Woods Pond Dam EAP Statement of PUrpose

I. STATEMENT OF PURPOSE

This Emergency Action Plan (EAP) has been prepared on behalf of the General Electric

Company (GE) for Woods Pond Dam, which is owned by GE. The purpose of this EAP is

to:

1. Define the responsibilities of various officials involved in emergency management;

2. Provide procedures designed to identifY, in a timely manner, unusual and unlikely

conditions which may endanger the structural integrity of Woods Pond Dam;

3. IdentilY actions to take to mitigate an ongoing or developing situation; and

4. Provide procedures to notilY the appropriate emergency management officials, if

possible, of impending or actual failure of the dam.

In accordance with the regulations of the Massachusetts Depatiment of Conservation and

Recreation, Division of State Parks and Recreation (DCR) (fOlmerly known as the

Environmental Management (DEM)) Regulations (Chapter 302 CMR 10.06, November 4, 2005), Woods Pond Dam is designated as a "large" size dam with a "significant" hazard

(Class II) potential. This size classification is based on the storage capacity that occurs

during the spillway design flood, while the hazard potential classification is based on the

presumption that failure of the dam may result in the loss of life and significant property

damage immediately downstream ofthe dam.

According to DCR Regulations (Chapter 302 CMR 10.11, November 2005), all dams

classified as "high" hazard (Class I) shall have an EAP. Ifthe Commissioner requires it, a

facility with a non-high hazard dam shall also provide an EAP. DCR requested that GE

prepare an EAP for Woods Pond Dam; GE prepared this EAP in response to that request and

as an exercise of good engineering judgment. The EAP conforms to the requirements for

EAPs outlined in Section 10.11 of the DCR Regulations. All EAPs are subject to approval

by the DCR. The dam owner shall submit a copy of the proposed EAP to the local and state

emergency agencies and all local emergency coordinators involved in the plan for review.

The owner shall incorporate applicable recommendations received and provide the

documented recommendations in appendices to the final version of the EAP. Annually, the

owner shall review the EAP and update it as necessary

Woods Pond Dam EAP Statement ofPurpose

The EAP shall, at a minimum, contain the following:

I. The identification of equipment, manpower, and material available for

implementation ofthe Plan;

2. A notification procedure for infolming local emergency agencies;

3. A topographic map showing the stream which will be flooded; and

4. A procedure for warning nearby local residents if failure of the dam is imminent

and a listing of addresses and telephone numbers of downstream residents that

may be affected by failure of the dam.

As described in Section IV, "Emergency Detection, Evaluation and Classification," two alert

conditions have been adopted for Woods Pond Dam:

I. Condition A - Failure Is Imminent or Has Occurred, and

2. Condition B - Potential Failure Situation Is Developing

A notification flowchart for each of these alert conditions is presented in this EAP.

II. PROJECT DESCRIPTION

A. Background

The original Woods Pond Dam was reportedly constructed around 1864 on the Housatonic

River near the towns of Lee and Lenox, Massachusetts, as shown on Exhibit I. The original

structure consisted of a 9-foot-high rock-filled timber crib overflow dam with a 14-foot-high

earthfill embankment parallel to the river channel as its east (left) abutment (left and right

refer to the side of the river channel when viewed looking in the downstream direction).

There are no design drawings or information available for any of the original project

structures. The original dam structure was replaced in 1989 with a new concrete overflow

spillway and new abutments of concrete and steel sheet pile. The earthfill (raceway) embankment separates the main river channel fi'om the 30 to 50-foot-wide raceway channel

along the left bank of the river. The raceway channel conveys flow to a small mill pond,

which at one time was a forebay for a water-powered mill that has since been retired. The

dam impounds the Woods Pond Reservoir, part of the Housatonic River Valley Wildlife

Management Area.

Prior to the 1989 rehabilitation, Woods Pond Dam was in a deteriorated condition. The original project could not pass a flood greater than about the lO-year flood event without

overtopping the earthen raceway embankment. Oveltopping of the embankment could have

caused a breach of the embankment, resulting in the uncontrolled release of the reservoir

water and sediments that had settled to the bottom ofthe reservoir.

Beginning in 1979, studies and investigations were performed to assess the condition of the

dam and develop recommendations for repair and rehabilitation of the dam. In 1983, the

timber planking and heavy gage sheet metal on the main overflow dam were replaced with an

18-inch-thick reinforced concrete cap. After placement of the cap, a leak developed at the

east abutment masomy wall, presumably due to the cut-off of natural seepage through the

timber crib dam. The joints in the masomy wall were filled with mortar shortly after the

1983 construction, temporarily stopping the leakage, but contributing to pressure buildup

behind the wall, causing lateral movement ofthe wall.

In 1988, a plan was developed to rehabilitate thc dam to provide a safer, more reliable project

structure. The rehabilitation was carried out in two stages. The first stage consisted of the

construction of a raceway closure structure with concrete stoplogs to divelt flow away fi'om

the raceway and mill pond. The purpose of the closure structure is to protect the raceway

embankment from oveliopping and potential failure during high flows. The second stage

consisted of the constmction of a replacement spillway and non-overflow gravity dam a ShOli

distance downstream of the original spillway. A rockfill berm was also conshucted on the

west (right) bank of the river between the original spillway and new spillway, and the top

2.5 feet the original timber crib spillway was demolished. The original sluice gate stmcture

at the downstream end of the raceway embankment was rehabilitated in 1991. This stmcture

controls the water level in the Mill Pond.

Additional modifications to the project were perfolmed in 2001 to address recommendations

made in the January 2001 Stmctural Integrity Assessment Repoli, and in 2003 to address

recommendations made in the May 2003 Stmctural Integrity Assessment Report. The 2001

modifications included repair to deteriorated masomy at the raceway closure stmcture;

removal of trees and other vegetation along the raceway embankment; regrading of the

raceway channel slopes; and removal of the masonry wall and reworking of the riprap at the upstream raceway embankment. The 2003 modifications included removal of trees and other

vegetation and the placement of riprap along the river side of the raceway embankment.

Based on the stmctural integrity assessment inspection conducted subsequently in

November 2004, the dam, raceway closure stmcture, and riprap constmcted in 1989 are in good condition, and there was no indication of deterioration or distress to the spillway and

tailrace.

MWH conducted a visual inspection in November 2005 and made recommendations for

some minor physical modifications. GE is in the process of implementing these

recommendations. For details, refer to 2005 Stmctural Integrity Assessment RepOli by

MWH, dated April 2006.

A general plan of the rehabilitated project is shown on Exhibit 2. Drawings for the 1989

rehabilitation are included in Appendix A for reference. The "General Design Report for

Woods Pond Dam Rehabilitation" for the 1989 rehabilitation was filed with the DCR and is

included in Appendix B of the March 1999 Woods Pond Dam Inspection Report.

