This month, we are excited to introduce a new section of Elite Tampa Bay called Kaptan’s Korner. Kaptan is our beloved rescue dog and starting this month we will be donating a page of advertising to an animal based charity in his honor.

We are also excited to introduce USF Health to Elite. USF Health provides the very best concierge medical care in the Tampa Bay area and will be providing content to Elite Tampa Bay for the next year. This month they discuss executive physicals and how early diagnosis of disease can literally save your life.

Over the last few months, we have been overwhelmed by your kind words, complements to our design team, and support. Thank you for helping us grow and allowing us to continue to bring you information on the best companies in the best city in our country.

The Introduction of Kaptan’s Korner:

This month we are introducing a special section of Elite Tampa Bay that is called Kaptan’s Korner.

Eight months ago we agreed to foster an English Bulldog. Within hours of bringing him home we became what rescues call foster failures (families that offer to foster dogs and cannot bear to give them back). We renamed him Kaptan (pronounced Captain) and he has brought joy to our family every moment since the day we brought him home. As a tribute to Kaptan; Elite Tampa Bay will now donate a full page ad to an animal based non-profit every month.

Kaptan has never met anyone that he doesn’t like; and his affable, clown like personality make us laugh on a daily basis. Although he loves his mommy and daddy he has one true passion in life… kitty cats. He patiently allowed our two cats to hiss at him, ignore him, taunt him, and generally torture him for six months before he won him over. They are now his best buddies. When being walked, you’d better believe that he will be pulling on his leash trying to drag you to the homes that he knows have cats living in them. It only seems right that for the first month the space will be donated to The Big Cat Rescue.

Big Cat Rescue is the largest accredited sanctuary in the world dedicated entirely to abused and abandoned big cats. They are home to over 100 lions, tigers, bobcats, cougars and other species most of whom have been abandoned, abused, orphaned, saved from being turned into fur coats, or retired from performing acts. Their dual mission is to provide the best home they can for the cats in their care and educate the public about the plight of these majestic animals, both in captivity and in the wild, to end abuse and avoid extinction.

They are one of our favorite charities & Carole Baskin and the team there are fiercely committed to the protection of not only Big Cats but of all animals.

An executive physical is a head-to-toe medical examination that is individualized based upon each client’s medical history, healthcare needs, and concerns. Cli-

ents are provided unparalleled access to multiple board certified specialty physicians in one convenient visit. An executive concierge schedules tests and screenings in ad-vance and escorts clients from one appointment to the next during the actual visit. Seamless navigation: Our concierge arranges all aspects of scheduling and escorts clients during the visit. Clients don’t have to think about where they need to be and at what time which allows them to free their mind, focus on their health and speak freely with the physician administering care.

The facility: The Monsour Executive Wellness Center was designed with aesthetics in mind inclusive of calming paint schemes and art, peaceful music and videos playing in exam rooms. Our goal is to provide a relaxing atmosphere so that clients relax and enjoy the overall experience.

Convenience: Prior to each client’s executive physical, a phlebotomist will visit their home or office to draw their blood. The results will be reviewed prior to their appointment and the physician will discuss the results with the client during the executive physical.

Empowerment: At the conclusion of the executive physical, clients are given a thumb drive that contains all of the test results from the day’s visit. This enables clients who travel to keep their medical records with them should they require unexpected medical services while away from home. It is also ideal for clients to share with their primary care physician.

WHO NEEDS AN EXECUTIVE PHYSICAL? The program is ideal for busy executives who don’t have time to spend “chasing” doctors. It eliminates the need for clients to obtain referrals should they require a visit with a specialist, it eliminates wasted time driving to and from multiple appointments and clients do not have to sit in waiting rooms for extended periods of time as the physicians are waiting for them. However, you don’t have to be an executive to have an executive physical as it is also ideal for those who prefer concierge medical services and have the means to pay for their physical; as insurance is not accepted.

Effient® (prasugrel) is indicated to reduce the rate of thrombotic cardiovascular (CV) events (including stent thrombosis) in patients with acute coronary syndrome (ACS) who are to be managed with percutaneous coronary intervention (PCI) as follows: [1] patients with unstable angina (UA) or non–ST-elevation myocardial infarction (NSTEMI); [2] patients with ST-elevation myocardial infarction (STEMI) when managed with primary or delayed PCI. The loading dose (LD) of Effient is 60 mg and the maintenance dose (MD) is 10 mg once daily. Effient is available in 5-mg and 10-mg tablets.

