk that the sample selected may not accurately reflect the population as a whole. This engagement was conducted in accordance with the Statements on Standards for Consulting Services , established by the American Institute of Certified Public Accountants.

    OBJECTIVE AND SCOPE:

    The objective of this engagement was to assist the Academic Records Task Force with internal audit procedures and analysis related to the findings in the Louisiana Department of Educations (LDOE) audit in the following two areas:

    Academic records review

    Graduation cohort rate review.

    PROCEDURES:

    The following procedures were performed during the course of this engagement:

    Obtained and reviewed the applicable LDOE and State Board of Elementary and Secondary Education Requirements as related to student data, academic records, and graduation records, including graduation cohort rates.

    Determined how student data, academic records and graduation records, including graduation cohort rates were maintained (both at EBRPSS and individual schools within the district) through discussion with EBRPSS and applicable school personnel and review of available documentation.

    Academic records review: For all exceptions identified by LDOE, reviewed applicable EBRPSS internal records and information reported by EBRPSS to LDOE to verify consistency of EBRPSS internal records with information the district reported to LDOE.

    Graduation cohort rate review: For all exceptions identified by LDOE, reviewed applicable EBRPSS documentation to determine if appropriate documentation was on file as per [Title 28, Part LXXXIII (Bulletin 111, 603 C.3.a)] based on exit code.

    Summarized the results of our procedures performed related to our reviews of academic records and graduation cohort rates.

    Where applicable, identified and documented control enhancement opportunities.

    ACADEMIC RECORDS TASK FORCE ASSISTANCE OVERVIEW:

    In its response to LDOEs audit findings, EBRPSS created a comprehensive corrective action plan that included detailed policies and procedures in the following areas:

    Graduate verification,

    Transcript data verification,

    Withdrawal/drop procedures,

    Grade correction procedures,

    Online grades procedures,

    Professional development and training, and

    Internal audit.

    Per discussion with management, P&N noted that all the above listed policies and procedures were either implemented or updated in April 2014. While the corrective action plan appears to include a robust list of actions required to ensure compliance with Title 28, Part CXV. Bulletin 741 Louisiana Handbook for School Administrators and Title 28, Part LXXXIII (Bulletin 111, 603 C.3.a), EBRPSS should consider implementing additional monitoring controls at both district and school levels to ensure completeness and accuracy of student academic records and graduation cohort rates. Examples of controls included in the corrective action plan and additional monitoring controls for managements consideration are detailed on the following pages.

    Related to academic records review, while P&Ns procedures were focused primarily on testing consistency of data between the student cumulative folder, district database and LDOEs Student Transcript System (STS); P&N noted instances where schools provided documentation related to identified variances to substantiate the grades within the district system. Similarly, for graduation cohort review, P&N noted instances where, for identified exceptions, schools provided additional documentation to substantiate correct Exit Codes in the system. It was noted that all in scope schools provided available documentation upon request and were receptive to working with P&N. Further, based on our site visits and documentation reviewed, we did not identify any indications of intentionally fraudulent behavior or coercion.

    OBSERVATIONS AND RECOMMENDATIONS :

    Academic Records Review Summary of Findings:

    Based on interviews and review of EBRPSS internal documentation, P&N noted the following observations. Detailed information related to findings by individual school is included on the following pages. In addition, a summary of results by school can be found at Appendix A.

    It should be noted our review was limited to reviewing applicable EBRPSS internal records and information reported by EBRPSS to LDOE to verify consistency of EBRPSS internal records per the student cumulative folders with information the district reported to LDOE. Our review did not include verification of student records to the electronic grade books, and the applicable regulations do not require inclusion of the grade book in the student cumulative folders. Further, exceptions identified below as primarily related to documentation deficiencies are based on information included in the cumulative folders. This does not imply that graduation requirements were not met for the remaining students with exceptions.

    It appears that established policies and procedures related to graduate verification, transcript data verification and grade correction were not consistently followed by the schools, causing discrepancies between EBRPSS internal records and information the district reported to LDOE. Part of this may be attributed to lack of appropriate monitoring controls.

    P&N noted that identified clerical and documentation issues were, in part due, to the manual nature of processes related to academic records maintenance and monitoring activities.

    Recommendations:

    1. Management should ensure that monitoring controls related to graduate verification, as detailed in the corrective action plan, are implemented and include signed certification by the school principals to the district stating that all of the graduates have been certified and deemed eligible to receive diplomas. In addition, periodic audits, as detailed in the corrective action plan, should be performed and monitored at the district level to ensure accuracy of the information reported by the schools.