B. Project Features

The present dam consists of a 140-foot-Iong concrete overflow spillway, a concrete non­

overflow gravity section at the west (right) abutment, and a concrete and steel sheetpile

raceway closure stmcture at the east (left) abutment, all constmcted since 1989. All the dam

stmctures are founded on bedrock. The dam has a maximum height of approximately 14 feet,

with an ogee-shaped spillway with a crest elevation of 948.3 feet National Geodetic Vertical

Datum (NGVD). The west (light) abutment is a concrete gravity structure with a sloped

downstream face and a top elevation of 954.0 feet NGVD. The raceway closure structure

forms the east (left) abutment of the dam and has a top elevation of 954.0 feet NGVD. The

drawings for the dam and raceway closure structure are provided in Appendix A. The CUlTent

structure is completely independent of the oliginal structure and does not rely on any of the

oliginal structures for support.

The current structures are founded on shallow bedrock, which is veliically bedded and is

generally fine grained, hard with valiable medium to close joint spacing. This was

detelmined during the 1988 geotechnical investigation program which included soil bolings

and water pressure testing of the rock. Details of the subsurface field investigation can be

found in the "General Design Report for Woods Pond Dam Rehabilitation" filed with the DCR in 1989 and included in the March 1999 Woods Pond Dam Inspection RepOli.

The raceway embankment upstream of the dam is earthen fill with liprap over the surface and

is no longer a water-retaining shucture serving no dam safety function. Failure of this

portion of the raceway embankment would have no impact on the safety or integrity of the

dam.

Downstream of the dam and raceway closure shucture is an earthen embankment protected with riprap along the length of the channel. This embankment is located between the tailrace

of the current overflow spillway and the raceway channel. The embankment has a crest

width of about 8 to 10 feet. The water elevation in the raceway is a few feet higher than the water elevation in the tailrace and river channel downstream of the dam.

III. POTENTIAL FLOOD LIMITS

Exhibit 3 shows a map of potential flood limits due to the failure of Woods Pond Dam under

"Sunny-day" and "Flood-day" conditions. Flood limits are for failure when the abutments

would be overtopped by two feet. "Sunny-day" means a fair-weather, non-rainy day. "Flood­

day" means a rainy, non-fair-weather day. The term "day" includes nighttime. The potential

flood limits are plotted on Exhibit 3 to show the maximum inundation area due to dam failure

to the point downstream where the water would be contained within the banks of the river.

These limits were estimated based on the infolmation and data provided in two repOlis

entitled:

1. "Woods Pond Dam, Phase 1 Inspection Report, Lee and Lenox, Massachusetts,

prepared for EPA Region 1, prepared by U.S. Anny Corps of Engineers, Omaha District, September 1987,"

2. "Flood Insurance Study, Town of Lee, Massachusetts, Berkshire County by Federal Emergency Management Agency, December 1, 1981."

IV. EMERGENCY DETECTION, EVALUATION, AND CLASSIFICATION

This EAP has been prepared to aid GE personnel in the timely detection, evaluation, and

classification of an existing or potential emergency situation at the dam. The EAP provides

two classifications of dam failure emergency according to their severity and urgency:

Condition A - Failure is Imminent or Has OccUlTed, and Condition B - Potential Failure

Situation Is Developing. The conditions or measures for detection and evaluation of an

existing or potential emergency are described below.

For purposes of this EAP, GE has designated a Facility Manager and an Alternate Facility

Manager (as specified in Section V). The Facility Manager shall have plimmy responsibility

for carrying out this Plan on behalf of GE. In the event that the Facility Manager is

unavailable, the Alternate Facility Manager shall be the point of contact and shall have all the duties and powers of the Facility Manager as set forth in this Plan. In addition, GE has pre­

selected contractors, also specified in Section V, to assist in emergency response under this

EAP.

A. Condition A - Failure is Imminent or Has Occurred

The following conditions may lead to the "Failure is Imminent or Has Occun-ed" emergency situation. These conditions should not be considered inclusive of all conditions that might

occur:

• Rapidly developing stmctural problems involving the spillway, raceway embankment,

abutments, and appul1enant stmctures. Some of the potential causes of developing

stmctural problems are listed on Exhibit 4.

The notification procedures of Section V, "Notification Flowcharts and Contact Numbers"

and given on the flowchart for Condition A (Exhibit 5), should be invoked if any of the

situations listed on Exhibit 4 are observed. An imminent situation at the dam site will

probably manifest itself as an excessive increase in flow in the Housatonic River and/or an

unexpected drop or lise in water level in the reservoir, river channel, raceway channel or mill

pond.

When failure is observed, the Observer (as defined in Section V) shall inform the Facility

Manager. The Facility Manager shall decide the severity of the problem and invoke the

notification procedure if appropriate.

The message fi'om the Facility Manager to the persons identified on the flowchart for this

situation (Exhibit 5) will contain the following information:

• Name and title (GE position) of the person making the call.

• A statement that this is an emergency call, due to a reported failure or imminent

failure of Woods Pond Dam.

• A statement that the person receiving the message must initiate contacts as per the

flowchart for "Condition A - Failure is Imminent or Has Occurred."

• A statement that warning and evacuation should begin immediately.

B. Condition B - Potential Failure Situation is Developing

The conditions that may lead to the "Potential Failure Situation is Developing" emergency

situation are also listed on Exhibit 4. Upon confinuing any of the situations listed on Exhibit

4, the notification procedures will be implemented by the Facility Manager. There will be

two messages from the Facility Manager for this situation. The first message will contain the

following infonnation:

• Name and title (GE position) of the person making the call.

• A statement that a potential failure situation is developing at Woods Pond Dam that

may lead to failure ofthe dam.

• A statement that the person receiving the message should put his or her contacts on

alert for a second notification message.

• A statement that emergency personnel should be ready for an evacuation process.

If failure of the dam becomes imminent, the second message from the Facility Manager shall

be as given in this section under "Condition A - Failure Is Imminent or Has Occurred."

If the dam failure is averted and the developing situation is mitigated, the emergency will be

called off via the same communication channels. In such an event, the message will contain

the following information:

• Name and title (GE position) of the person making the call.

• A statement that the potential failure situation at Woods Pond Dam has been averted,

and that the recipients' contacts should stand down fi'om emergency alert.

V. NOTIFICATION FLOWCHARTS AND CONTACT NUMBERS

The flowcharts shown in Exhibits 5 and 6 summarize who is to be notified and who is

responsible for notifying individuals and organizations in the order designated for the

following emergency situations:

A. Faillire Is Imminent or Has Occurred (Condition A): COiTesponds to situations where

a failure either has occlmed, is occurring, or obviously is just about to occur. The

notification flowchart for Condition A is printed on red paper.

B. Potential Failure Situation is Developing (Condition B): COl1"esponds to situations

where a dam failure may develop, but preplanned actions may prevent or mitigate

failure. Generally, there is more time available under this condition than under

Condition A to take preventative action and bring the situation under control. The

notification flowchart for Condition B is printed on yellow paper.