IMPORTANT SAFETY INFORMATION

WARNING: BLEEDING RISKEffient® (prasugrel) can cause significant, sometimes fatal, bleeding.Do not use Effient in patients with active pathological bleeding or a history of transient ischemic attack or stroke.In patients ≥75 years of age, Effient is generally not recommended, because of the increased risk of fatal and intracranial bleeding and uncertain benefit, except in high-risk situations (patients with diabetes or a history of prior myocardial infarction [MI]) where its effect appears to be greater and its use may be considered.Do not start Effient in patients likely to undergo urgent coronary artery bypass graft surgery (CABG). When possible, discontinue Effient at least 7 days prior to any surgery.Additional risk factors for bleeding include: body weight <60 kg propensity to bleed concomitant use of medications that increase the risk of bleeding

(eg, warfarin, heparin, fibrinolytic therapy, chronic use of nonsteroidal anti-inflammatory drugs [NSAIDs])

Suspect bleeding in any patient who is hypotensive and has recently undergone coronary angiography, percutaneous coronary intervention (PCI), CABG, or other surgical procedures in the setting of Effient.If possible, manage bleeding without discontinuing Effient. Discontinuing Effient, particularly in the first few weeks after acute coronary syndrome, increases the risk of subsequent cardiovascular events.

CONTRAINDICATIONS Effient is contraindicated in patients with active pathological

bleeding, such as from a peptic ulcer or intracranial hemorrhage (ICH), or a history of transient ischemic attack (TIA) or stroke, and in patients with hypersensitivity to prasugrel or any component of the product

WARNINGS AND PRECAUTIONS Patients who experience a stroke or TIA while on Effient

generally should have therapy discontinued. Effient should also be discontinued for active bleeding and elective surgery

Effient® (prasugrel) is indicated to reduce the rate of thrombotic cardiovascular (CV) events (including stent thrombosis) in patients with acute coronary syndrome (ACS) who are to be managed with percutaneous coronary intervention (PCI) as follows: [1] patients with unstable angina (UA) or non–ST-elevation myocardial infarction (NSTEMI); [2] patients with ST-elevation myocardial infarction (STEMI) when managed with primary or delayed PCI. The loading dose (LD) of Effient is 60 mg and the maintenance dose (MD) is 10 mg once daily. Effient is available in 5-mg and 10-mg tablets.

WARNING: BLEEDING RISKEffient® (prasugrel) can cause significant, sometimes fatal, bleeding.Do not use Effient in patients with active pathological bleeding or a history of transient ischemic attack or stroke.In patients ≥75 years of age, Effient is generally not recommended, because of the increased risk of fatal and intracranial bleeding and uncertain benefit, except in high-risk situations (patients with diabetes or a history of prior myocardial infarction [MI]) where its effect appears to be greater and its use may be considered.Do not start Effient in patients likely to undergo urgent coronary artery bypass graft surgery (CABG). 1 INDICATIONS AND USAGE1.1 Acute Coronary Syndrome: Effient® is indicated to reduce the rate of thrombotic cardiovascular (CV) events (including stent thrombosis) in patients with acute coronary syndrome (ACS) who are to be managed with percutaneous coronary intervention (PCI) as follows:

• Patients with unstable angina (UA) or non–ST-elevation myocardial infarction (NSTEMI).

• Patients with ST-elevation myocardial infarction (STEMI) when managed with primary or delayed PCI.

Effient has been shown to reduce the rate of a combined endpoint of cardiovascular death, nonfatal myocardial infarction (MI), or nonfatal stroke compared to clopidogrel. The difference between treatments was driven predominantly by MI, with no difference on strokes and little difference on CV death [see Clinical Studies (14)].It is generally recommended that antiplatelet therapy be administered promptly in the management of ACS because many cardiovascular events occur within hours of initial presentation. In the clinical trial that established the efficacy of Effient, Effient and the control drug were not administered to UA/NSTEMI patients until coronary anatomy was established. For the small fraction of patients that required urgent CABG after treatment with Effient, the risk of significant bleeding was substantial [see Warnings and Precautions (5.2)]. Because the large majority of patients are managed without CABG, however, treatment can be considered before determining coronary anatomy if need for CABG is considered unlikely. The advantages of earlier treatment with Effient must then be balanced against the increased rate of bleeding in patients who do need to undergo urgent CABG.2 DOSAGE AND ADMINISTRATIONInitiate Effient treatment as a single 60-mg oral loading dose and then continue at 10 mg orally once daily. Patients taking Effient should also take aspirin (75 mg to 325 mg) daily [see Drug Interactions (7) and Clinical Pharmacology (12.3)]. Effient may be administered with or without food [see Clinical Pharmacology (12.3) and Clinical Studies (14)].Dosing in Low Weight Patients: Compared to patients weighing ≥60 kg, patients weighing <60 kg have an increased exposure to the active metabolite of prasugrel and an increased risk of bleeding on a 10 mg once daily maintenance dose. Consider lowering the maintenance dose to 5 mg in patients <60 kg. The effectiveness and safety of the 5 mg dose have not been prospectively studied.4 CONTRAINDICATIONS4.1 Active Bleeding: Effient is contraindicated in patients with active pathological bleeding such as peptic ulcer or intracranial hemorrhage [see Warnings and Precautions (5.1) and Adverse Reactions (6.1)].4.2 Prior Transient Ischemic Attack or Stroke: Effient is contraindicated in patients with a history of prior transient ischemic attack (TIA) or stroke. In TRITON-TIMI 38 (TRial to Assess Improvement in Therapeutic Outcomes by Optimizing Platelet InhibitioN with Prasugrel),

