LEARNING OBJECTIVES At the completion of this live educational activity, participants will be better able to:• Identify timely testing and intervention strategies in patients at-risk for chronic

kidney disease (CKD).• Explain appropriate clinical measures to manage risk and increase patient

safety in CKD.• Recognize co-management and referral of patients to nephrology specialists,

when appropriate, in order to improve outcomes in CKD.• Review blood pressure management goals for patients on dialysis, especially

hemodialysis • Review approaches to diabetes management in patients with renal failure• Assess the current state of ESRD modality education, and the use of web and

technology-enabled self-management approaches• Recognize the role and availability of technology-supported patient self-

management in chronic disease