A copy of the flowchmis shall be prominently posted near the telephone or radio transmitter

of each person identified on the flowchatis as a caller. The key personnel of GE and police depatiments shall have copies in their offices and at homes (see Appendix C for EAP

distribution).

Detailed information supplementing the flowchmis is provided in Section N - Emergency

Detection, Evaluation, and Classification. The location of the dam, reservoir, and streams are

described in the Section II - Project Description. Potential inundation limits are discussed in

Section III - Potential Flood Limits. In addition to the individuals and agencies listed as

contacts in Actions 1 through 8 for Condition A and Condition B, GE has additional notification requirements under the Consent Decree for the GE-PittsfieldlHousatonic River

Site (the Consent Decree). The contacts for those notifications are listed in Action 9.

Paragraph 90 of the Consent Decree requires that notice be given "[i]n the event of any action

or occurrence during the performance of the Work which causes or threatens a release of

Waste Material from the Site that constitutes an emergency situation or may present an

immediate threat to public health or welfare or the environment." The phrase "constitutes an

emergency situation or may present an immediate threat to public health or welfare or the

environment" is further defined to mean "an unforeseen combination of circumstances, or the

conditions resulting from such circumstances, not normally anticipated to occur as part of the

Work, that require immediate action to avoid harm or an immediate threat of hmm to human

health, welfare or the environment."

VI. GENERAL RESPONSIBILITIES UNDER THE EAP

Failure of Woods Pond Dam would present a potential hazard to downstream areas. Areas of

concern at the project include a few houses located near the gravel pit and sewage disposal

plant. Numerous downstream roads and houses, primarily in the flood plains and close to the

banks of the river, would be flooded. Refer to the inundation map, Exhibit 3.

Once an emergency condition is identified, it is important to the success of the EAP that

responsible personnel respond immediately. Specific responsibilities of the persons or

organizations for the maintenance and operation of the dam and implementing various phases

of the plan are discussed below.

A. General Electric Responsibilities

GE has a plant in Pittsfield, Massachusetts, located approximately 12 miles upstream from

the dam. A member ofGE's Corporate Environmental Programs (CEP) stafffi'om that plant,

or a contractor designated by GE, will perform a routine visit to the dam site and its appurtenant stlUctures at least monthly (or more frequently as wan-anted by weather, river

levels, or other conditions). This person will be the "Observer," as defined in Section V

(''Notification Flowchatis and Contact Numbers") in case of an occun'ence at the dam. The person will carry a two-way radio and/or telephone and/or pager, and will be properly

equipped to perform a watch during nighttime surveillance and severe weather.

In addition, the Facility Manager or the designated contractor wiII conduct an inspection of

the dam and its appurtenant sttuctures at least quarterly to identify any changes in site

conditions that might indicate a problem for the dam. (This inspection wiII be combined with

one of the monthly site visits described above.). GE will provide a repOli on each such

quatierly inspection to the Lead Administrative Trustee (LAT) designated under Paragraph 113 of the Consent Decree.

GE wiII also arrange for a licensed professional engineer to conduct an assessment, at least

once every two years, of the structural integrity of Woods Pond Dam, and to provide to GE a

repOli on that assessment and any recommendations of measures necessmy to maintain dam

integrity. GE will provide a copy of each such report to the LAT.

The GE Facility Manager will have the authority to declare an emergency at Woods Pond

Dam or to change the degree of severity of an existing occurrence.

Woods Pond Dam EAP General Responsibilities Under The EAP

Upon receiving information of a threatening situation at Woods Pond Dam fi'om the Observer

(or fi'om other sources), it will be the primaty responsibility of the Facility Manager to direct

the designated person(s) of GE to proceed to the site. Included in this group will be one of

the persons who conduct routine inspections of the dam. It is the responsibility of this person

to know the signs of developing hazards such as those noted in Section IV, "Emergency

Detection, Evaluation and Classification," and to inform the Facility Manager immediately.

As a result of the site inspection, the Facility Manager will declare an emergency situation if

walTanted, and will designate the proper degree of severity. The Facility Manager will also

initiate all notification calls.

The Facility Manager will serve as the emergency coordinator for all local agencies and will

direct all mitigation effOlis at the dam site. The Facility Manager will also set up an appropriate central emergency headquarters fi'om which all emergency operations will be

directed.

In notifYing the contacts involved with the situation, the Facility Manager will briefly state (after having queried the Observer for the information):

I. Name of dam 2. Emergency condition

• Condition A - Failure is Imminent or Has Occurred, or • Condition B - Failure Situation is Developing

3. Severity of situation 4. Type of failure occurring or developing (e.g. overtopping, cracking or piping)

5. Time of observation, and time of failure, if occulTed and known, otherwise estimate

The Facility Manager will keep local authorities and news media infonned of changing

conditions at the dam site. However, the plan is not intended for the Facility Manager or his

designated person to assume the responsibility of governmental authotities duting an

emergency.

The Facility Manager is the EAP coordinator as described in Pati E of this section. The

Facility Manager will closely coordinate effolis with the staff at the dam site and local

agencies to serve as a communications hub for the duration of the emergency.

The officials who must be notified are shown in Section V ("Notification Flowcharts and

Contact Numbers") and on Exhibits 5 and 6 (Notification Flowcharts). As GE's top

communications representative and day-to-day manager during the emergency, the Facility

Manager will be at the top of the command chain and will speak for GE and engineering

personnel at the site.

B. Responsibility for Notification

The Facility Manager is authorized to notifY authorities of the emergency, including the

police in the Towns of Lenox, Lee and Stockbridge, as well as the Berkshire County Sheriffs

Department, Emergency Management offices and other federal, state and local agencies listed

on the notification flowcharts (Exhibits 5 and 6). The Facility Manager will have the

authority to issue statements to the press concerning the situation at the dam.

The town police and fire departments will serve as the lead authorities during emergency

situations. Once notified of an emergency condition, they will be responsible for notifYing

potentially affected downstream residents (listed in Appendix D) and coordinating warning

and evacuation procedures with other authorities, including the emergency management offices and other county and town departments (fire, ambulance, and transportation).

Major responsibilities ofthese authOlities include:

• Issuing wa1l1ings and evacuating residential and commercial buildings as needed

• Controlling traffic and placement of bani cades on roads

• Coordinating with federal and state agencies

• Coordinating utility services

• Setting up health centers

• Informing the public through radio and television media

• Providing shelter for evacuated people

• Maintaining law and order

The chiefs of police for the towns of Lenox, Lee and Stockbridge will serve as local

coordinators for their municipalities. These local coordinators will keep in contact with the

Facility Manager and will be responsible for requesting support from other local, county or

state agencies as needed. The Facility Manager will be responsible for advising these local

coordinators of the need to warn andlor evacuate local residents depending upon the

conditions at the dam site.