patients with a history of TIA or ischemic stroke (>3 months prior to enrollment) had a higher rate of stroke on Effient (6.5%; of which 4.2% were thrombotic stroke and 2.3% were intracranial hemorrhage [ICH]) than on clopidogrel (1.2%; all thrombotic). In patients without such a history, the incidence of stroke was 0.9% (0.2% ICH) and 1.0% (0.3% ICH) with Effient and clopidogrel, respectively. Patients with a history of ischemic stroke within 3 months of screening and patients with a history of hemorrhagic stroke at any time were excluded from TRITON-TIMI 38. Patients who experience a stroke or TIA while on Effient generally should have therapy discontinued [see Adverse Reactions (6.1) and Clinical Studies (14)].4.3 Hypersensitivity: Effient is contraindicated in patients with hypersensitivity (e.g., anaphylaxis) to prasugrel or any component of the product [see Adverse Reactions (6.2)].5 WARNINGS AND PRECAUTIONS5.1 General Risk of Bleeding: Thienopyridines, including Effient, increase the risk of bleeding. With the dosing regimens used in TRITON-TIMI 38, TIMI (Thrombolysis in Myocardial Infarction) Major (clinically overt bleeding associated with a fall in hemoglobin ≥5 g/dL, or intracranial hemorrhage) and TIMI Minor (overt bleeding associated with a fall in hemoglobin of ≥3 g/dL but <5 g/dL) bleeding events were more common on Effient than on clopidogrel [see Adverse Reactions (6.1)]. Suspect bleeding in any patient who is hypotensive and has recently undergone coronary angiography, PCI, CABG, or other surgical procedures even if the patient does not have overt signs of bleeding.Do not use Effient in patients with active bleeding, prior TIA or stroke [see Contraindications (4.1 and 4.2)].Other risk factors for bleeding are:

• Age ≥75 years. Because of the risk of bleeding (including fatal bleeding) and uncertain effectiveness in patients ≥75 years of age, use of Effient is generally not recommended in these patients, except in high-risk situations (patients with diabetes or history of myocardial infarction) where its effect appears to be greater and its use may be considered [see Adverse Reactions (6.1), Use in Specific Populations (8.5), Clinical Pharmacology (12.3), and Clinical Trials (14)].

• CABG or other surgical procedure [see Warnings and Precautions (5.2)].

• Body weight <60 kg. Consider a lower (5 mg) maintenance dose [see Dosage and Administration (2), Adverse Reactions (6.1), Use in Specific Populations (8.6)].

• Propensity to bleed (e.g., recent trauma, recent surgery, recent or recurrent gastrointestinal (GI) bleeding, active peptic ulcer disease, severe hepatic impairment, or moderate to severe renal impairment) [see Adverse Reactions (6.1) and Use in Specific Populations (8.7 and 8.8)].

• Medications that increase the risk of bleeding (e.g., oral anticoagulants, chronic use of non-steroidal anti-inflammatory drugs [NSAIDs], and fibrinolytic agents). Aspirin and heparin were commonly used in TRITON-TIMI 38 [see Drug Interactions (7), Clinical Studies (14)].

Thienopyridines inhibit platelet aggregation for the lifetime of the platelet (7-10 days), so withholding a dose will not be useful in managing a bleeding event or the risk of bleeding associated with an invasive procedure. Because the half-life of prasugrel’s active metabolite is short relative to the lifetime of the platelet, it may be possible to restore hemostasis by administering exogenous platelets; however, platelet transfusions within 6 hours of the loading dose or 4 hours of the maintenance dose may be less effective.5.2 Coronary Artery Bypass Graft Surgery-Related Bleeding: The risk of bleeding is increased in patients receiving Effient who undergo CABG. If possible, Effient should be discontinued at least 7 days prior to CABG.Of the 437 patients who underwent CABG during TRITON-TIMI 38, the rates of CABG-related TIMI Major or Minor bleeding were 14.1% in the Effient group and 4.5% in the clopidogrel group [see Adverse Reactions (6.1)]. The higher risk for bleeding events in patients treated with Effient persisted up to 7 days from the most recent dose of study drug. For patients receiving a thienopyridine within 3 days prior to CABG, the frequencies of TIMI Major or Minor bleeding were 26.7% (12 of 45

