1.报名表
报名表必须由本人亲自填写。所有栏目均要求填写
完整清晰、准确属实。若报考人自述部分预留空间
不够，可另附纸张（A4白纸）。请您将报名表复
印留底，以便查询。
2.附件（报考所需材料）
• 推荐信两封
• 学历和学位证书复印件一份
• 有效身份证件复印件两份
请将其中一份有效身份证件复印件贴在报名表
指定位置。
• 名片两张
请您按照要求贴在报名表的指定位置。
• 一寸彩色近照一张
请您提供半年以内的照片，贴在报名表指定位
置，并请您用铅笔在背面署名。
★
缺少任何材料或填写不清晰工整，都将视为报名无效。我们将在收到考生报名材料后的一周内给予反馈，在报名截止后统一审核筛选、安排入学面试。
请您使用报名表所附推荐信格式，无需中英文对
照。两位推荐人要求分别为报考人的直接领导和
一位在学术、业界、政府等方面有一定影响力的
权威人士。若没有直接领导，可另选一位权威人
士作为推荐人。推荐信必须由推荐人本人填写并
签名。
请您提供教育部批准的国民教育系列大专或大
专以上的学历、学位证书扫描件（包括工农兵
学生、成人高考、自考、专升本、通过教育部
统考的函授、电大），其它证件无效。
若您的报考学历是在境外或港、澳、台获得的，
须到教育部中国留学服务中心办理学历学位认
证手续，并提供教育部颁发的《国外学历学位
认证书》扫描件，否则不予批准录取。

清华五道口金融EMBA正在报名
具有国民教育系列大学本科或本科以上学历，毕业后满八年或八年以上全职工作经验，
其中包括五年或五年以上管理工作经验；
或具有国民教育系列大学专科或专科以上学历，毕业后满十年或十年以上全职工作经验，
其中包括五年或五年以上管理工作经验。
选拔方式
采取“申请审核-报名考试-择优录取”的方式进行，考察综合素质和管理潜能。申请审核包含材料审查和面试考核，
通过者方可报名参加全国硕士研究生统一入学考试中的EMBA联考笔试，并根据笔试成绩择优确定录取名单。
清华大学不接受考生调剂，报考清华大学的考生亦不能调剂到其他招生单位。
授课时间
学制两年。每个月集中授课四天，一般是周四至周日。
学历学位
完成全部课程和通过学位论文答辩后，依《清华大学章程》，经学校批准，可获得由学校颁发的毕业证书和学位证书。