Local coordinators will have the authOlity to release that information necessaty to assure the

safety of tile public and to expedite an orderly evacuation when required. GE will coordinate

press releases conceming conditions at the dam with local coordinators. The local public

infOlmation centers will be responsible for infonning the public of the provisions of this plan

and coordinating public input.

C. Responsibility for Evacuation

Following notification by the Facility Manager, the waming and evacuation of residents, as

necessary, will be performed by the local police, fire and civil defense departments.

Residents will be moved to pre-designated temporaty shelters.

The Lenox and Lee DPWs and the Stockbridge Highway Depatlment will take the lead roles

in placing and manning barricades on the affected roads, with assistance from police, to

prevent motorists from enteling the inundated areas. A map showing potential flood limits is

attached as Exhibit 3.

D. Responsibility for Duration, Security, Termination, and Follow-Up

As noted above under GE's responsibilities, the Facility Manager will keep local autholities,

primarily through the police depatiments and county emergency management offices, advised

of conditions at the dam by phone or messenger.

The local authorities will be responsible for termination of the emergency response activities,

with the advice of the Facility Manager.

For any emergency situation, after completion of all notification responsibilities, all officials,

including GE personnel, will maintain a log, report, or tape recording of all notifications and

decisions made by their offices for the duration of the emergency. The reports should include

the following infOlmation:

• Subject: (what happened and name or title of incident)

• Time and date:

• Location: (where incident OCCUlTed)

• Summary of incident:

E. EAP Coordinator Responsibility

The Facility Manager will be responsible for activities related to the EAP, including

reviewing and revising the plan, distributing copies of the flowchatis and plan, and

establishing training for personnel in charge of the dam (see Appendix C). Also, the Facility

Manager will be the contact should any questions arise about the plan.

VII. PREPAREDNESS

Preparedness actions are taken to prevent a dam failure or to help reduce the effects of a dam

failure and to facilitate a response to emergencies.

A. Surveillance

An emergency situation may be detected by inspection of the dam or by observation of

reservoir levels.

SUlveiIlance of the dam wiII be made in accordance with the procedures described in Section

VI.A (General Responsibilities under the EAP - General Electric Responsibility). If night

inspections are made, the inspector should be accompanied by a second individual, and they

wiII carry flashlights and a two-way radio (walkie-talkie) or cellular phone to communicate.

Any observance of a failure or potential failure situation will be recorded and action shall be

taken immediately. Such situations are given in Section IV ("Emergency Detection,

Evaluation, and Classification").

Based on experience, a breach could fully develop in an hour or less after breach initiation.

The flood wave could reach the downstream residences in a few minutes. Therefore,

readiness for quick implementation of the EAP is impOliant.

B. Response During Periods of Darkness

Once an emergency situation is detected, notification procedures during periods of darkness

will be the same as during daylight hours. Home phone numbers are provided on the

notification flowchmis to facilitate contact during non-business hours.

For any response actions taken during periods of darkness, at least two individuals should be

present, and they will cany flashlights and a two-way radio (walkie-talkie) or cellular phone

to communicate.

Due to the availability ofphones and two-way radios, no special procedure wiII be needed for

contacting people on the appropriate flowchart.

C. Access to Site

Access to dam site will be by car and foot.

D. Response During Weekends and Holidays

Once an emergency situation is detected, notification procedures during weekends and

holidays will be the same as during weekdays. Home telephone numbers provided on the

notification flowcharts will be used during weekends and holidays. Notification response

time during weekends and holidays is expected to be similar to response times for

emergencies occurring during weekdays.

E. Response During Periods of Adverse Weather

During a severe stom1, some roads may be under water that would be dry during fair weather conditions. Consequently, emergency-response vehicles may have to bypass roads that are

under water in order to get to the dam or affected areas.

Response during adverse weather (a storm) following observation of an emergency condition,

comprising notifications and waming to downstream areas, is expected to take more time

than under normal conditions.

F. Availability and Use of Alternative Communication Systems

Telephones will be the main system of communication on and off the dam site. Also, two­

way radios (walkie-talkies) will be available for site communication.

If the above methods fail, communications on-site and with emergency agencies in towns of

Lenox, Lee and Stockbridge will proceed by messenger.

G. Emergency Supplies and Resources

GE will be responsible for emergency supplies and resources for on-site use. GE will rely on

its designated contractor to assist in providing such supplies and resources, as described

below.

GE and its designated contractor will provide men and equipment for emergency repair of the

dam. GE's designated contractors and a list of the equipment they are responsible for

providing, are listed in Appendix E. Fill, comprised of earth and rock, would be obtained

fi-om available borrow areas. However, due to possibly rapid development of a breach and

consequently a ve1y Sh011 warning time, emergency repair of a dam will not be relied on as a

preventative or control action.

APPENDIXB

INVESTIGATION OF POTENTIAL FLOOD LIMITS

Areas and streams downstream from Woods Pond Dam (Valley Mill Dam) that would most

likely be inundated in case of a failure of the dam have been identified.

A dam break study was not perfonned for Woods Pond Dam because it was not needed as per

requirements of the DCR. The stream affected due to a failure would be the Housatonic

River. There would be some backwater effect on small hibutaries joining the Housatonic

River (fi'om upstream to downstream: a small urmamed stream, Washington Mountain Brook, Codding Brook, small unnamed stream, Goose Pond Brook and Hop Brook). Further

downstream, the backwater effect on the tributaries might not be significant. Exhibit 3 shows

a portion of the Housatonic River and hibutaries downstream fium the dam.

Although the DCR does not require a dam failure inundation map for "significant" hazard

(Class II) dams, the identification of roads and residences likely to be affected due to a failure of Woods Pond Dam was considered necessary for warning purposes. This required some

approximation of inundation boundaries along the Housatonic River. To draw these

inundation boundaries, guidance was obtained from two previous reports. First was the dam

break analysis made by U.S. Army Corps of Engineers, Omaha District (COE), presented in

the Woods Pond Dam Phase 1 Inspection Report, Lee and Lenox, Massachusetts, September

1987. The second report was the Flood Insurance Study for the town of Lee, Massachusetts,

Berkshire County, Community No 250028, December 1981 by the Federal Emergency

Management Agency (FEMA).

A hypothetical failure of Woods Pond Dam could occur either on a sunny day or during a

flood. For failure under the sunny day condition, COE assumed the failure to occur at a

reservoir elevation of 952.7 ft, 4.4 ft above the spillway crest. Failure under this condition

resulted in a maximum outflow of about 11,200 cfs and a water surface elevation of about

948.2 feet at a distance of 0.5 mile downstream from the dam. The FEMA study indicated a

100-year flood of about 11,700 cfs and a 500-year flood of about 16,300 cfs.

The water surface elevations computed by FEMA for the 100-year flood in the river reach

immediately downstream of the dam confilID the water surface elevation of 948.2 feet

estimated by COE. Further downstream, the dam break flood wave will attenuate resulting in

water surface elevations lower than the elevations based on a steady state flow of 11,200 cfs.