patients) in the Effient group, compared with 5.0% (3 of 60 patients) in the clopidogrel group. For patients who received their last dose of thienopyridine within 4 to 7 days prior to CABG, the frequencies decreased to 11.3% (9 of 80 patients) in the prasugrel group and 3.4% (3 of 89 patients) in the clopidogrel group.Do not start Effient in patients likely to undergo urgent CABG. CABG-related bleeding may be treated with transfusion of blood products, including packed red blood cells and platelets; however, platelet transfusions within 6 hours of the loading dose or 4 hours of the maintenance dose may be less effective.5.3 Discontinuation of Effient: Discontinue thienopyridines, including Effient, for active bleeding, elective surgery, stroke, or TIA. The optimal duration of thienopyridine therapy is unknown. In patients who are managed with PCI and stent placement, premature discontinuation of any antiplatelet medication, including thienopyridines, conveys an increased risk of stent thrombosis, myocardial infarction, and death. Patients who require premature discontinuation of a thienopyridine will be at increased risk for cardiac events. Lapses in therapy should be avoided, and if thienopyridines must be temporarily discontinued because of an adverse event(s), they should be restarted as soon as possible [see Contraindications (4.1 and 4.2) and Warnings and Precautions (5.1)]. 5.4 Thrombotic Thrombocytopenic Purpura: Thrombotic thrombocytopenic purpura (TTP) has been reported with the use of Effient. TTP can occur after a brief exposure (< 2 weeks). TTP is a serious condition that can be fatal and requires urgent treatment, including plasmapheresis (plasma exchange). TTP is characterized by thrombocytopenia, microangiopathic hemolytic anemia (schistocytes [fragment red blood cells] seen on peripheral smear), neurological findings, renal dysfunction, and fever [see Adverse Reactions (6.2)].5.5 Hypersensitivity Including Angioedema: Hypersensitivity including angioedema has been reported in patients receiving Effient, including patients with a history of hypersensitivity reaction to other thienopyridines [see Contraindications (4.3), Adverse Reactions (6.2)].6 ADVERSE REACTIONS6.1 Clinical Trials Experience: The following serious adverse reactions are also discussed elsewhere in the labeling:

• Bleeding [see Boxed Warning and Warnings and Precautions (5.1, 5.2)]

• Thrombotic thrombocytopenic purpura [see Warnings and Precautions (5.4)]

Safety in patients with ACS undergoing PCI was evaluated in a clopidogrel-controlled study, TRITON-TIMI 38, in which 6741 patients were treated with Effient (60-mg loading dose and 10 mg once daily) for a median of 14.5 months (5802 patients were treated for over 6 months; 4136 patients were treated for more than 1 year). The population treated with Effient was 27 to 96 years of age, 25% female, and 92% Caucasian. All patients in the TRITON-TIMI 38 study were to receive aspirin. The dose of clopidogrel in this study was a 300-mg loading dose and 75 mg once daily.Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials cannot be directly compared with the rates observed in other clinical trials of another drug and may not reflect the rates observed in practice.Drug Discontinuation: The rate of study drug discontinuation because of adverse reactions was 7.2% for Effient and 6.3% for clopidogrel. Bleeding was the most common adverse reaction leading to study drug discontinuation for both drugs (2.5% for Effient and 1.4% for clopidogrel).Bleeding: Bleeding Unrelated to CABG Surgery - In TRITON-TIMI 38, overall rates of TIMI Major or Minor bleeding adverse reactions unrelated to coronary artery bypass graft surgery (CABG) were significantly higher on Effient than on clopidogrel, as shown in Table 1.Table 1: Non-CABG-Related Bleedinga (TRITON-TIMI 38)