    2. Management should ensure that monitoring controls related to transcript data verification, as detailed in the corrective action plan, are implemented and include signed certification by the school principal to the district stating that a transcript data verification checklist was completed for all students.

    3. While the transcript data verification checklist provides a comprehensive review of student transcript information on an individual student level, management should consider implementing, at the school level, a completeness check to ensure the transcript data verification checklist has been completed for all students in grades 9 to 11.

    4. Management should ensure that monitoring procedures related to completeness and accuracy of transcript review and data entry into eSchoolPLUS for all transferring students, as detailed in the corrective action plan, are implemented and periodically audited.

    5. Management should consider implementing additional monitoring procedures to ensure that grade corrections are made in accordance with the established policies and deadlines. Management should consider implementing systematic (automated) controls that would prevent inappropriate changes being made in the system or creating system generated exception reports that would identify any changes made in the system outside of the established parameters. In addition, periodic audits, as detailed in the corrective action plan, should be performed at the district level to ensure that all grade changes are appropriately

    7. In designing its internal audit procedures, management should consider implementing a risk based approach that would focus initial efforts to schools that have high number of transferred students, students with grade changes and students in online courses or summer school.

    8. Management should continue to provide periodic training for all high school senior counselors, counselors, and principals on at least an annual basis, as detailed in the corrective action plan. Depending on the results of internal audits, management should determine if more frequent training should be required.

    9. Management should consider opportunities to further automate established processes related to academic records maintenance and monitoring activities through implementation of system exception reports and paperless student files.

    Academic Records Review Individual School Findings:

    The following details the number of students with LDOE reported exceptions per school and results of P&Ns review of EBRPSS internal records and information reported by EBRPSS to LDOE. Please note that while LDOE summarized its findings by exception type, P&N first summarized findings based on a total number of students with exceptions and then detailed exception types noted for those students.

    Arlington Preparatory Academy Since there were no LDOE reported exceptions, no additional procedures related to academic

    records review were performed by P&N for this school.

    Baton Rouge Magnet High School Since there were no LDOE reported exceptions, no additional procedures related to academic

    records review were performed by P&N for this school.

    Belaire High School For the six students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o Four instances in which not all grade documentation was included in the cumulative

    folder. o Six instances in which credits and/or grades in EBRPSS student cumulative folders did

    not match credits and/or grades submitted to LDOEs STS database. P&N noted that for one of the six instances, it appeared findings were related to documentation deficiencies in the cumulative folders rather than to the accuracy of information reported to LDOE

    Career Academy For the one student identified as an exception by LDOE, P&N reviewed applicable

    documentation noting the following: o One instance in which credits and/or grades in EBRPSS student cumulative folders did

    not match credits and/or grades submitted to LDOEs STS database. The school provided documentation to substantiate the grades reported to LDOE. It appeared that for this student, findings were related to documentation deficiencies in the cumulative folder rather than to the accuracy of information reported to LDOE.

    East Baton Rouge Laboratory Academy Closed School For the one student identified as an exception by LDOE, P&N reviewed applicable

    documentation noting the following: o The exception for this student was cleared, since, per applicable regulations, official

    student transcripts are not required to be included in the cumulative folder.

    East Baton Rouge Accelerated Academy For the two students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o The exception for one student was cleared, since, per applicable regulations, official

    student transcripts are not required to be included in the cumulative folder. o For the remaining student, the cumulative folder could not be located.

    Glen Oaks Senior High For the four students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o Exceptions for two students were cleared as follows:

    Per applicable regulations, official student transcripts are not required to be included in the cumulative folder.

    The school provided appropriate documentation required for compliance with the regulations.

    Istrouma Senior High School Closed School For the two students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o Exceptions for both students were cleared, as EBRPSS provided appropriate

    documentation required for compliance with the regulations.

    McKinley Senior High School For the 41 students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o Eight exceptions were cleared, as the school provided appropriate documentation

    required for compliance with regulations. o Of the remaining 33 students, P&N noted the following:

    23 instances in which not all grade documentation was included in the cumulative folder.

    33 instances in which credits and/or grades in EBRPSS student cumulative folders did not match credits and/or grades submitted to LDOEs STS database. P&N noted that for 12 of the 33 instances, it appeared that the errors within the cumulative folders were related to documentation deficiencies in the cumulative folders rather than to the accuracy of information reported to LDOE.

    Northdale Superintendents Academy For the seven students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o One exception was cleared, as the school provided appropriate documentation required

    for compliance with regulations. o For the remaining six students, P&N noted the following:

    One instance in which the cumulative folder could not be located. Four instances in which not all grade documentation was included in the

    cumulative folder. Six instances in which credits and/or grades in EBRPSS student cumulative

    folders did not match credits and/or grades submitted to LDOEs STS database. The school provided documentation for four students to substantiate the grades

    within the

    district

    system.