However, for a conservative estimate of the inundation limits under a sunny day failure, the

water surface elevations computed by FEMA conesponding to a flow of 11,700 cfs were

used. The inundation limits are shown on Exhibit 3 by a solid line.

Inundation downstream fi'om the dam during the spillway design flood or a flood

approaching the probable maximum flood (PMF) would be extensive compared to the solid

line shown on Exhibit 3. Contribution to the inundation area due to failure of the dam during

a flood would not be significant because of the small storage behind the dam at the spillway

crest elevation, only 460 acre-feet (COE). Due to flood surcharge, the storage will be about

5,300 acre-feet at elevation 954.0, the top of the abutments.

Under flood conditions, the COE assumed the dam to fail when the old canal (raceway)

embankment would have been oveliopped by two feet. For failure under this condition, the

maximum outflow would be about 16,000 cfs. This flow is nearly same as the 500-year flood

estimated by FEMA. The inundation limits based on this flood and assuming a steady state

flow are shown as dotted lines on Exhibit 3. The limits are conservative because a flood wave due to dam failure would attenuate and the water surface elevations would be lower

than those shown.

APPENDIXC

PLANS FOR TRAINING, UPDATING, AND POSTING THE EAP

Training

The Facility Manager will conduct a bienoial in-house EAP training session. In that session,

the Facility Manager will present detailed instmctions, as noted earlier in the plan, to GE

personoel to follow during a dam failure emergency. The instmction will include description

of the chain of command. Technical personoel will be trained in problem detection and

evaluation and appropriate remedial measures. Training may include testing the training and

readiness of key personoel responsible for action during an emergency to determine that they

know the required procedures and actions.

A statement on the training session will be included in the anoual review of the EAP review if a training session occurred during the previous year. The statement will include date of

session, attendance list, and summary of topics discussed. The annual review of the EAP will

include any plan revisions indicated by the outcome of the training.

Updating

GE will anoually review and update the EAP in accordance with DCR Regulations. Updates will reflect changes in names, titles, and phone numbers of people with responsibility to take

action during a dam emergency.

Review will identifY any new developments or other changes upstream or downstream, which

might necessitate revising the EAP. Ifthere is such a change, GE will promptly infotm DCR,

determine in consultation with agencies and others whether revision is necessmy, and

distribute any resulting revisions.

All updates and revisions will be immediately furnished to DCR (three copies) and all other

EAP holders (two copies) to keep the plan workable. Revised pages will be marked

"Updated Month Year".

Woods Pond Dam EAP Statement of Purpose

I. STATEMENT OF PURPOSE

This Emergency Action Plan (EAP) has been prepared on behalf of the General Electric

Company (GE) for Woods Pond Dam, which is owned by GE. The purpose of this EAP is

to:

I. Define the responsibilities of various officials involved in emergency management;

2. Provide procedures designed to identifY, in a timely manner, unusual and unlikely

conditions which may endanger the stlUctural integtity of Woods Pond Dam;

3. IdentifY actions to take to mitigate an ongoing or developing situation; and

4. Provide procedures to notifY the appropriate emergency management officials, if possible, of impending or actual failure of the dam.

In accordance with the regulations of the Massachusetts Department of Conservation and

Recreation, Division of State Parks and Recreation (DCR) (fOlmerly known as the Environmental Management (DEM» Regulations (Chapter 302 CMR 10.06, November 4,

2005), Woods Pond Dam is designated as a "large" size dam with a "significant" hazard

(Class II) potential. This size classification is based on the storage capacity that occurs during the spillway design flood, while the hazard potential classification is based on ~he

presumption that failure of the dam may result in the loss of life and significant propelty

damage immediately downstream of the dam.

According to DCR Regulations (Chapter 302 CMR 10.11, November 2005), all dams classified as "high" hazard (Class I) shall have an EAP. If the Commissioner requires it, a

facility with a non-high hazard dam shall also provide an EAP. DCR requested that GE

prepare an EAP for Woods Pond Dam; GE prepared this EAP in response to that request and

as an exercise of good engineering judgment. The EAP conforms to the requirements for

EAPs outlined in Section 10.11 of the DCR Regulations. All EAPs are subject to approval

by the DCR. The dam owner shall submit a copy of the proposed EAP to the local and state

emergency agencies and all local emergency coordinators involved in the plan for review.

The owner shall inCOlJlOrate applicable recommendations received and provide the

documented recommendations in appendices to the final version of the EAP. Annually, the

owner shall review the EAP and update it as necessaty

The EAP shall, at a minimum, contain the following:

1. The identification of equipment, manpower, and material available for

implementation of the Plan;

2. A notification procedure for infonning local emergency agencies;

3. A topographic map showing the stream which will be flooded; and

4. A procedure for waming nearby local residents if failure of the dam is imminent

and a listing of addresses and telephone numbers of downstream residents that

may be affected by failure ofthe dam.

As desClibed in Section IV, "Emergency Detection, Evaluation and Classification," two aleli

conditions have been adopted for Woods Pond Dam:

1. Condition A - Failure Is Imminent or Has Occurred, and

2. Condition B - Potential Failure Situation Is Developing

A notification flowchmt for each of these alert conditions is presented in this EAP.

II. PROJECT DESCRIPTION

A. Backgl'ound

The original Woods Pond Dam was reportedly constructed around 1864 on the Housatonic

River near the towns of Lee and Lenox, Massachusetts, as shown on Exhibit 1. The original

structure consisted of a 9-foot-high rock-filled timber crib overflow dam with a 14-foot-high

eatihfill embankment parallel to the river challllel as its east (left) abutment (left and right

refer to the side of the river challllel when viewed looking in the downstream direction).

There are no design drawings or information available for any of the original project

structures. The original dam structure was replaced in 1989 with a new concrete overflow

spillway and new abutments of concrete and steel sheet pile. The emihfill (raceway)

embankment separates the main river challllel from the 30 to 50-foot-wide raceway challllel along the left bank of the river. The raceway challllel conveys flow to a small mill pond,

which at one time was a forebay for a water-powered mill that has since been retired. The

dam impounds the Woods Pond Reservoir, part of the Housatonic River Valley Wildlife

Management Area.

Prior to the 1989 rehabilitation, Woods Pond Dam was in a deteriorated condition. The

original project could not pass a flood greater than about the lO-year flood event without overtopping the eatihen raceway embankment. Oveliopping of the embankment could have

caused a breach of the embankment, resulting in the uncontrolled release of the reservoir

water and sediments that had settled to the bottom of the reservoir.

Beginning in 1979, studies and investigations were performed to assess the condition of the

dam and develop recommendations for repair and rehabilitation of the dam. In 1983, the

timber planking and heavy gage sheet metal on the main overflow dam were replaced with an

18-inch-thick reinforced concrete cap. After placement of the cap, a leak developed at the

east abutment masomy wall, presumably due to the cut-off of natural seepage through the

timber crib dam. The joints in the masomy wall were filled with mortar shortly after the

1983 construction, temporarily stopping the leakage, but contributing to pressure buildup

behind the wall, causing lateral movement of the wall.