Bleeding Related to CABG - In TRITON-TIMI 38, 437 patients who received a thienopyridine underwent CABG during the course of the study. The rate of CABG-related TIMI Major or Minor bleeding was 14.1% for the Effient group and 4.5% in the clopidogrel group (Table 3). The sites contributing to the differences were primarily colon and lung. It is unclear if these observations are causally-related or are random occurrences.Other Adverse Events: In TRITON-TIMI 38, common and other important non-hemorrhagic adverse events were, for Effient and clopidogrel, respectively: severe thrombocytopenia (0.06%, 0.04%), anemia (2.2%, 2.0%), abnormal hepatic function (0.22%, 0.27%), allergic reactions (0.36%, 0.36%), and angioedema (0.06%, 0.04%). Because these reactions are reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure.Blood and lymphatic system disorders — Thrombocytopenia, Thrombotic thrombocytopenic purpura (TTP) [see Warnings and Precautions (5.4) and Patient Counseling Information (17.3)]Immune system disorders — Hypersensitivity reactions including anaphylaxis [see Contraindications (4.3)]7 DRUG INTERACTIONS7.1 Warfarin: Coadministration of Effient and warfarin increases the risk of bleeding [see Warnings and Precautions (5.1) and Clinical Pharmacology (12.3)].7.2 Non-Steroidal Anti-Inflammatory Drugs: Coadministration of Effient and NSAIDs (used chronically) may increase the risk of bleeding [see Warnings and Precautions (5.1)].7.3 Other Concomitant Medications: Effient can be administered with drugs that are inducers or inhibitors of cytochrome P450 enzymes [see Clinical Pharmacology (12.3)].Effient can be administered with aspirin (75 mg to 325 mg per day), heparin, GPIIb/IIIa inhibitors, statins, digoxin, and drugs that elevate gastric pH, including proton pump inhibitors and H2 blockers [see Clinical Pharmacology (12.3)].8 USE IN SPECIFIC POPULATIONS8.1 Pregnancy: Pregnancy Category B - There are no adequate and well-controlled studies of Effient use in pregnant women. Reproductive and developmental toxicology studies in rats and rabbits at doses of up to 30 times the recommended therapeutic exposures in humans (based on plasma exposures to the major circulating human metabolite) revealed no evidence of fetal harm; however, animal studies are not always predictive of a human response. Effient should be used during pregnancy only if the potential benefit to the mother justifies the potential risk to the fetus.In embryo fetal developmental toxicology studies, pregnant rats and rabbits received prasugrel at maternally toxic oral doses equivalent to more than 40 times the human exposure. A slight decrease in pup body weight was observed; but, there were no structural malformations in either species. In prenatal and postnatal rat studies, maternal treatment with prasugrel had no effect on the behavioral or reproductive development of the offspring at doses greater than 150 times the human exposure [see Nonclinical Toxicology (13.1)].8.3 Nursing Mothers: It is not known whether Effient is excreted in human milk; however, metabolites of Effient were found in rat milk. Because many drugs are excreted in human milk, prasugrel should be used during nursing only if the potential benefit to the mother justifies the potential risk to the nursing infant.8.4 Pediatric Use: Safety and effectiveness in pediatric patients have not been established [see Clinical Pharmacology (12.3)].8.5 Geriatric Use: In TRITON-TIMI 38, 38.5% of patients were ≥65 years of age and 13.2% were ≥75 years of age. The risk of bleeding increased with advancing age in both treatment groups, although the relative risk of bleeding (Effient compared with clopidogrel) was similar across age groups.Patients ≥75 years of age who received Effient had an increased risk of fatal bleeding events (1.0%) compared to patients who received clopidogrel (0.1%). In patients ≥75 years of age, symptomatic intracranial hemorrhage occurred in 7 patients (0.8%) who received Effient and in 3 patients (0.3%) who received clopidogrel. Because of the risk of bleeding, and because effectiveness is uncertain in patients ≥75 years of age [see Clinical Studies (14)], use of Effient is generally not recommended in these patients, except in high-risk situations (diabetes and past history of myocardial infarction) where its effect appears to be greater and its use may be considered [see Warnings and Precautions (5.1), Clinical Pharmacology (12.3), and Clinical Studies (14)].8.6 Low Body Weight: In TRITON-TIMI 38, 4.6% of patients treated with Effient had body weight <60 kg. Individuals with body weight <60 kg had an increased risk of bleeding and an increased exposure to the active metabolite of prasugrel [see Dosage and Administration (2), Warnings and Precautions (5.1), and Clinical Pharmacology (12.3)]. Consider lowering the maintenance dose to 5 mg in patients <60 kg. The effectiveness and safety of the 5 mg dose have not been prospectively studied.8.7 Renal Impairment: No dosage adjustment is necessary for patients with renal impairment. There is limited experience in patients with end-stage renal disease, but such patients are generally at higher risk of bleeding [see Warnings and Precautions (5.1) and Clinical Pharmacology (12.3)].8.8 Hepatic Impairment: No dosage adjustment is necessary in patients with mild to moderate hepatic impairment (Child-Pugh Class A and B). The pharmacokinetics and pharmacodynamics of prasugrel in

