Award Criteria Note: Failure to meet any of the criteria below will result in automatic elimination. 1. Applicant must serve as an instructor in a Head Start or Early Head Start Program providing services to children who live in Utah. This includes classroom teachers, teacher's assistants, and home visitors. 2. The local program must be a current member of UHSA. 3. Applicant must be a program employee for at least two years. 4. Applicant must have at least a CDA or an equivalent credential that has been recognized by OHS. Requirements 1. Include a one-page, double space, typed essay outlining your philosophy of educating young children. 2. Include a recommendation letter from a co-worker or your direct supervisor. Letter may be handwritten but can not be more than one page 3. Include a letter of support from a family you work/have worked with. Letter maybe handwritten but cannot be more than one page. Ranking Applications will be ranked on the following: 40 points- Essay- Educational philosophy 30 points- Letter from coworker or supervisor 30 points- Letter from family you are working with/have worked with. Applications are due to your local Head Start program by _______________ Winners from each program will be forwarded to the Utah Head Start Association by May 24.