In 1988, a plan was developed to rehabilitate the dam to provide a safer, more reliable project

structure. The rehabilitation was carded out in two stages. The first stage consisted of the

construction of a raceway closure structure with concrete stoplogs to diveli flow away fi'om

the raceway and mill pond. The purpose of the closure structure is to protect the raceway

embankment from overtopping and potential failure during high flows. The second stage

consisted of the construction of a replacement spillway and non-overflow gravity dam a short

distance downstream of the original spillway. A rockfill berm was also constmcted on the

west (right) bank of the river between the original spillway and new spillway, and the top

2.5 feet the original timber crib spillway was demolished. The original sluice gate stmcture

at the downstream end of the raceway embankment was rehabilitated in 1991. This stmcture

controls the water level in the Mill Pond.

Additional modifications to the project were perfOlmed in 2001 to address recommendations

made in the January 2001 Stmctural Integrity Assessment RepOlt, and in 2003 to address

recommendations made in the May 2003 Stmctural Integrity Assessment Report. The 2001

modifications included repair to deteriorated masonry at the raceway closure stmcture;

removal of trees and other vegetation along the raceway embankment; regrading of the

raceway channel slopes; and removal of the masonry wall and reworking of the riprap at the

upstream raceway embankment. The 2003 modifications included removal of trees and other vegetation and the placement of riprap along the river side of the raceway embankment.

Based on the stmctural integrity assessment inspection conducted subsequently in

November 2004, the dam, raceway closure stmcture, and riprap constmcted in 1989 are in good condition, and there was no indication of deterioration or distress to the spillway and

tailrace.

MWH conducted a visual inspection in November 2005 and made recommendations for

some minor physical modifications. GE is in the process of implementing these

recommendations. For details, refer to 2005 Stmctural Integlity Assessment Report by

MWH, dated Aplil 2006.

A general plan of the rehabilitated project is shown on Exhibit 2. Drawings for the 1989

rehabilitation are included in Appendix A for reference. The "General Design Report for

Woods Pond Dam Rehabilitation" for the 1989 rehabilitation was filed with the DCR and is

included in Appendix B ofthe March 1999 Woods Pond Dam Inspection Report.

B. Project Featm'es

The present dam consists of a 140-foot-Iong concrete overflow spillway, a concrete non­

overflow gravity section at the west (right) abutment, and a concrete and steel sheetpile

raceway closure stmcture at the east (left) abutment, all constmcted since 1989. All the dam

structures are founded on bedrock. The dam has a maximum height of approximately 14 feet,

with an ogee-shaped spillway with a crest elevation of 948.3 feet National Geodetic Vertical

Datum (NGVD). The west (right) abutment is a concrete gravity structure with a sloped

downstream face and a top elevation of 954.0 feet NGVD. The raceway closure structure

forms the east (left) abutment of the dam and has a top elevation of 954.0 feet NGVD. The

drawings for the dam and raceway closure structure are provided in Appendix A. The current

structure is completely independent of the original structure and does not rely on any of the

original structures for support.

The CutTent structures are founded on shallow bedrock, which is vertically bedded and is

generally fine grained, hard with vmiable medium to close joint spacing. This was

determined duting the 1988 geoteclmical investigation program which included soil borings

and water pressure testing of the rock. Details of the subsurface field investigation can be

found in the "General Design Report for Woods Pond Dam Rehabilitation" filed with the DCR in 1989 and included in the March 1999 Woods Pond Dam Inspection RepOit.

The raceway embankment upstream of the dam is emthen fill with riprap over the surface and

is no longer a water-retaining structure serving no dam safety function. Failure of this portion of the raceway embankment would have no impact on the safety or integrity of the

dam.

Downstream of the dam and raceway closure structure is an earthen embankment protected with riprap along the length of the channel. This embankment is located between the tailrace

of the current overflow spillway and the raceway channel. The embankment has a crest

width of about 8 to 10 feet. The water elevation in the raceway is a few feet higher than the

water elevation in the tailrace and river channel downstream of the dam.

III. POTENTIAL FLOOD LIMITS

Exhibit 3 shows a map of potential flood limits due to the failure of Woods Pond Dam under

"Sunny-day" and "Flood-day" conditions. Flood limits are for failure when the abutments

would be overtopped by two feet. "Sunny-day" means a fair-weather, non-rainy day. "Flood­

day" means a rainy, non-fair-weather day. The tenn "day" includes nighttime. The potential

flood limits are plotted on Exhibit 3 to show the maximum inundation area due to dam failure

to the point downstream where the water would be contained within the banks of the river.

These limits were estimated based on the information and data provided in two reports

entitled:

1. "Woods Pond Dam, Phase I Inspection Report, Lee and Lenox, Massachusetts,

prepared for EPA Region 1, prepared by U.S. Anny Corps of Engineers, Omaha

District, September 1987,"

2. "Flood Insurance Study, Town of Lee, Massachusetts, Berkshire County by Federal

Emergency Management Agency, December 1, 1981."

IV. EMERGENCY DETECTION, EVALUATION, AND CLASSIFICATION

This EAP has been prepared to aid GE personnel in the timely detection, evaluation, and

classification of an existing or potential emergency situation at the dam. The EAP provides

two classifications of dam failure emergency according to their severity and urgency:

Condition A - Failure is Imminent or Has OCCUlTed, and Condition B - Potential Failure

Situation Is Developing. The conditions or measures for detection and evaluation of an

existing or potential emergency are described below.

For purposes of this EAP, GE has designated a Facility Manager and an Alternate Facility

Manager (as specified in Section V). The Facility Manager shall have primary responsibility

for catTying out this Plan on behalf of GE. In the event that the Facility Manager is

unavailable, the Alternate Facility Manager shall be the point of contact and shall have all the

duties and powers of the Facility Manager as set forth in this Plan. In addition, GE has pre­

selected contractors, also specified in Section V, to assist in emergency response under this

EAP.

A. Condition A - Failure is Imminent or Has Occurred

The following conditions may lead to the "Failure is Imminent or Has Occurred" emergency

situation. These conditions should not be considered inclusive of all conditions that might

occur:

• Rapidly developing sttuctural problems involving the spillway, raceway embankment,

abutments, and appUlienant structures. Some of the potential causes of developing

structural problems are listed on Exhibit 4.

The notification procedures of Section V, ''Notification Flowchatis and Contact Numbers"

and given on the flowchati for Condition A (Exhibit 5), should be invoked if any of the

situations listed on Exhibit 4 are observed. An imminent situation at the dam site will

probably manifest itself as an excessive increase in flow in the Housatonic River and/or an

unexpected drop or rise in water level in the reservoir, river channel, raceway channel or mill

pond.

When failure is observed, the Observer (as defined in Section V) shall inform the Facility

Manager. The Facility Manager shall decide the severity of the problem and invoke the

notification procedure if appropriate.

