These queries enable the practice to analyze clinical data about their patients based on specific information, such as:

• Diagnosis

• Lab results

• Medications

• ADA standards of care

Reviewing patient reports can support your practice’s QDM efforts by:

• Identifying patients not at goal

• Finding patients overdue for an office visit

• Monitoring ADA standards of care

Developing Informatics Queries or using Clinical Decision Support non-compliant patient reports in Care360 EHR helps to facilitate and support a continuous quality improvement mindset.

Informatics Query Examples• Patients who have not reached

their treatment goal based on a lab value (eg, recent HbA1c test) or comorbidity

• Patients who are currently uncontrolled and have not had an HbA1c test in greater than 3 months

• Patients that are due for QDM metrics such as comprehensive foot and eye exams

Criteria Example #1 To identify patients not at goal for the HbA1c

In the Informatics Query Criteria, Test Names/Codes, select Show Analytes.

In Date Collected or Reported, set the desired date range.

Enter A1c in the Find text box and click Find Analytes.

From the displayed list of HbA1c tests, check to include them in the query.

In the From text box enter 7. In the To text box enter 999.

The Informatics Query in Care360 EHR can be used to identify appropriate candidates not at treatment goal and gaps in care.

The following demonstrates how to run queries based upon criteria in Care360 EHR.

To access the Informatics Queries, navigate to Reports > Informatics Query.

Navigate to Admin, Configure CDS Alerts

Check CMS122 / NQF0059 Hemoglobin A1c Poor Control, and click Save

Criteria Example #2: Identify patients who have uncontrolled diabe-tes and have not had an HbA1c test in greater than 3 months

A Clinical Decision Support report is a quick way to find patients who have not an HbA1c test within the ADA recommended timeframe (more than 3 months). CDS guidelines must be turned on, then reports of non-compliant patients are available.

Navigate to Reports, Quick View, Guidelines: Non-compliant Patients

The listing of Non-compliant patients is displayed.

Print the report if desired.

Criteria Example #3: Identify diabetic patients who have not had a Foot Exam

A Clinical Decision Support report is a quick way to find patients who have not had a foot exam within the ADA recommended timeframe. CDS guidelines must be turned on, then reports of non-compliant patients are available.

The listing of Non-compliant patients is displayed.

Print the report if desired.