    Scotlandville Magnet High School For the 13 students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o Exceptions for six students were cleared as the school provided appropriate

    documentation required for compliance with regulations. o Of the remaining seven students, P&N noted the following:

    Three instances in which not all grade documentation was included in the cumulative folder.

    Seven instances in which credits and/or grades in EBRPSS student cumulative folders did not match credits and/or grades submitted to LDOEs STS database. For four of the seven instances, it appeared that findings were related to documentation deficiencies in the cumulative folders rather than to the accuracy of information reported to LDOE.

    Tara High School For the 10 students identified as exceptions by LDOE, P&N reviewed applicable documentation

    noting the following: o Exceptions for four students were cleared as the school provided appropriate

    documentation required for compliance with regulations. o Of the remaining six students, P&N noted the following:

    Three instances in which not all grade documentation was included in the cumulative folder.

    Six instances in which credits and/or grades in EBRPSS student cumulative folders did not match credits and/or grades submitted to LDOEs STS database. For one of the six instances, the school provided documentation to substantiate the grades within the district system. It appeared that for this student, findings were related to documentation deficiencies in the cumulative folders rather than to the accuracy of information reported to LDOE.

    Woodlawn High School Since there were no LDOE reported exceptions, no additional procedures related to academic

    records review were performed by P&N for this school.

    Graduation Cohort Review Summary of Findings:

    Based on interviews and review of EBRPSS internal documentation, P&N noted the following observations. Detailed information related to findings by individual school is included on the following pages. In addition, a summary of results by school can be found at Appendix B, and a summary of results by cohort year can be found at Appendix C.

    From a total of 136 students identified as exceptions by LDOE for all open schools, six exceptions were cleared as the schools provided appropriate documentation required for compliance with [Title 28, Part LXXXIII (Bulletin 111, 603 C.3.a)] (Regulations).

    While it appears that in scope schools generally did not maintain appropriate documentation to ensure compliance with applicable regulations, P&N noted that schools provided additional documentation for 30 students to substantiate correct Exit Codes in the system. This information included documentation regarding students transfers, enrollment and/or graduation from the receiving schools. It appears that for those students, findings were related to documentation deficiencies rather than to the accuracy of Exit Codes in the system.

    For the remaining students in our sample, due to insufficient or a lack of documentation, it was not possible to determine if appropriate Exit Codes were used in the system.

    It appears that Exit Codes were erroneously entered for three students. This may be partially attributed to lack of appropriate monitoring controls

    of school ownership. Per Title 28, Part LXXXIII (Bulletin 111, 603 C.3.a), the schools are required to maintain documentation that supports Exit Codes for at least four years after the data has been used in School Performance Scores. Per discussion with EBRPSS management, it was noted that some of the students tested appear to have dropped during the period that is outside of the documentation retention period.

    Recommendations:

    1. Management should ensure that established monitoring procedures related to withdrawal/drop records, as detailed in the corrective action plan, are implemented and are being followed by the schools. Periodic audits, as detailed in the corrective action plan, should be performed at the district level to ensure that appropriate documentation is on file as required by the regulations.

    2. Management should consider implementing additional monitoring procedures around Exit Code entry in the system to ensure completeness and accuracy of the information in the system.

    3. Management should consider updating policies and procedures to define physical safeguarding controls around student records; including storage at the school, as well as transport to and storage at the warehouse. Furthermore, data ownership should be clearly defined and records should be periodically inventoried and reviewed to ensure compliance with data retention policies.

    4. Management should consider implementing additional monitoring procedures to ensure policies and procedures related to school closures and schools that are being transferred out of EBRPSS jurisdiction are being followed. Monitoring procedures should consider data ownership and procedures related to maintaining and monitoring of student records during required data retention periods.

    Graduation Cohort Rate Review Individual School Findings:

    The following details number of students with LDOE reported exceptions and results of P&Ns review of EBRPSS internal records for compliance with the regulations for each in scope school.

    Arlington Preparatory Academy For a total of one student identified as an exception by LDOE, P&N reviewed applicable

    documentation noting the following: o Appropriate documentation to ensure compliance with regulations was not on file.

    However the school provided additional documentation related to the transfer of this

    related to enrollment and/or graduation information of six students to substantiate appropriate Exit Codes in the system.