The message from the Facility Manager to the persons identified on the flowchart for this

situation (Exhibit 5) will contain the following information:

• Name and title (GE position) ofthe person making the call,

• A statement that this is an emergency call, due to a reported failure or imminent

failure of Woods Pond Dam,

• A statement that the person receiving the message must initiate contacts as per the

flowchart for "Condition A - Failure is Imminent or Has OCCUlTed,"

• A statement that waming and evacuation should begin immediately,

D, Condition D - Potential Failure Situation is Developing

The conditions that may lead to the "Potential Failure Situation is Developing" emergency

situation are also listed on Exhibit 4. Upon confirming any of the situations listed on Exhibit

4, the notification procedures will be implemented by the Facility Manager. There will be

two messages fi'om the Facility Manager for this situation. The first message will contain the

following information:

• Name and title (GE position) of the person making the call.

• A statement that a potential failure situation is developing at Woods Pond Dam that

may lead to failure ofthe dam.

• A statement that the person receiving the message should put his or her contacts on

alert for a second notification message.

• A statement that emergency personnel should be ready for an evacuation process.

If failure of the dam becomes imminent, the second message fi'om the Facility Manager shall

be as given in this section under "Condition A - Failure Is Imminent or Has Occurred."

If the dam failure is avelied and the developing situation is mitigated, the emergency will be

called off via the same communication channels. In such an event, the message will contain

the following information:

• Name and title (GE position) of the person making the call.

• A statement that the potential failure sihJation at Woods Pond Dam has been averted,

and that the recipients' contacts should stand down from emergency alert.

V. NOTIFICATION FLOWCHARTS AND CONTACT NUMBERS

The flowcharts shown in Exhibits 5 and 6 summarize who is to be notified and who is

responsible for notifYing individuals and organizations in the order designated for the

following emergency situations:

A. Failure Is Imminent or Has Occurred (Condition A): COiTesponds to situations where

a failure either has occUlTed, is occurring, or obviously is just about to occur. The

notification flowchart for Condition A is printed on red paper.

B. Potential Failure Situation is Developing (Condition B): COiTesponds to sihmtions

where a dam failure may develop, but preplanned actions may prevent or mitigate

failure. Generally, there is more time available under this condition than under Condition A to take preventative action and bring the situation under conh·ol. The

notification flowchart for Condition B is printed on yellow paper.

A copy of the flowcharts shall be prominently posted near the telephone or radio transmitter

of each person identified on the flowcharts as a caller. The key personnel of GE and police

depallments shall have copies in their offices and at homes (see Appendix C for EAP distribution).

Detailed infOimation supplementing the flowcharts is provided in Section N - Emergency

Detection, Evaluation, and Classification. The location of the dam, reservoir, and streams are described in the Section II - Project Description. Potential inundation limits are discussed in

Section III - Potential Flood Limits. In addition to the individuals and agencies listed as

contacts in Actions 1 through 8 for Condition A and Condition B, GE has additional

notification requirements under the Consent Decree for the GE-PittsfieldIHousatonic River

Site (the Consent Decree). The contacts for those notifications are listed in Action 9.

Paragraph 90 of the Consent Decree requires that notice be given "[i]n the event of any action

or occurrence during the perfOimance of the Work which causes or threatens a release of

Waste Material from the Site that constitutes an emergency situation or may present an

immediate threat to public health or welfare or the environment." The phrase "constitutes an

emergency situation or may present an immediate threat to public health or welfare or the

environment" is fil1lher defined to mean "an unforeseen combination of circumstances, or the

conditions resulting from such circumstances, not nOimally anticipated to occur as pall of the

Work, that require immediate action to avoid harm or an immediate threat of hann to human

health, welfare or the environment."

Woods Pond Dam EAP General Responsibilities Under The EAP

VI. GENERAL RESPONSIBILITIES UNDER THE EAP

Failure of Woods Pond Dam would present a potential hazard to downstream areas. Areas of

concern at the project include a few houses located near the gravel pit and sewage disposal

plant. Numerous downstream roads and houses, primarily in the flood plains and close to the

banks ofthe river, would be flooded. Refer to the inundation map, Exhibit 3.

Once an emergency condition is identified, it is important to the success of the EAP that

responsible personnel respond immediately. Specific responsibilities of the persons or

organizations for the maintenance and operation of the dam and implementing various phases

ofthe plan are discussed below.

A. General Electric Responsibilities

GE has a plant in Pittsfield, Massachusetts, located approximately 12 miles upstream from

the dam. A member ofGE's Corporate Environmental Programs (CEP) staff from that plant,

or a contractor designated by GE, will perform a routine visit to the dam site and its

appUltenant stmctures at least monthly (or more frequently as watTanted by weather, river levels, or other conditions). This person will be the "Observer," as defined in Section V

(,'Notification Flowcharts and Contact Numbers") in case of an occurrence at the dam. The

person will cany a two-way radio and/or telephone andlor pager, and will be properly equipped to perfOlID a watch dming nighttime surveillance and severe weather.

In addition, the Facility Manager or the designated contractor will conduct an inspection of

the dam and its appUltenant stmctures at least quarterly to identify any changes in site

conditions that might indicate a problem for the dam. (This inspection will be combined with

one of the monthly site visits described above.). GE will provide a report on each such

quatterly inspection to the Lead Administrative Trustee (LAT) designated under

Paragraph 113 of the Consent Decree.

GE will also arrange for a licensed professional engineer to conduct an assessment, at least

once evelY two years, of the structural integrity of Woods Pond Dam, and to provide to GE a

repOlt on that assessment and any recommendations of measures necessaty to maintain dam

integrity. GE will provide a copy of each such report to the LAT.

The GE Facility Manager will have the authority to declare an emergency at Woods Pond

Dam or to change the degree of severity of an existing occut1'ence.

Woods Pond Dam EM General Responsibilities Under The EAP

Upon receiving information of a threatening situation at Woods Pond Dam from the Observer

(or from other sources), it will be the primaty responsibility ofthe Facility Manager to direct

the designated person(s) of GE to proceed to the site. Included in this group will be one of

the persons who conduct routine inspections of the dam. It is the responsibility of this person

to know the signs of developing hazards such as those noted in Section IV, "Emergency

Detection, Evaluation and Classification," and to inform the Facility Manager immediately.

As a result of the site inspection, the Facility Manager will declare an emergency situation if

wan'anted, and will designate the proper degree of severity. The Facility Manager will also

initiate all notification calls.

The Facility Manager will serve as the emergency coordinator for all local agencies and will

direct all mitigation efforts at the dam site. The Facility Manager will also set up an appropriate central emergency headquarters from which all emergency operations will be

directed.

