PARKING: IF YOU ARE NOT A CITIZEN OR PERMANENT RESIDENT OF THE U.S.: Faculty Services (ISFS) by email at [email protected]. received you will need to sign the Termination form. NOTE: Regular students are not required to submit a resignation or termination letter. Employee Information Last Name First Name Middle Name University ID Submit a written resignation to your supervisor (e.g., letter, email , etc.). If a written resignation is not EMPCENTER TIMESHEET: Enter and submit your FINAL EmpCenter timesheet. If applicable, include hours worked and any leave that will be used. While this is an Estimate, it is the expectation that it is final. If the Estimate changes, notify your supervisor immediately. Return your OSU ID Card to your supervisor. Return all OSU keys and access cards to your supervisor. Return all OSU equipment borrowed to the Department or University. Email Business Affairs at baff.accounting @oregonstate.edu that you are leaving the University and destroy the credit card. If you are a card custodian or a budget authority for an OSU PCard, email Business Affairs at [email protected] with your name and last four digits of the card number that you are leaving the University, destroy the PCard, and notify other users that they will need to contact Business Affairs for a new card. Contact International Scholar and Graduate students: Contact Student Health Services at [email protected] or (541)737-7568. If you have questions regarding continuation of coverage. Classified and Unclassified Employees: Contact Employee Benefits if you have questions regarding continuation of your coverage through COBRA, life insurance, long-term care, etc. Email: [email protected] or call (541)737-2805. If your bank account is changing, or your final check will be a manual check (paper check), or you need to change your final check delivery, contact your Business Center to discuss check delivery arrangements.