    Belaire High School For a total of 16 students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o For one student, Exit Code 10 was erroneously added in the system. Per review of

    supporting documentation, the student transferred to another public school within the district and Exit Code 8 should have been entered upon transfer.

    o For the remaining 15 students, it was noted that records were destroyed when a new clerk cleaned out the storage office. As a result, appropriate documentation was not available for review.

    Broadmoor Senior High For a total of 26 students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o Four exceptions were cleared as the school provided appropriate documentation

    required for compliance with the regulations. o For the remaining 22 students, appropriate documentation was not available for the

    review.

    Northdale Superintendents Academy (Northdale Magnet Academy)

    The following three schools merged and were audited under Northdale Superintendents Academy: The CAN Academy of EBR, EBR Accelerated Academy (Northdale Magnet Academy North campus), and Northdale Magnet Academy (Northdale Superintendents Academy).

    For a total of 19 students identified as exceptions by LDOE, P&N reviewed applicable documentation noting the following:

    o Per school management, records for CAN Academy (3 students) as well as EBR Accelerated Academy (11 students) were sent to the Sharp Station warehouse when the schools merged and could not be located.

    o The remaining five students from Northdale Magnet Academy did not have appropriate documentation to ensure compliance with the regulations.

    Glen Oaks Senior High For a total of three students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o For all three students, appropriate documentation to ensure compliance with

    regulations was not on file However the school provided additional documentation

    o For all 11 students, appropriate documentation to ensure compliance with regulations was not on file. However, the school provided additional documentation including Transfer/Withdrawal Forms signed by parents for four students to substantiate appropriate Exit Codes in the system.

    Northeast High School For a total of six students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o Two exceptions were cleared as the school provided appropriate documentation

    required for compliance with regulations. o For the remaining four students, appropriate documentation to ensure compliance with

    regulations was not on file. However, the school provided additional documentation related to enrollment and/or graduation information of all four students to substantiate appropriate Exit Codes in the system.

    Scotlandville Magnet High School For a total of 16 students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o For all 16 students, appropriate documentation to ensure compliance with regulations

    was not on file.

    Tara High School For a total of ten students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o For one student, Exit Code 10 was erroneously added in the system. Per review of

    supporting documentation, the student transferred to another public school within Louisiana and Exit Code 9 should have been entered upon transfer.

    o For the remaining nine students, appropriate documentation to ensure compliance with regulations was not on file. However, the school provided additional documentation related to enrollment and/or graduation information of one student to substantiate appropriate Exit Code in the system.

    Woodlawn High School For a total of 18 students identified as exceptions by LDOE, P&N reviewed applicable

    documentation noting the following: o For one student, Exit Code 10 was erroneously added in the system. Per review of

    supporting documentation, the student received GED diploma and Exit Code 5 should have been entered upon transfer.

    o For the remaining 17 students, appropriate documentation to ensure compliance with regulations was not on file However the school provided additional documentation

    o For all three students, appropriate documentation to ensure compliance with regulations was not on file. However, the school provided additional documentation related to enrollment and/or graduation information of two students to substantiate appropriate Exit Codes in the system.

    Closed Schools Per discussion with management, it was noted that six schools included in the scope of LDOEs testing were closed. For a total of 45 students identified as exceptions by LDOE, P&N noted that documentation related to graduation cohort review was not available for the review due to the following:

    Central (1 student with LDOE noted exceptions) o Per discussion with management, P&N noted that this school left EBRPSS on July 1,

    2007. Appropriate documentation to ensure compliance with regulations was not on file. However, it appears that this school was closed outside of the documentation retention period, as defined by regulations.

    Istrouma Senior High (12 students with LDOE noted exceptions) o Per discussion with management, P&N noted that this school was turned over to the

    Recovery School district on July 1, 2012. Appropriate documentation to ensure compliance with regulations was not on file.

    Robert E. Lee High (26 students with LDOE noted exceptions) o Per discussion with management, P&N noted that this school closed on June 30, 2009.

    Records were sent to the Sharp Station warehouse when the schools closed and could not be located.

    McAuliffe Pre GED Center (2 students with LDOE noted exceptions) o Per discussion with management, P&N noted that this school closed on June 30, 2012.

    Records were sent to the Sharp Station warehouse when the schools closed and could not be located.

    Capitol PreCollege for Girls (1 student with LDOE noted exceptions) o Per discussion with management, it appears that records for this school were turned

    over to the Recovery School District/Charter School on July 1, 2009. Appropriate documentation to ensure compliance with regulations was not on file.

    East Baton Rouge Laboratory Academy (3 students with LDOE noted exceptions) o Per discussion with management, P&N noted that this school closed on June 30, 2012.