In notifYing the contacts involved with the situation, the Facility Manager will briefly state (after having queried the Observer for the information):

1. Name of dam 2. Emergency condition

• Condition A - Failure is Imminent or Has Occurred, or

• Condition B - Failure Situation is Developing

3. Severity of situation 4. Type of failure occUlTing or developing (e.g. overtopping, cracking or piping)

5. Time of observation, and time of failure, if occurred and known, otherwise estimate

The Facility Manager will keep local authorities and news media informed of changing

conditions at the dam site. However, the plan is not intended for the Facility Manager or his

designated person to assume the responsibility of govenunental authorities during an

emergency.

The Facility Manager is the EAP coordinator as described in Part E of this section. The

Facility Manager will closely coordinate efforts with the staff at the dam site and local

agencies to serve as a communications hub for the duration of the emergency.

The officials who must be notified are shown in Section V ("Notification Flowcharts and

Contact Numbers") and on Exhibits 5 and 6 (Notification Flowcharts). As GE's top

communications representative and day-to-day manager during the emergency, the Facility

Manager will be at the top of the command chain and will speak for GE and engineering

personnel at the site.

B. Responsibility foJ' Notification

The Facility Manager is authorized to notifY authorities of the emergency, including the

police in the Towns of Lenox, Lee and Stockbridge, as well as the Berkshire County Sheriffs

Depatiment, Emergency Management offices and other federal, state and local agencies listed

on the notification flowcharts (Exhibits 5 and 6). The Facility Manager will have the

authority to issue statements to the press concerning the situation at the dam.

The town police and fire departments will serve as the lead authorities during emergency

situations. Once notified of an emergency condition, they will be responsible for notifYing potentially affected downstream residents (listed in Appendix D) and coordinating warning

and evacuation procedures with other authorities, including the emergency management

offices and other county and town departments (fire, ambulance, and transportation).

Major responsibilities of these authorities include:

• Issuing warnings and evacuating residential and commercial buildings as needed

• Controlling traffic and placement ofbat rica des on roads

• Coordinating with federal and state agencies

• Coordinating utility services

• Setting up health centers

• Informing the public through radio and television media

• Providing shelter for evacuated people

• Maintaining law and order

The chiefs of police for the towns of Lenox, Lee and Stockbridge will serve as local

coordinators for their municipalities. These local coordinators will keep in contact with the

Facility Manager and will be responsible for requesting support fi'om other local, county or

state agencies as needed. The Facility Manager will be responsible for advising these local

coordinators of the need to warn and/or evacuate local residents depending upon the

conditions at the dam site.

Local coordinators will have the authority to release that infonnation necessary to assure the

safety of the public and to expedite an orderly evacuation when required. GE will coordinate

press releases concerning conditions at the dam with local coordinators. The local public

information centers will be responsible for informing the public of the provisions of this plan

and coordinating public input.

C. Responsibility for Evacuation

Following notification by the Facility Manager, the warning and evacuation of residents, as

necessary, will be perfonned by the local police, fire and civil defense departments.

Residents will be moved to pre-designated temporary shelters.

The Lenox and Lee DPWs and the Stockbridge Highway Depat1ment will take the lead roles

in placing and manning batl'icades on the affected roads, with assistance from police, to

prevent motorists fi'om entering the inundated areas. A map showing potential flood limits is

attached as Exhibit 3.

D. Responsibility for Duration, Security, Termination, and Follow-Up

As noted above under GE's responsibilities, the Facility Manager will keep local authorities,

primarily through the police depat1ments and county emergency management offices, advised

of conditions at the dam by phone or messenger.

The local authorities will be responsible for tennination of the emergency response activities,

with the advice of the Facility Manager.

For any emergency situation, after completion of all notification responsibilities, all officials,

including GE personnel, will maintain a log, report, or tape recording of all notifications and

decisions made by their offices for the duration of the emergency. The reports should include

the following information:

• Subject: (what happened and name or title of incident)

• Time and date:

E. EAP Coordinator Responsibility

The Facility Manager will be responsible for activities related to the EAP, including

reviewing and revising the plan, distributing copies of the flowchat1s and plan, and

establishing training for personnel in charge of the dam (see Appendix C). Also, the Facility

Manager will be the contact should any questions arise about the plan.

Woods Pond Dam EAP Preparedness

VII. PREPAREDNESS

Preparedness actions are taken to prevent a dam failure or to help reduce the effects of a dam

failure and to facilitate a response to emergencies.

A. Surveillance

An emergency situation may be detected by inspection of the dam or by observation of

reservoir levels.

Surveillance of the dam will be made in accordance with the procedures described in Section

VI.A (General Responsibilities under the EAP - General Electric Responsibility). If night

inspections are made, the inspector should be accompanied by a second individual, and they

will cany flashlights and a two-way radio (walkie-talkie) or cellular phone to communicate.

Any observance of a failure or potential failure situation will be recorded and action shall be

taken immediately. Such situations are given in Section IV ("Emergency Detection,

Evaluation, and Classification").

Based on experience, a breach could fully develop in an hour or less after breach initiation.

The flood wave could reach the downstream residences in a few minutes. Therefore,

readiness for quick implementation of the EAP is important.

B. Response During Periods of Darkness

Once an emergency situation is detected, notification procedures during periods of darkness

will be the same as dmlng daylight hours. Home phone numbers are provided on the

notification flowcharts to facilitate contact dmlng non-business hours.

For any response actions taken during periods of darkness, at least two individuals should be

present, and they will cany flashlights and a two-way radio (walkie-talkie) or cellular phone

to communicate.

Due to the availability ofphones and two-way radios, no special procedure will be needed for

contacting people on the appropdate flowchart.

Once an emergency situation is detected, notification procedures during weekends and

holidays will be the same as during weekdays. Home telephone numbers provided on the

notification flowchatts will be used during weekends and holidays. Notification response

time during weekends and holidays is expected to be similar to response times for

emergencies occurring during weekdays.

E. Response During Periods of Adverse Weather

During a severe storm, some roads may be under water that would be dry during fair weather conditions. Consequently, emergency-response vehicles may have to bypass roads that are

under water in order to get to the dam or affected areas.

Response during adverse weather (a storm) following observation of an emergency condition,

comprising notifications and warning to downstream areas, is expected to take more time

than under normal conditions.

F. Availability and Use of Alternative Communication Systems

Telephones will be the main system of communication on and off the dam site. Also, two­

way radios (walkie-talkies) will be available for site communication.

If the above methods fail, communications on-site and with emergency agencies in towns of

Lenox, Lee and Stockbridge will proceed by messenger.

G. Emergency Supplies and Resources

GE will be responsible for emergency supplies and resources for on-site use. GE will rely on

its designated contractor to assist in providing such supplies and resources, as described

below.

Woods Pond Dam EAP Preparedness

GE and its designated contractor will provide men and equipment for emergency repair of the

dam. GE's designated contractors and a list of the equipment they are responsible for

providing, are listed in Appendix E. Fill, comprised of earth and rock, would be obtained

from available borrow areas. However, due to possibly rapid development of a breach and

consequently a very short wal11ing time, emergency repair of a dam will not be relied on as a

preventative or control action.
