Medical BenefitsSuperior Technical Ceramics Corporation offers a medical plan administered by CIGNA Group. The Medical Plan features a deductible, office visit copayment, prescription drug coverage and coinsurance for certain services.
Through these plans you have access to thousands of network physicians and hospitals in the United States. You, the employee, and your dependents are responsible for ensuring the providers that you utilize are In Network. To access a listing of providers, logon to the CIGNA Group website. www.mycigna.com

Dental BenefitsThe Superior Technical Ceramics Corporation’s Dental Plan is designed to provide the dental coverage you need with the features you want. Take advantage of what this plan has to offer without compromising what matters most - including the freedom to visit the dentist of you and your dependents choice — an “in-network” dentist or an “out-of-network” dentist.
For the best savings, use a CIGNA Group participating dentist or specialist. You can find a dentist by visiting CIGNA Group website, www.mycigna. You can also call CIGNA Group 1-866-494-2111. Just show your dental plan card when you visit the dentist. If you choose a dentist who does not participate in our dental plan, your out-of-pocket expenses may be more, since you will be responsible for paying any difference between the dentist’s fee and the plan’s payment for the approved service.
If Dental work is required, request a pretreatment estimate from your Dentist. Your Dentist will contact CIGNA Group, then you and your dentist can review your care and costs before treatment. It’s a great way to be prepared and plan ahead.

Our vision plan is administered by Vision Service Plan which offers a Preferred Provider Network for you to choose a vision care provider. To access a listing of providers (private practice and retail centers) logon to Vision Service Plan website.

Income Protection Benefits- Coverage starts first of the month after 30 daysBasic Life and AD&D Insurance
Superior Technical Ceramics Corporation provides a Basic Life and AD&D benefit to eligible employees through CIGNA Group. The Life insurance benefit will be paid to your designated beneficiary in the event of death while covered under the plan. The AD&D benefit will be paid in the event of a loss of life or limb by accident while covered under the plan.
Disability Coverage
Disability insurance is designed to cover a portion of your salary when you are unable to work due to an accident or illness. The unexpected could happen at anytime. If you suddenly didn’t receive a paycheck, how would you pay your bills? It’s important to consider that sick and vacation time will only go so far.
The Short Term Disability Plan is offered at no cost to you. Please review the brief description below.
LIFE & AD&D CIGNA GROUP LIFE AND AD&D
Life (Employee)2 times annual compensation rounded to the next higher $1,000, OR maximum of $250,000. Providing you are still employed, benefits are
reduced to 50% at age 70, to 30% at age 75 and 20% at age 80.
AD&DIn the event of an accidental dismemberment, a benefit is
provided up to a scheduled amount corresponding to the loss. Please see your booklet for further details.

Combined Time Off (CTO)
Combined Time Off (CTO) is available to regular full time employees on a calendar year basis for the purpose of rest, relaxation and personal needs. CTO may be used for vacation, personal illness, illness of spouse or dependents, celebration of holidays that are not designated company holidays, children’s school events or other personal business. Please see your HR for more details.

401(k) Retirement
The Company has a 401K savings plan administered by John Hancock, operated and funded solely through employee contributions. Employees are eligible the first of the quarter following 6 months of employment, and will be notified of their eligibility by Human Resources. Participants can cease making contributions to the Plan at any time, but must wait until the beginning of each quarter to resume or change contributions. Participants are fully vested in their contributions to the Plan. Refer to the Summary Plan Document for more detailed information.
Auto Deferral Feature
All newly eligible employees will automatically have 3% withheld from paychecks and a default John Hancock account will be setup for them. Employees can opt out of this feature by completing a John Hancock Employee Change Request form and handing it in to Human Resources.
Profit-Sharing
Pension Plan
The Company has a fully funded, vested profit sharing plan in which employees participate after completing 1000 hours of service during the twelve (12) consecutive months preceding the enrollment dates of June 30 or December 31. Participants are subject to a six (6) year vesting schedule. The Summary Plan Description document is available in Human Resources for review.
Staff Development Benefit
In addition to any training or development activities employees may be required to attend as part of their regular job duties or responsibilities, all employees are encouraged to take advantage of educational opportunities that will enable them to improve their job skills. The Company’s educational assistance program provides financial support to employees who wish to improve their work-related skills and who have been employed for a minimum of one (1) year. Courses must be preapproved and job skill and undergraduate degree related. The Company will approve an employee to participate in one (1) course per semester. The annual per employee contribution cannot exceed $5,250 per year. All educational activities must be approved by your manager and Human Resources.

For domestic partners that do not qualify as dependents under Section 152 of the Internal Revenue Code, premium associated with domestic partner coverage will be paid by the employee with after-tax dollars and the fair market value of any employer contributions made on behalf of your domestic partner will be imputed as income to the employee. Unless otherwise requested, premiums will automatically be deducted on a pre-tax basis.