patients with severe hepatic disease have not been studied, but such patients are generally at higher risk of bleeding [see Warnings and Precautions (5.1) and Clinical Pharmacology (12.3)].8.9 Metabolic Status: In healthy subjects, patients with stable atherosclerosis, and patients with ACS receiving prasugrel, there was no relevant effect of genetic variation in CYP2B6, CYP2C9, CYP2C19, or CYP3A5 on the pharmacokinetics of prasugrel’s active metabolite or its inhibition of platelet aggregation.10 OVERDOSAGE10.1 Signs and Symptoms: Platelet inhibition by prasugrel is rapid and irreversible, lasting for the life of the platelet, and is unlikely to be increased in the event of an overdose. In rats, lethality was observed after administration of 2000 mg/kg. Symptoms of acute toxicity in dogs included emesis, increased serum alkaline phosphatase, and hepatocellular atrophy. Symptoms of acute toxicity in rats included mydriasis, irregular respiration, decreased locomotor activity, ptosis, staggering gait, and lacrimation.10.2 Recommendations about Specific Treatment: Platelet transfusion may restore clotting ability. The prasugrel active metabolite is not likely to be removed by dialysis.13 NONCLINICAL TOXICOLOGY13.1 Carcinogenesis, Mutagenesis, Impairment of Fertility: Carcinogenesis - No compound-related tumors were observed in a 2-year rat study with prasugrel at oral doses up to 100 mg/kg/day (>100 times the recommended therapeutic exposures in humans (based on plasma exposures to the major circulating human metabolite). There was an increased incidence of tumors (hepatocellular adenomas) in mice exposed for 2 years to high doses (>250 times the human metabolite exposure).Mutagenesis - Prasugrel was not genotoxic in two in vitro tests (Ames bacterial gene mutation test, clastogenicity assay in Chinese hamster fibroblasts) and in one in vivo test (micronucleus test by intraperitoneal route in mice).Impairment of Fertility - Prasugrel had no effect on fertility of male and female rats at oral doses up to 300 mg/kg/day (80 times the human major metabolite exposure at daily dose of 10 mg prasugrel).17 PATIENT COUNSELING INFORMATIONSee Medication Guide17.1 Benefits and Risks

• Summarize the effectiveness features and potential side effects of Effient.

• Tell patients to take Effient exactly as prescribed.• Remind patients not to discontinue Effient without first discussing it

• Inform patients that TTP is a rare but serious condition that has been reported with Effient.

• Instruct patients to get prompt medical attention if they experience any of the following symptoms that cannot otherwise be explained: fever, weakness, extreme skin paleness, purple skin patches, yellowing of the skin or eyes, or neurological changes.

• Inform patients that they may have hypersensitivity reactions including rash, angioedema, anaphylaxis, or other manifestations. Patients who have had hypersensitivity reactions to other thienopyridines may have hypersensitivity reactions to Effient.

17.4 Invasive Procedures: Instruct patients to:• inform physicians and dentists that they are taking Effient before any

invasive procedure is scheduled.• tell the doctor performing the invasive procedure to talk to the

1. Don’t skip meals to save up for the big one! Most nutrition plans have you eating more often to keep your metabolism working at constant pace. If you can keep to this, you won’t be as hungry when you get to the restaurant and it will be far easier to not engage in poor eating habits than if you were starving.

2. Drink plenty of water during the day and before the meal! Your water intake should be good anyway to help all of your body functions. However, here it can serve additional purposes. First, it can help to negate the effect that high sodium restaurant foods

may have on your body. And second, it is filling and adds ZERO calories. Try having an entire glass before eating your meal and you’ll be sure to feel fuller before you start.

3. Kindly ask the kitchen to make certain preparation adjustments for your food! No matter what you order you can ask the kitchen to prepare your meals without adding extra salt, getting dressings or sauces on the side, not cook your food in butter or limit the oil that they cook your food in. These small suggestions can have large nutritional benefits.

Perhaps you’ve started an exercise and nutrition regimen and you’re starting to see the results of all your hard work! However, it’s just then that one of your

close friends throws you a curve ball and says “why don’t we go out tonight to your favorite restaurant?” Immediately you get anxious because you know that this very event could plunge you back into that downward spiral and negate your hard work. You may even consider declining the invitation…….but you shouldn’t! You should be scared to eat out but rather be informed and have a game plan. We will examine 6 things you can do to stay the course, even in a restaurant.

4. Limit your portion size! Most restaurants tend to serve you way more than a normal portion when they bring you your food. Knowing this, you can do one of a couple of things. First, you could ask the server to bring a box with your meal and pack up half of it before you even take your first bite. This will take away the desire to over eat. Second, you could split the meal with a friend you are dining with. This again would accomplish the same goal and not leave you bringing home unhealthy left overs.

5. Stop eating when you’re satisfied! The words “food coma” or “eating for the sake of eating” should not enter your vocabulary. When you are full, STOP! Also, the order in which you eat your foods could be of a benefit too. Eat the protein and vegetable on the plate before eating the starch. They are usually more beneficial to the body and more satiating foods so you should become fuller quicker. You may not even want to dig into that pile of mashed potatoes using this trick.

6. Limit alcohol! Alcoholic drinks contain empty calories, lots of sugar and when you have enough of them, initiate really poor eating habits. Have a glass of red wine with a meal is not a big deal at all but 4 margaritas is like having a separate bad meal in addition to the one you are going to eat. You will be thanking yourself both that night AND the next morning with this suggestion.

So many people slip or trip and fall down each year. Many of them get hurt, some seriously. This is most common form of accident that causes injuries. Ok, you

slipped in some liquid or tripped over a piece of torn carpet, now who is responsible? The answer is not as easy as you think. Sometimes the property owner is responsible for your injuries but most of the time he or she is not.The majority of people that call me for this type of accident don’t really know why they fell. Most everyone seems to be under the misconception that if you fall down on someone else’s property, that they are responsible. Nothing could be further from the truth. The first question I ask all of them is, what did the property owner do wrong that caused you to fall? In order for the premises owner to be legally responsible for your injuries one of the following must be true.

1. The owner of the premises or an employee must have caused the spill, worn or torn spot, or other slippery or dangerous condition.

2. The owner of the premises or an employee knew of the dangerous condition but did nothing to remedy it.

3. The owner of the premises or an employee should have known of the dangerous condition because a “reasonable” person taking care of the property would have discovered and removed or repaired it.

The third situation is the most common but is the most difficult to prove. The most common fall is slipping in liquid in a super market or other retail store. It is rarely the fault of the premises owner that the liquid is there as it is typically spilled by another shopper. It is also nearly impossible to prove the premises owner or an employee had actual knowledge of this spill, as they always deny it. Therefore the only way to bring a claim against the store owner is to prove that the spill existed for a sufficient length of time that it should have been discovered and cleaned up. Surveillance is one way however, the insurance company will not turn that over to you voluntarily or may simply tell you it wasn’t working or was erased. If it benefits the store owner however, they usually hand deliver a copy. The only other way is to look for evidence that would help indicate that the dangerous condition had been there for a while. I always inquire about footprints, buggy tracks or if the liquid was dirty from traffic or already drying in areas. Photos of the area are invaluable in proving causation. Nearly everyone has a cell phone with a camera. Take photos of the area and of the witnesses. Get names and phone numbers of the witnesses as well.

The insurance company will nearly always maintain the position that it was your fault. They will blame it on the the shoes you were wearing or your inattentiveness to the surroundings. They also love to argue that no matter how bad the dangerous condition was, it was open and obvious and you should have avoided it.

So if you slip or trip and fall and suffer injuries you may very well have a viable claim against the owner of the property. The best way to determine that is to call an experienced lawyer like myself and let me evaluate it for you. It will cost you nothing and will greatly increase your chances of not only making a recovery but obtaining a significant one.

A Beautiful SmileBacteria can live in your mouth in the form of plaque, causing cavities and

gingivitis, which can lead to periodontal (gum) disease. In order to keep your mouth clean, you must practice good oral hygiene every day.

WHAT IS PLAQUE?Plaque is a sticky layer of material containing bacteria that accumulates on teeth, including where toothbrushes can’t reach. Many of the foods you eat cause the bacteria in your mouth to produce acids. Sugary foods are obvious sources of plaque, but there are others that you might not realize can cause harm. Starches—such as bread, crackers, and cereal—also cause acids to form. Plaque also produces substances that irritate the gums, making them red, sensitive, and susceptible to bleeding. This can lead to gum disease, in which gums pull away from the teeth and form pockets that fill with bacteria and pus. If the gums are not treated, the bone around the teeth can be destroyed and teeth may become loose or have to be removed.

HOW CAN I GET RID OF PLAQUE?The best way to remove plaque is by brushing and cleaning between your teeth every day. Brushing removes plaque from the tooth surfaces. Brush your teeth twice per day with a soft-bristled brush. The size and shape of your toothbrush should fit your mouth and allow you to reach all areas easily. Use an antimicrobial toothpaste containing fluoride, which helps protect your teeth from decay. Clean between the teeth once

a day with floss or interdental cleaners to remove plaque from between the teeth, where the toothbrush can’t reach. Flossing is essential to prevent gum disease.

HOW DO I BRUSH AND FLOSS MY TEETH?The American Dental Association recommends the following techniques for brushing and flossing your teeth:

BRUSHING• Place your toothbrush at a 45-degree angle against the gums.• Move the brush back and forth gently in short (tooth-wide) strokes.• Brush the outer tooth surfaces, the inner tooth surfaces, and the chewing surfaces of the teeth.• Use the tip of the brush to clean the inside surfaces of the front teeth, using a gentle up-and-down stroke.• Brush your tongue to remove bacteria and freshen your breath.

FLOSSING• Break off about 18 inches of floss and wind it around the middle fingers of each hand. Hold the floss tightly between your thumbs and forefingers.• Guide the floss between your teeth using a gentle rubbing motion.• When the floss reaches the gum line, curve it into a C shape against one tooth. Gently slide it into the space between the gum and the tooth.

• Bring the floss back toward the contact point between the teeth and move the floss up or down the other side, conforming the floss to the shape of the tooth.• Hold the floss tightly against the tooth. Gently rub the side of the tooth, moving the floss away from the gum with up-and-down motions.• Repeat this method on the rest of your teeth.

IS THERE ANYTHING ELSE I CAN USE TO CLEAN MY MOUTH?A mouth rinse, in addition to daily brushing and flossing, can increase the cleanliness of your mouth. Antimicrobial mouth rinses reduce bacteria and plaque activity, which cause gingivitis and gum disease. Fluoride mouth rinses also help reduce and prevent tooth decay. Always talk to your dentist about any new products you are interested in trying. Not everyone should use a fluoride mouth rinse. For instance, fluoride rinses are not recommended for children ages 6 or younger because they may swallow them. Always check the manufacturer’s label for precautions and age recommendations and talk with your dentist about the use of fluoride mouth rinse.

BAY AREA REAL ESTATE VALUES SEE BIG BUMPS

In 2013, the Tampa Bay housing market showed definitive signs of rebounding with historically low inventory and bidding wars over real estate, but will this progress continue this year? Economists and industry experts base their market predictions for the future on several key trends and patterns.

THE ROAD TO RECOVERY• The National Association of Realtors named Tampa one of the 10 housing markets anticipated to make turnaround.

• Non-distressed homes made up 67 percent of realtors’ sales last year, a significant improvement from 2011 when short sales and foreclosures accounted for over 50% of all real estate transactions.

• The upsurge in home values contributed to the significant

decline in the number of “underwater” homes, which plummeted from 45 to 30 percent.

• Tampa Bay housing sales and prices climbed to post-recession highs in 2013. According to a Tampa Bay Times article published in early January, more than 35,000 homes were sold last year and the median selling price was $155,000, the highest rate since 2008.

new study released by the rental truck company Penske and both Tampa and St. Petersburg were included among Livabilty.com’s Top 100 Most Liveable Cities in America (Tampa made the top 20 and St. Petersburg was ranked No. 59).

• The Forbes list is based around the “Equilibrium Home Price,” a measure that tracks the average housing prices for each market without factoring in uncharacteristic events like the 2008 housing collapse. The measure assumes that prices will eventually return to their previous level, so when homes fall below the equilibrium price, buyers are making a smart investment and can expect a good return. Since Tampa Bay homes are 13 percent below the Equilibrium Home Price, the Forbes list predicts 32 percent growth in our area over the next three years.

Moving forward into the remainder of 2014, our local real estate market looks strong. Interest rate hikes may affect the volume of home sales marginally, however many economists believe that the minimal impact that higher rates may have will be offset by heightened consumer confidence.

Look for a strong stable market in Tampa Bay for the remainder of the year.

Tampa Bay housing sales and prices climbed to post-recession highs in 2013. According to a Tampa Bay Times article published in early January, more than 35,000 homes were sold last year and the median selling price was $155,000, the highest rate since 2008.

The Tampa Bay AirFest Team has built an incredible airshow to include performers and a static aircraft line-up for 2014. Faced with the almost insurmountable task

of planning an air show in only three months time, coupled with the reality of a new fiscal environment, the 50+ member AirFest committee put together the best AirFest line-up ever! This would not have been possible without the tremendous support from the Tampa Bay area community.

This year’s show will feature stars of the Weather Channel’s Hurricane Hunters and Coast Guard Florida. This year’s AirFest ramp will be entirely packed with military aircraft, warbirds, military vehicles, equipment from all military branches, along with the displays of the incredible aviation and ground equipment of our local law enforcement partners.

Scheduled performers include: Mike Goulian (Extra 330SC), Rob Holland (MXS/RH), Melissa and Rex Pemberton (Edge 540 and Wingsuit demo), Matt Younkin (Beech 18), AeroShell (4 x AT-6), Randy Ball (Mig-17F), GEICO Sytypers (6 x SNJ-2), DAV Flight Team (B-25), Scott Yoak (P-51), Jerry “Jive” Kerby (T-28 & RV-8), Jim Tobul (F4U), Larry Labriola (L-39C), the USSOCOM Parachute Jump Team and the US Air Force Thunderbirds.

Military aircraft and equipment on static display will include: C-5, C-17, KC-10, KC-135, B-52, B-1, F-16, F-15, A-10, T-6, T-38, C-130H2, AC-130, WC-130, C-21, HH-60, Draken Aviation A-4K Skyhawk & MB-339CB, and many other vintage warbirds.

Don’t forget the gates open at 8 a.m. on Saturday and Sunday March 22 & 23. Expect airport style security, so don’t bring anything to the show that you wouldn’t bring on an airline flight. Coolers, glass containers, pets, fireworks, and large tote bags are all prohibited to ensure the security and safety of all attendees. Bicycles, roller blades, etc., are acceptable modes of transportation to the event; however, they will not be allowed on the flightline.

Due to overwhelming demand from the public, for the first time ever AirFest is offering limited reserved seating in the Flight Line Club for $15. This seating area includes covered and open air seating along with designated special access to food and drink concessions. Tickets can be purchased in advance by following the link on the left or on the day of show (this area will absolutely sell out so advance tickets are highly recommended).

In order to expedite entrance and maximize on-base parking, we ask that anyone with a military ID card please enter through the Bayshore gate and park in the new combined south ramp consolidated parking location.
